本願は、2009年3月31日に出願された米国仮特許出願第61/145,142号の利益を主張し、この米国仮特許出願は本明細書中に参考として援用される。
本開示は、健常人と比較して、シェーグレン症候群患者において示差的に発現されるマイクロRNA、およびシェーグレン症候群を診断および治療するための、上記開示マイクロRNAの使用に関する。
シェーグレン症候群は、免疫細胞によって涙および唾液を産生する腺が攻撃され、破壊される自己免疫障害である。この障害の顕著な特徴である症状は、口内乾燥およびドライアイである。シェーグレン症候群は、皮膚、鼻および膣の乾燥も引き起こす可能性があり、腎臓、血管、肺、肝臓、膵臓および脳を含めた体内の他の器官に影響を及ぼす可能性がある。シェーグレン症候群は、米国の1〜4百万人に影響を及ぼしており、女性は9倍この疾患を発症しやすい。シェーグレン罹患者の大多数は、診断される時点で40歳以上である。シェーグレン症候群は、一次状態として、または、全身性エリテマトーデス(「ループス」)または関節リウマチなどの他の自己免疫疾患に付随する二次障害として発生し得る。
マイクロRNA(miRNAまたはmiR)は、遺伝子発現を制御する小さな一本鎖RNA分子である。シェーグレン症候群患者が、健康な対照の被験体と比較して示差的なmiR遺伝子発現を示すことが本明細書に開示されている。いくつものmiRが、唾液の流れが不十分な患者と比較して唾液の流れが正常なシェーグレン症候群患者において、ならびに病巣スコアが高い(高度の炎症)シェーグレン症候群患者と病巣スコアが低い(低度の炎症)シェーグレン症候群患者において、示差的に発現されることも本明細書に開示されている。

II.用語および方法

健康な対照の被験体と比較して、シェーグレン症候群患者において示差的に発現されるマイクロRNAを同定することが本明細書に記載されている。唾液の流れが不十分なシェーグレン症候群患者と比較して、正常な唾液の流れを示すシェーグレン症候群患者において上方制御されるマイクロRNAも開示されている。470種のヒトマイクロRNAおよび64種のヒトウイルスマイクロRNAのマイクロアレイ発現解析を使用して、対照と比較して、シェーグレン症候群患者において少なくとも2倍、上方制御または下方制御される多数のマイクロRNAを同定した。場合によっては、マイクロRNA遺伝子産物の発現は45倍または92倍と同程度に変化した。したがって、被験体における1種または複数種の示差的に発現されるマイクロRNAの発現を測定することにより、被験体がシェーグレン症候群を有すると診断することができる。
マイクロRNA(miRNAおよびmiRとしても公知である)は、動物、植物およびウイルスならびに少なくとも1種の単細胞真核生物のゲノムにおいて見出されるより長い転写物から発現される短いRNA配列である(Molnarら、Nature 447巻:1126〜1129頁、2007年;Zhaoら、Genes Dev. 21巻:1190〜1203頁、2007年)。マイクロRNAは、標的遺伝子転写物の相補的部位に結合して翻訳抑制または転写物の分解を引き起こすことによって標的遺伝子の発現を制御する(Pillaiら、Trends Cell Biol. 17巻:118〜126頁、2007年)。これらの低分子RNA分子は、発生、細胞増殖、アポトーシス、代謝、形態形成および疾患(とりわけ、癌)に関連するいくつもの生物学的なプロセスに結びつけられている(KloostermanおよびPlasterk、Dev. Cell 11巻:441〜450頁、2006年)。
V.マイクロRNAの発現を検出および測定する方法
マイクロRNAは、種々の疾患を診断し、予後判定するためのバイオマーカーとして見込みがある非翻訳RNA分子である(Calinら、N Engl J Med 353巻:1793〜801頁、2005年;Johnsonら、Cell 120巻:635〜47頁、2005年;Voliniaら、Proc Natl Acad Sci U S A 103巻:2257〜61頁、2006年;Yanaiharaら、Cancer Cell 9巻:189〜98頁、2006年;Cumminsら、Proc Natl Acad Sci USA 103巻:3687〜92頁、2006年)。多数の予後バイオマーカーを組み合わせることにより、個々のバイオマーカーと比較して、シェーグレン症候群などの特定の疾患を診断し、治療する能力が改善される可能性がある。したがって、本明細書において提供される方法の一部の実施形態では、シェーグレン症候群を診断するため、およびシェーグレン症候群患者に対して予後を予測し、見込みのある治療を開発するためにマイクロRNAの発現プロファイルを使用する。
A.RT−PCR
マイクロRNAを含めたRNAを定量化するための方法は、当技術分野で周知である。一部の実施形態では、方法は、RT−PCRを利用する。一般に、RT−PCRによる遺伝子発現プロファイリングの最初のステップは、RNA鋳型をcDNAに逆転写することであり、続いてそのcDNAをPCR反応において指数関数的に増幅させることである。一般的に使用される2種の逆転写酵素は、トリ骨髄芽球症ウイルス逆転写酵素(AMV−RT)およびモロニーマウス白血病ウイルス逆転写酵素(MMLV−RT)である。しかし、当技術分野で公知の任意の適切な逆転写酵素をRT−PCRに使用することができる。逆転写ステップは、一般に、状況および発現プロファイリングの目的に応じて、特異的なプライマー、ランダムヘキサマーまたはオリゴdTプライマーを使用してプライミングする。例えば、抽出されたRNAを、GeneAmp RNA PCRキット(Perkin Elmer、CA)を製造者の説明書に従って使用して逆転写することができる。次いで、得られたcDNAをその後のPCR反応における鋳型として使用することができる。
B.遺伝子発現の連続分析(SAGE)
SAGEは、数多くの遺伝子転写物を、各転写物に対して個々のハイブリダイゼーションプローブを供給する必要なく同時かつ定量的に分析することを可能にする別の方法である。まず、転写物を一義的に同定するために十分な情報を含有する短い配列タグ(約10〜14塩基対)が各転写物特有の位置から得られれば、そのタグを生成する。次いで、多くの転写物を互いに連結して、配列決定することができる長い連続的な分子を形成し、多数のタグのアイデンティティを同時に明らかにする。転写物の任意の集団の発現パターンを、個々のタグの存在量を決定し、各タグに対応する遺伝子を同定することによって定量的に評価することができる(例えば、Velculescuら、Science 270巻:484〜7頁、1995年;およびVelculescuら、Cell 88巻:243〜51頁、1997年を参照されたい)。
ISHは、対象とする遺伝子の発現を検出し、比較するための別の方法である。ISHでは、核酸ハイブリダイゼーションの技術を単一細胞レベルまでに適用し、推定の基礎とし、細胞化学、免疫細胞化学および免疫組織化学の分野と組み合わせて、維持し同定されるべき細胞マーカーの形態を維持し同定することを可能にし、組織および血液の試料などの集団中の特異的な細胞に配列を局在化させることができる。ISHは、相補的な核酸を使用して、組織の一部分または一切片に(in situ)、または組織が十分小さければ、組織全体に(ホールマウントISH)1種または複数種の特異的な核酸配列を局在化させる、ハイブリダイゼーションの1種である。RNA ISHを使用してマイクロRNAの発現などの組織中の発現パターンをアッセイすることができる。
In situ PCRは、ISHの前の、PCRに基づく標的核酸配列の増幅である。RNAを検出するために、細胞内逆転写ステップを導入して、in situ PCRの前にRNA鋳型から相補的DNAを生成する。これにより、低コピーのRNA配列を検出することができる。
本明細書で提供される特定の実施形態では、アレイを使用してマイクロRNAの発現を評価すること、例えばシェーグレン症候群を診断または予知することができる。特定のマイクロRNAのセット(miR−126、miR−150、miR−183、miR−189、miR−200c、miR−212、miR−22、miR−30b、miR−326、miR−328、miR−494、miR−513、miR−548c、miR−564、miR−565、miR−574、miR−575、miR−585、miR−768−3p、miR−768−5p、miR−801、miR−9、ebv−miR−BART13およびebv−miR−BART19など)に特異的なプローブまたはプライマーを含むアレイについて述べる場合、そのようなアレイは、列挙したマイクロRNAに特異的なプローブまたはプライマーを含み、対照プローブ(例えば、インキュベート条件が十分であるかを確認するため)および場合によって追加のマイクロRNAに対するプローブをさらに含んでよい。典型的な対照プローブとしては、GAPDH、アクチンおよびYWHAZが挙げられる。一実施例では、アレイはマルチウェルプレート(例えば98ウェルプレートまたは364ウェルプレート)である。
アレイの固体支持体は、有機ポリマーから形成することができる。固体支持体の適切な材料としては、ポリプロピレン、ポリエチレン、ポリブチレン、ポリイソブチレン、ポリブタジエン、ポリイソプレン、ポリビニルピロリジン、ポリテトラフルロエチレン、ポリ二フッ化(difluroide)ビニリデン、ポリフルオロエチレン−プロピレン、ポリエチレンビニルアルコール、ポリメチルペンテン、ポリクロロトリフルオロエチレン(polycholorotrifluoroethylene)、ポリスルホン、水酸化二軸延伸ポリプロピレン、アミノ化二軸延伸ポリプロピレン、チオール化二軸延伸ポリプロピレン、エチレンアクリル酸、エチレン(thylene)メタクリル酸、およびそれらのコポリマーの混合物が挙げられるが、これらに限定されない(米国特許第5,985,567号を参照されたい)。
多種多様なアレイの形式を、本開示に従って使用することができる。一例としては、当技術分野で一般にディップスティックと称されるオリゴヌクレオチドバンドの直線状アレイが挙げられる。別の適切な形式としては、別々の細胞の二次元パターン(64×64アレイの4096個のます目など)が挙げられる。当業者に理解されている通り、これらに限定されないが、スロット(長方形)アレイおよび環状アレイを含めた他のアレイの形式が、同様に使用に適している(米国特許第5,981,185号を参照されたい)。一部の実施例では、アレイはマルチウェルプレートである。一実施例では、アレイは、糸、膜またはフィルムであるポリマー媒体上に形成される。有機ポリマー媒体の例は、厚さ約1ミル(0.001インチ)〜約20ミルのポリプロピレンシートであるが、フィルムの厚さは重要ではなく、相当広範囲にわたって変動してよい。アレイは、耐久性に加え、低バックグラウンドの蛍光を示す二軸延伸ポリプロピレン(BOPP)フィルムを含んでよい。
遺伝子発現は、上記の任意の技法、または当技術分野で公知の任意の他の方法を用いて評価することができる。本明細書に記載の通り、遺伝子発現は、例えば、標準の装置を使用して検出することができる標識したプローブを使用して測定することができる。例えば、マイクロアレイまたはRT−PCR(一般には遺伝子産物に特異的な標識したプローブを使用する)を用いた遺伝子発現の測定は、マイクロアレイスキャナーまたは標識を検出するための他の適切なスキャナーを使用して定量化することができる。一部の実施形態では、遺伝子発現を測定するために使用するデバイスはマイクロアレイスキャナーである。マイクロアレイスキャナーは周知であり、Agilent TechnologiesからのModel G250GB Microarray Scannerなど、市販されている。
多くのマイクロRNAがシェーグレン症候群患者において示差的に発現されることが本明細書に開示されている。そのように、シェーグレン症候群患者において下方制御される1種または複数種のマイクロRNAのレベルが上昇すること、またはシェーグレン症候群患者において上方制御される1種または複数種のマイクロRNAのレベルが低下することが、シェーグレン症候群の発症もしくは進行を阻害するため、および/またはシェーグレン症候群の1つまたは複数の徴候または症状(例えば、唾液の流れが低下すること)を軽減するために有益であり得る。
シェーグレン症候群患者を、対照(健康な対照の被験体など)と比較してシェーグレン症候群患者において上方制御される、または、対照(唾液の流れが正常なシェーグレン症候群患者など)と比較して唾液の流れが不十分なシェーグレン症候群患者において上方制御されるmiR遺伝子産物の発現を阻害する作用剤を治療有効量で患者に投与することによって治療する方法が本明細書で提供される。
B.下方制御されるマイクロRNAをコードする核酸分子の使用
シェーグレン症候群患者を、対照(健康な被験体など)と比較して、シェーグレン症候群患者において下方制御される、または、対照(唾液の流れが正常なシェーグレン症候群患者など)と比較して、唾液の流れが不十分なシェーグレン症候群患者において下方制御される単離されたマイクロRNA遺伝子産物を治療有効量で患者に投与することによって治療する方法も提供される。対照(唾液の流れが正常なシェーグレン症候群患者など)と比較して、唾液の流れが不十分なシェーグレン症候群患者において下方制御される単離されたmiR遺伝子産物を治療有効量で投与することによってシェーグレン症候群患者における唾液の流れを回復させる方法がさらに提供される。本明細書に記載の通り、miR遺伝子産物は、pri−miRNA、pre−miRNAまたは成熟miRNAであってよい。
治療剤は、治療を必要とする被験体に、当技術分野で公知の任意の適切な手段を用いて投与することができる。投与方法としては、管内投与、皮内投与、筋肉内、腹腔内投与、非経口投与、静脈内投与、皮下投与、膣投与、直腸投与、鼻腔内投与、吸入投与、経口投与または遺伝子銃による投与が挙げられるが、これらに限定されない。鼻腔内投与は、片方の鼻孔または両方の鼻孔を通して鼻および鼻腔内に組成物を送達することを指し、噴霧機構もしくは液滴機構によって送達すること、または核酸またはウイルスをエアロゾル投与することによって送達することを含んでよい。吸入による組成物の投与は、噴霧機構または液滴機構による送達を介して鼻または口を通すことができる。挿管を介して呼吸器系の任意の領域に直接送達することができる。非経口投与は、一般に注射によって実現される。注射剤は、従来の形態で、溶液もしくは懸濁液として、注射前の懸濁溶液に適した固形として、または乳剤としてのいずれかで調製することができる。注射溶液および注射懸濁液は、滅菌粉末、滅菌顆粒および滅菌錠剤から調製することができる。投与は、全身的または局所的であってよい。
シェーグレン症候群患者において示差的に発現されるマイクロRNAの同定
材料および方法
試料の取得およびマイクロRNAの単離
小唾液腺試料を、原発性シェーグレン症候群のコーカサス人種の女性から、および正常なボランティアから得た。すべての被験体は、Clinical Center of the National Institutes of Healthにおいて完全なリウマチ検査、口内検査および眼科検査ならびに実験室評価を受けた。すべてのシェーグレン症候群患者が原発性シェーグレン症候群に対する欧州−米国診断基準を満たした。生検時の患者の年齢中央値は、対照群については43.5歳(21〜58歳の範囲にわたる)、病巣スコアが低い群については58歳(23〜67歳の範囲にわたる)、および病巣スコアが高い群については51.5歳(37〜70歳の範囲にわたる)であった。
470種のヒトmiRNAおよび64種のヒトウイルスmiRNAに特異的なプローブを用いたAgilent miRNA Microarrays(V1)(Agilent Technologies)(スライドあたり8アレイ)を使用して小唾液腺miRNAをプロファイリングした。Agilent Small RNA Chipにおける最適miRNA曲線である(製造者の説明書により決定された)Agilent RNA 6000 Nano ChipにおいてRNA完全性の数値(RNA integrity number)が7を超え、NanoDrop 8000における260/280比および260/230比が2.00を超えた試料のみをマイクロアレイ解析に使用した。各アレイに対して、全RNA100ngを使用した。アレイを製造者のプロトコールに従って実行した。マイクロアレイを、Model G2505B Microarray Scanner(Agilent Technologies)を使用してスキャンし、データを抽出し、Agilent Feature Extraction Softwareを使用してアレイの品質管理を行った。
正規化。マイクロRNAアレイデータを、以下の通り、小唾液腺アレイにおいて同定された(推定)ハウスキーパーマイクロRNAのセットを使用して正規化した。ハウスキーパーマイクロRNAは、定義によれば、すべての組織試料において一定のレベルで発現される。したがって、2種のハウスキーパーの強度比は、正規化と無関係にデータセット内のすべてのアレイにわたって同じであるはずである。このような挙動をする可能性のあるハウスキーパーマイクロRNAを探索するために、すべてのアレイにおいて、Agilent Feature Extraction Softwareによって存在するとしてスコア化された「候補」マイクロRNA(すなわち、すべてのアレイにおいてgIsGeneDetected=1であるマイクロRNA;そのようなマイクロRNAが全部で132種あった)について考察した。アレイデータを、これらの候補マイクロRNAのそれぞれの強度に対して順々に正規化した。各場合において、正規化されたアレイにわたって、他の候補マイクロRNAすべてについて平均および変動係数(CV)を算出し、CV<0.22である候補マイクロRNAの数、すなわち、アレイを正規化するために使用したマイクロRNAに同調して発現が変動すると思われる他の候補マイクロRNAの数を係数した。
炎症性スコアが様々である15検体の小唾液腺試料に対して、TaqMan(商標)microRNA Assay(Applied Biosystems)を使用してリアルタイム定量PCR分析を行った;これらの試料は、本明細書に記載のマイクロRNAマイクロアレイ解析において使用した試料とは独立したものであった。出発材料10ng(感度限界内で確実に検出されるように最適化試験分析に基づいて決定された)を使用して、製造者の説明書に従って逆転写を行った。どちらもApplied Biosystemsから取得した(それぞれ製品番号4395188および4395460)マイクロRNAhsa−mir−768−3pおよびhsa−mir−574−3pを検出するために、特異的なマイクロRNAプライマーを使用した。簡単に述べると、逆転写させるために、Veriti 96ウェルサーマルサイクラー(Applied Biosystems、Foster City、CA)において、16℃で30分間、42℃で30分間、85℃で5分間、15μLのRT反応を行った。リアルタイムPCRを、ABI Prism 7500(Applied Biosystems)で行った。各PCR反応を3連で行った。20μLのPCR反応を、95℃で10分間、その後、95℃で15秒間の変性ならびに60℃で60秒間のアニーリングおよび伸長を40サイクルのサイクル条件で行った。
マイクロRNAマイクロアレイデータの解析
解析方法
Agilent GENESPRING(商標)プログラム9版および10版、ならびにBRB Arrayツールパッケージ安定リリース3.7.0版を用いて統計分析を行った。GENESPRING(商標)を用いて分析するためにアレイの製造者の説明書に従ってデータをインポートし、正規化した。

結果
表1は、正常な健康なボランティアの小唾液腺と比較して、シェーグレン症候群患者の小唾液腺において示差的に発現されるマイクロRNAの一覧を提供する。発現が2倍以上増加または減少するマイクロRNAが示されている(p=0.01に設定した統計的有意性)。マイクロRNAの発現における倍率変化(真ん中の列)、および、シェーグレン症候群患者由来の唾液腺と比較して、正常な唾液腺においてマイクロRNAが上方制御される(「上方」)か、下方制御される(「下方」)か(右側の列)についても列挙されている。この表において、シェーグレン症候群患者由来の小唾液腺は、それらの病巣スコア(炎症の尺度)または唾液の流れと関係なく一緒に分類されている。すべての正常なボランティアの小唾液腺の病巣スコアは0であった。

リーブワンアウト(Leave−one−out)交差検証法を使用して、誤分類率をコンピュータで計算した。(ベイズの)複合共変量予測に使用したT値を、abs(t)=10レベルで切り捨てた。同等のクラス分布率をベイズの複合共変量予測において使用する。ベイズの複合共変量予測からクラスに予測される試料についての予測確率の閾値は0.8である。

感度は、クラスA試料について、クラスAであると正しく予測される確率である。特異性は、クラスAでない試料について、Aでないと正しく予測される確率である。PPVは、クラスAであると予測された試料が実際にクラスAに属する確率である。NPVは、クラスAでないと予測された試料が実際にクラスAに属さない確率である。各分類方法および各クラスについて、これらのパラメータが以下の表9〜表15に列挙されている。
予測規則は、有意な遺伝子の重量(wi)および発現(xi)の内部合計によって定義される。発現は、デュアルチャネルデータについての対数比および単一チャネルデータについての対数強度である。試料は、合計が閾値(すなわち、Σiwixi閾値)を超えるとクラスNに分類される。複合共変量予測に対する閾値は−2237.041であり;対角線形判別予測に対する閾値は−1226.374であり;サポートベクターマシン予測に対する閾値は−3.933である。
遺伝子の重量
シェーグレン症候群患者の小唾液腺のマイクロRNAプロファイリング
試料およびアレイハイブリダイゼーション
全部で24検体の小唾液腺由来の試料を、マイクロRNAマイクロアレイ上でハイブリダイズさせた。これらの腺のうち8検体は健康な対照に由来し、16検体は原発性シェーグレン症候群(SS)患者に由来した。原発性シェーグレンの試料の半数は病巣スコアが12の広範な炎症を有し、半数は病巣スコアが1または2の低度の炎症を有した。病巣スコアが高い群と病巣スコアが低い群の両方の中で、患者の半数が、唾液の流れが不十分であり、半数が唾液の流れが保たれていた(図1)。これらの臨床パラメータを有する患者を選択して、正常な対照とSS患者との間だけでなく、炎症の程度が異なる患者間、または炎症のレベルが同様の患者の機能低下した唾液腺と機能が保たれている唾液腺との間で特異的なマイクロRNAのパターンに差異があるかどうかを試験するために、マイクロRNAの変化を探査することを可能にした。
正規化は、生物学的な変動ではなく技術的な変動を表す試料間の系統的な差異を取り除くことができるので、マイクロアレイデータ解析において重要なステップである。マイクロRNAアレイデータの解釈における主要な制限は、種々の正規化法の有用性および妥当性に明確な一致を欠くことである。mRNAマイクロアレイを解析するために開発された伝統的な正規化法の多くは、マイクロRNAデータセットとmRNAデータセットとの間に著しい差異があるので、マイクロRNAプロファイリングには適さない可能性がある(Pradervandら、RNA 15巻:493〜501頁、2009年)。例えば、mRNAと比較して、マイクロRNAの多くは非常に低いレベルで発現する、または全く発現せず、アレイ当たりのマイクロRNAがmRNAよりもずっと少ない(何万ものmRNAと比較してマイクロRNAは何百)。多くの正規化法では、mRNAの強度分布は疾患または実験条件にわたって不変であると想定されているが、この想定がマイクロRNAにあてはまるかは全く明白でない。
i)主成分分析
主成分分析(PCA)は、変動のほとんどを保持しながらデータセットの次元を縮小するための数学的方法である。マイクロアレイデータの場合、第1主成分は、データ内の最大量の変動性を説明する、発現パターンの線形結合である。第2主成分は第1主成分とは独立しており、残りの最大量の変動性などを説明する。したがって、PCAにより、実験変動性の大半を保持する2次元または3次元の多次元データセットの視覚化を可能にする教師なし分析がもたらされる。図3に唾液のマイクロRNAアレイのPCA分析結果を示す。各点は、最初の3主成分に沿った座標に従ってプロットしたアレイを表す。この分析により、すべてのSS試料(小さな円および大きな円)と健康な対照(中くらいの円)とが明白に分離し、ならびに病巣スコアが低い試料(大きな円)および病巣スコアが高い試料(小さな円)が互いに分離していることが示されている。したがって、この分析により、これらの種々の群を、それらのマイクロRNAプロファイルによって区別することができることの強力な証拠がもたらされる。
次に、正規化したマイクロアレイ発現データの階層クラスタリングを行った。階層クラスタリングは、別の教師なし分類法であり、マイクロRNAの発現の対での類似性の定義済みの尺度によって決定された試料を合併することによってクラスターを同定する。2つのクラスター間の距離を、すべての要素対の間の距離の平均として算出する、平均連結クラスタリングを使用した。PCAと同様に、健康な対照由来の小唾液腺試料と、病巣スコアが高いSS試料および病巣スコアが低いSS試料の両方とが明確に分離された(図4)。健康な組織と病巣スコアが高い試料とが分離したことは、それらが、細胞組成が非常に異なる組織を表すので、驚くべきことではなかった。しかし、健康な組織と病巣スコアが低い組織とが明白に区別されたことにより、マイクロRNAプロファイルが、組織学的差異が最小である小唾液腺間を区別するために十分な感度であることが示されている。さらに、miRNAプロファイルにより、炎症の程度が異なる試料間が良好に区別されただけでなく、病巣スコアが高い試料の中で、唾液の流れが保たれている試料と唾液の流れが低下した試料とが明白に分離された。
次に、示差的に発現されるマイクロRNAにより、健康なクラスのメンバーであること対SSクラスのメンバーであることを予測することができるかどうかを試験した。種々の予測アルゴリズム(すなわち、複合共変量予測、対角線形判別分析、1−最近傍、3−最近傍、最も近いセントロイド、サポートベクターマシンおよびベイズの複合共変量予測)のすべてにより、被験体が患者である、または健康な対照であると正確に分類され、事例の100%で1.0の感度および1.0の特異性の両方が得られた(表7)。分類器は、有意性のレベル0.001で、クラス間で有意に異なる58種のマイクロRNAで構成された。リーブワンアウト交差検証法を使用して誤分類率をコンピュータで計算した。
マイクロアレイで観察された差異を確認するため、およびシェーグレン患者のサブグループ間を区別するために選択されたマイクロRNAを使用することの潜在性を査定するために、正常群、病巣スコアが低い群および病巣スコアが高い群の間で反対方向に変化した2種のマイクロRNAを同定した。病巣スコアが上昇すると、これらのマイクロRNAの一方(hsa−miR−768−3p)は上昇し、他方(hsa−miR−574)は低下した。図5に例示した実験では、これらの2種のマイクロRNAの発現パターンを病巣スコアの予測子として使用することの実現可能性を、マイクロアレイの試験に使用した試料とは独立した15の試料のセットにおいて試験した。TaqMan(商標)リアルタイムPCRを用いて同じ試料において同時に決定したこれらの2種のマイクロRNA間のCt差異が示されている。病巣スコアが低い試料(FS:0〜2)、病巣スコアが中程度の試料(FS:5〜7)および病巣スコアが高い試料(FS:12)の間でこれらのCt差異に統計的有意差が見られた。Ct差異は、これらの2種のマイクロRNAの発現比の尺度であり、したがって試料の正規化とは無関係であることを示すことが重要である。
診断、疾患の活動性または組織損傷などの臨床的特性との強い関連を示すことに加えて、信頼性のあるバイオマーカーは、根底にある重要な生理学的プロセスを反映しなければならない。したがって、これらの知見の潜在的な生物学的重要性を、示差的に発現されるマイクロRNAに影響を受ける可能性がある生理学的プロセスを同定することによって査定した。まず、種々の臨床群の中で最も有意に示差的に発現されるマイクロRNAを同定した。これは、信頼水準75%で最大で1つの偽陽性を許容するように設定したBRB ArrayToolsのクラス比較アルゴリズムを使用して実現した。これにより、健康群、病巣スコアが低いSS群および病巣スコアが高いSS群の間で示差的に発現される94種のマイクロRNAが得られた(p<0.01)(表19)。
種々の臨床群の間で有意に示差的に発現したマイクロRNAの一覧(p<0.01)
A.非炎症性パターン(n=13):この群のマイクロRNAは、健康群と低病巣スコア群との間で>5倍に増加したが、病巣スコアが低い群と病巣スコアが高い群との間では基本的に変化しなかった(<50%の増加または減少)(すなわち、それらの発現レベルは炎症と相関しなかった)。

特異的なマイクロRNAが、炎症が激しい状況における機能低下に関連するかどうかをさらに探索するために、病巣スコアが高い患者を、唾液の流れが保たれているか、または唾液の流れが不十分であるかのいずれかと比較した。階層クラスター解析から予測された通り(図4)、病巣スコアが高い群において、唾液の流れが正常なものと唾液の流れが保たれているものとの間でいくつものmiRNAが示差的に発現された(表21)。

マイクロRNAバイオマーカーの供給源としてのヒト唾液由来エキソソーム
本実施例には、ヒト唾液由来のエキソソームからmiRNAを単離するための典型的なプロトコールが記載されている(Michaelら、Oral Dis. 16巻:34〜38頁、2010年も参照されたい)。
研究の被験体
被験体は、健康なボランティアに対するプロトコールまたはシェーグレン症候群の自然経過の研究において登録された。
腺の唾液の流れを刺激するために、患者に、舌の後外側表面に2%クエン酸溶液を、両側に綿棒で30秒毎に5秒塗布することを受けさせた。採取手順の間中、クエン酸刺激を30秒間隔で続けた。耳下腺の唾液を、以下の通り採取した。Carlson Crittendenの耳下腺採取器を、上顎第2大臼歯の反対側の頬側粘膜のStensonの管開口部の開口に両側的に置いた。耳下腺採取器を、内輪が管開口を囲むように粘膜に置いた。圧迫することによって真空を創出し、外輪からの吸引により採取器を粘膜に保持し、管開口部全面に設置する間、圧縮されたバルブを保持し、その後カップが適切な位置になったときにバルブをはずした。顎下の唾液/舌下の唾液を以下の通り採取した。耳下腺管の開口部を採取器で覆い、2%クエン酸を少なくとも5回舌に塗布した後、口底を乾燥させ、20秒間穏やかに吸引して氷上のチューブに唾液を採取した。次いで採取を停止し、2×2のガーゼを顎下腺管の開口部の全面に置き、舌に2%クエン酸を塗布した。唾液を穏やかに吸引して同じチューブに採取し、ガーゼを用いて再度採取を停止した。プロセス全体を最大8回繰り返した。
唾液中のエキソソームを単離するためのプロトコールを、尿中のエキソソームを単離するための先行の方法から適合させ、改変した(Zhouら、Kidney Int 74巻:613〜621頁、2008年)。採取した直後に唾液を氷上に置き、研究室に移して4℃で10分間、1500gで遠心分離した。次いで上清を取り出し、別のチューブに入れて4℃で15分間、17,000gで遠心分離して不必要な細胞小器官および細胞断片をさらに取り除いた。最初の遠心分離ステップの後、上清を滅菌チューブに移して、4℃で1時間、160,000gで超遠心分離した。超遠心分離した後、全唾液試料において粘性である水層を取り除き、エキソソームを含有するペレットをPBSで洗浄し、4℃で1時間、160,000gで再度超遠心分離した。2回目の超遠心分離の後、上清を取り除き、ペレットを簡単に乾燥させた。次いで試料を、タンパク質またはRNAを単離する準備をした。
エキソソームのタンパク質を分析する前に、10mMのトリエタノールアミン、250mMのショ糖および脱イオン水を混合することによって単離緩衝液の保存溶液を作製した。次いで、1Nの水酸化ナトリウムを用いて単離緩衝液のpHをpH7.6に調整した。脱イオン水を加えて単離緩衝保存溶液の総体積を50mLにした。溶液を−20℃で保管した。使用する直前に、単離緩衝液1mLにプロテアーゼ阻害剤を加えた(フッ化フェニルメチルスルホニル50マイクロリットル(2mg/ml)およびロイペプチン10マイクロリットル(1mg/ml)、どちらも−20℃で保管)。エキソソームを単離した後、ペレットを、プロテアーゼ阻害剤を含有する単離緩衝液100マイクロリットルに再懸濁させた。同体積の2×Laemmli緩衝液(Biorad、Hercules、CA)を加え、試料を60℃で10分間変性させた。ウェスタンブロットを用いてTSG101の存在を決定した。試料をNuPAGE Novex 4〜12% Bis−Tris Gel(Invitrogen、Carlsbad CA、USA)に供した。セミドライ転写ユニットを使用してタンパク質を膜に転写した。1:7500希釈したTSG101抗体(Abcam、(ab83)、Cambridge、MA)を用いてウェスタンブロットを行った。
エキソソームを単離した後、検出されたRNAがすべてエキソソーム由来であることを確実にするために、ペレットをRNase Aで処理して、いかなる残存細胞RNAも分解した。一部の試料を、RNase A(Puregene−Gentra Systems、Valencia、CA)、4mg/mlの溶液、脱イオン水中0.4mg/mlの作用濃度を用いて、37℃で10分間処理した。次いで、試料のエキソソームをmiRNeasy溶解緩衝液(Qiagen、Valencia、CA、USA)600マイクロリットルで溶解させ、後で使用するために−80℃で保管した、またはQiagenのmiRNeasy Kitを製造者のプロトコールに従って使用してすぐに処理した。すべてのRNA試料を、RNaseを含まない水50マイクロリットル中に溶出させた。エキソソームRNAの濃縮および沈殿の助けになるように、Novagenのペレットペイントを製造者のプロトコールに従って、軽微な改変をして使用した;RNA試料に2マイクロリットルのペレットペイントを加えた。ペレットペイントを加えた後、体積0.1の3M酢酸ナトリウムを試料に加え、試料を10秒間混合した。混合した後、体積2.5の100%エタノールを試料に加え、簡単にボルテックスした。次いで、試料を室温で2分間インキュベートし、4℃で5分間遠心分離した。遠心分離した後、エキソソームRNAを含有するペレットを70%エタノール200マイクロリットルで洗浄し、RNaseを含まない水に再懸濁させる前に風乾させた。次いでUV−Vis分光光度計(Nanodrop 8000)を使用してRNAを定量化し、Agilent 2100 Bioanalyzerを使用して、品質を査定し、RNase処理した試料と未処理試料の両方において低分子RNAの存在を検証した。
Exiqon miRNAマイクロアレイシステム(miRCURY LNA(商標)microRNA Array、10.0版)を使用して、耳下腺の唾液および顎下腺の唾液から単離されたエキソソームmiRNA、ならびにシェーグレン症候群患者の耳下腺の唾液中のエキソソームmiRNAに対してマイクロアレイハイブリダイゼーションを行った。使用した出発材料がアレイの製造者の推奨よりも下限にあったことを除いて製造者のプロトコールに記載の通りに試料の標識およびハイブリダイゼーションを行った。簡単に述べると、対照にスパイクしたmiRNAを250ngの唾液中のエキソソームマイクロRNAに加え、仔ウシ腸ホスファターゼで処理した。次いで、試料をHy3またはHy5のいずれかで標識し、変性させ、56℃で16時間、アレイ上でハイブリダイズさせ、洗浄し、Agilentスキャナー(Model G2505B)でスキャンした。データをAgilentのFeature Extractionアルゴリズムで処理した。
200μlから最大5mLまでの体積範囲の唾液試料により、定量PCRに適した量のエキソソームRNAが得られた。マイクロRNAは実に小体積の唾液から単離されたが、RNAの収量は少数の定量PCR反応に十分なだけのものであった。7日間−20℃で凍結させた唾液からエキソソームを単離することも可能であった。腺の唾液および全唾液の両方からエキソソームを単離することが可能であるが、全唾液は、その粘性および細胞のコンタミネーションにより、エキソソームを単離するための理想に満たない媒体になる。したがって、この試験は主に腺の唾液のみを焦点とした。

- 被験体がシェーグレン症候群を有すると診断するため、かつ/またはシェーグレン症候群を有する前記被験体を治療するための、前記被験体の生体試料中の少なくとも1種のマイクロRNA(miR)遺伝子産物の発現レベルの使用であって、前記少なくとも1種のmiR遺伝子産物がmiR−150、ebv−miR−BART13、ebv−miR−BART19、miR−768−3p、miR−574、miR−513、miR−188、miR−202、hcmv−miR−US4、miR−565、miR−509、miR−154、miR−99b、miR−564、miR−30bまたはmiR−409−3p遺伝子産物であり、ここで、
(i)前記被験体の前記生体試料における、対照と比較したmiR−150の前記レベルの上昇、ebv−miR−BART13の前記レベルの上昇、ebv−miR−BART19の前記レベルの上昇、miR−768−3pの前記レベルの上昇、miR−574の前記レベルの低下、miR−513の前記レベルの上昇、miR−188の前記レベルの上昇、miR−202の前記レベルの上昇、hcmv−miR−US4の前記レベルの上昇、miR−565の前記レベルの上昇、miR−509の前記レベルの上昇、miR−154の前記レベルの上昇、miR−99bの前記レベルの上昇、miR−564の前記レベルの上昇、miR−30bの前記レベルの上昇またはmiR−409−3pの前記レベルの上昇、またはそれらの組合せにより、前記被験体がシェーグレン症候群を有することが示され、ここで前記上昇または低下が診断上有意な量であり;かつ/または

- 前記対照と比較して、シェーグレン症候群を有する前記被験体の前記生体試料において、miR−150が少なくとも38倍増加する、ebv−miR−BART13が少なくとも21倍増加する、ebv−miR−BART19が少なくとも92倍増加する、miR−768−3pが少なくとも3倍増加する、miR−574が少なくとも2倍減少する、miR−513が少なくとも16倍増加する、miR−188が少なくとも7倍増加する、miR−202が少なくとも5倍増加する、hcmv−miR−US4が少なくとも6倍増加する、miR−565が少なくとも7倍増加する、miR−509が少なくとも18倍増加する、miR−154が少なくとも3倍増加する、miR−99bが少なくとも5倍増加する、miR−564が少なくとも29倍増加する、miR−30bが少なくとも4倍増加する、またはmiR−409−3pが少なくとも3倍増加する、またはそれらの組合せである、請求項1または請求項2に記載の使用。
- 前記生体試料が唾液腺である、請求項1から4までのいずれか一項に記載の使用。
- 前記唾液腺が小唾液腺である、請求項5に記載の使用。
- 前記唾液腺が耳下腺唾液腺である、請求項5に記載の使用。
- 前記生体試料が唾液である、請求項1から4までのいずれか一項に記載の使用。
- 前記生体試料が血液、血清または血漿である、請求項1から4までのいずれか一項に記載の使用。
- シェーグレン症候群と診断された前記被験体に対する適切な療法または第2の適切な療法を提供することをさらに含む、請求項1から9までのいずれか一項に記載の使用。
- 前記療法が、唾液産生を促進する作用剤を投与することを含む、請求項10に記載の使用。
- 前記療法がコルチコステロイドを投与することを含む、請求項10に記載の使用。
- 前記療法が、免疫抑制薬を投与することを含む、請求項10に記載の使用。
- 前記療法が、非ステロイド系抗炎症薬を投与することを含む、請求項10に記載の使用。
- シェーグレン症候群を有する患者において対照と比較して上方制御されるmiR遺伝子産物の発現を阻害する治療有効量の作用剤、またはシェーグレン症候群を有する前記患者において対照と比較して下方制御される治療有効量の単離されたmiR遺伝子産物の使用であって、シェーグレン症候群を有する患者を治療するための薬剤の調製における使用。
- 前記上方制御されるmiR遺伝子産物が、miR−150、ebv−miR−BART13、ebv−miR−BART19、miR−768−3p、miR−513、miR−188、miR−202、hcmv−miR−US4、miR−565、miR−509、miR−154、miR−99b、miR−564、miR−30bまたはmiR−409−3p遺伝子産物である、請求項15に記載の使用。
- miR遺伝子産物の発現を阻害する前記作用剤が、前記miR遺伝子産物に特異的なアンチセンス化合物である、請求項15または請求項16に記載の使用。
- 前記アンチセンス化合物がアンチセンスオリゴヌクレオチド、siRNAまたはリボザイムである、請求項17に記載の使用。
- 前記下方制御されるmiR遺伝子産物が、miR−183、miR−189、miR−200c、miR−22、miR−326、miR−328、miR−548c、miR−574、miR−585、miR−768−5pまたはmiR−9遺伝子産物である、請求項15に記載の使用。
- 前記下方制御されるmiR遺伝子産物がmiR−574遺伝子産物である、請求項15に記載の使用。
- 前記治療が、シェーグレン症候群を有する前記被験体における唾液の流れを回復させることを含む、請求項15から20までのいずれか一項に記載の使用。
- 病巣スコアが高いことまたは病巣スコアが低いことを診断するための、シェーグレン症候群患者の生体試料中の少なくとも1種のmiR遺伝子産物の発現レベルの使用であって、前記少なくとも1種のmiR遺伝子産物がmiR−150、miR−768−3p、miR−574、miR−513、miR−188、ebv−miR−BART19、miR−501、miR−126*、miR−342、miR−330、miR−135b、miR−142−5pまたはmiR−650遺伝子産物であり;
(i)シェーグレン症候群を有する前記患者の前記生体試料における、対照と比較した、miR−150の前記レベルの上昇、miR−768−3pの前記レベルの上昇、miR−574の前記レベルの低下、miR−513の前記レベルの上昇、miR−188の前記レベルの上昇、ebv−miR−BART19の前記レベルの上昇、miR−501の前記レベルの上昇、miR−126*の前記レベルの上昇、miR−342の前記レベルの上昇、miR−330の前記レベルの上昇、miR−135bの前記レベルの上昇、miR−142−5pの前記レベルの上昇、またはmiR−650の前記レベルの上昇、またはそれらの組合せにより、前記シェーグレン症候群患者の病巣スコアが高いことが示される;または
(ii)シェーグレン症候群を有する前記患者の前記生体試料における、対照と比較した、miR−150の前記レベルの低下、miR−768−3pの前記レベルの低下、miR−574の前記レベルの上昇、miR−513の前記レベルの低下、miR−188の前記レベルの低下、ebv−miR−BART19の前記レベルの低下、miR−501の前記レベルの低下、miR−126*の前記レベルの低下、miR−342の前記レベルの低下、miR−330の前記レベルの低下、miR−135bの前記レベルの低下、miR−142−5pの前記レベルの低下、またはmiR−650の前記レベルの低下、またはそれらの組合せにより、前記シェーグレン症候群患者の病巣スコアが低いことが示され;
前記上昇または低下が、診断上有意な量の上昇または低下である、
使用。 - 前記少なくとも1種のmiR遺伝子産物がmiR−768−3p遺伝子産物、miR−574遺伝子産物、またはその両方であり、前記対照と比較した、miR−768−3pの前記レベルの上昇、miR−574の前記レベルの低下、またはその両方により、前記シェーグレン症候群患者の病巣スコアが高いことが示される、請求項22に記載の使用。
- 前記対照が健康な被験体由来の生体試料である、請求項1から23までのいずれか一項に記載の使用。
- シェーグレン症候群を治療するための方法、またはシェーグレン症候群患者における唾液の流れを回復させるための方法で使用するための、シェーグレン症候群を有する患者において対照と比較して上方制御されるmiR遺伝子産物の発現を阻害する治療有効量の作用剤、またはシェーグレン症候群を有する前記患者において対照と比較して下方制御される治療有効量の単離されたmiR遺伝子産物。
- シェーグレン症候群を治療するための治療剤をスクリーニングするためのin vitro方法であって、(i)細胞培養物を候補作用剤と接触させるステップと;(ii)miR−150、ebv−miR−BART13、ebv−miR−BART19、miR−768−3p、miR−574、miR−513、miR−188、miR−202、hcmv−miR−US4、miR−565、miR−509、miR−154、miR−99b、miR−564、miR−30bおよびmiR−409−3p遺伝子産物から選択される少なくとも1種のmiR遺伝子産物のレベルを測定するステップを含み、対照と比較して、miR−150、ebv−miR−BART13、ebv−miR−BART19、miR−768−3p、miR−513、miR−188、miR−202、hcmv−miR−US4、miR−565、miR−509、miR−154、miR−99b、miR−564、miR−30bおよび/またはmiR−409−3pのレベルの低下、および/またはmiR−574のレベルの上昇により、候補作用剤がシェーグレン症候群を治療するための治療剤であると同定される方法。
- 表1〜6および表16〜21のいずれか1つに列挙されているmiR遺伝子産物から選択されるmiR遺伝子産物と特異的にハイブリダイズする少なくとも2種のオリゴヌクレオチドを含むアレイ。
- miR−150、ebv−miR−BART13、ebv−miR−BART19、miR−768−3p、miR−574、miR−513、miR−188、miR−202、hcmv−miR−US4、miR−565、miR−509、miR−154、miR−99b、miR−564、miR−30bおよびmiR−409−3pからなる群から選択されるmiR遺伝子産物と特異的にハイブリダイズする少なくとも2種のオリゴヌクレオチドを含む請求項27に記載のアレイ。
- 被験体がシェーグレン症候群を有すると診断するための、またはシェーグレン症候群を有する被験体に対する適切な療法を選択するための、請求項27または請求項28に記載のアレイの使用。
- 表1〜6および表16〜21のいずれか1つに列挙されているmiR遺伝子産物から選択されるmiR遺伝子産物に特異的な少なくとも2種のオリゴヌクレオチドプローブを含むキット。
- miR−150、ebv−miR−BART13、ebv−miR−BART19、miR−768−3p、miR−574、miR−513、miR−188、miR−202、hcmv−miR−US4、miR−565、miR−509、miR−154、miR−99b、miR−564、miR−30bおよびmiR−409−3pからなる群から選択されるmiR遺伝子産物に特異的な少なくとも2種のオリゴヌクレオチドプローブを含む、請求項30に記載のキット。
