Abstract
Description
多発性硬化症(「MS」)は、中枢神経系の炎症性自己免疫障害である(Compston and Coles, Lancet 372,1502−17(2008))。MSは、約1000人に1人の罹患率で、若年成人における神経障害の最も一般的な原因である(Polman and Uitdehaag,BMJ321,490−4(2000))。MSは、免疫系の関与、軸索および神経膠の急性炎症性損傷、機能の回復および構造修復、炎症後神経膠症、ならびに神経変性を伴う(例えば、Compston and Coles,2008を参照のこと)。これらの連続的プロセスは、回復を伴う発現、持続性の欠損、および二次性進行を残す発現によって特徴付けられる、臨床経過の根拠をなす(同上)。
多発性硬化症患者
本発明の方法および組成物は、例えば、再発−寛解型MS、一次性進行型MS、および二次性進行型MS等、あらゆる形態のMSの背景において、使用され得る。本発明の文脈におけるMS患者とは、例えば、症状の経歴、ならびに試験等磁気共鳴画像装置(MRI)、脊椎穿刺、誘発電位試験、および血液試料の研究所分析の活用による神経学的検査によって、MSの形態を有すると診断されている患者である。
多発性硬化症患者におけるリンパ球除去
本明細書で使用されるとき、「リンパ球除去」とは、循環リンパ球、例えば、T細胞および/またはB細胞を減少させることによる、免疫抑制の一種であり、リンパ球減少をもたらす。持続的リンパ球除去は、多発性硬化症を治療するために、例えば、自己骨髄移植(BMT)または全身リンパ節照射が使用されるときに見られる。例えば、Cox et al.,Eur.J.Immunol.35、3332−3342(2005)を参照のこと。例えば、リンパ球除去は、サイモグロブリン、シクロホスファミド、および全身照射を組み合わせた使用によって達成され得る。MS患者におけるリンパ球除去はまた、幾つかの薬物治療によっても達成され得る。例えば、ヒト化抗CD52モノクローナル抗体である、CAMPATH−1H(アレムツズマブ)は、MS患者を治療するためのリンパ球除去療法に使用されている。CAMPATH−lH−誘発型リンパ球減少は、臨床的かつ放射線学的に、中枢神経系炎症を効果的に低減させることが示されている(Coles et al.,Ann.Neurol.46,296−304(1999)、Coles et al.,2008)。
MS患者における二次性自己免疫
本明細書で自己免疫は、それが第1の(「一次性」)疾患、例えば、「一次性」自己免疫疾患の発現の後に生じるとき、「二次性自己免疫」と呼ばれる。二次性自己免疫は時に、例えば、リンパ球除去療法に続く、リンパ球減少を有するか、有したことのあるMS患者において生じる。一部の個人においては、二次性自己免疫は、リンパ球除去療法の後間もなく生じる(例えば、アレムツズマブを用いた治療)。他の個人においては、二次性自己免疫は、リンパ球除去療法の数ヶ月または数年後まで生じない可能性があり、それらの個人の一部においては、彼らが二次性免疫を発症する頃には、かなりのリンパ球回復(合計リンパ球数)が生じ得、彼らはもはやリンパ球減少症ではなくなる可能性がある。
IL−21の測定
本発明の方法において、IL−21は、幾つかの技術によって測定され得る。IL−21は、ガンマc関連サイトカインファミリーの一員であり、TおよびB細胞増殖、ならびにナチュラルキラー(NK)細胞の細胞毒性を促進するのに強力な活性を有する。IL−21は、主に活性CD4+T細胞(例えば、ThI7細胞)によって発現され、TヘルパータイプI(ThI)免疫応答に重要である(Weiss et al.,Expert Opin Biol.Ther.7,1705−1721(2007)、Sivakumar et al.,Immunology112,177−182(2004))。ヒトIL−21遺伝子は、162アミノ酸残基のポリペプチド前駆体および133アミノ酸残基(約15kD)の十分に進行した成熟タンパク質をコードし、遺伝子は、ヒト染色体4q26−27に配置される(Sivakumar et al.,2004)。IL−21(IL−2IR)に対する受容体は、休止抹消B細胞上、活性抹消血液単核細胞上、およびヒトリンパ節の胚中心内で見出さている(Marleau et al.,J.Leukocyte Biol.78、575−584(2005))。
IL−21SNP遺伝子型の検出
本発明の幾つかの方法において、患者が、リンパ球減少を有する一方で、上昇したIL−21を有する傾向があり(すなわち、有するリスクがある、または有する可能性がある)、故に二次性自己免疫疾患を発症するリスクがあるかどうかを予測するために、遺伝子型決定が使用される。「遺伝子型決定」は、生物検定の使用により個人の遺伝子を決定するプロセスに言及する。遺伝子型決定の方法は、当業者に周知であり、制限なしに、PCR、DNA塩基配列決定、アレル特異的オリゴ(ASO)プローブ、およびDNAマイクロアレイまたはビーズに対するハイブリダイゼーションを含む。遺伝子型決定は部分的であり得、すなわち、個人の遺伝子型の小さな断片のみが決定される。本発明の文脈においては、ある種のSNPのみが、検出される必要がある。SNPは、ゲノムの対応部分における単一のヌクレオチド−A、T、C、またはGが、種の員の間または個人における対合染色体の間で異なる場合に発生するDNA配列変異である。既知のヒトSNPは、National Center for Biotechnology Information(NCBI)でホストされる、パブリックドメインアーカイブSingle Nucleotide Polymorphism Database(dbSNP)内における、指定リファレンスSNP(refSNP or rs)識別番号である。
IL−21情報を獲得する時期
MS患者のIL−21(IL−21レベルまたはIL−21関連SNP遺伝子型)についての情報を獲得することは、患者のための治療および治療後モニタリング計画を選択するのに有用である。MS治療の前に情報が獲得される場合、患者は、リンパ球除去療法の後に二次性自己免疫疾患を発症する相対リスクについて、通知され得、それに応じて治療決定が下され得る。患者はまた、彼が「リスクがある」と分類される場合、強化された治療後モニタリング、例えば、専門医による、より頻繁でより徹底的な検査の必要性について通知され得る。したがって、IL−21情報は、MS治療におけるリスク管理(医師、薬剤師、および患者による)を改善する。
二次性自己免疫の治療
MS患者において生じる二次性自己免疫疾患は、疾患のタイプに基づいて治療され得る。本発明の幾つかの実施形態では、二次性自己免疫は、有効用量のIL−21拮抗薬を使用することによって治療され得る。IL−21拮抗薬は、IL−21活性を阻害する治療剤、例えば、IL−21とIL−21Rとの間の相互作用を阻害する薬剤であり得る。「有効用量」は、二次性自己免疫疾患の症状が緩和または予防されるような、患者におけるIL−21活性を阻害するのに十分な阻害剤の量に言及する。IL−21拮抗薬は、例えば、ヒトIL−21もしくはIL−2IRに対するキメラ、ヒト化、またはヒトモノクローナル抗体、あるいは可溶性IL−2IRタンパク質であり得る。また例えば、米国特許第7,410,780号および米国特許出願公開第20080241098号を参照されたく、それらの教示全体は、参照により本明細書に組み込まれる。IL−21拮抗薬を含有する薬学的組成物は、当業者に既知の方法に従って作製され得る。IL−21拮抗薬を含有する薬学的組成物は、当分野で既知の好適な方法、例えば、静脈注射、筋肉注射、または皮下注射を使用して患者に投与され得る。
MS患者を治療および試験するためのキット
本発明は、多発性硬化症を治療するためのキットを提供する。本発明のキットは、特に、リンパ球除去薬(例えば、アレムツズマブ)と、患者または医療提供者に、この薬物の禁忌、例えば、この薬物での治療後に二次性自己免疫疾患を発症するリスクの上昇についての潜在性を通知するための使用説明書と、を含み得る。リスクの上昇は、(i)上昇したIL−21、または(ii)SNP rs13151961におけるA/A、SNP rs6822844におけるG/G、およびSNP rs6840978におけるC/Cからなる群から選択される一塩基多型(SNP)の1つ以上の遺伝子型の存在に関連し得るか、またはそれによって示され得る。
MS治療に対するT細胞の応答性の評価
本発明は、MS患者において、リンパ球除去療法での治療に対するT細胞の応答性を評価するための方法を提供する。方法は、治療後に患者から獲得されたT細胞内のカスパーゼ−3の測定を伴う。治療を受けていないMS患者からのT細胞と比較して、上記T細胞中のカスパーゼ−3の増加(例えば、カスパーゼ−3タンパク質、RNAの転写、および/または活性レベル)は、治療された患者のT細胞がその治療に応答したことを示す。これらの方法は、治療されていないMSを持つ個人からのT細胞がアポトーシス抵抗性であり、この抵抗性が、カスパーゼ−3の低発現に関連するという我々の発見に基づく。しかし、リンパ球除去療法の後、カスパーゼ−3の発現は、T細胞内で有意に増加し、健康な個人において見られるレベルに達する。
実施例
以下の実施例では、全ての患者が再発寛解型多発性硬化症を有し、アレムツズマブが5日間、12〜24mg/日の静脈内注入によって与えられ、その後12ヶ月で再治療された、CAMMS−223およびCAMMS−224(REC 02/315および03/078)の2つの臨床治験のうちの1つの参加者であった。患者および対照は、リサーチ目的(LREC 02/263)の静脈切開に同意し、全て、血液採取の時点で少なくとも1ヶ月間、ステロイド類を含む他の疾患修飾薬に曝露されていなかった。
アレムツズマブ単回投与により、CD4+およびCD8+Tリンパ球が、1ヶ月目にベースライン値のそれぞれ5.6%および6.8%に、そして12ヶ月目にそれぞれ30.3%および40.8%(データ示さず)に減少された。
本実施例で行われる種々の検定には、入手可能性に従って、異なる長期横断的試料を使用した。測定するアレムツズマブ後のリンパ球細胞周期を測定する前置きとして、多発性硬化症を持つ治療されていない患者と正常な対照(図1Aおよび1B)との間で、刺激されていないか、またはミエリン塩基性タンパク質(MBP)もしくは甲状腺刺激ホルモン受容体(TSHr)で培養中の、T細胞の増殖応答について調査した。
A.末梢単核細胞培養物
末梢血単核細胞(PBMC)を、Ficoll−Paque密度勾配(Amersham Pharmacia Biotech)での遠心分離によって、ヘパリン添加血液から単離した。全PBMCを、直ちに1%ペニシリン、1%ストレプトマイシン、および10%ウシ胎仔血清(Sigma S5394)を含有する培養培地(RPMI)中に懸濁させ、106/mLの生存細胞の濃度に調製した(トリパンブルー排除によって測定)。受動的細胞死を誘発するために、PBMCを、増殖因子を付加することなく、培地のみの中で、72時間インキュベートした。PBMCを、抗CD3 mAbプレコートプレート(1μg/mL−BDPharmingen)中、可溶性抗CD28(1μg/mL:ご好意によりM.Frewin(University of Oxford)により供与)とともに48時間培養し、その後、活性化抗ヒトFas(クローンCHl 1、1μg/mL−Upstate Biotechnology、Lake Placid,NY)とともに18時間インキュベーションして、Fas媒介アポトーシスが誘発された。
B.アポトーシスの検出
アポトーシスT細胞は、CD3(Serotec MCA463APC)、CD4(Serotec MCA1267APC)、およびCD8(Serotec MCA1226APC)に対してアロフィコシアニン共役マウス抗ヒトモノクローナル抗体、FITC共役アネキシン−V、およびPropidium Iodide(BDPharmingen)で細胞を染色することによって検出した。蛍光性は、フローサイトメトリー(FACSCALIBUR:Becton Dickinson、Mountain View,CA)によって検出した。前方および側方散乱に基づき、生存およびアポトーシスリンパ球(減少したFScおよび増加したSScを有する)を含むように広いリンパ球ゲートが引き出された。そのゲート内で少なくとも15,000の事象を収集し、WinMDI 2.8ソフトウェアを使用して分析した。早期アポトーシス細胞は、アネキシンV+PI−として、および後期アポトーシス細胞もしくは壊死細胞は、アネキシンV+PI+として定義した(Aubry et al.,Cytometry 37、197−204(1999))。アポトーシス細胞死は、Q−VD−OPh(RnDSystems OPH001)での汎カスパーゼ阻害によって遮断される総細胞死(アネキシンV+PI−+アネキシンV+PI+)として定義した。
C.増殖検定
PBMCに細胞分裂追跡用色素、CFSE
(カルボキシフルオセイン二酢酸サクシニミジルエステル)(Lyons et al.Methods Cell Biol.63,375−398(2001))を負荷し、50μg/mLのミエリン塩基性タンパク質(MBP:RDI−TRK8M79/LYO)、またはマトリックス結合タンパク質に結合させた1μg/mLの甲状腺刺激ホルモン受容体細胞外ドメイン(TSHr:ご好意によりM.Ludgate(Cardiff University)により供与)とともに培養した。細胞中での10日間のCFSE染色後、特異的表面マーカー(CD4、CD8)で特定し、フローサイトメトリーによって分析した。前駆体頻度(親集団を離れ、少なくとも2回の細胞分裂を受けたリンパ球の割合として定義される)、および増殖指数(算定された親細胞の数で割った全ての生成における細胞の合計として定義される)を、Modfit LT 3.0(Verity Software)を使用して計算した。生存細胞の絶対数は、培養物中に含まれた個定数の不活性ビーズ(BDCALIBRITE、BDBiosciences)との比較によって測定した。
D.結果
多発性硬化症を持つ治療されていない患者と正常な対照(図1Aおよび1B)との間で、刺激されていないか、またはミエリン塩基性タンパク質(MBP)もしくは甲状腺刺激ホルモン受容体(TSHr)での培養物中のT細胞の増殖応答には、差異がなかった。逆に、多発性硬化症を持つ治療されていない患者からのT細胞の生存は、対照のものよりも≧4倍高く(p<0.005、図1C)、T細胞死の減少が、治療されていない多発性硬化症の特徴であることを示唆している。これは、治療されていない患者からのT細胞が、健康な対照と比較して、受動的およびFas媒介性の両方のアポトーシスに抵抗性であることを実証することによって、確認された(受動的:0.3%対6.7%、p=0.0016、およびFas媒介:2.9%対15.5%、p=0.0018、図1Eおよび1F)。
実施例2に記載した検定を使用して、アレムツズマブ後、自己抗原に応答するT細胞の割合(前駆体頻度)および増殖の程度(増殖指数)が、治療されていない患者および健康な対照と比較して有意に増加したことが見出された。例えば、3ヶ月時点には、刺激されていないT細胞の増殖は、治療されていない患者のものの>6.5倍であり、MBPおよびTSHr刺激に応答した増殖は、それぞれ900%および700%増加した(全てp<0.01:図1Aおよび1B)。T細胞のアポトーシスも、アレムツズマブ後有意に増加した。抗原刺激に応答して、6ヶ月時点でアポトーシスを受けたT細胞の割合は、ベースラインより10倍大きく(図1D、全ての抗原についてp<0.001)、培養の最後により少ない生存T細胞をもたらした(図1C)。受動的およびFas媒介アポトーシスもまた、アレムツズマブ後、健康な対照群で観察されたものの少なくとも2倍の比率で増加した(受動的:対照の6.7%と比較して、6、9、および12ヶ月時点で、それぞれ24.5%、22.2%、および17.9%、全てp<0.001;Fas媒介:対照の15.5%と比較して、6、9、および12ヶ月時点で、それぞれ37.8%、35.8%、および29.9%、全てp<0.01;図1Eおよび1F)。アレムツズマブ後のリンパ球アポトーシスの増加は、CD4+およびCD8+亜集団の両方で見られ(図IGおよびIH)、アレムツズマブ治療後少なくとも18ヶ月持続した(データ示さず)。
A.mRNA分析
PBMCを、エクスビボ直後、またはMBPもしくはポリクローナル刺激での培養後、MACS(登録商標)LS Columnに装填された1x107個の細胞につき、20μLの磁気ビーズ(Miltenyi Biotec;CD19 Microbead、CD3 Microbead、CD14 Microbead)を使用して、陽に分離した。時期的に保持された細胞を溶出し、洗浄し、RNAlater (商標)中−70℃で保管した(細胞純度は一貫して95〜98%、データ示さず).

治療されていない多発性硬化症患者からの、両方のアポトーシス経路に共通する実行型カスパーゼである、カスパーゼ3のT細胞のmRNA発現が、対照と比較して減少し、これは、刺激されていないPBMCおよびMBP刺激PBMCでは有意であったが(多重比較の補正後、それぞれ78%および87%、ともにp<0.05)、ポリクローナル刺激培養物については有意ではなかった(図2A)。CD14+細胞において同様の傾向が見られたが(しかしCD19+細胞では見られなかった)、この差異は多重比較の補正後は存続しなかった(図2B)。アレムツズマブ後、カスパーゼ3の発現は、増加するT細胞および単球内で有意に増加し、健康な対照に見られるレベルに達した(p<0.05、図2Aおよび2B)。試験した他の全ての遺伝子の発現(方法に列挙)は、アレムツズマブ後変化しなかった。
実施例5:アレムツズマブ後の二次性自己免疫は過剰なT細胞アポトーシスに関連する
リンパ球増殖およびアポトーシスの増加を治療に対する一般的応答として実証後、アレムツズマブ後、少なくとも3ヶ月を超えて持続する、新規自己免疫疾患の発症および/または正常な範囲を超える持続性の自己抗体として定義される、アレムツズマブ後のT細胞アポトーシスと自己免疫の発症との間の関係を試験した。この定義を用いて、自己免疫を持つ患者(n=10)に由来するT細胞は、同じ時点に研究された非自己免疫患者(n=10)からのT細胞と比較すると、治療後9ヶ月で、全ての培養条件で、有意により高いレベルのアポトーシス細胞死を示した(刺激されていない4.7%対14.4%、Fas媒介18.2%対32.1%、MBP 7.6%対17.6%、およびTSHr 9.5%対25.5%、全ての比較についてp<0.01;図3)。非病原性の抗体産生を除外した自己免疫疾患の発症という、自己免疫のより厳しい定義を適用した場合、自己免疫群の数を7に低減したにもかかわらず、差異が残った(刺激されていない、4.7%対15.4%;Fas媒介、18.2%対31.7%;MBP、7.6%対20.2%;TSHr、9.5%対13.4%;全ての比較についてP<0.02)。
A.IL−21検定およびスパイク
血清IL−21を、EBIOSCIENCEキット(88−7216−86)を使用して使用説明書の通り測定した。プレートを、マイクロプレートリーダー(モデル680、BioRad)を使用して、450nmで読み取った。刺激されていないPBMCおよびポリクローナル刺激PBMC(1μg/mLのプレート結合抗CD3および1μg/mLの可溶性抗CD28)を、5pg/mLおよび20pg/mLのrhIL−21(EBIOSCIENCE 14−8219)でスパイクした。CD4+およびCD8+のアポトーシスおよび増殖を、上述の通り評価した。
B.結果
ヒトT細胞のアポトーシスおよび増殖に対する外因性IL−21の影響をインビトロで試験した。健康な対照からのPBMCをrhIL−21でスパイクすることにより、刺激されていないおよびポリクローナル刺激のCD4+(図4A)およびCD8+(図4B)T細胞のアポトーシス死における、用量依存的な増加がもたらされた(全ての条件についてp<0.05)。刺激されていない細胞をrhIL−21でスパイクすることにより、CD4+およびCD8+の両方のT細胞において、小さいが有意な増殖がもたらされ、増殖指数(CD4+およびCD8+:それぞれ1.07対1.25、p=0.017;および1.09対1.32、p=0.017、図5A)、および前駆体頻度(CD4+ 0.007対0.014、p=0.016;CD8+ 0.007対0.015、p=0.026、図5C)の両方が増加した。IL−21は、ポリクローナル刺激に応答して増殖するCD4+もしくはCD8+T細胞の割合に影響を及ぼさず(図5D)、それらがすでに最大限に刺激されていたことを示唆している。しかしながら、IL−21は、CD8+の細胞増殖の程度において有意な増加をもたらした(増殖指数11.59対19.39、p=0.012、図5B)。
実施例6の全ての時点において、血清IL−21の濃度は、非自己免疫群と比較して二次性自己免疫を発症した患者において有意に高かった(全ての比較についてp<0.05、図6A)。その後にアレムツズマブ摂取に進んだ84人の患者からの、全ての治療前血清試料について調査した。これらの患者との少なくとも2年の経過観察後、彼らを「自己免疫」群(n=35:32人の患者が甲状腺疾患、1人がITP、1人がグッドパスチャー病、そして1人が脱毛症を持つ)、または「非自己免疫」群(n=49:一過性(6ヶ月を超えて持続しない)のみの自己抗体を持つか、または自己抗体を持たない患者)として分類した。これらの治療前血清は、対照よりも有意に高いIL−21の平均濃度を有したが、これは、完全に、二次性自己免疫の発症に進んだ患者の高IL−21レベルによって説明された。自己免疫の発症に進んだもの(464pg/mL)と進まなかったもの(229pg/mL、p=0.0002)との間には、高度に有意な差異があった(図6B)。
A.IL−21の遺伝子型の決定
染色体4q27上の4つの遺伝子、KIAA1109−ADAD1−IL2−IL21を含有する強い連鎖不均衡の領域にある、合計で、4つのSNP、rs13151961、rs6822844、rs4833837、およびrs6840978を試験した。4つの全てのSNPは、Applied Biosystems Assay−On−Demand(AoD)産物として入手可能であった。SNPの遺伝子型の決定は、Applied Biosystems TaqMan手順を使用して、製造業者の推奨する条件に従って行った。ポリメラーゼ連鎖反応(PCR)は、Applied Biosystemsの384ウェル9700 Viper PCR機で行い、その後遺伝子型を、SDS Software Version 2.1を使用して、7900 High Throughput Sequence Detection System(SDS)上で求めた。各個人の遺伝子型を二重に決定した。多発性硬化症に関連する遺伝因子:HLA−DRBl *1501、rs2104286(IL2RA)、およびrs6897932(IL7R)について、全ての個人の遺伝子型をさらに決定した。
B.結果
遺伝的変異とIL−21産生との間に関連があるかどうかを決定するために、IL−21遺伝子をコードする染色体4q27上の連鎖不均衡(LD)のブロック内にある4つの一塩基多型(SNP)について、73人の被験者の遺伝子型を決定し、彼らのアレムツズマブ前血清IL−21濃度を測定した。4つの全てのSNPについての軽微な対立遺伝子頻度は、公開されているデータと一致していた:rs13151961G(14.5%)、rs6822844T(14.6%)、rs4833837G(38.0%)、およびrs6840978T(18.1%)(Glas et al,Am.J.Gastroenterol.104,1737−1744(2009))。4つのSNPのうちの3つ(rs13151961 A/A、rs6822844 G/G、およびrs6840978 C/C)における遺伝子型が、有意に高いレベルの血清IL−21(p値:それぞれ0.0076、0.0098、および0.0067)に関連することが見出された。rs4833837における遺伝子型は、IL−21産生に影響を及ぼさなかった。rs4833837と他の3つのSNPとの間のLDは低く(r2<0.15)、したがって、ハプロタイプrs13151961(A)−rs6822844(G)−rs6840978(C)上のSNPが、IL−21産生の増加に関連している可能性が最も高い。HLA−DRBl *1501、rs2104286(IL2RA)、およびrs6897932(IL7R)についての遺伝子型頻度は、他の多発性硬化症を持つ任意抽出患者について公開されているデータと相違しなかった(International Multiple Sclerosis Genetics Consortium(IMSGC),Lancet Neurol.7,567−569(2008)、Yeo et al.,Ann Neurol 61,228−236(2007))。

(i)前記患者からの血液試料中のIL−21、または(ii)SNP rs13151961におけるA/A、SNP rs6822844におけるG/G、およびSNP rs6840978におけるC/Cからなる群から選択される、一塩基多型(SNP)の1つ以上の遺伝子型の存在または非存在についての情報を獲得するステップと、

前記助言後、前記薬物を前記患者に提供するステップと、を含む、方法。 - 前記提供するステップの前に、前記患者から、告知に基づく同意を獲得することをさらに含む、請求項72に記載の方法。
