Abstract
Description
本出願は、2007年4月13日に出願された同時係属出願No.11/735372の一部継続出願である。
ステップ802は、器具を所定の場所に前進させるステップを記述している。所定の場所は典型的には幽門の近傍に設けられる。例えば幽門の前、幽門を横切る、幽門の後、食道内、胃と食道との接続部、又は胃の中のような他の場所も可能である。例えば、アンカー構成要素は、幽門の前に、胃洞内に、幽門を横切って、十二指腸球部内に、小腸内に、又は他の適切な部位に配置され得る。
ステップ804は、器具が所定の場所に前進した後に、圧力源を展開圧力に調節するステップを記述している。図9Aは、展開場所に配置された器具の断面図を示している。明瞭化のために前記断面図が医療用観察器械又は結合ユニットとは独立していることに注意すべきである。図9Aを参照すると、器具は参照符号900として全体的に描かれている。器具は、送出管902とアンカー構成要素904とを含んでおり、前記アンカー構成要素904は二つの固定構成要素906の間に配置されてシース908内に捕捉されている。明確に示されるように、スリーブ910は送出管902の管腔の内側で裏返しにされている。
ステップ806は、スリーブを展開するステップを記述している。ここで図9Bを参照すると、流体を潅流バッグから放出するために弁が開放される。流体が管腔を介して送出管902の端部に到達すると、スリーブ910は送出管902の遠位部912から反転することにより展開される。反転工程は、X線透視又は本技術分野で知られた他の技法によって監視され得る。スリーブが展開するとき、潅流バッグ内の圧力は、手動ポンプを使ってブラダ側の中に追加の空気を加えることによって維持される。図9C及び9Dに示されるように、スリーブの展開は、完全な反転が完了して加圧された例えば造影剤溶液のような流体がスリーブを通って勢い良く流れてスリーブの反転が完了したことを知らせるまで継続する。反転することは、スリーブが曲がりくねった生体組織を通ることを可能にする一方で、スリーブの完全な展開を可能にする。また、反転は、幽門を大きくは越えないで延びる位置からのスリーブの展開も可能にする。つまり、器具は、スリーブの最も遠位の最後の場所に近接した場所からのスリーブの展開を可能にする。
ステップ808及び810は、アンカー構成要素を展開するために器具を位置決めするステップ及びアンカー構成要素を展開するステップを記述している。スリーブの展開後に、医療用観察器械の遠位端及び残りの器具構成要素は、アンカー構成要素を展開するための目標の固定位置に位置決めされる。目標の固定位置は、食道、胃と食道との接続部、胃、又は小腸であり得る。例えば、目標の固定位置は、幽門の前、胃洞、幽門を横切って、幽門の後、十二指腸球部、小腸、又はスリーブ及びアンカー構成要素の配置に適した他の部位であり得る。
アンカー構成要素が展開された後に、ステップ812において、正確な展開の確認及び医療用観察器械の除去が実施される。図9Gに示されるように、医療用観察器械及び送出管902及びシース908は除去される。正確な展開の確認は、例えば内視鏡による可視化、超音波による可視化、X線による可視化、及び同等のもののような本技術分野で知られた手段によって達成される。
本発明の範囲を限定する意図なしに、次に示す例は、本発明の様々な実施形態がどのように作られ及び/又は使用されるかを説明している。
例1
この例は、本発明の一つの様相によるスリーブ及びアンカー構成要素の製造を例証する。一方の側にFEPの熱接着層を有するePTFEを含む実質的に無孔性の複合フィルムが用いられた。複合フィルムは、次に示す特性、即ち、約25mmの幅、約0.0025mmの厚さ、約0.6MPaより大きいイソプロピル・アルコール泡立ち点、及び例えば最強の方向の長さ方向における約309MPaの引張強さを有していた。
この例は、例1のアンカー構成要素及びスリーブをこの例の展開管の中へ装填することを説明している。
この例では、生体消化性アンカー構成要素が製造され、及び模擬された胃腸部環境内での長期に渡るそれぞれの重量の腐食減少量が詳しく調べられた。
この節は、フィルムの引張強さの測定について記載している。最大引張力が、2714シリーズ・コード・ヤーン把持器付きのインストロン(Instron)型番5560引張試験機(マサチューセッツ州、カントン)を使って、10個の試料に対して測定されて平均化された。顎部の間隔は10.2cmであり、クロス・ヘッド速度は200mm/分であった。最大引張力の10個の測定値の平均が用いられた。10個の試料幅の平均値が計算された。ミツトヨ挟みゲージ型番547−400(日本、中津川)を使って厚さが測定された。厚さの10個の測定値の平均が用いられた。引張強さが、最大引張力及び試験された試料の横断面積の商として計算された。
Claims (27)
- スリーブと、
前記スリーブの少なくとも一部分に取付けられた非円形のアンカー構成要素と、
管腔を有する送出管であって、前記スリーブがほぼ完全に反転されて該送出管の前記管腔内に収容されている送出管と、
を具備する医療用器具。 - 前記送出管は、内視鏡の作業チャネルの内側に収まるように形成されている、請求項1に記載の医療用器具。
- 前記内視鏡の前記作業チャネルは約10mm以下の直径を有する、請求項2に記載の医療用器具。
- 前記送出管の近位端部に取外し可能に結合されたコネクタを更に具備する、請求項1に記載の医療用器具。
- 前記送出管の遠位端部に取外し可能に結合されたシースを更に具備する、請求項1に記載の医療用器具。
- 前記アンカー構成要素はニッケルチタン合金を含む、請求項1に記載の医療用器具。
- 前記アンカー構成要素は生体消化性材料を含む、請求項1に記載の医療用器具。
- 前記アンカー構成要素はX線不透過材料を含む、請求項1に記載の医療用器具。
