Abstract
Description
本願は、米国仮特許出願第60/978,511号(名称「Cooled Ablation Catheter Devices and Methods of Use」、2007年10月9日出願)の優先権を主張し、この出願は、本明細書に参考として援用される。
心房細動は、異常な電気信号の発生によって不整脈が生じる、心臓の疾患である。本疾患の治療を成功させるための実証されているプロトコルは、開心術(「maze」手術とも称される)であって、いくつかの細長い(すなわち、約2−10cmの)損傷が、心臓の上方室(「心房」)内の心内膜内に生成される。これらの損傷は心房内の過剰な電気刺激の流動を阻止し、洞結節からの刺激によって心収縮を適切に調節することを可能にする。
Claims (25)
- カテーテル装置であって、
血管アクセスのために寸法設定および成形されるカテーテルであって、近位端と遠位端との間に延在し、かつ、少なくとも1つの内部流体内腔を有する細長本体を含む、カテーテルと、
該カテーテル本体の該遠位端の近位に配置される電極先端であって、該先端は外壁と自身を通る少なくとも1つの流体通路を有する熱質量とを含む、電極先端と、該細長本体の内部流体内腔と流体的に連絡する冷却チャンバであって、該熱質量に対して近位に位置し、該熱質量を含む該電極先端の近位部分を冷却するように適合される、冷却チャンバとを備えており、該先端は該熱質量内の該流体通路と流体的に連絡する複数の潅水開口をさらに含む、装置。 - 前記冷却チャンバは、前記先端の複数の外壁の間に、該先端の実質的に全幅にわたって延在する、請求項1に記載の装置。
- 前記内部熱質量は、高い熱伝導性を有する物質を備える、請求項1に記載の装置。
- 温度センサは、少なくとも部分的に、前記熱質量内に配置される、請求項1に記載の装置。
- 前記複数の潅水開口のうちの少なくとも1つは、前記温度センサの遠位に配置される、請求項4に記載の装置。
- 前記熱質量は、前記先端の実質的に全幅にわたって延在する、請求項1に記載の装置。
- 前記熱質量から遠位に配置される第2の冷却チャンバをさらに備える、請求項1に記載の装置。
- 前記熱質量を通る前記流体通路は、前記冷却チャンバに対して、該熱質量を通る流体通路内の流体圧の増加をもたらすように寸法設定および成形される、請求項1に記載の装置。
- 前記冷却チャンバへの流体流入口は、該冷却チャンバの流体流出口からオフセットされている、請求項1に記載の装置。
- 前記複数の潅水開口のうちの少なくともいくつかは、前記冷却チャンバに対して遠位に配置される、請求項1に記載の装置。
- カテーテル装置であって、
血管アクセスのために寸法設定および成形されるカテーテルであって、近位端と遠位端との間に延在し、かつ、少なくとも1つの内部流体内腔を有する細長本体を含む、カテーテルと、
該カテーテル本体の遠位端の近位に配置される電極先端であって、該先端は、外壁と自身を通る少なくとも1つの流体通路を有する熱質量とを含む、電極先端と、該細長本体の内部流体内腔と流体的に連絡する近位の冷却チャンバであって、該熱質量の近位に配置され、かつ、該熱質量の近位表面を冷却するように適合される、近位の冷却チャンバと、該熱質量の遠位に配置される遠位の冷却チャンバとを備え、流体路が、該近位の冷却チャンバから、該少なくとも1つの流体通路を通って、該遠位の冷却チャンバ内へと延在し、該先端は、該遠位の冷却チャンバと流体的に連絡する複数の潅水開口をさらに備えており、
該熱質量を通る該少なくとも1つの流体通路は、該近位または遠位の冷却チャンバに対して、該少なくとも1つの流体通路内の圧力の増加をもたらすように寸法設定および成形される、装置。 - 前記細長本体は、可撓性材料から形成される、請求項11に記載の装置。
- 少なくとも部分的に前記熱質量内に配置される、温度センサをさらに備える、請求項11に記載の装置。
- 前記近位の冷却チャンバは、前記先端の複数の外壁の間に、該先端の実質的に全幅にわたって延在する、請求項11に記載の装置。
- 前記遠位の冷却チャンバは、前記先端の複数の外壁の間に、該先端の実質的に全幅にわたって延在する、請求項11に記載の装置。
- 前記熱質量は、高い熱伝導性を有する材料を備える、請求項11に記載の装置。
- 前記熱質量は、前記先端の実質的に全幅にわたって延在する、請求項1に記載の装置。
- 前記近位の冷却チャンバへの流体流入口は、該近位の冷却チャンバの流体流出口からオフセットされている、請求項1に記載の装置。
- 前記複数の潅水開口のうちの少なくともいくつかは、前記冷却チャンバに対して遠位に配置される、請求項1に記載の装置。
- 組織を焼灼する方法であって、
電極先端を提供するステップであって、該電極先端は、自身を通る少なくとも1つの流体通路を有する熱質量と、該熱質量の近位に配置され、かつ、該熱質量の近位表面と熱的に連絡する近位の冷却チャンバと、少なくとも1つの遠位潅水開口とを有する、ステップと、
焼灼エネルギーを該電極先端に送達するステップと、
該近位の冷却のチャンバ内へ、該熱質量内の該少なくとも1つの流体通路を通して、冷却流体を送達することにより、該電極先端の近位部分を冷却するステップと
を含む、方法。 - 遠位の冷却チャンバ内へ前記冷却流体を送達するステップをさらに備える、請求項20に記載の方法。
- 前記熱質量を通る前記少なくとも1つの流体通路は、前記近位または遠位の冷却チャンバに対して、該少なくとも1つの流体通路内の圧力増加をもたらすように寸法設定および成形される、請求項21に記載の方法。
- カーテル装置であって、
血管アクセスのために寸法設定および成形される焼灼カテーテル先端であって、外壁および自身を通る少なくとも1つの流体通路を有する熱質量を備える、焼灼カテーテル先端と、該熱質量の近位に配置され、かつ、冷却流体の流動を受容するように適合される、近位の冷却チャンバと、該カテーテル先端を囲む環境に冷却流体を送達するための複数の潅水開口とを備えており、第1および第2の流体内腔が、該近位の冷却チャンバへと延在し、かつ、該近位の冷却チャンバ内へのおよび該近位の冷却チャンバからの閉ループ冷却流路を画定し、第3の流体内腔が、該複数の潅水開口と流体的に連絡し、該カテーテル先端の外部へと冷却流体の開ループ送達を提供する、装置。 - 前記遠位の冷却チャンバは、前記熱質量に対して遠位に配置される、請求項23に記載の装置。
- 前記第3の流体内腔は、前記遠位の冷却チャンバと流体的に連絡する、請求項24に記載の装置。
