Abstract

Description
本発明は、疾患を治療するための、同種細胞注入の使用に関連する。より具体的には、本発明は、対宿主性移植片(GVH)病の毒性なしに、抗腫瘍効果を産生することを可能にする、同種細胞治療方法に関連する。
同種異系細胞治療は、いくつかの型の悪性腫瘍およびウイルス性疾患の重要な治療法である。同種異系細胞治療は、患者に対する細胞の注入または移植を含み、ここで注入または移植された細胞は患者以外のドナーから得る。同種異系細胞治療プロトコールのために利用されてきた同種異系ドナーのタイプは、HLAのマッチした同胞、マッチした非血縁のドナー、部分的にマッチした家族メンバーのドナー、血縁の臍帯血ドナー、および非血縁の臍帯血ドナーを含む。同種異系ドナー細胞を、通常骨髄の採取、末梢血の採取、または出生時の臍帯血の採取によって得る。このマッチしたドナーの必要性が、同種異系細胞治療プロトコールの大きな制限である。HLAのマッチングの必要性なしに有効である、同種異系細胞治療の方法を提供することが、本発明の目的である。

従来技術の同種細胞治療プロトコールにおいて、移植片中のT細胞が、治療の有用なGVT効果および有害なGVH効果を媒介する原因である。本発明の目的を達成するために、新規メカニズムが記載され、ここで移植片中のT細胞は免疫効果を直接媒介せず、その代わりに宿主免疫系を刺激して、内在する疾患に対する有効な免疫反応を媒介するよう作用する。

本発明の方法は、宿主DCの活性化および1型サイトカインストームの状況におけるTAAの新規分断を誘発する。これらは、腫瘍に対する宿主の1型適応免疫反応の新規発達に必要な条件である。
本発明のミラー効果メカニズムを刺激するために、増強された性質を有する同種異系細胞を産生する好ましい方法は、以下のものを含む:(1)正常なスクリーニングされたドナーから、白血球除去血輸血によって単核細胞原材料を採取する;(2)原材料からCD4T細胞を単離する;(3)CD4+細胞を抗CD3および抗CD28モノクローナル抗体(mAb)で標識する;(4)標識したCD4+細胞を、CD4+細胞上のmAbを架橋し得る薬剤でコートした生物分解性マイクロスフィアまたはナノスフィアと混合する;(5)遠心分離によって生物分解性スフィアおよび標識CD4+細胞を濃縮する;(6)混合物を、106細胞/mlを超える細胞密度で、外来性のサイトカインなしに、血清を含まない培地で培養する;(7)インキュベーター中で2日間妨害せずに細胞を培養する;(8)さらなる標識薬剤およびコートした生物分解性スフィアを加える;(9)新しい培養混合物を遠心分離し、続いて細胞を含まない培養液体積の50−90%を除去する;(10)透析フィルターを通して細胞を含まない馴化培養液の90%を透過させる;(11)残る10%の馴化培養液を新しい培養液でもとの体積に戻し、そしてこの補充した馴化培養液を細胞混合物に戻し加える;(12)工程8から11を少なくとも毎日、少なくとも6日間の全体培養期間で繰り返す。
本明細書中で提供される好ましい方法の実施において、単核細胞の開始集団(原材料)を、ドナーから、好ましくは白血球除去血輸血手順によって採取する。原材料を提供するためにリクルートされたドナーは、健康であり、そして外来性の病原体からフリーでなければならない。ドナーは、好ましくは意図されるレシピエントに対して完全にミスマッチのHLAを有する。望ましくはないが、部分的にHLAのマッチしたドナー(意図されるレシピエントの同胞のような)からの原材料も、本発明の方法において使用し得る。部分的にマッチした原材料は、レシピエントの免疫系が非常に弱体化しており、ミスマッチドナーの細胞の注入がGVH病反応を引き起こし得る場合にのみ使用する必要がある。免疫系が弱体化した個人の場合でさえ、依然としてミスマッチ細胞を使用することが好ましい。これらの患者においてGVH病の危険性を最小限にするために、ミスマッチドナー細胞の投与量を抑制し得るか、またはミスマッチ細胞を注入の直前に照射し得る。

処理の次の工程は、精製CD4+細胞のエキソビボ培養である。CD4細胞を、少なくとも6日間、持続性および一定の活性化刺激にさらすことが好ましい。細胞を活性化するために、好ましくは最初に抗CD3および抗CD28mAbのような活性化薬剤で標識し、そして活性化薬剤を次いで架橋して活性化シグナルをCD4細胞に伝達する。細胞を標識するために、細胞を最初に、血清を含まない培養液中で1mlあたり107細胞の細胞密度に調整する。正常なバッチは、10mlの培養液中約108のCD4細胞を含む。mAbをそれぞれ、少なくとも1mlあたり1マイクログラム、好ましくは1mlあたり10マイクログラムの最終濃度で細胞に加える。細胞を室温または好ましくは4℃で、15分から30分間回転するか、または端と端を接した混合装置で、mAbと共にインキュベートするべきである。細胞を次いで洗浄して、過剰なmAbを除去し、そして血清を含まない培養液に1mlあたり107細胞で再懸濁するべきである。
好ましい架橋方法は、標識細胞を活性化薬剤と反応性の薬剤でコートした生物分解性ナノスフィアまたはマイクロスフィアと混合することである。例えば、生物分解性スフィアを、抗CD3および抗CD28mAbのFc領域に特異的なmAbでコートし得るか、または活性化薬剤がマウス由来である場合には、コートする薬剤はポリクローナル抗マウス抗体であり得る。標識細胞を、少なくとも1:1、好ましくは最低3:1、および最も好ましくは最低5:1のスフィア対細胞比で、コートされた生物分解性マイクロスフィアと混合する。スフィア/細胞混合物を、室温で、または好ましくは4℃で、15から30分間、好ましくは標識細胞とよく混合する。
CD4細胞への活性化シグナルを増強するために、よく混合した標識細胞/スフィア混合物を、遠心機で500から800rpm、4℃で2から10分間遠心沈殿する。その力は、細胞を固く「ペレット」にするほど大きくなく、細胞を濃縮するのにちょうど十分な大きさであるべきである。遠心力が細胞およびスフィアを相互作用させ、架橋およびCD4細胞へのシグナル伝達を増加させ、増強された活性化を与える。細胞を、好ましくはガス透過性バッグの培養容器において回転させる。遠心後、柔軟なバッグ容器のマッサージおよび振動によって細胞を静かに再懸濁させ、そして37℃で5%二酸化炭素の環境のインキュベーター内に置く。
CD4細胞を、エキソビボ培養処理中に、密接な細胞対細胞の接触を保つことも好ましい。密接な細胞対細胞の接触を、細胞を高い細胞密度で、好ましくは1mlあたり106細胞またはそれより高い密度で培養することによって達成し得る。細胞対細胞の接触および活性化シグナルの伝達を増強するために、細胞を頻繁な遠心分離にかけることも望ましい。
培養の最初の2日間、細胞をインキュベーター中で妨害しないでおかなくてはならない。
3日目に、さらなるマイクロスフィアおよびmAbを培養に加え、そして完全に混合する。100mlの培養に、抗CD3および抗CD28mAbをそれぞれ100マイクログラム、3−5×108のコートした生物分解性マイクロスフィアと共に加える。
生物分解性スフィアを含む培養中で細胞を高密度に維持することは、培養液の頻繁な交換を必要とする。高い細胞密度は、高率の代謝廃棄物の蓄積および利用可能な栄養素の消費を引き起こす。それに加えて、生物分解性スフィアの加水分解が、培養液のpHを酸性にする。しかし、早すぎる培養液の交換は、外来性のサイトカインが利用されない培養に有害であり得る。外来性サイトカインはヒトに注入された場合に毒性であり得、そして培養細胞を生存のために外来性サイトカインの存在に依存性にし得るので、細胞治療プロトコールにおいて使用する細胞を処理する場合に、外来性サイトカインを使用しないことが好ましい。従って、本発明の方法は、細胞処理において透析工程を含む。
除去した培養液の、10,000ダルトンまたはそれより小さい孔サイズを有する膜を通した透析は、代謝廃棄物を通過させながら、内因性サイトカインの保持を可能にする。好ましい実施態様において、培養の50−90%の培養液を少なくとも毎日除去し、そして除去した培養液の90%が透析膜を通過する。
透析膜を通過した培養液を廃棄し、一方10%の保持された培養液を、新しい培養液でもとの体積まで戻し、そして次いでT細胞/スフィア培養に戻し加える。保持された培養液は、培養液の除去前と同じ濃度で内因性サイトカインを含む。
工程8から11を、少なくとも1日1回、最低3日間(培養において全部で6日)反復する。処理の典型的なバッチの流れにおいて、培養を100mlの培養液体積中約108の精製CD4細胞で始める(1日目)。記載した方法によって、細胞は6日目から8日目までに約1−5×109細胞に増殖する。この細胞数に達した時に、細胞をガス透過性バッグ中で1000mlの培養液中に再懸濁し得、そして工程8から11を、さらに3から6日まで少なくとも毎日反復する(培養の9日目から14日目)。この時間に、培養中の全細胞は約1−5×1010細胞に増殖する。
細胞を、培養の6日目以降いつでも、またはそのバッチ培養で少なくとも109細胞が入手可能である場合に回収し得る。最大限のサイトカイン産生を保証するために、回収のタイミングは、細胞を最後の工程8−11サイクルから24時間後に処方および注入するように起こるべきである。
回収した細胞を、細胞が注入時および循環中に活性化されたままであることを保証するために、コートされた生物分解性マイクロスフィアで架橋された細胞表面に結合した活性化mAbと共に処方する。
十分な数の処方CD4細胞を、疾患の症状を改善するために、レシピエントに投与する。典型的には、107から1010細胞の1回投与量を、好ましくは109細胞の1回投与量を単回の設定で注入する。注入を、単回の109細胞の投与量として、または好ましくはいくつかの109細胞の1回投与量に分割してのいずれかで投与する。注入の頻度は、3から30日ごと、またはもし望ましいまたは示されるならば、より長い間隔でさえもあり得る。注入の量は一般的に、患者当たり少なくとも1回の注入、および好ましくは許容されるなら少なくとも3回の注入、または疾患の症状が改善されるまでである。細胞を、50−250ml/hrの速度で静脈内に注入し得る。
本発明の方法によってできた細胞は、同時培養した場合に、自然免疫系の細胞を急激に活性化する。この活性化は、本発明の方法によって産生された細胞に発現するCD40Lおよび宿主自然免疫細胞に発現するCD40分子の相互作用によって起こる。宿主PBMCおよび本発明の方法によって産生された同種異系ドナー細胞の同時培養時に、マクロファージおよび樹状細胞は、補助刺激細胞表面分子およびMHCクラスIおよびII分子をアップレギュレートし、IFN−ガンマ、TNF−アルファ、IL−1、IL−12、およびI型インターフェロンのような炎症促進性のサイトカインを産生する。これは「サイトカインストーム」を産生し、これはBMTプロトコールにおいて同種異系ドナーリンパ球の注入によって産生されるサイトカインストーム環境とほとんど同じである。
(マイクロスフィア調製)
マイクロスフィアを調製するために、溶媒蒸発法を使用した。0.15から0.25dl/gの固有粘性を有する、Lactel(登録商標)(Birmingham Polymers、Birmingham、AL)50/50DL−PLG製品番号50DG020を、ポリマーとして使用した。DL−PLGの粉末を、20mlの塩化メチレンに、最終的にDL−PLGの5%w/v比まで溶解した。5%のDL−PLG溶液を次いで、125mlの0.1Mグリシン/HCl緩衝液pH1.1中2.4%のヒドロキシプロピルメチルセルロースに、室温で(25±2℃)1000rpmで絶えず攪拌しながら1滴ずつ加えた。有機溶媒の完全な蒸発まで(約3時間)攪拌を維持した。マイクロスフィアを、1000rpm、40℃で5分の遠心分離によって回収し、続いて蒸留水によって3サイクル洗浄し、ろ過および一晩乾燥させた。マイクロスフィアのサイズは、<10%の最大CVで、3.0から7.0μmの範囲であった。スフィアを次いで、吸着法を用いてポリクローナルヤギ抗マウス抗体でコートした。抗体を5%のヒト血清アルブミン(HSA)を含む30mlのPBS溶液に、10μg/mlの濃度で懸濁した。この溶液を使用して、乾燥したマイクロスフィアを1mlあたり約2×108粒子の濃度で再懸濁した。マイクロスフィアおよびポリクローナル抗体を逆さにして、40℃で8時間混合した。マイクロスフィアを次いで、HSAを含むPBSで3回洗浄し、ろ過および乾燥した。乾燥した粒子を、使用の前に70%のイソプロパノールの溶液中で保存した。
下記の実施例に関して、同種異系細胞産物を、好ましい実施態様において記載された方法によって調製した。簡単には、1.2×1010の末梢血単核細胞(PBMC)を、白血球除去血輸血によって健康なドナーから採取した。PBMCを洗浄し、そして一晩室温で保存した。PBMCを、ビオチン化抗CD4mAbで標識し、そして2次抗ビオチンmAb磁気粒子(Miltenyi Biotec、Germany)と混合することによって、CD4+細胞に関して濃縮した。CD4+細胞を次いで、磁気化カラム(MACS(登録商標))を通過させることによって選択した。1.3×108のCD4+が選択され、そしてLifeflask(Baxter)ガス透過性バッグにおいて、100mlのXVIVO−15培養液中に置いた。CD4+細胞を5%CO2の雰囲気中で、37℃で一晩インキュベートした。次の日、非接着細胞を洗浄し、そして抗CD3および抗CD28mAbで標識して、そしてヤギ抗マウス抗体でコートした生物分解性マイクロスフィアと3:1の比で懸濁した。懸濁液を、1000rpmで5分間遠心分離し、そして培養バッグを手でマッサージすることによって静かに再懸濁した。懸濁液を72時間インキュベートし、そして細胞を再標識および新しいマイクロスフィアと懸濁した。懸濁液を1600rpmで8分間遠心分離し、上清を除去し、そして体積の90%を透析フィルターに通した。保持された上清を、細胞懸濁液に戻し加え、そして新しい培養液で体積を100mlに戻した。この処理を、培養9日目まで毎日反復した。10日目に、できた細胞を下記で記載する実施例に使用した。
同種異系細胞産物の試料を、表現型分析のために1日目および10日目に取った。細胞の免疫表現型決定のために、ヒトCD4、CD8、CD14、CD19、CD56、CD4/CD25、CD4/DR、CD4/CD45RA、CD4/CD45RO、CD4/CD62L、CD4/CD154(FasL)、CD4/TRAILに対する、モノクローナル抗体(Becton−Dickinson、San Jose、CA、USA)を用いた直接蛍光技術によって、表面の標識を行った。細胞内サイトカインを検出するために、単核細胞をFACS透過処理溶液(Becton−Dickinson)によって透過処理した。フローサイトメトリー分析を、Cellquestソフトウェアを用いて、FACSort装置(Becton−Dickinson)によって行った。その結果を、全ての免疫表現型に関して、10,000回から計算した染色細胞のパーセントとして報告する。
同種異系細胞産物のサイトカインプロファイルを決定するために、細胞質ゾルRNAをRNeasyキット(Qiagen)を用いて精製し、そしてRoche First Strand cDNA合成キットを用いて逆転写した。プライマーおよびプローブを、Applied Biosystemsから購入したか、またはPrimer Expressソフトウェアを用いてデザインした。リアルタイムPCR増幅および産物の検出を、ABI Prism 7700において、製造会社の推薦する手順によって行った。遺伝子産物を、1日目および10日目に100,000の値に設定したGAPDH発現と相対的に表す。
ステージ3の卵巣癌患者由来のPBMCを、密度勾配精製および軟膜の単離によって調製した。宿主PBMCを、同種異系細胞産物と50:50の比で混合し、そして24穴プレートで7日間培養した。同種異系細胞を、緑色細胞追跡色素、5−クロロメチルフルオレセイン二酢酸(CMFDA)で標識した。培養を3組セットアップした。
培養7日目の終わりに、各ウェルにおいて2%より少ない生きた細胞が緑色に染まり、それらは宿主PBMCによって拒絶されたことを示す。
本発明の方法によって産生された同種異系細胞産物の、宿主癌患者PBMCを刺激して1型サイトカインを産生させる能力を決定するために、同種異系細胞を、好ましい実施態様において記載されたように調製し、9日目に回収し、そして24穴培養プレートにおいて癌患者由来の1×106PBMCと混合し、そして5%のCO2を含む加湿雰囲気において、37℃で48時間インキュベートした。
実施例番号3からの宿主CD3+T細胞およびCD14+単球を、エフェクターおよび補助刺激マーカーに関して表現型を分析した。
K562標的細胞に対するNK活性を、生きたK562細胞を染色するためにDIO膜色素(Molecular Probes、Eugene、OR、USA)を、および死んだ細胞を染色するためにヨウ化プロピジウム(Sigma)核色素を用いて、フローサイトメトリーアッセイによって評価した。特異的な溶解のパーセントを、以下の式によって計算した:
癌細胞株NCI−H23(肺癌)、Caki−1(腎臓細胞癌)およびACHN(腎臓細胞癌)を、MHCI、MHCII、細胞死受容体FasおよびTRAIL−R2および補助刺激分子CD80およびCD86の発現に関して分析した。細胞株を次いでトランスウェル(transwell)プレートの下部で培養した。上部ウェルにおいて、正常ドナー由来の宿主PBMCおよび同種異系細胞産物を100:1の細胞比で混合した。培養を96時間インキュベートした。
