Description
背景
癌症免疫治疗的主要目的是通过激活先天性免疫应答和适应性免疫应答促进肿瘤清除(tumor elimination),并且依赖于接种策略的开发,其中,接种策略被赋予了诱导肿瘤特异性淋巴细胞同时克服免疫耐受机制的双重能力。利用癌症免疫治疗能够有效地借助正确设计的癌症疫苗实现肿瘤清除以及持久的减轻。免疫治疗中靶向恶性细胞以及避免正常细胞的特异性,也能够引起最小的毒性。
免疫应答通常是由两种极化应答描述的,Th1(T-辅助细胞,T-helper)应答和Th2应答。Th1应答介导细胞免疫力,并且对于免疫介导的肿瘤根除(tumor eradication)是至关重要的,Th2应答则介导体液免疫力。Th2应答能够保护抵抗某些传染病,但是不被期望用于抗肿瘤应答。Th1和Th2应答是反向调控(counter-regulatory),提高Th1应答会下调Th2应答,反之亦然。
尽管Th1免疫力对于肿瘤根除是必要的,但在某些患者中,肿瘤在存在Th1环境下也能够继续生长。因而,肿瘤能够成功抑制并避免保护性Th1应答。成功的免疫治疗需要促进Th1环境,使肿瘤逃避机制和天然耐受循环失效,其中,肿瘤逃避机制和天然耐受循环防止正常组织的免疫破坏。
许多阻碍抗肿瘤免疫力的机制都是CD4+CD25+FoxP3+T调控(Treg)细胞活性的结果。重要的是,这些Treg细胞促进肿瘤耐受的出现和持久,并且在早期肿瘤发展中是主要的免疫逃避机制。在肿瘤发展过程中观察到的Treg扩张可能是由于天然存在Treg(nTreg)的扩增,或者由于CD4+CD25-FoxP3-T细胞转化为CD4+CD25+FoxP3+Treg(iTreg)。Treg通过抑制效应细胞CD4+、CD8+和自然杀伤(NK)细胞的功能以及抑制树突状细胞活化来干扰免疫应答。由于Treg是利用免疫细胞根除肿瘤的主要屏障之一,但使用药物或者抗体时它们的治疗耗竭(therapeutic depletion)或者它们的功能失活会提高对癌症免疫治疗的应答。然而,Treg的选择性清除或失活会构成主要的挑战,因为这些细胞与活化的常规非抑制性T细胞具有相同的表面标记物。例如,基于抗体的途径,会无差别地靶向Treg以及活化的效应T淋巴细胞。
利用含有CRCL(Chaperone Rich Cell Lysate,富含分子伴侣的细胞裂解物)的抗癌症疫苗已经显示会提供免疫刺激作用。CRCL是通过等电聚焦技术从肿瘤细胞裂解物产生的。CRCL-处理的树突状细胞以及巨噬细胞会抵抗Treg抑制,并且产生促炎症细胞因子和免疫刺激能力。然而,CRCL对Treg的直接作用似乎是不太可能,因为Treg的存活,FoxP3表达以及免疫抑制功能在暴露至CRCL之后得到维持。
许多化疗药物(即环磷酰胺,甲磺酸伊马替尼(imatinib mesylate))已经被描述为Treg功能的负调节剂,进一步提高癌症疫苗效率。适应性转移同种异体T细胞也能够提高肿瘤免疫治疗的效力,通过提供辅助的“危险”信号至宿主免疫细胞。这种作用部分来自于活化的T淋巴细胞产生细胞因子,这促进了产生保护性I-型免疫应答。然而,I型细胞因子包括IFN-γ对Treg的作用是不一致的。作为细胞介导免疫力的关键效应细胞因子,外源或者自分泌的IFN-γ都能够消极地调控Treg产生。与该观点类似,其他的研究已经发现IFN-γ会增强活化诱导的细胞死亡,并且该细胞因子可能会通过增强内源性途径和外源性途径依赖性凋亡调节效应T细胞的扩张和持久。
发明内容

详细描述
本发明包括抑制患者中疾病抗原免疫耐受和/或使肿瘤逃避机制失效的方法。该方法还包括刺激抵抗疾病抗原的免疫力,特别是Th1免疫力。免疫耐受的抑制以及抗疾病抗原免疫力的刺激都可以通过给药治疗组合物来产生,其中,治疗组合物包括疾病抗原源和同种异体Th1细胞,优选同种异体活化的Th1细胞作为佐剂。通常,同种异体Th1细胞是被活化的效应/记忆Th1(emTh-1)细胞。该治疗组合物还可以包括疾病相关抗原和/或含有疾病相关抗原的裂解物,优选从患病组织分离的分子伴侣蛋白的混合物,例如富含分子伴侣的细胞裂解物(CRCL)。
在本发明中,抑制免疫耐受的方法可以包括削弱Treg细胞的抑制活性。该方法可以抑制幼稚T细胞(CD4+CD25-FoxP3-)转化为Treg细胞(CD4+CD25+FoxP3+),这里称为iTreg。自然存在Treg(nTreg)和iTreg细胞二者的抑制活性可以被降低或者消除。本发明的方法有利地在患者中抑制Treg的免疫抑制活性,同时能够促进常规效应T淋巴细胞的功能。治疗组合物对Treg的抑制活性依赖于IFN-γ,其在优选组合物中是由em-Th1细胞产生的,而这又会提高宿主先天和适应性免疫细胞产生IFN-γ。
本文中提到的“抑制免疫耐受”涉及抑制患者免疫系统耐受疾病抗原的存在的能力,包括任何天然耐受和/或抑制肿瘤逃避患者免疫系统。
本文所描述的方法可以用于治疗患者中的多种疾病。该疾病可以是癌性的或非癌性的。癌性疾病可以包括产生肿瘤的癌症以及不会产生肿瘤的癌症例如恶性血液病(hematologicalmalignancies)。非癌性疾病可以包括例如由病原体或多个病原体造成的疾病。肿瘤可以出现于患者的多个位置,包括乳房、肺、肝、胃、皮肤、胰腺等。患者可以是人或者非人。
本发明的方法可以包括为患者给药治疗组合物,其中,给药该组合物能够抑制患者免疫系统产生的疾病和/或疾病抗原耐受。为患者给药治疗组合物能够使患者中的肿瘤逃避机制失效,并引起更好的肿瘤根除。优选地,给药治疗组合物能够抑制作为CD4+CD25+FoxP3+的Treg细胞的活性。本文中所提到的“Treg”细胞涉及作为CD4+CD25+FoxP3+的调节T细胞,并且包括nTreg和iTreg二者。本文中所提到的幼稚T细胞是作为CD4+CD25-FoxP3-的T细胞。
本发明的方法包括以多种方式抑制Treg细胞的活性。例如,可以通过抑制幼稚T细胞转化为iTreg细胞抑制Treg细胞的活性。该方法可以包括给药本文所描述的治疗组合物,其干扰幼稚细胞转化为Treg细胞,例如通过调控FoxP3的活性。FoxP3是一种转录因子,其是能够造成免疫抑制活性的细胞的标记物。细胞中,FoxP3的不存在或者恢复是细胞不能发挥抑制功能的指示。
本发明的方法还能够负调控患者的nTreg细胞,以便增强免疫耐受的抑制。除了作为对刺激的应答衍生自幼稚T细胞的iTreg细胞之外,患者还可以具有nTreg细胞。在本发明的方法中所使用的治疗组合物也可以抑制患者中nTreg和/或iTreg细胞的活性。
这些方法还可以包括给药IFN-γ,以便抑制Treg细胞的活性和/或包含CD40L。IFN-γ和/或CD40L可以用作治疗组合物的一部分。在治疗组合物中使用IFN-γ和/或CD40L可以阻断Treg的免疫抑制活性,而不会阻碍效应T淋巴细胞的活性。优选地,IFN-γ是从活化的Th1细胞自然产生的,从而这些细胞也能够在表面表达CD40L,其能够下调宿主免疫细胞中IL-12的表达,引起宿主IFN-γ产生的提高。
本发明的方法还可以包括使幼稚T-细胞偏向CD4+CD25+Tbet+GATA3-表型(Tbet+细胞)分化。给药治疗组合物能够使幼稚T-细胞偏向Tbet+细胞分化,这是患者中Th1免疫环境的指示。Tbet+的存在能够引起促进Th1环境,以及抑制由于促进偏离产生Treg细胞而造成的免疫耐受活性。
本发明包括给药如在上面方法中所述抑制Treg细胞活性的治疗组合物。该治疗组合物可以包括活的免疫细胞,其中至少一部分是T细胞。这些T-细胞优选是Th1表型的效应/记忆T-细胞(CD45RO+,CD62LLo)(CD4+T-细胞,其产生IFN-γ而不产生IL-4)。效应/记忆Th1细胞是在配制和引入患者的时候被活化,并且在本文中称为emTh-1细胞。优选的活化方法是通过在T-细胞上交联CD3和CD28表面分子。其他活化方法也在本发明的范围内。优选地,emTh-1细胞在被活化之后表达CD40L,并产生大量炎性细胞因子(例如IFN-γ、GM-CSF和TNF-α)。优选地,这些emTh-1细胞对于患者是同种异体的。可以如在例如授予Har-Noy的美国专利No.7,435,592中所描述的,进行emTh-1的活化,以及如在例如授予Har-Noy的美国专利No.7,402,431中所描述的,在缓冲液中进行配制以给药。这两份专利均通过参照并入本文。活化的emTh-1细胞可以用于本文所描述的治疗组合物。这些活化的e-mTh1细胞可以按照商标ALLOSTIM从Immunovative Therapies,Ltd.of Israel获得。表现出与这些细胞类似功能特征的其他活化的同种异体T-细胞也在本发明的范围内。尽管对本发明的包含活化的emTh-1细胞治疗组合物进行了描述,但含有是用其他方法制备但具有类似功能特征的细胞的治疗组合物也都在本发明的范围内。同样在本发明范围内的是emTh-1细胞的无生命组分,例如IFN-γ和CD40L分子。
除了emTh-1细胞外,治疗组合物还可以包含其他组分,这些组分能够强化患者中Th1环境的维持和/或抑制患者中疾病抗原的免疫耐受。在一些优选实施例中,治疗组合物还可以含有与疾病相关的抗原。该治疗组合物可以包含一种以上疾病抗原。如果在组合物中包含超过一种抗原,则这些抗原可以来自相同抗原源或者不同抗原源。在制剂中可以使用任何抗原源,例如这些抗原可以来源于活细胞或生物体,原材料可以是冻/融裂解物,其被辐射灭活(或其他灭活方法),作为完整细胞或生物体或其裂解物来使用。在一些优选实施例中,肿瘤细胞或者肿瘤细胞裂解物可以用作抗原的细胞原材料。细胞原材料可以来自自体或同种异体细胞源或来自细胞系。抗原还可以来自编码抗原的裸DNA或RNA。细胞核材料可以单独或与病毒载体结合使用。抗原源的另一实例是抗独特型抗体或其模拟抗原的部分,或其他模拟抗原结构的方法。抗原致敏(antigen-pulsed)或转染的树突状细胞(DC)也可以作为组合物中的抗原源。DC可以被肽或完整蛋白质、重组蛋白或mRNA或编码抗原的DNA致敏,或者DC可以与含有抗原的细胞融合,或者DC可以被病毒载体转染例如含有抗原的反转录病毒、慢病毒、腺病毒,或者这些抗原源可以单独使用而不使用DC。
在治疗组合物中也可以使用一种以上肿瘤相关抗原(TAA),TAA的例子包括:MART-1、gp100、酪氨酸酶、MelanA、TRP-1,肿瘤特异性突变基因产物例如CDK-4、β-链蛋白(β-catenin、MUM-1),致癌基因例如p53和ras(K-和H-ras)、癌症测试抗原例如MAGE、GAGE和NY-ESO1,过表达的自身抗原例如MUC1、细胞周期蛋白B1、Her2-neu、CEA、WT、p53、SART-1、PRAME、p15,和病毒抗原例如HPV E7、EBV-衍生抗原和端粒酶。
在优选实施例中,抗原组分可以包含一种以上从死的受感染组织或者肿瘤分离的分子伴侣蛋白(也称为热休克蛋白)。热休克蛋白(HSP)是针对细菌、真菌和寄生虫病原体的免疫应答的主要靶标。肿瘤衍生热休克蛋白(hsp)-肽复合物(特别是hsp70和grp94/gp96)已经被证明为有效的疫苗,在动物和人中产生抗肿瘤免疫应答。该方法利用应力蛋白(stressproteins)的肽结合性质,其负责多种细胞过程中作为分子伴侣的功能。
在细胞外环境中的某些分子伴侣也能够调节先天和适应性免疫力,因为他们能够伴随多肽并与宿主的免疫系统相互作用,特别是专门的抗原呈递细胞。利用来自肿瘤的HSP进行免疫接种,能够表现出抗肿瘤应答,并且下调免疫抑制机制。HSP的免疫原性可以来自与它们相关的抗原蛋白。
分离用作抗原源的分子伴侣的优选方法由Katsantis描述于US PatNo.6,875,849。其他的方法由Srivastava描述于US PatNos.6,797,480;6,187,312,6,162,436;6,139,841;6,136,315;和5,837,251。HSP也可以被抗原致敏包括肽、完整细胞或细胞裂解物。
在一个实施例中,肿瘤衍生富含分子伴侣的细胞裂解物(CRCL)用作抗原源,并且是通过利用在下面所描述的自由溶液等电聚焦(free solution-isolectric focusing,FS-IEF)技术富集主要分子伴侣蛋白而获得的。该技术是快速且有效的方法用于与传统技术相比获得多达5-20倍的抗原材料并且用更少的时间。从临床观点来看,就从可能受限的肿瘤来源获得高产量以及从肿瘤收获到患者治疗的快速轮转时间而言,多分子伴侣蛋白复合物富集的FS-IEF方法可以是期望的。
另外,在疫苗中常规使用的抗原也可以用于本发明的药物组合物,包括完整的微生物或微生物的部分例如活的减毒完整微生物、灭活微生物、重组肽和蛋白、糖蛋白、糖脂、磷脂、合成肽或破碎的微生物、多糖,其单独使用或者与载体元件缀合,例如载体蛋白,也可以被使用。
通常,任何能够用于治疗或者预防疾病的抗原或抗原组合都可以用于治疗组合物。衍生自炎性病原体的抗原也可以用作抗原源,并且在本文中称为致病抗原。作为抗原来源的疾病的例子可是:白喉,破伤风,脊髓灰质炎,狂犬病,百日咳,甲型、乙型和丙型肝炎,EBV,CMV,疱疹病毒1和2,黄热病,伤寒,水痘,天花(small pox,天花),麻疹,腮腺炎,德国麻疹,日本脑炎,脑膜炎,流感,肺炎球菌感染,轮状病毒感染,艾滋病(HIV-1和2),癌症,HTLV1和2,梅毒,HPV,结核病,莱姆病(Lyme disease),RSV感染,锥虫病,登革热,疟疾,炭疽热,埃博拉病毒,兔热病,耶尔森氏菌,西尼罗河病毒,由衣原体、淋病奈瑟菌(Neisseria gonorrheae)、肺炎链球菌(Streptococcus pneumoniae)、卡他莫拉菌(Moraxella catarrhalis)、金黄色葡萄球菌(Staphylococcus aureus)或流感嗜血杆菌B型(Haemophilus influenza type B)引起的细菌性疾病,疟疾,利什曼病,李氏杆菌病等。
在一个优选实施例中,治疗组合物包括emTh-1细胞以及还包括CRCL裂解物。emTh-1细胞以及CRCL裂解物独立地促进Th1免疫力,但是添加emTh-1细胞提供了佐剂以提高Th1免疫力的产生并且还促进Treg活性的抑制。另外,当在治疗组合物中组合这两种组分时,可以观察到协同应答,从而该组合比任何单独的组合物都更有效。
治疗组合物还可以包含能够干扰降低或者抑制Treg细胞免疫耐受活性的其他组分或者因子。在一个实施例中,该治疗组合物可以包含外源IFN-γ。IFN-γ可以是纯化的,或者重组IFN-γ。治疗组合物还可以包含抗-TGF-β抗体。TGF-β通常会提高Treg细胞活性。在治疗组合物中包含抗-TGF-β抗体可以降低Treg细胞的活性。
可以通过所有常规使用的途径给药治疗组合物,包括肠胃外,真皮内,肌内,皮下或粘膜途径。在某些实施例中,也可以通过节内或者瘤内给药组合物。同种异体emTh-1细胞优选来自刻意HLA-错配的供体。在治疗组合物中的优选剂量为针对真皮内途径至少大约1×107细胞,更优选针对经脉内途径为大约5×107~1×109细胞。在该范围外能够初步产生期望免疫应答的剂量也在本发明的范围内。
治疗组合物可以被一次或者更多次给药,在每次给药时采用不同的路线。如果给药超过一次,则组合物的再次剂量可以在首次剂量给药之后至少3天,优选至少7-14天进行给药。如果需要,可以给药其他剂量的治疗组合物。在一个优选实施例中,emTh1细胞首先被真皮内给药,然后真皮内在相同位置给药CRCL组合物。这些真皮内注射重复大约3~6次,每次间隔大约3~10天。然后,为了活化疫苗接种方案所产生的记忆细胞,可以通过输注注射仅给药emTh1细胞。
本发明还包括在患者中刺激治疗免疫作用的方法,包括降低Treg细胞的免疫抑制活性,通过为患者给药包含同种异体Th1细胞的治疗组合物,其中Treg细胞活性的降低和/或抑制会诱导患者中抵抗疾病的治疗效果。本发明还包括破坏患者中免疫抑制功能的方法,其包括降低Treg细胞的免疫抑制活性,通过为患者给药包含同种异体Th1细胞的治疗组合物,其中Treg细胞活性的降低和/或抑制会降低患者中的免疫抑制功能。

小鼠-在特定的无病原体的条件下饲养小鼠,并根据University of Arizona InstitutionalAnimal Care and Use Committee的指南进行看护。从国家癌症研究院(Bethesda,MD)获得雌性BALB/c(H2d),C57BL6(H2b),严重复合型免疫缺乏症SCID(H2d)和裸鼠(H2d)。从Jackson Immunoresearch Laboratories(Sacramento,CA)购买IFN-γ-受体-/-(H2d)小鼠。从Jackson Immunoresearch Laboratories获得FoxP3-EGFP(H2d)小鼠,其在外源启动子控制下功表达绿色荧光蛋白(GFP)和FoxP3。GFP表达能够精确地鉴定和分离FoxP3+Treg。从Jackson Immunoresearch Laboratories获得同源系Thy1.1小鼠(Cby.PL(B6)-Thy1a/ScrJ)。这些动物携带T淋巴细胞特异性Thy1.1等位基因。使用抗=Thy1.2抗体,可以将来自Thy1.2小鼠的供体T细胞与受体Thy1.1小鼠T细胞进行区分。这些小鼠均在6~8周龄时进行使用。
制备同种异体活化的emTh-1细胞-根据Har-Noy et al,Leukemia Research(2009)33:525-538和Har-Noy et al.,Leukemia Research(2008)32:1903-1913所述,体外产生emTh-1细胞并活化。收集来自C57BL/6小鼠的脾细胞,并利用氯化铵钾(ACK)缓冲液进行处理以裂解血红细胞。然后使用CD4+微珠和autoMACSTM分离装置(Miltenyi Biotec,Auburn,CA)。通过流式细胞术对正选择和负选择的细胞进行常规分析,以检验每个级分的纯度。然后在RPMI培养基(Gibco/BRL,Gaithersburg,MD)中对这些CD4+T淋巴细胞进行扩增,其中RPMI培养基补充了10%热灭活胎牛血清(Gemini Bio-products,Woodland,CA)、抗-CD3和抗-CD28-包被的顺磁珠(CD3/CD28 T-细胞扩增珠,Dynal/Invitrogen),起始的珠∶CD4+细胞比例为3∶1,以及存在20IU/mL重组小鼠(rm)IL-2,20ng/mL rmIL-7,10ng/mLrmIL-12(Peprotech,New Jersey)和10μg/mL抗-小鼠IL-4mAb(Becton Dickenson)。从第3至第6天,每天向培养物中添加额外含有rmIL-2,rmIL-7,抗-IL-4mAb和CD3/CD28珠的完整培养基,以保持稳定的细胞密度(0.5-1×106细胞/mL)。计算所添加珠的量,以维持在细胞扩增时,1∶1的珠∶细胞比例。在培养6天后,CD4+T细胞的扩增大约是第0天时所接种细胞数目的60至100倍。在第6天,收集细胞,并通过物理破坏和通过磁体进行脱珠。直接使用这些细胞或者保存在液氮中以后使用。采用类似的规程,以从C57BL/6小鼠产生emTh-1细胞。
从健康供体外周淋巴细胞产生人emTh-1细胞,其中外周淋巴细胞是采用淋巴细胞分离介质(1.077;Eurobio)进行密度离心而分离的。然后采用人CD4微珠(Miltenyi Biotec)分离T细胞,并与人T-细胞扩增珠(Dynabeads;Invitrogen)进行培养,其中在存在20IU/mL重组人IL-2(rhIL-2),20ng/mL rhIL-7,10ng/mL rhIL-12(Peprotech)和10g/mL抗-人IL-4 mAb (BD Bio-sciences)时进行培养。根据前面针对小鼠细胞的段落中所描述的,进行维持人CD4 T-细胞培养物。涉及人的研究得到了institutional review board(IRB00005448;FWA00004218)的批准,并且被通知符合Declaration of Helsinki的规定。
磁力细胞分选-收获并解离从BALB/c或C57BL6小鼠分离的脾。使用小鼠CD4+CD62L+幼稚T细胞或CD4+CD25+T调节细胞分离试剂盒以及autoMACSTM分离器根据制造商的指南(Miltenyi Biotec,Auburn,CA),通过磁力细胞分选CD4+CD62L+,CD4+CD25+和CD4+CD25-T淋巴细胞。通过该技术分离的CD4+CD25+T淋巴细胞表达高水平的转录因子FoxP3,并且具有免疫抑制性能。
CD4 + CD62L + T细胞转化为CD4 + CD25 + FoxP3 + Treg-如前所述,从Balb/c小鼠脾细胞中分离CD4+CD25-CD62L+幼稚T细胞,并在96孔板(100细胞/孔)中在完全培养基中进行培养,利用抗-CD3/抗-CD28T细胞扩增珠(Dynabeads,Invitrogen)进行活化,其中T淋巴细胞∶珠比例为1∶1,并且存在TGF-β(5 ng/ml),在37摄氏度下培养72小时。利用同种异体活化的CD4+emTh-1细胞上清液,处理一些孔。通过流式细胞术分析确定CD4+CD25+FoxP3+和CD4+CD25+FoxP3-细胞的百分比。
流式细胞术分析和抗体—在含有3%热灭活胎牛血清和0.09%叠氮钠(Sigma Chemical)的PBS中对细胞(~106)进行清洗,并首先与Fc受体阻断Ab(BD Biosciences Pharmingen,San Diego,CA)孵育5分钟,然后与饱和量的荧光染料-缀合Ab适当组合孵育40min。然后使用FACS calibur(Becton Dickinson Immunocytometry Systems,San Jose,CA)对细胞进行清洗和分析。针对每个样品收集最少10,000个事件,并利用CellQwest Pro software(BectonDickinson Immunocytometry Systems)进行数据分析。对于FoxP3检测,采用Allophycocyanin(APC)抗-小鼠FoxP3染色组合按照提供者的指导(Clone FJK-16,eBioscience,San Diego,CA),在体对通过磁力细胞分选纯化的CD4+CD25+或CD4+CD25-T细胞或者产生的转化的CD4+CD25+Treg进行固定,渗透以及染色,并通过流式细胞术进行分析。为了监控CD4+CD25+Treg,首先用异硫氰酸荧光素(FITC)-缀合抗-CD4(rat IgG2b;BD BiosciencesPharmingen)和藻红蛋白(PE)-缀合抗-CD25(rat IgG1;BD Biosciences Pharmingen)抗体对细胞进行染色。然后,采用上面所述的eBioscience FoxP3组合对细胞进行染色。通过采用抗-小鼠Tbet-藻红蛋白和抗-小鼠Gata-3-藻红蛋白单克隆抗体(BD Biosciences Pharmingen)的细胞外染色对转录因子Tbet和Gata-3(分别由Th-1和Th-2细胞表达)的表达进行评估。从BD Biosciences Pharmingen(PE-缀合大鼠IgG1,FITC-缀合大鼠IgG2a)或eBioscience(APC-缀合大鼠IgG1)购买同种型对照抗体。
T细胞扩增和抑制检验—使用Miltenyi分离试剂盒从脾细胞和淋巴结细胞纯化CD4+CD25+和CD4+CD25-T细胞。将细胞在96孔板中在37摄氏度下,在完全培养基中或利用活化的emTh-1细胞上清液中培养48小时,并用平板-结合抗-CD3,可溶抗-CD28和IL-2进行活化。在其他实验中,将未处理的,活化的emTh-1细胞上清液预处理或新分离的CD4+CD25-T细胞(1×105)在圆底96孔板中与经过处理或未经处理的CD4+CD25+T细胞(1×105)共培养48小时。在所有共培养物中添加抗-CD3/CD28T细胞扩增珠-Dynabeads(细胞/珠=1/1)。然后添加溴脱氧尿苷(BrdU)(Millipore Corporation of Massachusetts)持续额外12小时。根据制造商的程序(Millipore Corporation of Massachusetts),通过酶联免疫检验(ELISA)对细胞进行固定,并掺入BrdU。培养均设置6批。
通过利用1.077淋巴细胞分离介质的密度离心分离的人外周血淋巴细胞进行类似的实验。使用人调控T-细胞分离试剂盒(Miltenyi Biotec)从总外周血单核细胞纯化CD4+CD25+和CD4+CD25-T细胞。在全培养基或利用人emTh-1上清液将细胞在96孔板中在37摄氏度下培养24小时,并且利用平板-结合抗-CD3(5ng/mL),可溶性抗-CD28(5ng/mL)和IL-2(20IU/mL)进行活化。然后采用细胞扩增试剂盒,根据制造商的程序(Invitrogen)对CD4+CD25-应答T细胞进行染色。将所标记的细胞与CD4+CD25+T细胞(1x105)利用人抗-CD3/CD28T-细胞扩增珠(细胞∶珠比例1∶1)共培养,并在72小时后按照制造商的指导通过流式细胞术进行分析细胞分裂。
通过ELISA检测细胞因子的产生—使用酶联免疫吸附检验(ELISA)根据制造商的程序(eBiosciences)确定细胞培养上清液中IFN-γ和TNF-α的浓度。
12B1白血病细胞和肿瘤产生—通过利用人bcr-abl(b3a2)融合基因反转录病毒转化BALB/c骨髓细胞获得鼠白血病细胞系12B1。这些细胞表达p210bcr-abl蛋白。这是严重的白血病,100%致死剂量(LD100)是在尾部经脉注射后100个细胞。在RPMI培养基(Gibco/BRL,Gaithersburg,MD)中培养细胞(37℃,5%CO2),其中,RPMI培养基(Gibco/BRL,Gaithersburg,MD)补充了10%热灭活胎牛血清(Gemini Bio-products,Woodland,CA)。从Dr Wei Chen(Cleveland Clinic,Cleveland,OH)获得12B1细胞,并按照常规进行检测,发现不含支原体(Mycoplasma)。
为了产生肿瘤,首先在PBS(Gibco/BRL)对12B1细胞进行清洗,然后计数和调节至浓度为5×104细胞/mL。对雌性BALB/c小鼠在右侧腹股沟皮下注射0.1mL(5×103细胞),并且监控肿瘤的产生。
CRCL制备—制备12B1肿瘤-衍生CRCL。简言之,将肿瘤组织在含有去污剂的缓冲液中进行匀浆,并将所获得的高速上清液在Rotofor设备(Bio-Rad)中在15W恒定功率下进行FS-IEF。收集级分,并分析分子伴侣蛋白含量。汇集感兴趣的级分,并通过透析、去除去污剂和离心浓缩制备为疫苗。使用Limulus Amebocyte Lysate(LAL)检验试剂盒(Cambrex Bio Science,Walkersville,MD)根据制造商的指导,确定CRCL制备物中所包含的内毒素水平。CRCL中内毒素的水平低于培养基对照中的水平(<0.01EU/μg)。将12B1 CRCL用于对小鼠进行体内接种疫苗。
体内肿瘤生长和组合免疫治疗—在第0天,在右侧腹股沟处,为BALB/c小鼠注射5×103个活12B1细胞。在第+3,+7以及+14天,给药同种异体(C57BL6)活化的CD4+细胞(105个细胞/小鼠)或12B1-衍生CRCL疫苗(25μg/小鼠)或细胞+CRCL。这些处理是在右侧脚底肉垫按照总体积100微升提供的。每隔一天,监控肿瘤生长,并在肿瘤体积达到4000mm3时,将小鼠进行麻醉。将不同处理组之间的无肿瘤存活进行比较。
体内免疫细胞的耗尽—通过在第-1天,第+3天和第5天,腹膜内注射抗-asialo GM1抗体(25μl/小鼠,利用PBS稀释至1/8;Wako Chemicals,Osaka,Japan),使小鼠耗尽NK细胞。
统计—通过log-rank统计产生和分析Kaplan-Meier曲线,以确定处理组之间的存活百分比和差异。在其他实验中,Student’s t检验用于确定组之间的显著差异(p<XXXX)。
使用磁力活化细胞分选,从小鼠脾分离CD4+CD25-CD62L+幼稚T细胞。使用T-细胞扩增珠(细胞∶珠比例1∶1)活化细胞,并重悬浮来自不同肿瘤细胞系的培养基中(12B1,B16,4T1),并用emTh-1细胞的上清液进行处理。图1A,经过处理的细胞在96孔板中37摄氏度下孵育72小时,并使用流式细胞术分析确定FoxP3表达。图1B显示了从独立的实验收集的数据。对这些数据进行作图,并采用Student’s t检验对图进行分析。
如图1A和1B所示,Th-1细胞上清液在体外削弱由肿瘤细胞或TGF-β诱导的从幼稚CD4+CD25-FoxP3-T细胞分化CD4+CD25+FoxP3+Treg。Treg扩展可以来自CD4+CD25-FoxP3-T细胞转化为CD4+CD25+FoxP3+,或者来自天然存在Treg的扩增。转录因子FoxP3对于诱导Treg抑制功能是必需的,并且在非调控细胞中它的表达将它们转化为免疫抑制细胞。检验效应-记忆Th-1细胞所产生的可溶性因子的功能。特别的,进行一些研究以观察TGF-β是否可以负调控在体内由不同分泌TGF-β的肿瘤诱导的从幼稚T细胞产生Treg。已知12B1,B16黑色素瘤和4T1细胞能够在培养物中分泌TGF-β。利用这3种细胞系之一培养幼稚CD4+CD62L+T细胞,都促进它们分化为FoxP3+T细胞(图1A)。然而,通过在培养过程中存在活化的emTh-1细胞上清液,显著阻断了肿瘤诱导的FoxP3表达(图1A,B)。
利用T-细胞扩增珠(细胞∶珠比例1∶1)具有或没有TGF-β(5ng/mL)对CD4+CD25-CD62L+幼稚T细胞进行培养72小时,其中,存在或者不存在emTh-1上清液(emTh-1sup)。然后,通过流式细胞术对细胞进行分析。图2A和图2B是来自10次独立实验的代表性点图或柱状图。图C是在总CD4+T淋巴细胞中CD4+CD25+FoxP3-活化的T细胞的百分比。P<0.01,与不使用emTh-1上清液培养的细胞相比的显著差异。图2D是在CD4+CD25-CD62L+T细胞中确定的转录因子FoxP3,Tbet和GATA-3的表达,其中CD4+CD25-CD62L+T细胞被利用T-细胞扩增珠具有或没有TGF-β1培养,采用或没有采用emTh-1sup处理。三次实验的代表性结果显示于图2D。
图2A-2G中的数据显示emTh-1上清液显著地抑制TGF-β-诱导的幼稚T细胞转化为FoxP3+T淋巴细胞(图2A和2B)。此外,同种异体emTh-1上清液放大了活化的效应(CD25+FoxP3-)细胞的数目(图2C)。观察到该作用是剂量依赖性的(图2E)。TGF-β-诱导的从FoxP3EGFP-转基因小鼠分离的幼稚CD4+CD25-FoxP3-T细胞转化,是在存在或者不存在emTh-1上清液时进行的。然后对GFP-阳性(即表达FoxP3)细胞进行分选,并且评估他们的抑制活性。这些结果证明这些残余表达FoxP3的iTreg的功能被削弱(图2F)。另外,添加到之前已经转化的FoxP3+iTreg的emTh-1上清液显著地削弱了他们的免疫抑制功能(图2G)。
对效应-记忆Th-1上清液进行评估,以观察他们是否可以将TGF-β-诱导的Foxp3+T细胞偏向促炎细胞系分化。转录因子T-bet和GATA-3分别主要是由Th1或Th2细胞表达的。流式细胞术分析显示在存在TGF-β+Th-1cell上清液时培养72小时后获得的大部分CD4+T细胞表达低水平的Gata-3和FoxP3。大部分所获得的细胞展示出Tbet-阳性表型,这与Th-1极化一致(Fig 2D)。因此,这些结果说明emTh-1细胞产生可溶性的因子,其能够将幼稚T细胞的TGF-β-依赖性极化从FoxP3+Treg转向促炎症Th-1谱系。
利用T-细胞扩增珠培养CD4+CD25-CD62L+幼稚T细胞72小时,其中,存在或者不存在emTh-1上清液,采用或者不采用针对(图3A)小鼠IFN-γ或(图3B)小鼠TNF-α的阻断抗体。在图3C和3D中,从IFN-γR-/-小鼠脾分离CD4+CD25-CD62L+幼稚T细胞,并利用T-细胞扩增珠培养72小时,其中,存在或者不存在TGF-β1处理的emTh-1上清液,存在或者不存在emTh-1上清液。通过流式细胞仪确定表达FoxP3的细胞的百分比。
检验Th-1细胞所产生主要细胞因子的参与。在来自Th-1细胞培养物中检测到高水平的IFN-γ和TNF-α。使用IFN-γ而不是TNF-α的阻断抗体中和会破坏emTh-1上清液的作用,恢复TGF-β诱导的幼稚细胞向表达FoxP3的T细胞转化(图3A和3B)。该数据显示重组IFN-γ,而不是TNF-α会削弱TGF-β诱导产生FoxP3+T细胞的负调控(图3A和3B)。使用IFN-γR-/-小鼠确认了这些结果。图3C和3D中所示数据显示从IFN-γR-/-小鼠分离的CD4+幼稚T细胞转化为FoxP3+T细胞并不受Th-1细胞上清液调节(图3C和3D)。因此,IFN-γ展现了负责Th-1介导的抑制FoxP3+T淋巴细胞产生的主要机制。
在图4A中,从BALB/c小鼠淋巴组织分离CD4+CD25+nTreg,并用平板-结合抗-CD3(5ng/mL),可溶性抗-CD28(5ng/mL)和IL-2(20IU/mL)培养指定时间,其中添加或者不添加emTh-1上清液。然后,采用流式细胞术测定FoxP3表达。在图4B中,利用平板-结合抗-CD3,可溶性抗-CD28和IL-2添加或不添加emTh-1上清液,培养CD4+CD25+nTreg 48小时。然后,采用完全培养基对细胞进行充分清洗。利用抗-CD3/抗-CD28 T-细胞扩增珠刺激应答CD4+CD25-T淋巴细胞,其中在不存在(CD25-)或存在未处理nTreg(CD25-+未处理nTreg)或在存在emTh-1上清液-处理的nTreg(CD25-[nTreg]emTh-1sup)时。在48小时后,采用BrdU掺入检验确定应答CD4+CD25-T-淋巴细胞增殖。NS,非显著;P<0.001,与利用未处理Treg培养的应答CD25-T细胞相比的显著差异。在图4C中,首先,利用emTh-1上清液对CD4+CD25-T淋巴细胞进行处理([CD25-]emTh-1sup)或不进行处理(未处理CD25-),持续48小时,利用完全培养基充分清洗,并与新分离的CD4+CD25+nTreg(+nTreg)共培养48小时。然后,采用BrdU掺入检验确定应答CD25-T细胞的增殖。P<0.001。在图4D中,如图4C所述对共培养物中的IFN-γ浓度进行测定。
在图4E中,从人PBMC分离CD4+CD25+Treg,并暴露至人emTh-1细胞上清液持续24小时。然后添加Cell Trace Violet-标记的CD25-应答T细胞至未处理Treg(CD25-+未处理Treg)或emTh-1预处理Treg(CD25-+[Treg]emTh-1sup)。在图4F中,首先对人CD4+CD25-T淋巴细胞用emTh-1上清液进行处理([CD25-]emTh-1sup),或者不进行处理(未处理CD25-),然后用Cell Trace Violet标记,并与新分离的CD4+CD25+Treg(+Treg)共培养。如在材料和方法中所描述的,采用流式细胞术分析,确定应答CD 25-T-细胞增殖。
图4A中的结果证明FoxPro3表达在nTreg中不受emTh-1上清液削弱。分离自小鼠淋巴组织的nTreg被抗-CD3和抗-CD28抗体以及IL-2活化。将细胞培养在完全培养基或emTh-1上清液中持续48小时,然后清洗,之后与新分离的CD4+CD25-细胞共培养额外48小时。使用BrdU掺入检验分析emTh-1上清液处理或未处理的nTreg抑制CD4+CD25-细胞增殖的能力。数据显示emTh-1上清液(图4B)或IFN-γ显著抑制Treg抑制CD4+CD25+常规T-细胞增殖的能力。采用分离自外周血淋巴细胞的人CD4+CD25+Treg和CD4+CD25-应答T细胞获得类似的结果。
利用emTh-1上清液孵育T细胞48小时,并与新分离的CD4+CD25+FoxP3+nTreg共培养。利用emTh-1上清液预处理的CD4+CD25-T细胞的增殖(图4C)和IFN-γ的产生(图4D)都不受Treg的抑制。在将emTh-1上清液预先孵育的人CD4+CD25+应答T细胞暴露于CD4+CD25+Treg时,获得了类似的结果(图4F)。这些数据表明emTh-1细胞不仅削弱Treg的抑制功能,而且诱导效应T细胞对Treg介导抑制的耐受。
在第0天,在幼稚Balb/c小鼠的右侧腹股沟注射5×103个活12B1细胞。在图5A中,在第+3,+7和+14天,进行接种疫苗。如在“材料和方法”中所述,在右侧脚底肉垫进行处理。监控小鼠的存活,并展示在Kaplan-Meyer图中。在图5B中,在第0天,在SCID小鼠的右侧腹股沟注射5×103个活12B1细胞,并用PBS,活化的emTh-1或活化的emTh-1+CRCL的组合,通过在第+3,+7和+14天脚底肉垫注射进行处理。在图5C中,为具有免疫活性的Balb/c小鼠注射肿瘤细胞,并用PBS或活化的emTh-1+CRCL的组合在第+3,+7和+14天进行处理。在一些组小鼠中,如“材料和方法”中所述,在第-1,+3和+5天,使用i.p.注射抗-asialo GM1抗体,耗尽NK细胞。在所有试验中,监控小鼠的存活,并展示在Kaplan-Meyer图中。在图5D中,在第0天,为C57BL/6小鼠注射5×105个B16细胞。然后用B16衍生CRCL+emTh-1+B16CRCL对动物进行处理(第3,5和7天)。如在材料和方法中所述,每隔一天对肿瘤体积进行监控。使用Kaplan-Meyer分析描述动物存活。
为了诱导肿瘤特异性免疫力,对基于同种异体效应-记忆Th-1细胞的免疫治疗进行评估,以观察是否它可以与个性化疫苗组合,如来自相同类型肿瘤的CRCL。如图5的结果所示,同种异体emTh-1细胞可以被安全地体内递送,和与肿瘤富含衍生分子伴侣蛋白的小鼠裂解物(CRCL)组合能有效地处理具有12B1白血病的小鼠。使用在幼稚Balb/c小鼠中建立的12B1肿瘤,并确认基于同种异体效应-记忆Th-1细胞的免疫治疗能够有效地与CRCL接种疫苗有效地组合,实现受处理动物的显著的无肿瘤存活(图5A)。采用B16疟疾模型获得类似的结果,其中,疟疾模型由B16肿瘤携带C57BL/6小鼠构成,其被B16-衍生CRCL+Balb/c小鼠产生的emTh-1细胞处理(参见图5D)。
利用SCID和裸鼠确定由CRCL+活化的emTh-1诱导的抗肿瘤应答中T细胞的作用。在如上所述,接种肿瘤细胞之后3,7和14天,使用CRCL+活化的emTh-1对携带12B1肿瘤的SCID小鼠进行处理。CRCL+活化的emTh-1免疫治疗不改善携带12B1肿瘤的SCID小鼠的存活(图5B)。使用裸鼠获得了类似的结果。这些数据表明同种异体Th-1+CRCL组合免疫治疗针对12B1肿瘤的保护作用依赖于T细胞介导免疫应答。与这些数据一致,在免疫活性小鼠中使用抗-asialo-GM1抗体体内耗尽NK细胞不会显著削弱同种异体Th-1+CRCL处理的治疗效果,这表明NK细胞在由联合治疗诱导的抗肿瘤免疫应答中不发挥主要作用(图5C)。因此,基于同种异体活化的emTh-1细胞结合CRCL疫苗的免疫治疗的肿瘤保护作用依赖于T淋巴细胞。
在第0天,为幼稚BALB/c小鼠注射5×103个12B1细胞(在右侧腹股沟皮下注射)并进行接种疫苗,如图5所述。在第45天,利用在右侧腹股沟皮下提供的5×103个12B1细胞以及在左腹股沟皮下提供1x106个A20细胞再次对存活的无肿瘤小鼠进行再次攻击。每隔一天,确定肿瘤体积。在图6A中,在对照小鼠中监控A20肿瘤体积。在图6B中,在emTh-1+CRCL-处理的小鼠中监控A20肿瘤体积。在图3C中,在对照族中监控12B1体积。在图3D中,在emTh-1+CRCL-处理的动物中监控12B1肿瘤体积。结果是2次独立试验的代表性结果。
对于经过CRCL+同种异体活化的emTh-1细胞处理存活的小鼠,再次通过在右侧腹股沟肠胃外施加12B1肿瘤细胞以及在相对的腹股沟施加无关的B细胞白血病(A20,H-2D)而再次攻击。在处理组和对照组中,在所有8只小鼠中均产生A20肿瘤(图6A和6B)。在CRCL+同种异体效应-记忆Th-1细胞组中,8只小鼠中有5只被保护抵抗12B1肿瘤再次攻击,两只显示出肿瘤生长的延迟,而同时所有对照小鼠均产生12B1肿瘤(图6C和6D)。因此,这些结果表明在多数受处理的动物中,由CRCL+同种异体活化的emTh-1细胞构成的联合免疫治疗诱导了长期肿瘤特异性免疫力。
在图7A中,emTh-1+CRCL免疫治疗诱导肿瘤特异性杀伤T淋巴细胞。为携带B16肿瘤的小鼠注射对照PBS,或者用B16-衍生CRCL以及同种异体emTh-1细胞进行处理,如在材料和方法中所述。在最后接种疫苗之后7天,收获脾细胞并与CRCL(25微克/mL)和50U/mL IL-2孵育3天。然后在尼龙毛柱(nylon wool column)上纯化T淋巴细胞,并与B16肿瘤细胞或无关的4T1乳腺癌细胞靶孵育36小时,如所示(效应T细胞∶靶肿瘤细胞比例为20∶1)。测定细胞毒性。对照T,利用来自注射PBS的小鼠的T淋巴细胞培养的肿瘤细胞;+T[emTh-1],利用来自经CRCL+emTh-1处理的小鼠的T淋巴细胞培养的肿瘤细胞。在图7B中,emTh-1细胞在体内,使CD4+CD25-FoxP3-幼稚T淋巴细胞偏向CD4+CD25+FoxP3-effector T细胞分化,而不是Treg。将从Thy1.2FoxP3EGFP转基因BALB/c 小鼠分离的CD4+CD25-FoxP3-幼稚T淋巴细胞(107细胞)转移至携带12B1肿瘤的致癌Thy1.1BALB/c小鼠。利用emTh-1细胞(+emTh-1细胞)或对照PBS(无emTh-1细胞)对动物进行处理。受体Thy1.1小鼠的内源T细胞表达Thy1.1而不表达Thy1.2抗原,这允许特异性追踪和鉴定Thy1.1小鼠中的Thy1.2T淋巴细胞。收集脾并解离,使用流式细胞术在CD4+T-细胞进行门控之后,通过评估Thy1.2GFP+细胞的频率确定所转移幼稚Thy1.2CD4+CD25-FoxP3-T细胞在体内向GFP+(表达FoxP3)Treg的转化。在图7C中,emTh-1细胞弱化体内Treg抑制功能。在不存在(CD25-)或存在Treg时,利用抗-CD3/抗-CD28 T-细胞扩增珠刺激应答CD4+CD25-T淋巴细胞,所述Treg是从未处理的携带肿瘤的小鼠(CD25-+[Treg]未处理)或emTh-1(CD25-+[Treg]emTh-1)处理的携带肿瘤的小鼠的引流淋巴结分离的。在使用BrdU掺入检验48小时后,测定应答CD4+CD25-T-淋巴细胞增殖。
对从CRCL+emTh-1细胞-处理的小鼠分离的T细胞的抗肿瘤功能进行分析。从接受联合治疗的动物的脾纯化的淋巴细胞能够特异性杀死亲代肿瘤细胞但不杀死无关靶癌症细胞(图7A)。EmTh-1细胞能够使转移至携带12B1肿瘤的致癌Thy1.1小鼠的幼稚Thy1.2CD4+CD25-FoxP3-T淋巴细胞偏向Thy1.2CD4+CD25+FoxP3-效应T-细胞而不是Thy1.2CD4+CD25+FoxP3+Treg分化(图7B)。此外,从利用emTh-1细胞处理的携带肿瘤的动物分离的Treg的抑制功能显著降低(图7C)。这确认了emTh-1细胞对Treg在体外观察到的作用也发生在体内,并且还证实emTh-1放大CRCL疫苗效率的机制包括抑制肿瘤诱导Treg。

Claims (9)
1.包含活化的Th1细胞和疾病抗原源的组合物在制备药物中的用途,其中所述药物用于在患有疾病的患者中抑制免疫耐受和促进治疗效果,
其中,所述Th1细胞和所述疾病抗原在相同的位置被给药,
所述活化的Th1细胞为同种异体Th1细胞,并且所述活化的Th1细胞降低在患者中降低Treg细胞的免疫抑制活性,其中降低所述Treg细胞的活性会:i)在患者中降低疾病抗原的免疫耐受;ii)在患者中引起对疾病的治疗效果;或iii)在患者中降低免疫抑制功能,以及
其中所述Treg细胞为CD4+CD25+FoxP3+,
所述疾病包括恶性血液病和产生肿瘤的癌症,所述肿瘤位于患者乳房、肺、肝、胃、皮肤或胰腺。
2.根据权利要求1所述的用途,其中通过降低幼稚T细胞(CD4CD25-FoxP3-)转化为iTreg细胞,抑制Treg细胞的抑制活性。
3.根据权利要求2所述的用途,其中所述Treg细胞的抑制活性是通过抑制nTreg细胞达到的。
4.根据权利要求1所述的用途,其中:
通过真皮内注射给药所述Th1细胞和疾病抗原;和/或
在给药所述疾病抗原源之前,给药所述活化的Th1细胞。
5.根据权利要求1所述的用途,其中,所述Th1细胞是同种异体emTh-1细胞。
6.根据权利要求1所述的用途,其中,所述疾病抗原源是富含分子伴侣的细胞裂解物。
7.根据权利要求1所述的用途,其中,以大约3-10天的间隔给药所述Th1细胞和疾病抗原源至少三次,直到患者展现出治疗效果。
8.根据权利要求1所述的用途,其中所述疾病抗原源以所述疾病抗原相关的裂解物的形式提供的。
9.根据权利要求1所述的用途,其中所述组合物进一步包含:
i)IFNγ;
ii)与TGF-β相互作用并降低TGF-β活性的组分;或
iii)CD40L。
