Claims (15)

  1. 下結節伸展部に刺激を与える装置において、
    複数の電極を備えた先端部と、基端部とを有するリードであって、前記複数の電極は、ぺーシング電極として機能しているときにぺーシング信号を供給する、又は感知電極として機能しているときに心拍数をモニタするように構成される前記リードと、
    感知モード又はぺーシングモードで選択的に機能するよう動作可能な回路と、を備え、
    前記回路は、前記複数の電極に電気的に接続されると共に、同複数の電極のうちのどれが最も確実に信号を伝導する遅い伝導路であるかを決定し、その最も遅い伝導路の電極にぺーシング信号を供給し、それによって前記最も遅い伝導路の電極を感知電極として機能する状態からぺーシング電極として機能する状態に切り替える、装置。
  2. 前記複数の電極は2個の電極を含む、請求項1に記載の装置。
  3. 前記複数の電極は3個の電極を含む、請求項1に記載の装置。
  4. 前記リードは前記先端部に設けられたねじ部を備え、前記ねじ部は、前記リードの前記先端部のチップから延在し、前記基端部に向かって第一電極および第二電極を越えて延在するが、第一電極および第二電極を越えて第三電極までは延在していない、請求項1〜3の何れか一項に記載の装置。
  5. 前記複数の電極は4個の電極を含む、請求項1に記載の装置。
  6. 前記回路は、
    バッテリと、
    チャージスイッチを介して前記バッテリと電気的に並列接続されたキャパシタと、
    感知回路と、
    前記複数の電極の各々に対して少なくとも一つが対応するように設けられる複数のぺーシングスイッチと、
    前記複数の電極の各々に対して少なくとも一つが対応するように設けられる複数の感知スイッチと、を備え、
    前記感知モード時、前記複数の感知スイッチが前記感知回路を前記複数の電極に電気的
    に接続すると共に、前記複数のぺーシングスイッチが前記キャパシタを前記複数の電極から電気的に切り離し、
    前記ぺーシングモード時、前記複数のぺーシングスイッチのうちの一つが前記キャパシタを前記最も遅い伝導路の電極に電気的に接続すると共に、前記感知スイッチのうちの一つが前記感知回路を前記最も遅い伝導路の電極から電気的に切り離す、請求項1〜5の何れか一項に記載の装置。
  7. 前記ぺーシングモード時、前記最も遅い伝導路の電極以外の全ての電極が感知電極として機能し続ける、請求項6に記載の装置。
  8. 前記ぺーシング信号は、前記下結節伸展部に刺激を与えるために、前記ぺーシング電極の作動、停止、および調整のうちの少なくとも一つを生じさせる、請求項1に記載の装置。
  9. 前記装置は房室結節の血管を通じて下結節伸展部に案内されるように構成され、前記装置はさらに、前記リードを房室結節の血管を通じて下結節伸展部に案内するカテーテルを備え、同カテーテルは湾曲部を持つチップを有する先端部を備えている、請求項1〜8の何れか一項に記載の装置。
  10. 前記湾曲部は10度〜60度の角度をなす、請求項9に記載の装置。
  11. 請求項1〜10の何れか一項に記載の装置を準備するステップと、
    以下の二つのステップ、すなわち、
    前記装置が心臓の下結節伸展部近くに配置されるよう同装置を移動させるステップ、及び
    前記下結節伸展部に刺激を与えるために、前記装置の作動、停止、および調整のうちの1つ以上を行うステップ、
    を実施するための指令を与えるステップと、
    を備える方法。
  12. 下結節伸展部に刺激を与えるために、請求項1〜10の何れか一項に記載の装置を使用する方法。
  13. 下結節伸展部に刺激を与える装置であって、
    第一電極および第二電極を備えた先端部と、基端部とを有するリードと、
    前記先端部に設けられたねじ部とを備え、
    前記ねじ部は、前記リードの前記先端部のチップから延在し、前記基端部に向かって第一電極と第二電極を越えて延在している装置。
  14. 第一電極および第二電極の近くに、第三電極をさらに備え、
    前記ねじ部は、第一電極および第二電極を越えて第三電極までは延在していない、請求項13に記載の装置。
  15. 感知回路をさらに備え、
    前記電極は、刺激機能から感知機能へ切り替え可能である、請求項13に記載の装置。
