Abstract
Description

更なる他の態様では、本発明は、各種の独特な臨床的な癌治療製剤を提供する。幾つかの実施態様では、これらの癌治療製剤は、細胞外マトリックス物質のゲル、シート、又は注射可能な調製物の形態であってよい。注射可能な調製物は、静脈内投与に適切なものとして説明されてよい。
更なる他の態様では、本発明は、全癌細胞が増殖し、且つ、実質的に除去されたECMが回収され、ワクチンとして使用されてよい調製物を提供する。これらの順化ECM調製物は、したがって、本質的に癌細胞を含まないものとして開示され、本明細書に記載の全細胞含有調製物の有益な抗癌及び抗腫瘍増殖特性の全てではないかもしれないが、多くを提供すると期待される細胞と組織分泌因子/ペプチド/有機及び無機分子との比較的濃縮された組み合わせを有する。全細胞を含まないことによって、全細胞の投与についての任意の予期されない懸念事項を避け得る。
他の態様では、本発明は、本明細書に記載の各種の形態にあるECMを、T細胞サプレッサー(シクロホスファミド)、サイトカイン(IL−21)、サイトカイン顆粒球/マクロファージコロニー刺激因子(GM−CSF)、ホルモン(メラトニン)、免疫抑制酵素(1−メチル−トリプトファン)、COX−2インヒビター(シクロオキシゲナーゼ−2)、オリゴヌクレオチド(CpGオリゴヌクレオチド)、又はこれらの任意の組み合わせなどの他の活性剤と共に使用し得る製剤及び/又は治療計画を提供する。
更なる他の態様では、本発明は、意図する患者の腫瘍及び/又は癌細胞組織/生検組織をSISなどのECM物質上で増殖させる、カスタマイズされたECMワクチンを提供する。前記細胞が培養により増殖する機会を得た時点で、細胞を不活化又は除去し、ECM物質を洗浄し、次いで、ECMを洗浄した物質を、患者のためのワクチン又はアジュバントとして使用する。この手法は、個々の患者の腫瘍に特異的且つ特有である可能性がある癌組織抗原の標的を可能にする。さらに、この本発明の態様は、患者の臨床的必要性によって要求される継続する追加ワクチン接種を提供するのに十分な量まで、回収した腫瘍物質のECM上での増大を可能にする。

用語の定義がその用語の一般的に使用される意味から逸脱する場合は、出願人は、特に示していない限り、以下に提示する定義を用いることを意図する。
材料及び方法
本実施例は、本発明の実施において使用されてよい材料及び方法の幾つかの例を提示する。
小腸粘膜下層(SIS)は、Cook Biotech,Inc.(West Lafayette,IN)から得た。その物質は、滅菌されて凍結乾燥された細胞外マトリックスのシートとして提供された。実験等級の物質は、SIS調製物の使用のために提供され、ブタの空腸を回収し、10から20cmの長さで生理食塩水溶液に入れることによって調製されたことが開示されている(31〜33)。全ての腸間膜組織を除去した後に、空腸切片を裏返して、粘膜を外科用メスの持ち手及び湿らせたガーゼを用いて長手方向にこする動作によって磨耗した。漿膜及び筋層を、次いで、同じ手法を用いてゆっくりと除去した。残存する組織は、過酢酸を用いて消毒し、高純度の水で広範囲にわたってすすぎ、移植前にエチレンオキシドを用いて滅菌した。
RCMは、Cook Biotech,Inc.(West Lafayette,IN)から得た。簡潔には、腎被膜は、解体した直後に成体のブタの腎臓から切除した。それを流れる水道水で全体的にすすぎ、希釈した過酢酸のエタノール溶液で殺菌し、汚染する可能性がある細菌及びウイルスを除去した(34)。殺菌後に、RCMを高純度の水ですすぎ、酸を除去して、凍結乾燥によってシート状にした後に、エチレンオキシドガスを使用して移植前に滅菌した。
PAIII細胞株は、LWラットの自発性前立腺腫瘍から得た。PAIII細胞は、増殖又は疾患のパターンを変化させずに、多数の継代の間、LWラットに移植された。PAIII細胞がLWラットの脇腹に皮下移植される際に、大きな転移アデノカルシノーマが40日以内に発症したが、初期の腫瘍は10日以内に触診可能である。原発腫瘍から、PAIII細胞は、頻繁に肺に転移する。PAIII腫瘍は、ホルモン非依存的であり、大半の治療に対して耐性を示す(35)。
GFT細胞ワクチンは、動物内で増殖した腫瘍から回収した細胞の、グルタルアルデヒド固定化腫瘍(GFT)懸濁物である。GFT細胞ワクチンは、腫瘍組織から調製した(36)。具体的には、3グラムの皮下腫瘍組織をLobund−Wistarラットから回収して、ワクチン調製に使用した。皮下腫瘍は、LWラットにおける自然発生の転移性前立腺アデノカルシノーマから単離した前立腺アデノカルシノーマ細胞を投与することによって生産された(37)。
Unibersity of Notre Dameで維持されている繁殖コロニーから得られたLWラットを全ての試験に使用した。このモデルにおいて、大きな腫瘍が、約99%のラットにおいて1×106PAIII細胞の皮下投与の後に皮下に発生した。
当該モデルにおいて、オスの3から4月齢のLWラットの脇腹に、1×106PAIII細胞を皮下に投与した。14から21日後に、触診可能な腫瘍が存在し、40日までに、転移巣が肺に存在した。切除を伴う試験のために、動物は無菌手術に備えた。視認できる腫瘍を切除するが、切除は徹底的なものではなく、十分な腫瘍床を、未処理の固体の100%が腫瘍の再増殖を起こすように残す。
単相のSIS又はRCMのシートを2×2cmの切片に切断して、修飾イーグル培地(MEM)に入れる。PAIII細胞(1×106)又はPAIII皮下ラット腫瘍から直接回収した細胞(1×106)を、SIS又はRCMに重ねて、37℃でインキュベートした。SIS上でGFT細胞ワクチンを作製するために、細胞を接着させたSISに、次いで、グルタルアルデヒド固定化(GFT)を実施して、洗浄する。グルタルアルデヒド固定化は、2.5%のグルタルアルデヒド(v/v)中で60分に亘って37℃においてインキュベートし、次いで、培地で洗浄することを伴う。
破傷風毒素を用いた攻撃後の生存対非生存の結果を、2の自由度でカイ二乗検定を用いて群の間で比較した。p≦0.05である際に、差が有意であると解した。腫瘍重量の平均についての結果は、ウィルコクソンの順位和検定を用いて群の間で比較し、p≦0.05の際に有意であると解した。
腫瘍細胞ワクチン及び癌アジュバントのin vivo活性
本実施例は、in vivoにおける有効な癌ワクチンアジュバントとしての本発明の利用を実証する。
処理なし=11.64グラムの平均腫瘍重量;
GFT細胞=10.54グラムの平均腫瘍重量;
GF SIS=12.31グラムの平均腫瘍重量;
GF SIS+GFT細胞=4.77グラムの平均腫瘍重量。
ECMによる癌細胞増大の支持
本実施例は、細胞外マトリックス物質上での癌細胞集団の拡張のための方法の提供についての本発明の利用を実証する。本実施例は、癌ワクチン調製物において有用な高度に免疫原性の細胞集団を調製するための発明の利用も実証する。癌の場合では、多数の重要な抗原は、結合組織マトリックスによって発現され、腫瘍細胞の細胞外マトリックスとの相互作用に関するようである。細胞外マトリックス上で増殖させた癌細胞ワクチンは、かくして、本実施例に従って調製され、ワクチン接種のための改善されたワクチン抗原組成物として使用されてよい。
本発明は、本発明の実施に使用されて良い細胞外マトリックス物質のタイプの他の例を示す。本実施例は、ブタ筋膜細胞外マトリックス物質(FEM)を利用する。
過去の研究者は、純粋な細胞株のSIS上におけるin vitro増殖能を評価していた。例えば、Badylak et al(38)は、NIH Swissマウス3T3線維芽細胞、初代ヒト線維芽細胞、ケラチノサイト、内皮細胞、及び確立されたラット骨肉腫細胞株の培養を支持することが可能であることを示した。本実施例は、SISに由来する本明細書に記載の細胞外マトリックス調製物が、癌細胞の増殖を支持することを実証する。特に、前立腺癌細胞株及び皮下腫瘍(Lobund−Wistar(LW)ラットにラットPAIII細胞を接種することによって生じた腫瘍)から直接回収した混合細胞集団が、本明細書に記載の条件下において物質上で増殖することが示された。
手術による切除後の腫瘍の再増殖を予防するためのワクチンアジュバントとしてのSIS
研究の初期において、本発明者は、前立腺癌を予防するという、PAIIIラット腫瘍から直接回収されたグルタルアルデヒド固定化腫瘍(GFT)細胞の能力を開示した(36)。これに基づいて、本実施例は、ワクチン接種が、手術による切除後の腫瘍の再増殖を阻害することを実証する。
ワクチンは、皮下腫瘍から回収した腫瘍細胞を、3日間に亘ってSIS上で培養して増殖させ、その後にグルタルアルデヒド固定化(GFT)及び洗浄を実施することによって調製した(SIS上のGFTワクチン)。グルタルアルデヒド固定化は、2.5%グルタルアルデヒド(v/v)中で60分に亘って37℃で細胞をインキュベートし、次いで、培地で洗浄することを伴う。5ラットの一群は切除のみを実施し、1つの群はGFT細胞ワクチンを腫瘍床に適用し、1つの群はSISを腫瘍床に適用し、並びに1つの群はSIS上のGFT細胞ワクチンを腫瘍床に適用した。3週間後の平均腫瘍再増殖(腫瘍重量(グラム)±標準偏差)についての結果は、図5に示しており、以下のようであった:
切除のみ:11.64±2.14グラム、4/5が肺への転移を有していた;
SIS単独:13.61±1.46グラム、4/5が肺への転移を有していた;
GFT細胞ワクチン:9.50±1.27グラム、3/5が肺への転移を有していた;
SIS上のGFT細胞ワクチン:3.98±1.37グラム、2/5が肺への転移を有していた。
第二の試験では、移植前に、細胞を28日間に亘ってSIS上で培養した。この試験の結果は、図6に示しており、以下のようなものであった:
切除のみ:14.9グラム±2.12、6/6が肺への転移を有していた;
SISのみ:15.6グラム±1.82、5/5が肺への転移を有していた;
GFT細胞ワクチン:11.8グラム±1.46、4/5が転移を有していた;
SIS上のGFTワクチン:6.01グラム±1.17、2/5が肺への転移を有していた。
癌を予防するワクチン用のアジュバントとしてのSISゲルの作用
固体のSISマトリックスに取り込まれたワクチンの移植は組織の切開を必要とするため、全ての適用に実用的であるわけではない可能性がある。かくして、本実施例は、SISなどの細胞外マトリックスを用いたゲル形態における癌に対するワクチンを提供するという本発明の利用並びにワクチンアジュバントとしてのその使用を実証する。
0.25mlのSISゲル;

を皮下投与した。
生理食塩水:1.02g(±0.37)、5/6ラットが肺への転移を有していた、
GFT細胞ワクチン:0.86g(±0.11)、6/10ラットが肺への転移を有していた、
SISゲル中のGFT細胞ワクチン=0.19(±0.14)、1/10ラットが肺への転移を有していた。
癌の治療のためのワクチンアジュバントとしてのSISゲルの作用
本実施例は、予防手段及び治療手段としての免疫効力の強化を提供するための本発明の利用を実証する。
治療群:平均腫瘍重量(g)±SD,肺への転移
生理食塩水(対照):9.2±2.2g、6/6が転移を有していた;
SIS単独:8.6±1.8g、6/6が転移を有していた;
GFT細胞ワクチン:5.8±0.9g、4/6が転移を有していた;
SISゲル中のGFT細胞ワクチン:5.0±0.8g、3/6が転移を有していた;
SISシート上のGFT細胞ワクチン:2.1±1.1g、3/6が転移を有していた;
GFTワクチン及びECMアジュバントの安全性
本実施例は、ヒトを含む動物の治療のための臨床的に許容される調製物としての本発明の利用を実証する。特に、本実施例は、前記調製物が、組織損傷を誘導せず、かつ、自己免疫疾患を生じさせないことを実証する。
SISはin vivoに配置した際に腫瘍組織の増殖を促進しない
本実施例は、腫瘍/癌細胞調製物を用いて抗腫瘍活性を提供するものとしての細胞外マトリックスの利用を実証する。本実施例は、本発明の調製物が、それ自体によって腫瘍及び/又は癌の増殖を誘導することはないことも実証する。
偽手術(対照);
SISを用いた腫瘍の内包(腫瘍は下層の血管床から切除しなかった);
完全な腫瘍の切除(全ての肉眼で確認できる腫瘍を除去した);又は
完全な腫瘍の切除後に、SIS(約3×3cm)を切除した腫瘍床にかぶせる
の1つに分類した。
シートSIS、臨床応用のために提案する投薬計画
本実施例は、シート様の調製物におけるSISの使用によって、癌の治療及び/又は腫瘍増殖の低減/阻害を提供するためのSIS中の本発明のワクチンの利用を実証するために提示する。
細胞外マトリックスに基づくワクチンは開示されていないが、不活化同種異系全前立腺癌細胞株を含む前立腺癌ワクチンの使用が開示されている(Michael,el al)(2005)(47)。本試験において、12ヶ月に亘る一ヶ月ごとの8×106の不活化全細胞の経皮注射を、最初の二回は標準的なアジュバントであるalum中で、ホルモン耐性前立腺癌を有する患者に投与した。最初の二回の投与において使用したアジュバントは、カルメットゲラン桿菌であった。最初の三回の投与は1週間毎に与え、その後1ヶ月に一度与えた。この手法によって、毒性の兆候を示すことなくPSA(前立腺特異的抗原)速度における統計的に有意な低減が誘導された。さらに、疾患の進行の所定の点までの時間の中央値が、約28週から58週に増大した。
数少ない研究が、腫瘍の手術による切除と組み合わせてワクチン接種を利用することに注目している。Pilla et al(2006)(49)は、手術による切除後に、2週間毎の4回までの治療において、腫瘍由来のヒートショックプロテインgp96ペプチド複合体ワクチンを進行した黒色腫患者に投与した。その手法は、11/18の患者において手術後における疾患の安定化をもたらした。Berd et al(1997)(50)は、臨床的に明らかなリンパ節への転移を有する黒色腫患者に、一週間毎又は一ヵ月毎の予定で不活化自己全細胞ワクチンを投与した;この手法は、手術単独によるものよりも優れた生存率をもたらした。
ワクチンの経皮投与
本実施例は、組織に基づくアジュバント癌調製物の経皮適用可能な製剤を提供するための本発明の利用を実証する。
SISは同種異系細胞株に基づくワクチンのための効果的なアジュバントである
上記実施例に示したように、PAIII前立腺癌細胞又はLobund−Wistar(LW)ラット中の前立腺腫瘍から直接回収した細胞を用いたワクチンは、同系の動物中の保護免疫を刺激する。対照的に、同種異系細胞は、同種の遺伝的に異なる個体から得られたものである。かくして、PAIII細胞株は全てのLWラットの間で移植可能であり、かつ、同系であると解されるが、Mt−Lu及びMat−Ly−Lu細胞株は、Copenhagenラットに由来する。これら後者の2種の細胞株は、LWラットに移植した際にも腫瘍が発生しない。RFL−6細胞株は、本発明者が、線維芽細胞抗原が切除後の腫瘍再増殖に対する保護免疫を強化するか否かを決定するために評価した、同種異系ラット線維芽細胞株である。

GF Mat−Ly−Lu+RFL:5.9(3.06)、1/6の肺が陽性;
GF Mat−Ly−Lu+SIS上のRFL:2.4(1.77)、0/6の肺が陽性。
SISは、異種細胞株に基づくワクチンのための効果的なアジュバントである
癌の免疫療法に共通の問題は、免疫寛容についての問題である。各種の機構によって、宿主免疫系は、単純に、腫瘍に効果的に反応することができない。多くの場合には、腫瘍が「自己」であると認識される。かくして、腫瘍抗原と類似あり、且つ、異物として強く認識される抗原が有利であろう。これに関して、他の種(異種)からの細胞株が有益であろう。本発明によれば、異種腫瘍細胞に基づくワクチンは、宿主の腫瘍を攻撃し得るものとして、強力な免疫反応を提供するであろう。

ワクチンアジュバントとして作用する他の細胞外マトリックス(ECM)の機能
本実施例は、各種の異なる細胞由来のマトリックスを使用してワクチンを提供するための本発明の利用を実証する。
細胞を含まないワクチン調製物としての順化ECM組織物質の調製物の提案
本実施例は、ECM順化ワクチン又はワクチンアジュバントの本質的に細胞を含まない調製物を提供するための本発明の利用を実証する。当該順化ECMは、ワクチン又はワクチンアジュバントとして使用されてよい。
ECMワクチン及び第二の活性成分を用いた併用療法の提案
本実施例は、化学療法剤などの他の活性剤と共にECMワクチン調製物を含む治療法を提供するための本発明の利用を示す。単独又は併用における、以下に記載するもの並びに同じクラス/生物学的機能/生物学的活性の他のものなどの薬剤の包含も、臨床治療のための本明細書に提示する各種の用途において有用であろう。幾つかの場合において、前記併用は、ECMの抗癌活性及び/又は有効性を更に改善することが期待される。幾つかの実施例において、選択された化合物は、ECMと混合又は化学結合などによって結合されてよい。この様な併用剤が含んで良いものの幾つかの例を以下に挙げる。
患者に合わせたECMワクチンの提案
本実施例は、治療すべき標的患者から組織を用いてECMの専用ワクチン調製物を提供するための本発明の利用を実証する。意図する患者由来の組織を用いて実際に調製することで、本発明によるワクチン調製の手法は、とりわけ、非自己免疫反応と関連する有害な効果の可能性を低減及び/又は最小化するであろう。さらに、ECM上における回収した腫瘍組織の増大は、患者の臨床的進行によって示される継続する追加免疫接種に十分な物質の生産を可能にする。これは、本明細書に記載の調製物の抗原活性を保持しながら達成される。
Claims (22)
- 免疫原量の興味ある癌抗原調製物;及び
癌ワクチンアジュバント
を含む、癌ワクチンとしての使用に適切な組成物であって、
前記癌ワクチンアジュバントが、細胞外マトリックス物質に特徴的な調製物を含み、前記癌ワクチンアジュバントの存在下における前記興味ある癌抗原調製物の免疫原量が、前記癌ワクチンアジュバントの非存在下において保護反応を刺激するのに十分な興味ある癌抗原調製物の免疫原量未満である、組成物。 - 前記興味ある癌抗原調製物がヒト前立腺細胞を含む、請求項1に記載の組成物。
- 前記癌ワクチンアジュバントが、小腸粘膜組織に由来する細胞外マトリックス物質を含む、請求項1に記載の組成物。
- 興味ある癌抗原の免疫原性を高めることが可能な細胞外マトリックス物質を含む癌ワクチンアジュバントを得る工程;
前記癌ワクチンアジュバントを免疫原量の興味ある癌抗原と組み合わせる工程;
を含む方法によって調製される、請求項1に記載の組成物であって、
