本発明は、薬物療法の分野にある。詳細には、本発明は、慢性的アルコール摂取またはアルコールに対する胎児期曝露によって生じる脳疾患または脳損傷を、ペルオキシソーム増殖因子活性化受容体(PPAR)アゴニストを投与することによって治療するため、予防するため、または回復させるための方法に関する。
アルコール依存および乱用は世界中で最も費用のかかる医療上の問題の一つであり、それらの影響は、女性および若年者全般における過剰飲酒の発生率が増加しているため、増大し続けている。過度の飲酒は認知機能障害および脳に対する永続的な構造的損傷を引き起こす恐れがある。ウェルニッケ-コルサコフ症候群は、アルコールに伴う神経変性の最も廃疾性が高く臨床的に重大な形態の一つであるが、その病因の大部分は予防可能であるチアミン欠乏症に関係している。これとは対照的に、認知障害および運動障害をもたらす、白質縮小、脳室拡大、小脳変性、ならびに上前頭連合皮質、前帯状領域および視床下部の内部でのニューロン損失を含む、より有病率の高いアルコール関連脳病変の発生病理は明らかになっていない。
本発明は、アルコール誘発性脳損傷および脳疾患およびFASの発生においてインスリン抵抗性の増大が果たす重要な役割、ならびにPPARアゴニストが脳損傷を予防または治療する能力に関する。アルコールを慢性的に経口摂取した動物に対するPPARアゴニストの投与は、酸化ストレス(例えば、脂質過酸化)およびDNA損傷に起因する損傷を含むアルコール摂取に応じて起こる脳損傷を軽減または予防する。このため、本発明は、動物におけるアルコール誘発性脳疾患を治療するため、予防するため、または回復させるための方法であって、前記動物にPPARアゴニストの治療的有効量を投与する段階を含む方法に関する。
慢性的エタノール曝露の成体ラットモデルに関する一般的方法
雄性(約200g)のLong Evansラット(Charles River Laboratories, Cambridge, MA)に対して、エタノールがカロリー含有量の0%または37%(9.25%v/v)を構成する等カロリー液体飼料(BioServ, Frenchtown, N.J.)を6週間与えた(Yeon et al., Hepatology 38:703 (2003))。固形飼料を与えた対照群も試験対象とした。同等な食物消費および体重の維持を確かなものとするためにラットを毎日モニターした。屠殺後に脳を摘出し、パラフィン包埋のためにHistochoice固定液(Amresco Corp., Solon, OH)中に浸漬固定するか、または側頭皮質、視床下部および小脳のマイクロダイセクションのために未加工のまま切片化した。未加工の組織ブロックをドライアイス-メタノール浴中で急速凍結(snap-frozen)させ、mRNAおよびタンパク質の試験のために-80℃で保存した。
固定した脳を、統一された目標物に沿って冠状面で切片化し、パラフィン中に包埋した。パラフィン包埋切片(8μm厚)をヘマトキシリンおよびエオシンで染色するか、またはDNA損傷および脂質過酸化を検出するためにそれぞれ8-ヒドロキシデオキシグアノシン(8-OHdG)(Oxis Research)もしくは4-ヒドロキシノネナール(4-hydroxynonenol)(HNE)(Chemicon International, Temecula, CA)に対するモノクローナル抗体で免疫染色した。免疫染色の前に、脱パラフィン処理して再水和させた切片を、リン酸緩衝食塩水(10mMリン酸ナトリウム、0.9%NaCl、pH 7.4;PBS)中の0.1mg/mlサポニンにより室温で20分間処理し、その後に内因性ペルオキシダーゼ活性を消失させるためにメタノール中の3%過酸化水素により10分間処理し、続いて非特異的な結合部位をブロックするためにSuperBlock-TBS(Pierce Chemical Co., Rockford, IL)中にて室温で30分間処理した。切片を0.5〜1μg/mlの一次抗体とともに4℃で一晩インキュベートした。免疫反応性は、ビオチン化二次抗体、アビジン-ビオチン-西洋ワサビペルオキシダーゼ複合体(ABC)試薬、および色素原としてのジアミノベンジジンを用いて検出した(Vector Laboratories, Burlingame, CA)(Lam et al., J. Biol. Chem. 269:20648 (1994))。組織切片をヘマトキシリンで対比染色し、光学顕微鏡法によって検査した。陰性対照(非関連抗体)および陽性対照(グリア酸性線維素タンパク質)の反応を並行して行った。切片には符号を付した上で検査した。
全RNAを、TRIzol試薬(Invitrogen, Carlsbad, CA)を製造元のプロトコールに従って用いて脳組織から単離した。RNAの濃度および純度を、260nmおよび280nmで測定した吸光度から決定した。RNA(2μg)を、AMV First Strand cDNA合成キット(Roche Diagnostics Corporation, Indianapolis, IN)およびランダムオリゴデオキシヌクレオチドプライマーを用いて逆転写させた。リアルタイム定量的RT-PCRを用いて、インスリン、IGF-IおよびIGF-II成長因子、それらの対応する受容体、ニューロン(Hu)、アストロサイト(グリア酸性線維素タンパク質;GFAP)、オリゴデンドログリア(ミエリン結合糖タンパク質-1;MAG-1)、ミクログリア(AIF1)および内皮細胞(エンドセリン-1;ET-1)の遺伝子、アセチルコリンエステラーゼ(AChE)およびコリンアセチルトランスフェラーゼ(ChAT)のmRNAレベルを測定した。並行した反応で測定したリボソーム18S RNAのレベルを用いて、mRNA転写物の相対的存在量を算出した(Myers et al., Trends Biochem. Sci. 19:289 (1994);Baltensperger et al., Science 260:1950 (1993))。PCR増幅は、2.5ngの元のRNAテンプレートから生成されたcDNA、各300μMの遺伝子特異的な順方向および逆方向プライマー(表1)ならびに12.5μlの2×QuantiTect SYBR Green PCR Mix(Qiagen Inc, Valencia, CA)を含む25μlの反応物中で行った。増幅されたシグナルを、BlO-RAD iCycler iQ Multi-Color RealTime PCR検出システム(Bio-Rad, Hercules, CA)を用いて連続的に検出した。用いた増幅プロトコールは以下の通りである:最初に95℃で15分間の変性および酵素活性化、95℃×15秒間、55〜60℃×30秒間および72℃×30秒間を45サイクル。アニーリング温度は、iCyclerソフトウエアに備わっている温度勾配プログラムを用いて最適化した。
未加工の凍結組織(約100mg)を、プロテアーゼ阻害薬(1mM PMSF、0.1mM TPCK、1μg/mlのアプロチニン、1μg/mlのペプスタチンA、0.5μg/mlのロイペプチン、1mM NaF、1mM Na4P2O7)を含む5倍容積のNP-40溶解緩衝液(50mM Tris-HCl、pH 7.5、1% NP-40、150mM NaCl、1mM EDTA、2mM EGTA)中でホモジナイズした。タンパク質濃度をビシンコニン酸(BCA)アッセイ(Pierce, Rockford, IL)を用いて決定した。診査的試験により、20%の特異的結合を達成するために必要なタンパク質の量および放射性標識リガンドの濃度を決定した。インスリン受容体結合アッセイは、100μgのタンパク質を用いて行った。IGF-I結合アッセイは試料当たり25μgのタンパク質を必要とし、IGF-II受容体結合アッセイは10μgタンパク質を用いて最適化した。エタノール曝露との関連で成長因子の結合を評価するためには競合的平衡結合アッセイを用いた。全結合量に関しては、2つずつの個々のタンパク質試料を、結合緩衝液(100mM HEPES、pH 8.0、118mM NaCl、1.2mM MgSO4、8.8mMデキストロース、5mM KCl、1%ウシ血清アルブミン)および100nCi/mlの[125I](2000Ci/mmol;50pM)インスリン、IGF-IまたはIGF-IIを含む100μlの反応物中でインキュベートした。非特異的結合の測定のためには、反復試験試料を0.1μMの非標識(非放射性)リガンドの添加の点を除いて全く同じく調製した。
ヒト組換え[125I]インスリン、IGF-IおよびIGF-IIはAmersham Biosciences(Piscataway, NJ)から購入した。非標識ヒトインスリンはSigma-Aldrich(St. Louis, MO)から購入した。組換えIGF-IおよびIGF-IIはBachem(King of Prussia, PA)から入手した。8-OHdGおよびHNEに対するモノクローナル抗体はOxis Scientific(Foster City, CA)から購入した。他のすべての精製化学薬品および試薬は、CalBiochem(Carlsbad, CA)またはSigma-Aldrich(St. Louis, MO)から購入した。
実験は各群当たり9匹のラットを用いて実施した。データはグラフ中に平均±S.E.M.として表した。群間比較はスチューデントのT検定を用いて行った。統計学的分析はNumber Cruncher Statistical System(Kaysville, UT)を用いて行った。有意差および傾向に対応するP値はグラフの上に表示されている。
慢性的エタノール消費により引き起こされる成体ラット脳における神経変性
組織病理学的検討は、小脳、および海馬を含む側頭葉に的を絞ったが、これはこれらの構造がエタノール媒介性神経毒性の公知の標的であり、それらを分子的および生化学的アプローチを用いて評価した。ヘマトキシリンおよびエオシンで染色した脳切片は、エタノール摂取ラットの小脳皮質における構造異常を示し、これにはプルキンエ細胞の損失および内顆粒層の内部の細胞の密度低下が含まれた(図1A〜D)。これに対して、側頭葉および海馬では、細胞損失の明らかな証拠を含め、明確な組織病理学的異常がみられなかった。
細胞種におけるエタノール誘発性病的変移
慢性的にエタノールを与えることが側頭皮質、視床下部および小脳の内部の細胞集団における病的変移を引き起こしたか否かを明らかにするために、リアルタイム定量的RT-PCR試験を用いて、Huニューロン性リボソームRNA結合タンパク質(Datta et al., Cell 91:231(1997);Hetman et al., J. Neurosci. 20:2567(2000);Dudek et al., Science 275:661(1997))、オリゴデンドログリアに関するミエリン結合糖タンパク質-1(MAG-1)、アストロサイトに関するグリア酸性線維素タンパク質(GFAP)、および内皮細胞に関するエンドセリン-1(ET-1)をコードするmRNA転写物を測定した。検出された各々の特異的mRNA転写物のng量を、同一の試料中で測定された18S RNAレベルに対して標準化し、各群当たり9匹の動物からの結果を統計学的に分析した。この試験により、側頭葉(図3A)および小脳(図3C)ではHu遺伝子発現のエタノールに関連した低下がみられるが、視床下部(図3B)ではみられないことが示された。MAG-1発現はエタノール摂取ラットの視床下部で有意に低下したが、側頭葉および小脳ではそうではなかった(図3D〜3F)。GFAP発現の有意な増大がエタノール摂取ラットの小脳で検出されたが、側頭葉および視床下部ではそうではなかった(図3G〜3I)。ET-1発現はエタノール摂取ラットの側頭葉で有意に増大したが、視床下部または小脳ではそうではなかった(図3J〜3L)。
インスリン、IGF-IおよびIGF-IIポリペプチド、ならびにインスリン、IGF-IおよびIGF-II受容体のmRNA発現に対するエタノールの影響
リアルタイム定量的RT-PCR試験により、対照およびエタノール曝露させた脳の両方において、インスリン、IGF-IおよびIGF-IIポリペプチドならびにインスリン、IGF-IおよびIGF-II受容体に対応するmRNA転写物が検出された(図4および5)。インスリン遺伝子は視床下部で最も大量に発現され、そのレベルは側頭葉および小脳よりも20〜40倍の高さであった(図4A〜4C)。IGF-I遺伝子も視床下部で最も大量に発現されたが、その平均レベルは側頭葉および小脳よりも3〜4倍の高さであった(図4D〜4F)。全体的には、IGF-IIはインスリンおよびIGF-Iよりも大量に発現され、この場合も最も高度な発現は視床下部においてであった(図4G〜4I)。慢性的にエタノールを与えることは、側頭葉におけるIGF-I発現(図4D)および小脳におけるIGF-II発現(図4I)の平均レベルを有意に低下させた。それ以外の点では、慢性的にエタノールを与えることは脳におけるインスリン、IGF-IおよびIGF-II発現に対して有意な影響を及ぼさなかった。
エタノールによるインスリン受容体およびIGF受容体の結合の障害
成長因子および成長因子受容体の発現に対する、慢性的にエタノールを与えることの影響にばらつきがあったことから、エタノールが別の機構を通じてインスリン/IGFシグナル伝達を障害させうるか否かを明らかにすることが関心の対象となった。有効なリガンド結合はインスリンおよびIGFのシグナル伝達カスケードにとって決定的に重要であり、ニューロン生存性の低下を含む、エタノール曝露脳で報告されているインスリンシグナル伝達障害の下流での影響の多くは、CNSにおけるインスリンまたはIGF-Iの結合の低下によって媒介されうると考えられる。側頭皮質、視床下部および小脳の膜タンパク質抽出物をトレーサーとして[125I]で標識したインスリン、IGF-IまたはIGF-IIとともに、過剰な非放射性リガンドの存在下または非存在下でインキュベートすることによって、平衡結合アッセイを行った。それらの試験により、側頭皮質および小脳よりも視床下部においてインスリン、IGF-IおよびIGF-II受容体結合(fmol/mg)がより高レベルであることが示された(図6)。慢性的にエタノールを与えることは、側頭葉、視床下部および小脳においてインスリンおよびIGF-I受容体結合を有意に低下させた(図6A〜6F)。加えて、IGF-II受容体結合は対照ラットと比較してエタノール曝露の小脳において有意に低く(図6I)、一方、側頭葉および視床下部ではIGF-II受容体結合がエタノール曝露群において相対的に保持されていた(図6G〜6H)。
アセチルコリン恒常性におけるエタノール媒介性障害
アセチルコリンはCNSの認知系および運動系において主要な機能的役割を果たしている。アセチルコリン産生はコリンおよびアセチル-Co-Aの十分な供給を必要とする。アセチル-Co-Aはエネルギー代謝によって産生されるが、それはインスリンおよびIGF-I刺激によって主導される。最近の研究では、コリンアセチルトランスフェラーゼ(ChAT)の発現はインスリンおよびIGF-I刺激によって調節されることが実証されている(Minana et al., J. Neurochem. 75:954 (2000))。このため、インスリンおよびIGF-Iシグナル伝達機構のエタノール性阻害がChATの欠乏を伴うか否かを明らかにすることが関心の対象となった。アセチルコリンの定常レベルはアセチルコリンエステラーゼ(AChE)による負の調節を受けるため、AChE mRNAレベルを測定することも関心の対象となった。リアルタイム定量的RT-PCR試験により、ChAT mRNA発現の平均レベルは視床下部(図7B)および小脳(図7C)では有意に低下するが、側頭葉(図7A)ではそうでないことが実証された。これに対して、慢性的にエタノールを与えることは、調べた3つの脳領域のいずれにおいてもAChEの発現は有意に変化させなかった(図7D〜7F)。
ヒトにおける慢性的エタノール曝露に関する一般的方法
対照例および慢性的アルコール依存患者からのヒトの死後貯蔵脳組織を、オーストラリアのシドニー大学組織資源センター(Tissue Resource Center at the University of Sydney)から入手した。症例はすべて慢性的エタノール乱用の証拠が文書記録されており、他の薬物乱用の証拠はない。対照被験者は年齢および性別に関して合致し、エタノール消費が低レベルであることが文書記録されているものとした。2つの脳領域を調べた:小脳皮質(前上虫部領域)および前頭葉内の前帯状回。これらの領域を試験のために選択した理由は、それらがエタノール神経毒性の主な標的であるためである。これらの領域のホルマリン固定してパラフィン包埋した切片をヘマトキシリンおよびエオシン色素で染色し、光学顕微鏡法により検査した。隣接組織切片は免疫組織学的染色に供した。同じ領域からの組織の未加工の急速凍結ブロックをmRNA発現および受容体結合の測定のために用いた。
慢性的エタノール消費による、成体脳における神経変性の発生
ヘマトキシリンおよびエオシンで染色した切片により、慢性的アルコール依存患者の小脳皮質における構造異常が実証された。これらの異常には、プルキンエ細胞の斑状損失、内顆粒層の内部の細胞密度の低下およびバーグマングリアの増殖が含まれた。これに対して、帯状回では、アルコール依存患者における細胞損失の明らかな証拠を含め、明確な組織病理学的異常がみられなかった。隣接切片の免疫組織化学染色により、アルコール依存患者におけるGFAPおよびHNEに対する細胞標識の著しい増加、ならびに8-OHdGに対する限局的な免疫反応性の増加が判明した。アルコール依存患者の小脳では、顆粒層、および皮質の下層をなす白質中核においてGFAP免疫反応性の増加が検出され、脂質過酸化を反映するHNE免疫反応性の増加は、皮質のプルキンエ細胞層および顆粒細胞層ならびに皮質下白質に局在していた。これに対して、8-OHdG免疫反応性は、アルコール依存患者において、小脳皮質のプルキンエ層および顆粒層の内部の散在性細胞ならびに皮質下白質で検出された。
細胞種におけるエタノール誘発性病的変移

インスリン、IGF-IおよびIGF-IIポリペプチド、ならびにインスリン、IGF-IおよびIGF-II受容体のmRNA発現に対するエタノールの影響
リアルタイム定量的RT-PCR試験により、対照例およびアルコール依存患者の脳の両方において、インスリン、IGF-IおよびIGF-IIポリペプチドならびにインスリン、IGF-IおよびIGF-II受容体に対応するmRNA転写物が検出された(図8および9)。対照脳では、インスリン遺伝子およびIGF-II遺伝子の発現は小脳虫部よりも帯状回において高度であり、一方、IGF-1 mRNAレベルはこの2つの構造物において同程度であった(図8)。全体的には、IGF-IIはインスリンおよびIGF-Iよりも大量に発現された。慢性的アルコール乱用は、帯状回および小脳虫部におけるインスリン遺伝子発現(図8A〜8B)ならびに小脳におけるIGF-I発現の平均レベルを有意に低下させたが、帯状回ではそうではなかった(図8C〜8D)。興味深いことに、IGF-II mRNAレベルはアルコール依存患者の小脳では有意に増大したが、帯状回ではそうではなかった(図8E〜8F)。
慢性的アルコール依存患者の脳において元のまま保たれるインスリン受容体基質(IRS)遺伝子発現
成長因子により刺激される、IRS分子を通じてのシグナル伝達に対する主な応答には、細胞の成長および生存性の増加、ならびにアポトーシスの阻害が含まれる(Eves et al., Mol. Cell. Biol. 18:2143 (1998);Condorelli et al., Mol. Cell. Biol. 21:3025 (2001);Halestrap et al., Biochem. Soc. Trans. 28:170 (2000);Hirsch et al., Cell. Biol. Toxicol. 14:141 (1998);Xu et al., J. Biol. Chem. 278:26929 (2003);Yeon et al., Hepatology 38:703 (2003);Dahia et al., Hum. Mol. Genet. 8:185 (1999);Urso et al., Life Sci. 28:1053 (1981))。インスリン/IGF-Iによって活性化されるシグナル伝達経路の完全性を検討するために、IRS-1、IRS-2およびIRS-4 mRNA転写物のレベルを測定した。IRS-3については調べなかったが、これはこのアイソフォームが齧歯動物の脂肪組織でのみ発現されるためである。リアルタイム定量的RT-PCRにより、対照例および慢性的アルコール依存患者の脳の両方においてIRS-1、IRS-2およびIRS-4 mRNA転写物が検出された(図10)。対照の帯状回組織では、IRS-1発現が最も高度であり、続いてIRS-2、さらにIRS-4の順であった(図10A、10C、10E)が、小脳虫部では、IRS-2およびIRS-1が同程度に多く、IRS-4は最も少なく発現された(図10B、10D、10F)。慢性的アルコール乱用は、帯状回および小脳虫部のいずれにおいても、IRS-1、IRS-2およびIRS-4の平均レベルに対して有意な影響を及ぼさなかった。

妊娠期間中の慢性的エタノール曝露のラットモデルに関する一般的方法
妊娠したLong-Evansラットに対して、エタノールがカロリー含有量の0%、8%、18%、26%または37%と等価である0%、2%、4.5%、6.5%、9.25%(v/v)を構成する等カロリー液体飼料(BioServ, Frenchtown, N.J.)を与えた。慢性的エタノール曝露のインビボモデルを作製するためには、これらの濃度のエタノールが一般的に用いられる(Vander Top et al., Alcohol Clin. Exp. Res. 29:882 (2005))。これらの液体飼料を妊娠第6日に開始し、全妊娠期間にわたって継続した。同等な食物消費および体重の維持を確かなものとするためにラットを毎日モニターした。小脳はエタノール神経毒性の主な標的であるため(Maier et al., Alcohol 23:49 (2001);Maier et al., Alcohol Clin. Exp. Res. 23:726 (1999);Mohamed et al., I. Cytol. Exp. Neurol. 97:35 (1987))、小脳を用いて、発達中のCNSにおけるインスリンおよびIGFシグナル伝達に対する、妊娠中のエタノールに対する慢性的曝露の影響を検討した。出生直後に採取した未加工の組織をドライアイス-メタノール浴中で急速凍結させ、mRNAおよびタンパク質の試験のために-80℃で保存した。加えて、小脳をHistochoice固定液(Amresco Corp., Solon, OH)中で浸漬固定してパラフィン中に包埋した。組織切片をヘマトキシリンおよびエオシンで染色し、光学顕微鏡法によって検査した。
インスリン受容体またはIGF-I受容体の分子を、ウサギポリクローナル抗体(1μg/ml)およびプロテインAセファロース(Amersham-Pharmacia, Arlington Heights, IL)を用いて個々の100μgタンパク質試料から免疫沈降させた(Ausubel et al., (2002) Current Protocols in Molecular Biology. John Wiley & Sons New York)。非放射性アッセイ(Chemicon International, Temecula, CA)を製造元のプロトコールに少し変更を加えたものに従って用いて、免疫沈降物における受容体チロシンキナーゼ活性を測定した。手短に述べると、ストレプトアビジンをコーティングしたウェル上に捕捉させたビオチン化合成ペプチド基質のチロシンリン酸化を、西洋ワサビペルオキシダーゼ結合抗ホスホ-チロシンおよびSuperSignal West Pico化学発光性基質(Pierce Chemical Co., Rockford, IL)を用いて検出した。発光はTopCount装置(Packard Instrument Co., Meriden, CT)にて測定した。プロテインA上に捕捉された免疫沈降物を、チロシンキナーゼ活性のレベルを反応物中の受容体タンパク質の含有量に対して標準化する目的で、デンシトメトリーを用いるウエスタンブロット法に供した。
インビトロ実験を用いて、小脳ニューロンにおける受容体結合ならびにコリンアセチルトランスフェラーゼ(ChAT)およびアセチルコリンエステラーゼ(AChE)の発現に対するエタノールの影響を検討した。初代ニューロン培養物を生後第8日のラット仔小脳組織(Nikolic et al., Genes Dev. 10:816 (1996))から作製した。培養物は、5%ウシ胎仔血清、4mMグルタミン、100μM非必須アミノ酸混合物(Gibco-BRL, Grand Island, NY)、25mM KClおよび9g/Lグルコースを補充したダルベッコ変法イーグル培地(DMEM)中に維持した。エタノール処置は、培養物(6ウェル培養皿または96ウェルプレートに播いた)を、50mMエタノールをリザーバートレイから気化させた密封チャンバー内に入れることによって達成した(de la Monte et al., Alcohol Clin. Exp. Res. 24:716 (2000);Banerjee et al., Alcohol Clin. Exp. Res. 22:2093 (1998))。対照培養物は、リザーバートレイに水を加えた点を除いて全く同じく処置した。チャンバーには75%窒素、20%酸素および5%二酸化炭素を含む気体を流した。37℃での96時間のインキュベーション後に、細胞を収集してインスリン、IGF-IおよびIGF-II受容体結合に関して測定した。または、細胞を12時間血清欠乏状態においた後に10nmインスリン、10nM IGF-I、25nM IGF-IIまたは媒体により16時間刺激して、RNAを収集してChATおよびAChEの発現を測定した。成長因子により10分間刺激した同時並行的な96ウェル培養物をATPの測定のために用いるか、または16時間刺激したものを生存度およびChATまたはAChEの免疫反応性を測定するために用いた。受容体結合に関連した膜コレステロール含有量の役割を調べるためには、対照細胞およびエタノール曝露細胞を媒体、ロック緩衝液(154mM NaCl、5.6mM KCl、2.3mM CaCl2、1.0mM MgCl2、3.6mM NaHCO3、5mM グルコース、5mM Hepes、pH 7.4)中にある10mMメチル-β-シクロデキストリン(MβCD)または10mMの水性可溶性コレステロールによって3時間処置し、その後に収集して結合アッセイを行った。成長因子による刺激下でのニューロン生存度および機能に対する膜コレステロール含有量の影響を検討するためには、全く同じく処置した96ウェル培養物を媒体、10nMインスリン、10nM IGF-Iまたは25nM IGF-IIにより10分間刺激し、その後にATP含有量に関して分析するか、または16時間刺激したものを生存度およびChATまたはAChEの免疫反応性に関してMICEアッセイを用いて測定した(de la Monte et al., Biotechniques 26:1073 (1999))。ATP含有量はATPLiteアッセイ(Packard, Meriden, CT)を用いて測定した。生存度はCyQuantアッセイ(Molecular Probes, Eugene, OR)。免疫反応性は、マイクロタイター免疫細胞化学ELISA(MICE)アッセイ(de la Monte et al., Biotechniques 26:1073 (1999))を用いて培養ウェル中にて直接測定した。
コレステロール含有量は、Amplex Redアッセイキット(Molecular Probes, Eugene, Oregon)を製造元のプロトコールに従って用いて測定した。予備的試験により、脂質(クロロホルム:メタノール)抽出物におけるコレステロールレベルおよび群間差は、製造元が示しているRIPA緩衝液抽出物中で測定したものと同程度であった。このため、組織試料の脂質抽出物による分析を行う必要はなかった。手短に述べると、組織ホモジネートを上記の通りにRIPA緩衝液中に調製した。1×反応緩衝液(キットに付属)中に系列希釈した試料を、100μlの最終反応容積中にて、150μM Amplex Red試薬、1U/ml西洋ワサビペルオキシダーゼ、1U/mlコレステロールオキシダーゼおよび0.1U/mlコレステロールエステラーゼとともにインキュベートした。反応物を37℃で30分間インキュベートし、蛍光をFluorocountマイクロプレートリーダー(Packard Instrument Co., Meriden, CT)にて測定した(Ex 560nm/Em 590nm)。キットに付属のコレステロール標準物質を用いて標準曲線を同時に作成した。コレステロールのレベルを試料中のタンパク質濃度に対して標準化した。
出生時体重および小脳発達に対する、妊娠中のエタノールに対する慢性的曝露の用量効果
8%、18%および26%エタノール群における仔の出生時体重は、対照と比較して有意に低くなかった。しかし、37%エタノールを含む食餌を与えた雌親からの仔は、平均体重が対照と比較して有意に低かった(P<0.05;表4)。8%エタノール食餌群の仔は平均体重が対照と比較して高かったが(P<0.01)、それにもかかわらず、CNS遺伝子発現および機能に関して他のエタノール曝露群と類似した複数の異常があり、このことは発達中のCNSにおけるエタノール誘発性神経毒性は出生時体重が正常でも高くても起こりうることを示唆している。低エタノール曝露群における出生時体重の増加に関して考えられる一つの解釈は、2型糖尿病患者の子孫に起こるものと同様に、仔が主としてインスリン抵抗性の影響を呈したというものである。用いた最高濃度のエタノールで達成された血中アルコールレベル(表3)は、ヒトにおいて中毒性であると法的にみなされ、救急処置のために救急治療室に搬送されたアルコール依存患者で以前に観察されている範囲内にあった(Fulop et al., Am. J. Med. 80:191 (1986);Jagger et al., Neurosurgery 15:303 (1984))。
妊娠した雌親に対して、カロリー含有量に対するパーセンテージまたは(v/v)として種々の濃度のエタノールを含むLieber-DiCarli等カロリー液体飼料を妊娠第6日から与え、全妊娠期間にわたって継続した。仔の体重を出生後直ちに秤量した。母体血液を出産後に採取して血中アルコール濃度を測定した。データは結果の平均±S.D.を示している。データは、Tukey-Kramer post-hoc有意性検定を伴うANOVAを用いて統計学的に分析した。有意なP値は対照と比較した際のものである(*P<0.05;**P<0.005)。
細胞密度、細胞構築および細胞遊走に関係するエタノール用量依存的な構造異常
対照小脳は、外顆粒細胞および内顆粒細胞、プルキンエ細胞ならびに分子層および稠密な顆粒細胞層に対応する明瞭な境界を伴う、皮質の境界明瞭な小葉(ひだ形成)および層構造を有し、散在性のアポトーシス性(凝縮性または断片化)核はわずかであった。エタノール用量が増えるに伴って、小脳皮質小葉は進行性に単純化し、皮質層構造はより不明瞭になり、顆粒層内部の細胞密度は低下した(図13A、13D、13G、13J、13M)。皮質小葉の減少は皮質表面の平坦化および広幅化ならびに溝形成の限定(浅い溝)を伴っていた。皮質層の境界線の減少は、内顆粒細胞層およびプルキンエ細胞層の幅広化および不規則層構造、ならびに外顆粒細胞層および分子層の狭小化を伴っていた。外顆粒細胞層のより強拡大の像により、エタノール用量依存的な進行性の細胞密度低下が示された。37%エタノール群では、細胞損失およびアポトーシスが目立ち、残りの細胞種は、細胞の多くが神経芽の要素(高密度のクロマチンを伴う、密に並んだ小さな円形または長円形の核)ではなくグリアの形態学的特徴(薄い小嚢状の核)を有しているという点で、対照とは異なった(図13B、13E、13H、13Kおよび13N)。アポトーシスの前のDNA切断に対応する一本鎖DNAを検出するための免疫組織化学染色により、標識核の密度のエタノール用量依存的な増加が判明した(図13C、13F、13I、13L、13O)。しかし、37%エタノール群における顕著な細胞損失およびアポトーシスは核標識の相対的な低下を伴っており、これはおそらく多くの細胞が既にアポトーシスを起こしているためと考えられる。
小脳内の細胞種におけるエタノール誘発性病的変移
小脳形成不全およびアポトーシスのエタノール用量依存的な増大が、残りの細胞集団における病的変移を生じさせたか否かを明らかにするために、リアルタイム定量的RT-PCR試験を用いて、Huニューロン性リボソームRNA結合タンパク質(Hu et al., J. Neurosci. Res. 78:637 (2004);Kumagai et al., J. Neuroimmunol. 93:37 (1999);Szabo et al., Cell 67:325 (1991))、オリゴデンドログリアに関するミエリン結合糖タンパク質-1(MAG-1)、アストロサイトに関するグリア酸性線維素タンパク質(GFAP)、ミクログリアに関するアログラフト炎症因子-1(AIF-1)(Imai et al., Biochem. Biophys. Res. Commun. 224:855 (1996);Ito et al., Stroke 32:1208(2001))、および内皮細胞に関するエンドセリン-1(ET-1)をコードするmRNA転写物を測定した。検出された各々の特異的mRNA転写物のng量を、同一の試料中で測定された18S RNAレベルに対して標準化し、各群当たり8〜12匹の動物からの結果を統計学的に分析した。これらの試験により、エタノールによって媒介されるHuおよびMAG-1発現の低下ならびにGFAPおよびAIF-1発現の増大が示されたが、ET-1には有意な変化はみられなかった(図14)。Huに対する阻害作用ならびにGFAPおよびAIF-1に対する刺激作用はエタノール用量依存的であったが、MAG-1発現に対する阻害作用は用いたすべてのエタノール投与量で同程度であった。
インスリン、IGF-IおよびIGF-II、ならびにインスリン、IGF-IおよびIGF-II受容体の小脳発現に対するエタノールの影響
リアルタイム定量的RT-PCR試験により、対照およびエタノール曝露させた小脳の両方において、インスリン、IGF-IおよびIGF-IIポリペプチド遺伝子、ならびにインスリン、IGF-IおよびIGF-II受容体に対応するmRNA転写物が検出された(図15)。インスリンおよびIGF-IIはIGF-Iよりも大量に発現された。妊娠中のエタノールに対する曝露はインスリン遺伝子発現の有意な低下を生じさせたが、それは用量依存的ではなく、すなわち、低レベルのエタノール曝露もインスリン遺伝子発現を阻害した(図15A)。妊娠中のエタノールに対する曝露はまた、IGF-I(図15C)およびIGF-II(図15E)のレベルのわずかな低下も引き起こしたが、これらの傾向は用量依存的ではなかった。インスリン受容体およびIGF-I受容体のmRNAレベルは同程度であり、いずれもIGF-II受容体発現よりも1.5〜2.0倍の高さであった(図15B、15D、15F)。妊娠中のエタノールに対する慢性的曝露は、インスリン、IGF-IおよびIGF-II受容体発現をいずれも有意には阻害しなかったが、用いた最高濃度ではIGF-II受容体発現が対照と比較して低下した。同一試料中で測定したリボソーム18S(図15G)および28S(図15H)のレベルはすべての群で同程度の高さであった。RT-PCRによって得られた結果に一致して、ウエスタンブロット分析では、対照およびエタノール曝露小脳におけるインスリン受容体およびIGF-I受容体発現のレベルが同程度であることが示された。
エタノールによるインスリン受容体およびIGF受容体の結合の障害
有効なリガンド結合はシグナル伝達カスケードにとって決定的に重要であり、ニューロン生存性の低下を含む、エタノール曝露脳で報告されているインスリンシグナル伝達障害の下流での影響の多くは、CNSにおけるインスリンまたはIGF-Iの結合の低下によって媒介されうると考えられる。小脳膜タンパク質抽出物を、トレーサーとして[125I]で標識したインスリン、IGF-IまたはIGF-IIとともに、過剰な非放射性リガンドの存在下または非存在下でインキュベートすることによって、平衡結合アッセイを行った。平衡結合試験により、IGF-I受容体およびIGF-II受容体に対する特異的結合(fmol/mg)はインスリン受容体に対するよりも高レベルであることが示された。エタノール曝露群はすべて、インスリン、IGF-IおよびIGF-II受容体に対する結合が対照と比較して有意に低下していた。加えて、エタノール曝露は、インスリン受容体およびIGF-I受容体の結合を、IGF-II受容体結合(約50%)よりも大きな度合い(約80%)で障害させた。しかし、受容体結合が障害される度合いはエタノール用量依存的ではなかった。
インスリン受容体およびIGF-I受容体のチロシンキナーゼ活性に対する、妊娠中のエタノールに対する曝露の影響
エタノールに伴う受容体結合の障害がインスリン受容体およびIGF-I受容体のチロシンキナーゼ活性、ならびにインスリン受容体およびIGF-I受容体タンパク質の発現の低下と関連づけられる度合いを特徴づけるための試験を行った。受容体タンパク質レベルはデジタル画像デンシトメトリーを用いるウエスタンブロット法によって測定した。免疫沈降物における受容体チロシンキナーゼ活性を、非放射性発光を利用するアッセイを用いて測定した。これらの試験により、インスリン受容体およびIGF-I受容体チロシンキナーゼ活性のレベルはいずれも有意に低下しているが、エタノール曝露仔からの小脳組織におけるインスリン受容体およびIGF-I受容体タンパク質発現のレベルは対照と比較して同程度であることが示された。結合アッセイの結果に一致して、エタノールに伴う受容体チロシンキナーゼ活性の低下は用量依存的ではなく、種々の子宮内レベルの食餌性エタノールに曝露された仔からの小脳において同程度に低下した。結果は免疫沈降物中のインスリン受容体およびIGF-I受容体のタンパク質レベル(デンシトメトリーを用いるウエスタンブロット分析により検出した)に対して標準化されているため、エタノールに伴うインスリン受容体およびIGF-I受容体のチロシンキナーゼ活性の低下は、成長因子受容体発現の変化に起因するのではなかった。CNSニューロンにおけるエタノールで障害されたインスリンおよびIGF-Iシグナル伝達の主な転帰は、グルコース利用およびATP産生の不足に起因するエネルギー代謝の低下である(de la Monte et al., Cell. Mol. Life Sci. 62:1131(2005))。種々のレベルのエタノールに対する慢性的な子宮内曝露の影響をエネルギー代謝に関連して明らかにするために、小脳組織ホモジネート中のATP含有量を、発光を利用するアッセイを用いて測定した。それらの試験により、小脳ATP含有量のエタノール用量依存的な進行性の低下が示され、18%、26%または37%のエタノールを含む食餌に曝露された仔から得た試料では対照との有意差が検出された。
アセチルコリン恒常性におけるエタノールに伴う障害
アセチルコリンはCNSの認知系および運動系において主要な機能的役割を果たしている。アセチルコリン産生はコリンおよびアセチル-Co-Aの十分な供給を必要とする。アセチル-Co-Aはエネルギー代謝によって産生されるが、それはインスリンおよびIGF-I刺激によって主導される。最近の研究では、コリンアセチルトランスフェラーゼ(ChAT)の発現はインスリンおよびIGF-I刺激によって調節されることが実証されている(Rivera et al., J. Alzheimers Dis. 8:247 (2005))。このため、インスリンおよびIGF-Iシグナル伝達機構のエタノール性阻害がChATの欠乏を伴うか否かを明らかにすることが関心の対象となった。アセチルコリンの定常レベルはアセチルコリンエステラーゼ(AChE)による負の調節を受けるため、AChE mRNAレベルを測定することも関心の対象となった。リアルタイム定量的RT-PCR試験により、エタノール曝露小脳組織におけるChAT発現の平均レベルの有意な低下(図16A)およびAChE発現の増大(図16B)が示された。ChAT mRNAのレベルは、用いた最も低いエタノール濃度で急峻に低下し、ChAT発現のさらなる低下はエタノール曝露の用量を増やしてもわずかに過ぎなかった(図16A)。これに対して、AChE mRNAレベルはエタノール用量とともに漸増的に増大した(図16B)。
インビトロでの短期的エタノール曝露による、インスリン、IGF-IおよびIGF-II受容体の結合、受容体チロシンキナーゼ活性、ならびに対応する成長因子により刺激されるChAT発現の障害
インビボ観察所見を検証し、エタノールがインスリン/IGF抵抗性を引き起こす考えられる機序を特徴づけるために、初代小脳ニューロン培養物を用いるインビトロ実験を行った。以前の研究により、エタノールに曝露させた小脳ニューロン培養物では、インスリンおよびIGF-Iにより刺激される受容体チロシンキナーゼ活性のレベルの低下が示されている(de la Monte et al., Cell. Mol. Life Sci. 62:1131(2005))。エタノールのこの影響がリガンド-受容体結合の障害によって媒介されるか否かを明らかにするために、初代ラット小脳対照またはエタノール曝露させた(50mMに96時間)ニューロン培養物から採取した膜タンパク質を用いて平衡結合アッセイを行った。用いるエタノールの濃度は、ヒトアルコール依存患者で検出されている範囲内とした(Fulop et al., Am. J Med. 80:191 (1986);Jagger et al., Neurosurgery 15:303 (1984))。ChATおよびAChEの免疫反応性はMICEアッセイ(細胞ELISA)を用いて培養細胞において直接測定し、その値を細胞密度に対して標準化した。これらの結果から、インスリン受容体(図17A)、IGF-I受容体(図17B)およびIGF-II受容体(図17C)の結合のレベルの有意な低下が実証された。加えて、エタノール処置したニューロン細胞では、ATPの基礎レベル、およびインスリン、IGF-IまたはIGF-IIによる刺激下でのレベル(図17D)、ならびに基礎的な、インスリンおよびIGF-Iによる刺激下でのChAT免疫反応性(図17E)も、対照と比較して有意に低下していた。AChE免疫反応性は成長因子刺激によって顕著には変化しなかったが、AChE発現はIGF-IIで刺激したものの方が対応する非刺激対照細胞と比べて高く、非刺激エタノール曝露細胞の方が非刺激対照細胞と比較して高かった(図17F)。
インスリン受容体およびIGF受容体の結合ならびにChATの刺激の障害の考えられる機序
膜コレステロール含有量は、細胞表面受容体に対するリガンド結合に影響を及ぼしうる。例えば、膜におけるコレステロール含有量の減少または増加は、成長因子の結合およびシグナル伝達の変化または障害と関連している(Cho et al., Am. J. Physiol. Heart Circ. Physiol. 286:H1881 (2004);Huo et al., J. Biol. Chem. 278:11561 (2003);Meuillet et al., Biochim. Biophys. Acta 1454:38 (1999);Peiro et al., J. Biol. Chem. 275:37846 (2000))。受容体結合に関して観察された違いが膜コレステロール含有量と相関するか否かを明らかにするために、小脳膜抽出物におけるコレステロールレベルを測定した。仔の脳の分析(各群当たりN=8)により、エタノール曝露させた小脳膜では対照と比較してコレステロール含有量の有意な低下が示された(図18A)。小脳膜コレステロール含有量は8%エタノール食餌群では対照と比較して約50%減少したが、より高用量のエタノールでは膜コレステロールのそれ以上の減少はわずかしか観察されなかった(図18A)。同様に、50mMエタノールに対する96時間のインビトロ曝露も、小脳ニューロン膜コレステロール含有量を有意に減少させた(図18B)。
