Abstract
【解決手段】装置は、中空の外側シャフトの中に内側シャフトを備える。装置の内側シャフトの一方の端部は、深さを決定するために硬骨または軟骨の欠損の中に挿入するのに好適であり、外側シャフトの他方の端部は、インプラントを保持するのに好適である。インプラントは、欠損の深さが決定されるとその欠損に適合するように切断され、次いで欠損に挿入される。インプラントを切断するための切断装置が開示される。送出装置の中にインプラントを挿入するためのインプラントカプセル装填機も開示される。

Description
本発明は、軟骨および硬骨の欠損の修復を行うための器具および方法に関する。
米国特許番号5,152,763(Johnsonら)、5,919,196(Bobicら)および6,358,253(Torrieら)に開示されるように、インプラントによる処置の1つのタイプは、患者の体の健康な部分から採取され、欠損内に移植される、健康な硬骨または軟骨のプラグを挿入することを伴う。あるいは、例えば米国特許番号4,186,448(Brekkeら)、5,607,474(Athanasiouら)および5,716,413(Walterら)に開示されるように、インプラントは、多孔質で生体適合性を有する発泡体またはポリマー等の合成材料から成り得る。
本発明は、深さが不明の硬骨および/または軟骨の欠損へのインプラントの測定、サイジングおよび送出を可能にする、硬骨および/または軟骨のインプラント送出ツールを提供する。欠損は、硬骨および軟骨の損傷に限定されない。欠損は、健康な硬骨または軟骨の一部が移植のために除去された後に硬骨または軟骨の組織内に残る穴など、意図的につくり出し得る。意図的につくり出された欠損はまた、靭帯または腱の修復手術の間に自系または同種異系の移植片を挿入するためにつくり出された、硬骨または軟骨組織内の穴をも含む。本装置は、膝などの関節における骨軟骨の欠損の関節鏡検査による修復に有用であり、また、該装置が到達可能ないかなる硬骨または軟骨の欠損の処置にも好適である。さらに、該装置は、硬骨および軟骨の移植片と同様、合成のインプラントとの使用にも好適である。本明細書において、「インプラント」には、合成材料から作成されたインプラントと、硬骨および軟骨の移植片であるインプラントとが含まれる。
欠損に挿入するのに好適な遠位端および近位端を有する内側シャフトと、を備えた硬骨または軟骨のインプラント送出装置であって、該内側シャフトと外側シャフトとがスライド可能に係合するように、該内側シャフトが、該外側シャフトの該内腔内にはまるように適合される、装置。
該外側シャフトから突出したインプラントを受けるための手段と、該突出部分を切断するための切断刃と、を有するインプラント切断装置と、を備えた、インプラント送出システム。
該インプラントが該搭載装置内に配置されている際は、前記内側シャフトの前記近位端が前記外側シャフトの前記近位端から突出し、前記インプラントの長さが、該内側シャフトの突出部分の長さと等しい、該搭載装置の前記遠位端に該インプラントを挿入する工程と、
該内側シャフトの該近位端が該欠損の底部に接触するまで、該内側シャフトの該近位端を該欠損内に挿入する工程と、
該外側シャフトの該近位端が、該欠損を取り囲む組織の表面に接触するまで、該外側シャフトを近位方向に前進させ、それにより該インプラントの一部が該外側シャフトの該遠位端を超えて伸びるようにする工程と、
該外側シャフトの該遠位端を超えて突出した該インプラントの一部を切断し、残りの部分を該外側シャフト内に配置させておく工程と、
該搭載装置の該遠位端を、該欠損を覆うように配置する工程と、
該内側シャフトを遠位方向に前進させて、切断後に残った該インプラントの一部を該欠損内に押し出す工程と、を備えた、方法。

欠損に挿入するのに好適な遠位端および近位端を有する内側シャフトと、を備え、該内側シャフトと外側シャフトとがスライド可能に係合するように、該内側シャフトが、該外側シャフトの該内腔内にはまるように適合される、キット。
図1は、近位端34および遠位端32を有する本発明のインプラント送出装置30の1つの実施形態を示す。好ましい実施形態において、該送出装置30は、関節鏡検査での使用に適した長さ、すなわち、約6〜8インチの長さを有する。インプラント送出装置30は、縦軸に沿って内腔4を有する、中空で管状の外側シャフト1(図3にも示す)を備える。内腔4は、遠位端32から近位端34まで、外側シャフト1の全長にわたって伸びる。図9A〜9Cおよび図11A〜11Cも、内腔4を図示する。インプラントが送出装置30内にあるときにインプラントが見えるように(図1には示されていない)、外側シャフト1の遠位端32は、外側シャフト1を貫く1つ以上のスロット5を有し得る。スロット5は、インプラントが送出装置30内に配置される間に見えることを可能にするものであればいかなる形状でもあり得、透明な材料で覆われ得る。

Claims (2)
- 近位端および遠位端と、縦軸と、外側シャフトの縦軸に沿った内腔とを有する管状の外側シャフトと、
遠位端および近位端を有し、該近位端は欠損に挿入されるのに好適である内側シャフトと、
を備えた硬骨または軟骨のインプラント送出装置であって、
該内側シャフトと外側シャフトとがスライド可能に係合するように、該内側シャフトが、該外側シャフトの該内腔内にはまるように適合され、
該内側シャフトが、該内側シャフトの外側表面の部分の周りに摩擦部材を備え、該摩擦部材はリング状である、装置。 - 明細書に記載の装置。
