Abstract
Description
本発明は縫合糸及び縫合糸技術における改良に関し、詳しくは、外科処置において高強度の縫合糸溶融ループを作製するための材料及び装置に関する。
ポリマー縫合糸は、様々な溶着又は連結処理、例えば、縫合糸のセクションの一部を部分溶融するに十分に加熱して相互溶着させる、溶接のような処置に特に適している。縫合部位では、縫合糸を直接熱すると周囲組織も加熱されてしまうので、縫合糸の被溶着部分又は被溶着セクションに非熱的エネルギーを加えて当該部分の縫合糸材料を部分加熱することが好ましい。詳しくは、縫合糸材料の被溶着セクションに超音波エネルギーを有効付加して摩擦熱を誘起させることにより、これらセクションを相互溶着又は溶接して連結することができる。
米国特許出願番号第5,417,700号及び同3,515,848号には、縫合糸を超音波溶接する装置及び方法が開示される。これらの特許には、例えば、縫合糸材料の被連結セグメントに2つの異なる方向の何れかから機械的エネルギーを付加することが記載される。プラスチック製の縫合糸の場合は、被連結セグメントの軸に実質的に平行な方向から機械的エネルギーを加えて連結し、金属製縫合糸材料の場合は、縫合糸の被連結セグメントの軸に実質的に直交する方向から機械的エネルギーを加えて連結させる。これらの特許には、溶接用の球状先端部を使用して金属製縫合糸材料を連結することも記載される。
縫合糸の超音波溶接法は既知であるが、縫合糸溶接部の強度及び信頼性を、縫合糸結紮又はその他のループ閉止装置に代わる、又はそれらの強度を上回るに十分なものとするように縫合糸溶接プロセスを制御することは従来より困難又は不可能であった。
ある実施例では、細長材料セグメントの、連結部にあって且つ溶融部分よりも尚外側(つまり相互に接触していない)の第1端部位置の第1材料部分及び第2端部位置の第2材料部分の各断面積は、連結部の外側の細長材料セグメントにおける各断面積とほぼ等しい。
細長材料は好適な材料、例えば、ポリマーのような材料からなる単一のフィラメント又はファイバから構成され得、好ましい実施例では外科用縫合糸材料のような熱可塑性ポリマーとされる。
細長材料の各セグメントは連結部位置で溶接により連結されることが好ましい。溶接は色々のエネルギー、例えば、超音波、レーザー、アーク放電、熱エネルギーのようなエネルギーを用いて実施され得る。
細長材料自体は、多数のファイバからなる単一の撚り線を含み得、又は多数の撚り線を含み得る。多数の撚り線を含むものである場合、各撚り線は相互に捻られ、編み組され、あるいはそうでなければシース及びコア形態におけるように相互連結され得る。
細長材料は、その形態を問わず、その連結部に主軸方向での張力が加わると連結部の第1材料部分及び第2材料部分が実質的に張力負荷を受け、連結部の溶融部分が実質的に剪断力負荷を受ける。好ましい実施例では以下の式が満たされる。

凸面状の縫合糸接触面の曲率半径は凹面状の縫合糸接触面のそれよりも大きいことが好ましい。第1縫合糸接触面及び第2縫合糸接触面を共に凸面化した場合は、各表面の曲率半径は異なるか又は実質的に同一であり得る。
本発明の他の様相によれば、上述したような超音波溶接装置は、パターン化した第1縫合糸接触面及び第2縫合糸接触面を持つ第1部材及び第2部材を含み、パターン化された各第1縫合糸接触面及び第2縫合糸接触面は相補的又は非相補的なものであり得、また、第1及び第2の各部材における第1縫合糸接触面及び第2縫合糸接触面の各パターンは定期的又は不定期的に変更され得る。
図1に示すように、本発明に従う溶融ループ10は、熱その他形式のエネルギーを加えると接着又は溶融する、外科用縫合糸のような細長材料の1つ以上のセグメント12を含む。細長材料として好適な材料には、これに限定しないが、ポリマー、特には熱可塑性材料、例えばナイロン(ポリアミド、PA)、ポリプロピレン、ダクロン(Dacron:商標登録)(ポリエステルの好適な形式のもの)、ポリグリコール酸(PGA)、ポリグリコネート、ポリジオキサノン、が含まれる。細長材料は、単一のフィラメント又はファイバを含み得、又は複数のフィラメント(以下、マルチフィラメントとも称する)を含み得る。マルチフィラメント形態のものである場合、各フィラメントは例えば編み組みした縫合糸材料のように編んだものであることが好ましい。
各材料セグメントは溶接を使用して溶融させる準備段階で予め結紮され得、又は単にオーバーラップされ得る。
第1材料部分16及び第2材料部分18の、連結部14における連結セグメントの断面積変化が一様であるその他の断面ジオメトリも本発明の範囲内のものとする。

ここで、Awは溶融部分20の剪断面積(即ち、第1材料部分16及び第2材料部分18の間の溶融層であって且つこの溶融層の断面積ではない面積)であり、τfwは溶融部分を破壊する剪断応力であり、Auは、連結部14で且つ溶融部分の外側(突き合わされていない)の第1端部及び第2端部付近における第1材料部分16及び第2材料部分18の合計断面積であり、σfuは、第1端部及び第2端部付近で且つ溶融部分の外側(突き合わされていない)の第1材料部分及び第2材料部分を破壊する引張応力である。
図10A及び図10Bに示すように、第1部材30の第1縫合糸接触面30Aはz軸及びx軸の周囲で凹状とされ、第2部材32の第2縫合糸接触面32Aはz軸の周囲で凸状とされている。例示した縫合糸の第1材料部分16及び第2材料部分18は円形断面を有しているが、そうした特定ジオメトリに限定される必要はない。第1縫合糸接触面の輪郭により、少なくとも第1縫合糸接触面30Aと第1材料部分16との間における、第1縫合糸接触面30Aの全長さ及び幅に渡る接触は実質的に連続したものとなる。溶融部分20の剪断面積は比較的大きいので、溶融部分の強度は比較的高くなると思われる。
図10A及び図10Bに示すように、第2縫合糸接触面32Aの凸状湾曲部の半径は、第1縫合糸接触面30Aの凹状湾曲部の半径と等しいあるいはそれ未満であることが好ましい。図12A及び図12Bに示すように、その何れもが凸状湾曲部を有する縫合糸接触面を備える溶接ホーン及びアンビルを有する装置では、それら凸状湾曲部の表面の各半径は溶融部分の所望の面積に依存して、相互に異なるかまたは実質的に同じであり得る。
図15A、図16、図17Aに示すように、第1部材30の第1縫合糸接触面30Aは、この第1縫合糸接触面30Aと第1材料部分16との間の接触面積を増大させるチャンネル又は溝を設けたジオメトリを有し得る。図15B、図16、図17Bにも示すように、第2部材32は、溶接プロセス中に縫合糸の第1材料部分16及び第2材料部分18を整列させたままで拘束する複数の部材を含み得る。第2部材の連結部分は溶接プロセス後に分離し、かくして、縫合糸ループを移動あるいはそうでなければ操作することなく、連結部材を溶接装置による拘束状態から釈放する。図15A、図15B、図16Aには、連結部位置で連結された材料の各セグメントの下方で第2部材32を相互に連結させた構成の超音波溶接装置が例示される。連結部材は溶接プロセス中は図15A及び16Aに示すように係合状態に維持され、溶接プロセス後はヒンジ又はピボット動作して分離することにより図15Bに示すように縫合糸ループを釈放する。
図18、図19A及び図19Bには、縫合糸の被溶接部分である第1材料部分16及び第2材料部分18を第2部材32の壁内で相対的に拘束し、整列させ又は方向付ける更に他の構成を有する溶接装置が例示される。本実施例の装置では、水平方向ではなくむしろ垂直方向の溶融部分20を有する溶接連結部を創出させる。この装置では、第1部材30は、第1部材の各側部から垂直方向に伸延する第2部材32の2つのセクションと相補形状を成し且つこれらセクションと嵌合する。第1部材の第1縫合糸接触面30A及び第2部材の第2縫合糸接触面32Aは先に議論したように実質的に平坦であるが、キャンバ角を設けあるいは輪郭を変更しても良い。図19Aに示すように、材料の第1セグメント12において重複する第1材料部分16と第2材料部分18とは、第2部材32の複数の部材の内部で斜行整列状態で相互に連結される。溶接プロセス中、電源から送られる超音波エネルギーが機械的エネルギーに変換され、かくして第1材料部分16と第2材料部分18との間に局部的な摩擦が生じる。摩擦熱により可塑状態となった時点で第1材料部分16及び第2材料部分18を加圧し、これらの部分を流動及び溶融させて垂直方向の溶融部分20を形成する。各材料セグメントを加熱して可塑状態にするに際し、矢印35の方向から各材料セグメントに圧力が行使され、かくして、これらの材料セグメントは垂直方向の溶融部分20の内部で流動及び溶融する。
先に示した各実施例におけるように、第2部材32の複数の部材は、縫合糸ループを釈放するべく溶接プロセス後は分離され得る。
図26Eには、M1の一部をM2の横断方向面上に配置して成る、M1及びM2からなる単一のストランド構成が例示される。この実施例でもストランドは同中心又は非同中心的な構成を有し得る。
図27A〜図27Eには、本発明に従う、複数材料構成のフィラメント又はファイバから成る別の実施例での各断面13が例示される。先に説明したように、細長材料からなるループには、1つ以上の複数材料構成のファイバが含まれ得る。異なる材料であるM1及びM2は夫々溶融温度が異なり得、また、所望に応じてファイバの個別の容積部分中に隔置され得る。そうした個別の容積部分は、所望に応じて、例えば、ある程度平行な軸線に沿った構成のものとすることができる。例えば、低融点の材料M1を、比較的高融点の材料M2の周囲に配置し得る。この場合、溶接プロセスの周囲温度を検出及び制限することで、高融点の材料の溶融が防止されると共に、細長材料の一体性が維持され得る。
以上、本発明を実施例を参照して説明したが、本発明の内で種々の変更をなし得ることを理解されたい。

第1縫合糸接触面を有し且つ振動して超音波周波数での機械的エネルギーを伝達するように動作自在の第1部材と、
第2縫合糸接触面を有する第2部材と、
第1部材を第2部材に関して移動させるための移動手段にして、第1及び第2の前記縫合糸接触面間の間隙を、該第1及び第2の前記縫合糸接触面間の移動の関数として調節自在の移動手段と、
溶接作業中、被溶接材料の2つ以上の材料セグメントを、前記第1及び第2の前記縫合糸接触面間の間隙において所定の整列状態下に受け且つ維持するための固定手段と、
溶接作業における温度に相当する温度信号を発生するように動作自在の温度検出手段と、
を含む超音波溶接装置。 - 温度検出手段が、第1部材及び第2部材に連結したサーミスタを含む請求項47の超音波溶接装置。
- 温度検出手段が、第1部材及び第2部材に連結した熱電対を含む請求項47の超音波溶接装置。
- 温度検出手段が、第1部材及び第2部材に連結したファブリ・ペロー干渉計温度センサを含む請求項47の超音波溶接装置。
