Abstract
Description
CD8+T細胞の主要な生物学的機能は、体内の病原体感染細胞を除去することである。感染細胞のT細胞による認識に関わる機構は、現在では分子レベルで良く確立されており、T細胞受容体複合体(TCR)と主要組織適合遺伝子複合体クラスI分子(MHC I)に結合した抗原由来ペプチドとの相互作用に依存する。該細胞が産生するすべてのタンパク質抗原は、最終的に分解され、生成ペプチドが、細胞表面上のMHC I分子により提示される。
T細胞の発生は胸腺で起こり、その際、各T細胞クローンが最終的に特異なTCRを発現するように、TCRのα及びβ遺伝子セグメントが再編成される[13]。表面TCRを産生する発生中の胸腺細胞は、CD4及びCD8コレセプターを発現し、自己ペプチド−MHCリガンドに対する各TCRの特異性及び親和性に依存して、複雑な成熟過程を受ける。自己ペプチド−MHC分子に親和性のないTCRを発現する胸腺細胞は、プログラム細胞死の機構により死滅する。胸腺中の細胞上に発現される自己ペプチド−MHCリガンドに対し、親和性の強いTCRを発現する潜在的に有害な胸腺細胞は、物理的削除[14]、機能的不活性化[15]、又は受容体編集[16]を介して除去される。胸腺間質細胞上の自己ペプチド−MHCリガンドに対し、低いが有意な親和性を示すTCRを発現する胸腺細胞だけが、胸腺選択に生き残る[17]。
T細胞のTCRが高い親和性を示す外来ペプチド−MHCリガンドに、未だ接触したことのない該T細胞は、「ナイーブ」T細胞と呼称される。こうした細胞は、健康な若年成人における二次リンパ器官中のT細胞の大半を占める。ナイーブT細胞は、脾臓、リンパ節、及び粘膜リンパ器官(腸のパイアー斑など)を含めた二次リンパ器官中を継続的に再循環する[18、19]。個々のナイーブT細胞は、二次リンパ器官中を平均して数カ月循環すると推定される[20、21]。この通常寿命中でのナイーブCD8+T細胞の生存は、自己ペプチド−MHC複合体の低親和性TCR認識[22]、及びIL−7受容体を介したシグナル伝達[23、24]によって維持されている。TCR及びIL−7受容体を介したシグナルは、ナイーブT細胞の生存に必要であるが、こうしたシグナルによって、該T細胞が正常数のT細胞を含有する宿主中で増殖することはない。対照的に、T細胞欠損宿主中に移入されると、ナイーブT細胞は増殖する。この「恒常性維持的」増殖も、IL−7[23、24]、及び自己ペプチド−MHC複合体の低親和性TCR認識[25]に依存するが、IL−2にも、CD28共刺激性(co−stimulatory)受容体にも依存しない[26]。若年者では、新たなナイーブT細胞が、胸腺により恒常的に産生され、二次リンパ器官に移送されることにより、老化したナイーブT細胞と置き換わる。対照的に、胸腺産生力が低下又は欠失した年配者では、老化細胞を残存するナイーブT細胞の増殖によって置き換え得る。
二次リンパ器官のT細胞領域中を移動するナイーブCD8+T細胞は、体内の任意の細胞中、最高量のMHC分子を構成的に発現する大型で形状不規則な樹状細胞(DC)の濃密な網目に接触する[27]。感染や組織損傷がない場合、二次リンパ器官中の全DC集団は、CD80、CD86などの共刺激性分子の低発現を特徴とする休止状態で存在する[28]。この状態では、DCは、ナイーブT細胞の生存を維持する、低親和性の自己ペプチド−MHCリガンドの提示において重要な役割を演じることはほぼ確かである。
免疫応答が最高潮の抗原特異的CD8+T細胞は、エフェクター機能を発現し、したがって「エフェクター細胞」と呼称されることもある[52]。エフェクター細胞は、特徴的な1組の接着受容体を発現する。ナイーブ細胞と異なり、そうした細胞は、その決定的な特徴、即ち、適切なペプチド−MHCクラスI複合体を表示する標的細胞を直接死滅させる能力に寄与する、パーフォリン及びグランザイムを発現する[53]。エフェクターT細胞は、T細胞領域から離れ、多くの非リンパ組織、特に炎症を起こした抗原沈着部位の中へ移動する。細胞溶解能を有するエフェクターCD8+T細胞の非リンパ器官中への移動は、あらゆる身体部分からのペプチド−MHCクラスI複合体を表示する細胞を除去する有効な方法である。
最高潮の増殖後に非常に大多数のエフェクター細胞が死滅するが、それでも、抗原が炎症との関わりで初回に提示された場合、抗原経験T細胞の安定集団が長期間生き残る[52]。多くの点から、記憶細胞は、基礎活性化状態に戻ったエフェクター細胞と見なすことができる。実際のところ、数連の証拠から、エフェクター細胞が記憶細胞の前駆体であることが示唆される[60、61]。
前立腺特異抗原(PSA)は、前立腺の線房及び導管に沿った円柱上皮細胞により、専ら産生されるカリクレイン様セリンプロテアーゼである[66〜68]。PSAは、前立腺管の内腔中に分泌され、約0.5〜5mg/mlの範囲のやや高濃度で精漿中に存在する[69]。生理学的には、PSAは、主要なゲル形成タンパク質のセメノゲリンI及びII、並びにフィブロネクチンを切断し、精子の運動性を増加させるように、精漿中で機能する[67、70、71]。PSAは、アミノ酸261個の不活性プレプロPSA前駆体として翻訳される。プレプロPSAは、プレ領域(シグナルペプチド)及びプロペプチドを構成する追加の24残基を有する。プロペプチドを放出すると、酵素活性のある、アミノ酸237個の成熟細胞外形態が生成する。PSA同様に前立腺組織中で選択的に発現されるヒト腺性カリクレイン2(hKLK2)は、プロPSAの活性化を担っている[72]。PSAは、69位アスパラギンに結合したN結合型オリゴ糖を含有することが示された[73]。
近年になって幾種もの腫瘍ワクチン戦略が開発されてきた。その大部分は、免疫系が認識できる腫瘍抗原の特定に依存している。DNAワクチンの接種は、腫瘍抗原に対する体液性、細胞性双方の免疫応答を誘発するそのような一手法を代表する。動物モデルを用いた試験によれば、防御的な抗腫瘍免疫応答を誘発するために、DNAワクチンの接種を使用する妥当性が実証された。しかし、人間の癌細胞が発現する大部分の腫瘍抗原は、免疫原性が弱いため、臨床状況でDNAワクチンの効力を増強するための戦略の開発が必要となる。DNAワクチンの免疫学の理解、及びCTL活性に関するDNAワクチンの効力の増加に使用される戦略の最近の進歩を以下に考察する。
DNAワクチンは、単純なプラスミドDNA発現ベクターを普通意味する。それには、真核性プロモーターとポリアデニル化配列との間に挿入された所望の抗原、細菌性の抗生物質耐性遺伝子及び細菌性の複製起点をコードするcDNAが含有される。真核性プロモーター及びポリアデニル化配列は、哺乳類細胞中で適切な抗原発現を行うために必要であり、抗生物質耐性遺伝子及び複製起点は、細菌中でのベクターの産生を可能とする。
防御的抗腫瘍応答を発現する際のDNAワクチンの有用性は、マウスにおけるモデル腫瘍抗原を用いて初めて実証された。SV40ラージT抗原[146]、β−ガラクトシダーゼ[147]、ヒト癌胎児性抗原(CEA)[148]、ヒトパピローマウィルスE7[149]、又はヒトPSA[150]をコードするプラスミドによるDNA免疫は、対応する抗原を発現する同系腫瘍細胞の致死性接種から、マウスを防御することが示された。枯渇試験によって、腫瘍拒絶におけるCD8+細胞傷害性Tリンパ球の役割が証明された[147、149]。まとめると、これらの試験によって、腫瘍細胞を標的とした抗原特異的免疫応答を誘発するために、DNAワクチンを使用することの妥当性が実証されている。しかし、これらの試験に使用した抗原はすべて、事実、自己抗原を示す「正規の」腫瘍抗原より、通常はるかに免疫原性が高い外来タンパク質であった。
マウスにおける試験によって、抗原特異的CTLがDNAワクチンで誘導される頻度は、ウィルス誘発応答と比較して約10分の1であり、DNAで単回免疫した後のエフェクターCTLの一次応答は、わずかに遅れて免疫してから12〜15日で最高となる[164、165]。CTL一次応答のこうした定性的差異は、プラスミドDNA投与後に産生する微量の抗原、及びインビボでのAPCの非効率的ターゲッティングに部分的に起因すると思われ、このことは相俟って、ナイーブT細胞の頑強な初回抗原刺激及び増殖を保証するには不十分である。
ここでは、HPV関連肛門異形成[213]、転移性結腸直腸癌[212]、B細胞リンパ腫[214]、転移性メラノーマ[215、216]、及び前立腺癌[211、217]の患者において、腫瘍関連抗原を標的としたDNAワクチンに関して最近行ったいくつかの第I相臨床試験を考察する。これらの試験では、DNAワクチン接種の分野内での現在の進歩を良く代表している、異なったDNA送達技法及びアジュバントを多種採用した。
自己抗原より外来抗原に対して免疫応答を誘発する方が、当然ながら容易である。その理由から、抗原をできる限り外来性にし、それでもなお自己タンパク質に対して免疫を誘発することが、有益である。これを実施する一戦略は、ナイーブな自己タンパク質に外来タンパク質を結合することである。このための汎用タンパク質は、キーホールリンペットシアノゲンである。KLHに対する免疫応答の誘発は、結合しているタンパク質に対して免疫応答を誘発することがしばしばある。
ここでは、組換え非ヒト霊長類PSAを含有するワクチン製剤を作製する、各ステップの説明がなされている。
一方法では、CMVプロモーターの制御下で非ヒト霊長類PSAを発現する、プラスミドを含有するDNAワクチンが調製される。該プラスミドは、細菌性複製起点及びカナマイシン抵抗性遺伝子などの抗生物質抵抗性遺伝子のような、プラスミドの産生に必須の他のDNA配列を含有する。該プラスミドは、半強度リン酸緩衝塩水などの適切な担体中、0.5〜5μg/μlの濃度で混合される。投与には、半インチの27ゲージ針を付けたツベルクリン型シリンジ中に該プラスミド混合物を装入する。該プラスミド混合物20〜50μlを三角筋上の皮膚中又は前腕上に皮内注入する。
電界の印加は、電気穿孔法を行うことによって使用することができる。適切な電気穿孔法は、そのすべてが参照により本明細書に組み込まれている、米国特許第6010613号、第6603998号及び第6713291号に開示されている。
Claims (25)
- 非ヒト霊長類PSAと、人間に投与するための医薬として許容可能な担体とを含む物質組成物。
- 非ヒト霊長類PSAと、人間に投与するための医薬として許容可能な担体とを含む、人間のためのワクチンである、請求項1に記載の物質組成物。
- 前記物質組成物がワクチンであり、非ヒト霊長類PSAが、ヒトPSAに対して抗体を産生するヒト免疫応答を誘発する、請求項1に記載の物質組成物。
- 前記物質組成物がワクチンであり、ヒト免疫応答が、ヒトPSAに対する抗体の産生を生じる、請求項1に記載の物質組成物。
- 前記物質組成物がワクチンであり、ヒト免疫応答が、ヒトPSAを含有するヒト細胞に対して、細胞傷害性細胞性免疫を生じる、請求項1に記載の物質組成物。
- ヒトPSAに対する免疫応答を与えることを目的として、人間に投与するワクチンを調製するための、非ヒト霊長類PSAの使用。
- 人間において抗原に対する免疫応答を与えることを目的として、人間に投与するワクチンを調製するための、抗原に対する非ヒト霊長類DNA配列の使用。
- ヒトPSAに対する免疫応答を与えることを目的として、人間に投与するワクチンを調製するための、PSAに対する非ヒト霊長類DNA配列の使用。
- 人に非ヒト霊長類PSAを供与するために、前記人に非ヒト霊長類DNA配列を導入するステップを含む、人間を治療する方法。
- ヒト免疫応答を誘発するために、人に非ヒト霊長類PSAを導入するステップを含む、人間において前立腺癌を治療する方法。
- 人に非ヒト霊長類PSAを導入するステップを含み、ヒト免疫応答が、ヒトPSAに対する抗体の産生を生じる、人間において前立腺癌を治療する請求項10に記載の方法。
- ヒトPSAを含有する細胞に対する細胞傷害性細胞性免疫を含む、免疫応答を誘発するために、人に非ヒト霊長類PSAを導入するステップを含む、人間において前立腺癌を治療する請求項10に記載の方法。
- 人に非ヒト霊長類PSAを導入するステップを含み、ヒト免疫応答が、ヒトPSAを含有する細胞に対して、細胞傷害性細胞性免疫を生じる、人間において前立腺癌を治療する請求項10に記載の方法。
- 非ヒト霊長類抗原を発現する核酸ワクチンをヒト細胞中に送達する方法であって、

- 人間においてヒトPSAに対する免疫応答を誘発する方法であって、非ヒトPSAの遺伝子由来の遺伝子配列を人に導入するステップを含み、前記非ヒトPSA遺伝子配列は、人において非ヒトPSAとして発現され、導入される前記非ヒト遺伝子配列は、ヒトPSAの遺伝子由来の遺伝子配列と塩基対相同性を備え、前記相同性は、88%以上で98%以下の範囲にある方法。
- 相同性が、92%以上で99%以下の範囲にある、請求項18に記載の方法。
- ヒト抗原に対する免疫応答を与えることを目的として、人間に投与するワクチンを調製するために、非ヒト霊長類抗原を発現するベクターの使用。
- 前記ベクターがDNAベクターである、請求項20に記載のベクターの使用。
