Claims (43)
- 細長いカテーテル(102)と、該カテーテル内に閉じ込められた複数のニードル(402)を備え、前記複数のニードルは展開されると、処置すべき腫瘍を取り囲む構造体を共に画定し、前記ニードルが、前記腫瘍を取り囲む組織の外郭を加熱して閉塞させることより、前記腫瘍への血液供給を断つように作用する、腫瘍の処置のための装置。
- 前記ニードルが可撓性をもつ請求項1に記載の装置。
- 前記ニードルは、延びたときに、湾曲した形状又は角度の付いた形状をとるように配置される、請求項1又は2に記載の装置。
- 前記ニードルは、組織の外郭を加熱するために高周波パワーを供給する電極として機能するように配置される、請求項1乃至3のいずれか1項に記載の装置。
- 前記構造体の隣接するニードルが逆の極性をもって作用する電極として配置される、請求項4に記載の装置。
- 前記カテーテル(102)の長手方向の軸に沿って延び、ケージ内の前記組織の特性を測定するための測定装置を有する、細長いプローブ(200)を含む、請求項1乃至5のいずれか1項に記載の装置。
- 前記カテーテル(102)の長手方向の軸に沿って延びる細長い前記プローブ(200)を有し、前記構造体のニードルが、前記プローブ(200)の極性とは異なる極性を有する電極として配置される、請求項6に記載の装置。
- 前記測定装置が血圧センサ、インピーダンスセンサ又は温度センサを備える、請求項6に記載の装置。
- ケージ内の前記組織中の血流を測定するためのドップラ式血流計を有する、請求項1乃至8のいずれか1項に記載の装置。
- ケージ内の前記組織の少なくとも一部を吸引するためのルーメンを有する、請求項1乃至9のいずれか1項に記載の装置。
- 前記腫瘍の領域における組織を切断するためのカッターを有する、請求項1乃至10のいずれか1項に記載の装置。
- 吸引すべき前記組織を切断するためのカッター(154、160)を有する、請求項10に記載の装置。
- 第1の構造体を規定する第1の複数のニードルと、前記第1の構造体内にこれよりも小さい第2の構造体を規定する第2の複数のニードルとを有する、請求項1乃至12のいずれか1項に記載の装置。
- 前記ニードルは内側の構造体と外側の構造体との間にある組織部分の加熱及び塞栓のために配置される、請求項13に記載の装置。
- 前記構造体内にマイクロ波エネルギーを与えることによって、前記腫瘍及び外郭を閉塞させるように構成された、請求項1に記載の装置。
- 前記内側の構造体と前記外側の構造体との間にマイクロ波エネルギー又は高周波エネルギーを与えることによって前記組織部分が閉塞される、請求項14に記載の装置。
- 前記ニードルは、中央の電極ニードルと、この中央ニードルを中心として離間された一連の外側ニードルとを有するアレイを形成するように配置されており、前記外側ニードルは、前記中央ニードルを中心として一定間隔で離間され、端面が平坦な円筒状のアレイに構成された操作端部を有する、請求項1に記載の装置。
- 前記中央ニードルが前記外側ニードルに対して移動可能であって、前記外側ニードルに対して前記装置の長手方向に沿って軸方向にスライド可能である、請求項17に記載の装置。
- 前記中央ニードルは、吸引及び/又はフラッシングのための1つ以上のルーメンを有する、請求項17又は18に記載の装置。
- 前記中央ニードルは、電磁エネルギーを前記組織に与えるための軸方向に離間した複数の導電部を有し、前記導電部が電磁パワー源によって個々に通電可能である、請求項17乃至19のいずれか1項に記載の装置。
- 前記複数の導電部は、軸方向に延びる絶縁部によって離間されており、前記軸方向に延びる絶縁部は、吸引及び/又はフラッシングのための1つ以上の開口部を有する、請求項20に記載の装置。
- 前記外側ニードルは、その端部近傍に直線状の導電部を有し、前記中央ニードルが直線状とされ、前記直線状の導電部が前記中央ニードルに対して平行である、請求項17乃至21のいずれか1項に記載の装置。
- 前記ニードルのカバーを有し、該カバーは組織を貫通する先端部を有しており、前記カバーは、前記ニードルを露出させるために、前記装置の主部に対してスライド可能であり、
前記先端部は拡張可能であって、一連の拡張用溝を有することにより、包まれて覆われた構成から、拡がった動作可能な構成へと、前記ニードルを配置及び展開可能である、請求項17乃至22のいずれか1項に記載の装置。 - 前記ニードル間で電磁パワーを切り替えるための切替システムを有する、請求項17乃至23のいずれか1項に記載の装置。
- 前記外側ニードルは、電磁パワーによる通電のために略円筒状をしたパターンに配置され、前記外側ニードルの間で前記組織の焼灼を行い、中空円筒状の焼灼済の組織部分を形成する、請求項17乃至24のいずれか1項に記載の装置。
- 前記外側ニードルのうちの少なくとも1本が、前記中空円筒状の組織部分に対する閉塞端の焼灼を行うために、前記中央ニードルと共に通電されるように配置され、その先端又は近傍における前記中央ニードルの第1の導電部が、前記閉塞端の焼灼を行う通電のために配置されており、前記閉塞端が略円盤状の形状を有する、請求項25に記載の装置。
- 前記外側ニードルのうちの少なくとも1本は、焼灼がなされる組織の囲いの内部に腫瘍又は他の組織を入れるために、前記中央ニードルと共に通電されて、中空の略円筒状部分に対して第1端部から離間した第2の閉塞端の焼灼を行うように構成され、前記中央ニードルの第2の導電部は、前記第2の閉塞端の焼灼を行う通電のために、前記第1の導電部から離間して設けられる、請求項26に記載の装置。
- (a)カテーテル(102)を用いて複数のニードル(402)を配置し、該ニードルが、処置すべき腫瘍を取り囲む構造体を共に規定するようにし、(b)前記腫瘍を取り囲む前記構造体によって規定される組織の外郭の加熱及び塞栓を行うことで、前記腫瘍への血液供給を断つことを含む処置方法。
- 前記ニードルが可撓性をもち、湾曲した形状又は角度の付いた形状を取るように前記ニードルを配置する、請求項28に記載の方法。
- 組織の外郭を加熱するために、前記ニードルに対して高周波パワー又はマイクロ波パワーを与えるステップを有する、請求項28に記載の処置方法。
- 前記構造体の隣接するニードルが、互いに逆の極性をもつ電極を規定する、請求項30に記載の方法。
- 前記カテーテル(102)の長手方向の軸に沿って延びる細長いプローブ(200)を前記腫瘍に挿入して、前記構造体内の組織の特性を前記プローブで測定することを含む、請求項28乃至31のいずれか1項に記載の方法。
- 前記カテーテル(102)の長手方向の軸に沿って延びる細長いプローブ(200)を前記腫瘍に挿入して、前記プローブ(200)の極性とは異なる極性をもつ、前記構造体のニードルにパワーを与えることを含む、請求項32に記載の方法。
- 血圧、インピーダンス又は温度を測定するステップを有する、請求項32に記載の方法。
- 前記構造体内の組織中の血流を測定することを含む、請求項28乃至34のいずれか1項に記載の方法。

- 前記構造体内にマイクロ波エネルギーを与えることによって前記腫瘍及び前記外郭の塞栓を行う、請求項28に記載の方法。
- マイクロ波エネルギーを、前記内側の構造体と前記外側の構造体との間の前記組織部分に与える、請求項39に記載の方法。
- 略直線状の作用部を有するニードルによる略円筒状のアレイを提供し、焼灼がなされる円筒状の組織を腫瘍の周りに形成するために前記アレイにおける前記ニードル間に電磁パワーを与えることを含む、請求項28に記載の方法。
- 前記アレイ内部に中央ニードルを提供し、該中央ニードルと、円筒状アレイにおける前記ニードルのうちの少なくとも1本との間に電磁パワーを与えて、前記円筒状の組織に対して1つ以上の、焼灼がなされる閉塞端、すなわち腫瘍が処置される器官の外面から離れている場合における、前記円筒状の組織の両端部を形成する、請求項42に記載の方法。
