Description
a)該被験者の体液及び/又は細胞(特に白血球)のサンプルを、任意に単離して、Rv0079、Rv0569、Rv1733c、Rv1738、Rv1813c、Rv1996、Rv2007c(FdxA)、Rv2029c(PfkB)、Rv2030c、Rv2031c(HspX、Acr、16-kDaアルファ結晶性相同体)、Rv2032、Rv2626c、Rv2627c、Rv2628、Rv3126c、Rv3129、Rv3130c、Rv3132c、Rv3133c(DosR)、Rv0080、Rv1737c(NarK2)、Rv1735c及びRv1736c(NarX)から成るミコバクテリア NRP /休止状態(DosR)レギュロン配列の群から選択される一以上のポリペプチド又はその断片と接触させること;及び、
b) ミコバクテリア感染を示す増殖又はサイトカイン産生によって測定して前記ポリペプチドに対する免疫応答を検出すること。
アミノ酸配列同一性は、二つの(ポリ)ペプチド配列が、例えばデフォルトパラメーターを用いるプログラムGAP又はBESTFITによって至適に整列された場合、他で定義したような少なくともある割合の配列同一性を共有することを意味する。GAPは、マッチの数を最大にし、ギャップの数を最小にするように2つの配列をそれらの全長にかけて整列させるために、Needleman and Wunsch グローバル・アライメント・アルゴリズムを使用する。一般に、GAPデフォルトパラメーターは、ギャップ創造ペナルティ=8及びギャップ拡大ペナルティ=2を用いる。タンパク質については、該デフォルト・スコアリング・マトリクスは、ブロサム(Blosum)62 (Henikoff & Henikoff, 1992, PNAS 89, 915-919)である。配列のアライメント及び配列同一性割合のスコアは、GCGウィスコンシン・パッケージ、バージョン10.3(Accelrys Inc.(9685 Scranton Road, San Diego, CA 92121-3752, USA)から入手可能)のようなコンピュータープログラムを用いて決定できる。或いは、類似度又は同一性の割合は、FASTA、BLASTなどのようなデータベースをサーチすることによって決定することもできる。
試験被験者
試験は、20人のTB患者、23人の健康なツベルクリン皮膚試験(TST)陽性個体及び21人の健康な非感染コントロールを含んだ。我々の第一の目的が、新規の潜在性抗原に対するT細胞応答をスクリーニングすることであったために、認識が最も起こりそうな結核菌感染期を仮定しなかった。それ故、TB患者及びTST転換者という異質な組を試験被験者として選択した。
この試験のために、結核菌H37Rvを堅いスクリューキャップを持つチューブ中で24時間増殖し、収集し、先に記載されたように(16)溶解した。さらに低酸素ライセートと称されるこのライセートをアセトンで沈殿させ、PBSで透析した。この低酸素培養物の培養濾液を、セントリプレップ濃縮機で濃縮した。結果として生じた標本のタンパク質濃度をBCA試験(Pierce, Rockford, Ilinois)によって測定した。該低酸素のライセート及び低酸素の培養物濾液は、スターテン(Statens)血清研究所(Copenhagen, Denmark)によって提供された。標準通気研究室条件下で培養された結核菌のライセートは、国立公衆衛生及び環境研究所(Bilthoven, the Netherlands)によって提供された。
HspXに反応することが分かっている2人のTB患者及び2人のTST陽性個体から得たPBMCを用いて、低酸素条件下で増殖された結核菌のライセート (n=4)又は培養濾液(n=4)の何れかに対して、8つの長期T細胞株を作成した。比較のために、3人のTB患者及び1人のTST陽性個体からのPBMCを結核菌のライセートで刺激することによって作成した4つのさらなる結核菌-特異的T細胞株を、標準的な通気の研究室条件下で培養した。T細胞株は、先の開示(24)の通りに作成した。要するに、PBMCを上記で特定された5 μg/ml抗原の存在下で、24ウェルプレート(Nunc, Roskilde, Denmark)中で、1×106 細胞/ウェルでインキュベートした。6日後、25 U/mlのインターロイキン-2 (Cetus, Amsterdam, The Netherlands)を加え、培養をさらに2〜3週間継続した。T細胞を凍結し、使用まで液体窒素中で保管した。
T細胞(5×104/ウェル)を、抗原提示細胞(1.5×104/ウェル)として、自家性の又はHLA-DRが一致した照射されたPBMCと共に、三通り、96ウェルの平底マイクロタイタープレート(NUNC)中、抗原の存在下又は非存在下で培養した。10%のプールされたヒト血清、40 U/mlのペニシリン及び40 μg/mlのストレプトマイシンを補充したイスコーブ(Iscoves)改変DMEM(Gibco, Paisley, Scotland)を標準培養培地として用いた。表Iに示した全部で25の組換え潜在性抗原を、最終濃度0.33 μM、標準結核菌ライセート、1 μg/mlの濃度の該低酸素のライセート及び培養物濾液で試験した。マイトジェンPHA (2 μg/ml)をポジティブコントロールとして用いた。37℃及び5% CO2での3日間の培養の後、IFN-γの検出のために上清(三通り毎にプールされた50 μl/ウェル)を集め、及びT細胞の増殖を[3H]チミジン取込みによって先に開示されたように(22)測定した。増殖は刺激(stimulation)指数として表現され、刺激ウェルにおける毎分のカウントとして算出され、非刺激ウェルにおける毎分のカウントで割り算される。刺激指数≧4が陽性応答として事前に定義された。
PBMC(1.5×105/ウェル)を三通り、96-ウェルの丸底マイクロタイタープレート中で、37℃、5% CO2で、潜在性抗原の存在下又は非存在下で、標準培養培地で培養した。全ての実験を通して同じ抗原及び同じバッチを使用した。Rv1733cバッチの不足のために、この抗原のための試験被験者の数を17人の健康なコントロール、18人のTST+個体及び16人のTB患者に限定した。6日目、IFN-γの検出のために上清を収集し(75 μl/ウェル、三通りについてプールした)、T細胞の増殖を他で(22)開示されたように測定した。
該上清中のIFN-γ濃度をELISA (U-CyTech, Utrecht, The Netherlands)により測定した。該アッセイの検出限界は20 pg IFN-γ/mlであった。ELISAサンプルは二つ組で試験した。非刺激培養物の平均値を、刺激培養物の平均値から減算した。陽性応答を、刺激されたT細胞株の上清においてIFN-γレベル≧50 pg/mlとして、及び、PBMC培養物からにおいて≧100 pg/mlとして、予め定義した。
培養物上清におけるIFNγ TNFα、IL-10、IL-5、IL-4及びIL-2のレベルを、ひとつのサンプルにおいて多数のサイトカインの同時検出を可能にする、ヒトTh1/Th2 サイトカイン (BD Biosciences)のためのCytometric Bead Array (CBA)キットを用いて測定した。アッセイは、製造者の説明書に従って行った。
PBMCを解凍し、PBS/0.5 % BSA (37℃)中に10×106 cell/mlで再懸濁した。CFSEを最終濃度5 μMで加え、37℃で10分間、暗中でインキュベートした。インキュベーション後、FCS (10%)を加え、細胞をPBS/0.5% BSA中で2回洗浄した。標識されたPBMC(1×106 cell/well)を24ウェルプレート上で、標準培養培地で、PPD (5 μg/ml)、Rv1733c組換えタンパク質(20 μg/ml)、Rv1733cペプチドプール(ペプチド当たり10 μg/ml)、PHA (2 μg/ml)の何れかの存在下で、又は培地のみで培養した。6日後、細胞をPBS/0.1% BSAで洗浄し、CD4で染色し、続いて、CD4陽性細胞の増殖を、フローサイトメーターを用いたCFSE希釈の測定によって評価した。
各研究グループにおける応答者の割合を比較するため、カイ二乗試験を用いた。半(Median)IFN-γ応答を、全3つの研究グループを比較するためにクルスカル-ウォリス(Kruskal-Wallis)試験を用いて、非パラメトリカリー(non-parametrically)に評価し、二つのグループを比較する場合に、マン-ホイットニーU検定法をhoc 試験後に用いた。本研究の第一の目的が潜在的な免疫原性の潜在性抗原の初期スクリーニングであるために、ボンフェローニコレクションを適用しなかった。TST陽性個体がTB患者よりも一般によく25潜在性抗原を認識するという過程を試験するために非パラメトリックなフリードマン試験(ランクによる分散)を適用した。<0.05のA P値は、投擲的に有意であると見なした。統計分析のために、ウィンドウズ(登録商標)用のSPSS 10.0を用いた。
免疫原性潜在性抗原の選択
抗原を、NRP、酸素制限の間及び低用量酸化窒素曝露の間に誘導されることが発見された(17)、48遺伝子(表2)から成る、近年同定された結核菌の休止状態レギュロンから選択した。それらの遺伝子のほとんどが、機能が未知の仮定上のオープンリーディングフレームであるため、このポストゲノム的抗原探索のための遺伝子の選択は、タンパク質の機能に基づくことはできない。それ故、我々は、遺伝子をそれらの誘導レベルに基づいて選択することを選んだ。この目的のために、潜伏時の3つの異なるインビトロモデルにおいて、「Voskuil et al.」によって観察されたような折り畳みに基づいて、平均折り畳み(mean fold)誘導をそれぞれ個々の遺伝子について算出した(17)。48の候補遺伝子のデータから、25の最も強力に誘導する遺伝子が、組換えタンパク質のクローニングと発現のために選択された(表1;図7i及び図8i)。それらの仮定上のタンパク質は続いて、サイズがかなり異なっている(9〜74 kDa)潜在性抗原を直接比較することを可能にするために、等しいモル濃度で試験した。25の抗原は全て、結核菌超音波処理で作成した短期T細胞株により認識される。
結核菌潜在性抗原に対する応答におけるPBMCによるインターフェロン-γ産生
続いて、該25の潜在性抗原を、20人のTB患者、23人のTST陽性の健康な個体及び21人の未感染のコントロール被験者のPBMCによるIFNγ産生の誘導について試験した。それぞれの個々の潜在性抗原について、グループ当たりの応答(IFNγ≧100 pg/ml)試験被験者の割合を算出した(表1)。我々は、20人のTB患者及び23人のTST陽性個体を合わせて、全43人の結核菌感染個体の応答者の割合も算出した。後者の分析は、19の潜在性抗原が、感染個体の過半数(56%)によって認識されるRv1733cと共に、結核菌感染個体の少なくとも5%によって認識されることを示した。残りの試験された6つの抗原、Rv0572c、Rv2623、Rv2624c、Rv3127、Rv3131、Rv3134cは、結核菌感染個体によって認識されないか、又は極めてわずかにしか認識されなかった。増殖データを分析したとき、同様の認識プロフィールが見出された(示さず)。
潜在性抗原に異なる応答をするTB患者及びTST陽性個体
TB患者及びTST転換者のグループにおける半(median)IFN-γ応答を、各潜在性抗原について測定した。25の潜在性抗原をひとつのグループと見なすと、半IFN-γ応答は、TB患者よりも、結核菌に潜在的に感染したとみなされるTST陽性個体において一貫して有意に高かった(P<0.01;フリードマン試験)。抗原認識におけるこの相違をさらに分析するため、我々は、IFN-γ応答と各潜在性抗原の間の比と、同じ個体における低酸素結核菌ライセートにたいする総応答を算出した。この分析は、T細胞の普遍的な応答性において起こり得る個体間変動を矯正する。上記のフリードマン分析が繰り返し、それらの比の中央値(medians)を比較すると、潜在性抗原がTST陽性個体によって優先的に認識されるということが確証された(P<0.01)。
頻繁に認識される潜在性抗原に対するインターフェロン-γ応答
潜在性抗原、即ちRv1733c、Rv2029c、Rv2627c及びRv2628は、潜在的に感染した個体に優先的に広く認識され、IFN-γ産生によって測定される最も強いTh1応答を誘導する。夫々の試験グループについて、それらの4抗原に対するIFN-γ応答を図3に示す。TST陽性個体のグループにおける、Rv1733c、Rv2029c、Rv2627c及びRv2628に応答しての半IFN-γ産生は、それぞれ213、281、107及び51 pg/mlであった。反対に、それらの4つの抗原に対するより低いIFN-γ応答がTB患者において見られ、夫々の中央値は98、16、<10及び<10 pg/mlであった。このIFN-γ応答における相違は、抗原Rv2029cについて統計的に有意であった(P=0.03)(図3)。
健康なコントロールによる潜在性抗原の認識
幾分予想外なことに、25の潜在性抗原の16が、健康な個体の少数のT細胞によって認識されるが(表1)、しかし、その免疫応答の強度は結核菌感染個体におけるものより一般的に低い。全ての健康な非BCGワクチン接種のコントロールがTST陰性であり、また、結核菌特異的免疫優性抗原ESAT6及びCFP10に対するインビトロ応答も、このグループでは陰性であったことから(データは示さず)、潜在性抗原に対する観察された応答は、結核菌複合体種による感染によるものではないと結論付けられた。しかしながら、21人の健康なコントロールのうちの10人(48%)が、低酸素結核菌ライセートに著しく応答し、その応答者の間の半IFN-γ応答は563 pg/mlであった(図3及び4)。この発見は、環境のミコバクテリアへの以前の曝露の結果としてのミコバクテリアの抗原に対するT細胞のクロス反応性と矛盾しない。潜在性抗原に対する応答が、主として低酸素結核菌ライセートに強く応答する健康なコントロールのグループにおいて観察されたために(図4)、それらの応答は、クロス反応性環境ミコバクテリアへの以前の曝露を反映している可能性が高い。Rv1733cのみが、低酸素結核菌ライセートに強く応答しない健康なコントロールによって時折認識され、その半IFN-γは41 pg/mlであり、ミコバクテリア抗原以外の抗原に対するクロス反応性の可能性を示唆した。
Rv1733cのペプチドに対する応答における、CFSE標識PBMCの増殖
潜在性抗原によるタンパク質特異的活性化を確認するために、最も頻繁に認識される抗原、Rv1733cのペプチド特異的増殖を測定した。Rv1733cに応答することが知られているTST陽性個体及び健康なコントロールからのPBMCをCFSE標識し、Rv1733cの組換えタンパク質又はペプチドプールによって刺激した。6日後、細胞をCD4について染色し、CD4 T細胞の増殖を、フローサイトメーターを用いて評価した。PPD及びRv1733c組換えタンパク質による刺激はいずれも、CD4+ T細胞の増殖を強く誘導した。また、幾つかのRv1733cのペプチドプールは、特にペプチド16を含むプールにおいて、CD4+ T細胞の増殖を誘導することができた。これは、TST陽性個体からのPBMCについての図5に示す。このHLAタイプのドナー(DRB1*15)のためのRv1733cのエピトープのコンピュータ予測は、結果として、可能性のある幾つかのエピトープの発見をもたらし、その二つはペプチド16に位置し(VDEPAPPARAIADAALAALG及びVDEPAPPARAIADAALAALG)、ペプチド16に応答するCD4 T細胞の観察された増殖と一致する。Rv1733cのペプチドもまた、Rv1733cの組換えタンパクに対して応答する健康なコントロールのPBMCを用いたCD4細胞の増殖を誘導し、これは、この応答が抗原特異的であることを示す。
潜在性抗原への応答における他のサイトカインの産生
ELISA(実施例1〜6)によるIFNγに加えて、他の一連のサイトカインを組換え抗原で刺激したPBMCの6日目の上清中で測定した。TB患者(n=10)及びマントー陽性個体(n=10)のPBMCを、組換えタンパク質Rv1733c、Rv2029c(PfkB)、HspX (Rv2031c)、Rv2032、Rv2626c、Rv2627c、Rv2628、Rv3129、ESAT-6及びCFP10で刺激し、及びさらに陽性コントロールとして、結核菌の低酸素ライセートで刺激した(図6)。IFNγについてのCBAデータは、ELISAによって行われた観察を確証した。TNFα及びIL-5応答は、明白なゆがみなしで両方のグループで見出された。IL-2及びIL-4について極めて乏しい応答が検出された;また、何らかの検出可能な応答があった場合、それらはTB患者において観察された。興味深いことに、潜在性抗原の多くについて、著しいIL-10応答がマントー陽性において観察され、TB患者では観察されなかった(図6)。
曝露前(即ち、予防的な)TBワクチンについて、前臨床のTBワクチン試験が、低投与量エアロゾル感染誘発モデル (Williams et al. 2005, Tuberculosis (Edinb). 2005; 85(1-2):29-38)を含む、関連するマウス、モルモット及び非ヒト霊長類モデル(Brandt et al. Infect. Immun. 2000; 68(2): 791-795; Olsen et al. Infect. Immun. 2004; 72(10):6148-50; Langermans et al. Vaccine 2005; 23(21):2740-50) における進行性のスクリーニング及び試験を含むことは、TB分野で一般的に承認されている。曝露後(即ち、治療上の)TB患者については、現存の動物モデルはヒトTB疾病を部分的にのみ模倣し(McMurray, Clin. Infect. Dis. 2000; 30 Suppl 3:S210-2)、ヒトへの推定は不明確である。TBワクチン開発の分野における現在の考えは、新規のTBワクチンを用いて、BCGによって誘導された免疫応答を追加免疫することに向けられている(例えば、McShane et al. Nat Med. 2004;10(11):1240-4; Orme Tuberculosis (Edinb). 2005;85(1-2):13-7))。しかしながら、免疫応答の追加免疫は、BCGが最初の場所において応答を初回抗原刺激した場合にのみ達成される。
Claims (19)
- 脊椎動物におけるミコバクテリア感染に対する免疫応答を誘導するための方法であって、該方法は、結核菌休止状態(DosR)レギュロン配列:Rv0079、Rv0569、Rv1733c、Rv1738、Rv1813c、Rv1996、Rv2007c、Rv2029c(PfkB)、Rv2030c、Rv2031c(HspX)、Rv2032、Rv2626c、Rv2627c、Rv2628、Rv3126c、Rv3129、Rv3130c、Rv3132c、Rv3133c(DosR)、Rv0080、Rv1737c(NarX)、Rv1735c及びRv1736c(NarK2)、及びそれらの類似体、相同体又は断片から成る群から選択される一以上のポリペプチドの源(source)を含む組成物を前記脊椎動物に投与する工程を含む方法。
- 前記ポリペプチドが、Rv1733c、Rv2029c(PfkB)、Rv2627c、Rv2628、Rv0080、Rv1737c(NarK2)、Rv1735c及びRv1736c(NarX)から成る群から選択される、請求項1に記載の方法。
- 前記ポリペプチドが、Rv1733c、Rv2029c及びRv2627cから成る群から選択される、請求項2に記載の方法。
- 前記ポリペプチドが、TB休止状態レギュロン(DosR)配列Rv1733c、Rv2029c(PfkB)及びRv0080から得られる、請求項2に記載の方法。
- TB休止状態(DosR)レギュロン配列:Rv0079、Rv0569、Rv1733c、Rv1738、Rv1813c、Rv1996、Rv2007c、Rv2029c(PfkB)、Rv2030c、Rv2031c(HspX)、Rv2032、Rv2626c、Rv2627c、Rv2628、Rv3126c、Rv3129、Rv3130c、Rv3132c及びRv3133c(DosR)、Rv0080、Rv1737c(NarX)、Rv1735c及びRv1736c(NarK2)、及びそれらの類似体、相同体又は断片から成る群から選択される一以上のポリペプチドの源を含み、及び任意にアジュバントを含む、ミコバクテリア感染に対する免疫化のための組成物。
- 前記ポリペプチドが、Rv1733c、Rv2029c(PfkB)、Rv2627c、Rv2628、Rv0080、Rv1737c(NarK2)、Rv1735c及びRv1736c(NarX)から成る群から選択される、請求項5に記載の組成物。
- 前記ポリペプチドが、Rv1733c、Rv2029c及びRv2627cから成る群から選択される、請求項6に記載の組成物。
- 前記ポリペプチドが、TB 休止状態レギュロン(DosR)配列Rv1733c、Rv2029c(PfkB)及びRv0080から得られる、請求項6に記載の組成物。
- 前記アジュバントが、ポリI:C、CpG、LPS、脂質A及びそれらの誘導体、IC31、QS21、リポペプチドPam3Cys、細菌のフラゲリン、DDA/MPL、DDA/TDB及び可溶性LAG3から成るアジュバントの群から選択される、請求項5〜8の何れかに記載の免疫化のための組成物。
- 前記ミコバクテリアのポリペプチドが、ミコバクテリアTB複合体種 ミコバクテリウム・ツベロクロシス(ヒト型結核菌)、ミコバクテリウム・ボビス(ウシ型結核菌)、ミコバクテリウム・ボビス(ウシ型結核菌)BCG、ミコバクテリウム・アフリカヌム(M. africanum)、ミコバクテリウム・カネティー(M. canetti)及びミコバクテリウム・ミクロティ(M. microti)から選択される、請求項5〜9の何れかに記載の免疫化のための組成物。
- 前記ポリペプチドの源が、前記ポリペプチドをコードする組換えDNA分子であり、任意にベクター及び/又はゲノム中に含まれる、請求項5〜10の何れかに記載の免疫化のための組成物。
- 前記ポリペプチドの源が、組換えミコバクテリア中にあり、好ましくはミコバクテリア・ボビス カルメット-ゲラン杆菌(Bacillus Calmette-Guerin)(BCG)中にある、請求項5〜11の何れかに記載の免疫化のための組成物。
- 前記ポリペプチド断片が合成ペプチドであり、好ましくは18〜45のアミノ酸長であり、任意に重複し又はライゲートされ、任意にさらなるアミノ酸、免疫刺激部分及び/又は溶解性を増強しインビボ安定性を上昇するための保護基を含む、請求項5〜12の何れかに記載の免疫化のための組成物。
- 抗体又はその断片又はCD40リガンド又はその変異体から選択される、CD40結合分子をさらに含む、請求項5〜13の何れかに記載の免疫化のための組成物。
- 4-1BBレセプターを刺激することのできる、アゴニストの抗-4-1BB抗体又はその断片をさらに含む、請求項5〜14の何れかに記載の免疫化のための組成物。
- 前記組成物が、潜伏期に特異的でないミコバクテリア抗原をさらに含む、請求項5〜15の何れかに記載の免疫化のための組成物。
- 以下の工程を含む、被験者におけるミコバクテリア感染を診断する方法:
a)被験者の体液から単離されたサンプル及び/又は被験者の白血球から単離されたサンプルを、Rv1733c、Rv2029c(PfkB)、Rv2627c、Rv2628、Rv0080、Rv1737c(NarK2)、Rv1735c及びRv1736c(NarX)から成るTB休止状態レギュロンによってコードされるタンパク質の群から選択される、一以上のポリペプチド又はその断片と接触させること;及び、
b)ミコバクテリア感染を示す、抗体の前記ポリペプチド又はその断片への結合によって、体液性免疫応答を検出すること及び/又は;
c)特異的増殖及び/又はサイトカイン産生及び/又は細胞外又は細胞内活性化マーカーの発現によって、細胞性免疫を検出すること。 - 前記ポリペプチドが、Rv1733c、Rv2029c、Rv2627c及びRv0080から成る群から選択される、請求項17に記載の方法。
- 請求項1〜4で定義された一以上のポリペプチド及び前記ポリペプチドに結合する抗体をアッセイ及び定量化するための試薬を含む、請求項17又は18に記載の診断方法を実行するための診断キット。
