Description
この出願は、2004年12月15日に出願された米国仮出願第60/636,169号、2005年7月11日に出願された米国仮出願第60/698,112号、2005年9月14日に出願された米国仮出願第60/717,485号の優先権を主張し、これらはすべて、引用を以て本願に組み込まれるものとする。
乳癌は、治療に対する臨床所見及び対応に関して一様性のない病気である。この多様性は、乳癌のサブタイプ毎に癌細胞を構成する分子が異なることによる。しかしながら、現在治療標的として利用されているのは、2種類の分子的特性にすぎない。これらは、抗エストロゲン剤の標的であるエストロゲン受容体と、ハーセプチンの標的であるHER2である。これらの分子を標的にするための努力により、極めてプロダクティブ(productive)であることが立証されている。しかしながら、これら2種類の標的を有しない腫瘍の治療は、一般的には、増殖細胞を標的とする化学療法で治療されることがしばしばある。重要な正常細胞の一部も増殖するので、正常細胞も化学療法によって同時に損傷を受ける。化学療法は、毒性の強さと関係がある。ER又はHER2に加えて、腫瘍における分子標的の同定(identification)は、新しい抗癌治療において重要である。
本発明は、新規な予後予測及び治療標的であるHTPAP遺伝子について記載するもどである。HTPAP遺伝子は、標準の化学療法(ドキソルビシン、シクロホスファミド及びパクリタキセルを含む)での治療後であっても、増幅すると、乳癌患者に予後不良をもたらす。HTPAPの増幅は、腫瘍サイズ、治療、陽性の腋窩リンパ節の数、年齢及びホルモン受容体の状態、HER2増幅、及びcMYC増幅に関して、独立した予後予測因子(prognosticator)である。さらに、cMYCは、ハーセプチンに対する反応の予測因子であって、cMYC増幅とHER2増幅/過剰発現とを有する患者について、ハーセプチンが化学療法に加えられると、癌再発率は75%低下する。一方、cMYC増幅を有しない患者の場合、再発率の低下は45%にすぎない。HER2増幅又は過剰発現を有する乳癌患者の約30%は、cMYCが増幅する。トラスツズマブによるHER2シグナル伝達が阻害されると、cMYCの役割は、増殖スイッチからアポトーシススイッチ(pro-apoptotic switch)に変化するものと思われる。本発明は、以下の臨床的適用を含むものである: トラスツズマブと、HER2シグナル経路を標的にする他の薬物を使用して、患者毎の治療法を決定する方法を最適化すること; トラスツズマブ又はHER2標的療法後にも再発危険性の高い患者を同定することにより、血管新生を標的にするベバシツマブ(アバスチン)等の他の薬物又は標的療法の追加を行なう将来の臨床的研究に関する患者選択の方法を最適化すること; PCRを用いて、HER2とcMYCの両遺伝子の増幅状態の検出をシングルチューブアッセイ(single tube assay)で行ない、乳癌患者の反応の予後予測及び予測を行なうこと; アポトーシス抑制シグナルを他の活性腫瘍遺伝子から阻害し、そのアポトーシス活性の間接的調節を行なうことによって、cMYC標的療法を合理的に開発すること。
商業的に入手可能なFISHプローブの価格が高い理由の1つは、プローブである細菌人工クローン(BAC)を直接蛍光標識するのが困難で高コストであるためである。本発明は、パラフィン包埋組織に対して10年以上前から行われている全ゲノム増幅法及び効率的FISH法とを組み合わせることにより、既知のアンプリコンを表すBACクローンを効率的に蛍光標識する方法を提供するものである(図2に概要を示す)。この標識及びFISH法は、商業的に入手可能なプローブと比べて、ログオーダで安価である。パラフィンブロック組織サンプルを30,000以上の乳癌及び大腸癌の症例に使用し、臨床追跡情報(clinical follow up information)及び施された治療に対して全て注釈が付けられてあり(annotated)、臨床的相関の研究に対して、他とは異なる特異な情報源が提供される。FISH法と組織マイクロアレイ(TMA)を組み合わせることにより、単一の顕微鏡用切片を用いて100以上の症例をスクリーニングすることが可能になり、何千もの症例の中で複数のアンプリコンのスクリーニングを実現することができる。当該分野の専門家であれば、当該分野で広く知られたいかなる方法についてもは、FISH適用のためのプローブを標識するのに用いられることは容易に認識するであろう。さらに、FISHは増幅の決定に用いられるから、その他多くの定量法又は半定量法を、限定するものではないが、抗体を用いたアッセイ(例えばELISA(酵素免疫測定法))及びqt PCRに用いられることができる。
パイロット実証プロジェクトにおいて、National Surgical Adjuvant Breast and Bowel Project (“NSABP”)B−28のトライアルから987以上の症例をスクリーニングし、アリアマイシン(Ariamycin)シクロホスファミドを4サイクル行なったものと、同じ薬剤投与後パクリタキセルを4サイクル行なったものとを比較した。この研究では、組織マイクロアレイを構築し、FISHアッセイは、アレイCGHデータに基づいた10の異なる内部開発プローブに対して行なった(2セットは、HER2及びMLN64と、APPBP2及びPPMIDであり、互いに極めて近い)。アンプリコン及びそれらの染色体位置は次のとおり示される。

HER2とcMYCは両方とも、乳癌において予後に関連することは既に示されている。HER2は、ハーセプチンの治療標的である。しかしながら、化学療法で治療された患者におけるそれらの予後予測的役割は、明確に例証されていない。他方では、HTPAPは、ホスファチジン酸ホスファターゼ相同ドメイン及び5'膜貫通ドメインを含有する蛋白、並びに蛋白質が分泌されたことを示すシグナルペプチドに翻訳する新規な遺伝子である(図33参照)。HTPAP用のFISHプローブ(クローンRP-11-513D5)の生成に用いられた細菌人工染色体クローンは、その中に3種類の遺伝子(HTPAP、WHSC1L1及びDDHD2)だけを有している。これらの中で、遺伝子増幅と、乳癌細胞株における発現との相関に関する他の研究では、HTPAPは、この領域が増幅されるときに過剰発現するものであることが示されている[Pollack JR, Sorlie T, Perou CM et al., Proc Natl Acad Sci USA 2002; 99(20):12963-12968; Hyman E, Kauraniemi P, Hautaniemi S et al., Cancer Res 2002; 62(21):6240-6245; Ray ME, Yang ZQ, Albertson D et al., Cancer Res 2004; 64(l):40-47]。乳癌における遺伝子発現のマイクロアレイ解析によるデータの検討において、HTPAPの過剰発現は、94の他の遺伝子を有する乳癌患者の予後不良と関連があることが報告されている[Jenssen TK, Kuo WP, Stokke T, Hovig E.]。乳癌における遺伝子発現と患者の生存との関係が報告されている[Hum Genet 2002; 111(4-5):411-420]。これらの結果は、HTPAP遺伝子の増幅が、標準的化学療法による治療後でも、乳癌の独立した予後予測因子であることを示している。
cMYC増幅による悪性形質転換が起こった細胞は、HER2増幅の作用によりアポトーシスの運命を脱出できるように思われるが、これはまた、それら細胞を、生存へのHER2信号伝達に依存させることになる(図2b)。それゆえ、トラスツズマブによりHER2シグナルが阻害されると、そのような癌細胞ではcMYCのアポトーシス促進作用をトリガすることができる。これは、NSABPのB−31トライアルの一部として回収された腫瘍試料のレトロスペクティブ分析で検証された。そこでは、HER過剰発現腫瘍を有すると診断された患者がランダムに選ばれ、化学療法又は、化学療法+ハーセプチンが施された。この分析の結果では、HER2とcMYC遺伝子の両方について増幅を有する腫瘍は、トラスツズマブに感受性である(sensitive)。
Claims (21)
- 乳癌を治療する方法であって、
乳癌患者のHTPAPの発現量又は増幅を測定し、
HTPAPの発現量又HTPAPの増幅が増加した患者に対し、HTPAPのホスファチジン酸ホスファターゼ活性を妨げる少なくとも1種の化合物を治療上有効な量を投与することを含んでいる、方法。 - HTPAPの発現量は、酵素免疫測定法、放射免疫測定法及びフローサイトメトリー法からなる群から選択される方法により測定される請求項1の方法。
- 酵素免疫測定法では、患者から得た上清の可溶性HTPAP蛋白濃度が測定される請求項2の方法。
- HTPAPの発現量は、リアルタイムの定量的ポリメラーゼ連鎖反応によって測定される請求項1の方法。
- 少なくとも1種の化合物は、抗HTPAP特異的抗体である請求項1の方法。
- 抗HTPAP特異的抗体は、ヒト化モノクローナル抗体である請求項5の方法。
- HTPAPの増幅は、蛍光インサイチューハイブリダイゼーション法によって測定される請求項1の方法。
