Abstract
トランスジューサ(1、2、7、8)と、制御手段(5)と、出力手段(7)とを有する、光透過及び検出システムから構成される装置である。前記トランスジューサーは患者の体における複数の部位に置かれ、前記部位において吸収され、及び/又は反射された光を測定し、血管運動活性に関わる信号を集める。出力としては、症状が存在しているか否かを簡単に指示するために、前記トランスジューサー(1、2、7、8)から集められた前記血管運動信号を詳細に表示する形式を採用すればよい。例えば、患者の足裏に置かれた2つのトランスジューサからの信号の間の相違を測定することにより、片側のDVTの存在が検出できる。本発明は、例えば、DVT及び糖尿病性末梢神経障害があるか否かという兆候の提供にも用いられる。

Description
本発明は、人体の個々の部位における微小循環内の血管運動活性に対する評定により、深部静脈血栓症(DVT、Deep Vein Thrombosis)、糖尿病性末梢神経障害、重症虚血肢、自律神経機能、動脈と静脈の疾患などを含む疾患の臨床症状を検出することに関し、特に深部静脈血栓症(以下、DVTという)及び糖尿病性末梢神経障害の検出に関するものである。
脚部にかかるDVTは、静脈に凝血塊が形成されることにより、脈管が部分的又は完全に閉塞されてしまう症状である。凝血塊の形成は、外科処置や外傷、又は長期間の不活動状態(例えば、長時間飛行、身体障害)による血液停滞に起因する脈管障害が原因となる。知覚の結果として、DVTの場合では、軽い痛み及び腫れから、致命的な肺塞栓に至る。
DVT検出にかかる臨床診療に利用された従来の検査には、例えば、静脈造影検査や複式超音波検査のような画像検査がある。静脈造影検査では、放射線にオペークな画像媒体の注入、及び専門的な読み出しを要するX線画像が必要であり、患者に危険や不快な感覚を与え、時間も費用もかかるので、プライマリ・ケア又は一般開業医(以下、GPという)には向いていない。複式超音波検査の場合も、同じく時間と費用がかかるプロセスで、高度に熟練した医師が必要なので、プライマリ・ケア又はGPには向いていない。
プレディスモグラフィは、従来からの低コストで比較的に速い検査であり、訓練を受けたプライマリ・ケア又はGPに利用されている。しかし、プレディスモグラフィの場合は、患者が検査中に運動得ざるを得ないので、すべての患者には適用できない。また、この検査は熟達した操作員を必要とし、いつも確実な結果が得られるとは限らない。さらに、血液における凝固剤を測定し、その他の検査とともに用いるほうが良いと推薦されるD-ダイマー試験検査もある。プレディスモグラフィ及びD-ダイマー検査は、できる限り、多くのDVTのない患者が、複式超音波検査又は静脈造影検査において、より面倒な画像検査を受けなくてもすむように、前置のスクリーニング法として利用される。
本発明は、前記従来の技術の欠点に鑑みてなされたもので、その改善を求めることをその目的とする。
そこで、本発明は、人体の個々の部位における微小循環内の血管運動活性に対する評定により、疑われたDVT、糖尿病性末梢神経障害、重症虚血肢、自律神経機能、動脈と静脈の疾患などを含む疾患の臨床症状をモニターして評定するための、光透過及び検出システムから構成される装置を提供する。
微小循環内の血管運動活性は、微細脈管の連続的な収縮・拡張のプロセスであり、血圧調節、温度調節、組織の酸化と栄養摂取といった若干の重要な機能を果たす。前記プロセスの制御は局部的な制御でもあり、全身的な制御でもある。局部的な制御は近くの組織からの化学信号により活性化され、全身的な制御は自律交感神経系によるものであり、主として、中核温度及び体血圧を調節する。得られた局部的な血液量の変動に基づいて、局部的及び全身に多くの生物学的な過程に関する情報が提供される。
本発明は、好ましい実施形態において、人体の一つ又は複数の部位に置かれる複数のウェーブ・トランスジューサと、前記部位において吸収され及び/又は反射された光を測定し、対応した部位での絶対値及び/又は部位同士の間の微分値に関わる信号を提供するための制御手段とを含む、光透過及び検出システムから構成されている。前記トランスジューサとしては、赤外ウェーブ・トランスジューサが好ましい。
本発明は、トランスジューサにより、トランスジューサーの下の微小循環の血液量の変動を連続的にモニターするものである。光吸収は血液量に比例しており、又は、それとは逆に、光反射は血液量に反比例している。安定した環境で座り又は仰臥した休憩中の患者にとって、血液量の主要な変動は、全身的なコントロールの表れである。さらに、四肢における全身的な血管運動コントロールは対称的である。従って、トランスジューサは、健常者のそれぞれの足裏に置かれた場合、それぞれのトランスジューサからの信号が同一でなくても似たものになる。片側のDVTの存在の検出において、2つのトランスジューサからの信号の間の相違を測定することにより行うことができるのは、病変した脚部における末梢の体積が流出抵抗の増大に従い拡大するためである。これは、変動された周波数及び位相特性を病変した脚部の血管変動にもたせ、これにより、両側の対称性が影響されることになる。
本発明の他の局面においては、トランスジューサから受信された信号は、全身及び末梢における自律神経の障害の評定に用いられる。従来の全身的な自律神経機能検査では、通常ECG波形から得られた心拍の変動を解析する。しかしながら、トランスジューサによれば、体全体の皮膚における大部分の点から集められた信号から、心拍動を見ることができる。従って、心拍変動は前記信号から入手できる。心拍成分の変動の解析は、次に、トランスジューサからの信号の低周波数変動と比較することによって行われ、これにより末梢と全身的な自律機能とを直接的に比較することは可能である。前記2つの変動源は、健常者の体においては相似しているが、末梢神経障害のみになった患者の体においては、相似していない。
足部の血管運動活性によるDVT、血管病及び神経機能の評定の利点は、患者が運動しなくてもよい受動的検査を適用することができることである。神経機能検査は、例えば、ヴァルサルヴァ手技(valsalva manoeuvre)や呼気障害の軽度等級付け(mild graduation of exhalation impedance)のようなストレステスト(stress testing)により補強されるのが好ましい。患者の体において用いられた前記部位が容易にアクセスでき、従来の検査と比べると、低コストの機器に低い技能レベルが必要なだけであり、確実な結果が提供される。
これまでの血管運動活性及び係る生物学的な過程に対する認識は不足しているので、本発明のような血管運動活性による臨床症状の評定につき発表された文献は極めて少ない。発明者らは、血管運動信号により、健康者と不健康者の体に共に存在している複数の生物学的な過程に関わる重要な情報が提供されていることを発見した。
以下、次のような図面を参照して実施形態のみにより本発明を説明する。
図1と図2を参照すると、本発明は、図2に示すように、LEDと、適切なアンプ3、4を有する光検出器とからなるトランスジューサ1、2を含む、光透過及び検出システムから構成されている。トランスジューサ1、2を皮膚に付けると、中央制御ユニット5は、光検出器2からのミッドスケールの電圧の検出に適用される電圧によりLED1を駆動することにより、トランスジューサをキャリブレーションする。光検出器2の信号は、A/D1及びA/D2によりデジタル化される。LEDへの駆動電圧は、D/A1及びD/A2の出力により生じる。キャリブレーション工程が終了すると、中央制御ユニット5は適用できるサンプリングレートで光検出器4(図2)からのデータを集める。DVT検出には、6Hzのサンプリングレートが採用される。キーパッドのようなユーザ入力装置6と、LCDスクリーンやLED表示器のような出力ディスプレイが用いられる。また、PC接続、プリンター或いはその他のフォームのデータ記録装置に用いられる入力/出力ポートも用意される。
図3a〜図3cは本発明における2つのトランスジューサ1、2を微分信号の解析に用いる2チャンネルのシステムを利用した好ましい実施形態を示している。DVT検出のために、図3aに示すように、トランスジューサ1、2が患者の足裏に置かれている。図3bにおける配置は、DVTの概略位置が示されている。血管運動信号が相似していると、DVTは大腿にあり、それとは逆に、血管運動信号が相似していない場合、DVTは脹脛にある。図3cにおける配置は上肢と下肢との間の脈波トランシット時間を示しており、それにより、動脈壁の硬化が示される。図4は、2チャンネルのシステムにより健常者の足裏から得られた信号を示している。各トランスジューサからの信号は同一でなくても相似している。2つの信号の間の相違を測定することにより、片側のDVTの存在が検出される。
ユーザに示される出力としては、症状が存在しているか否かを簡単に指示するために、トランスジューサ1、2により集められた血管運動信号(図4に示す如く)が詳細に表示する形式を採用すればよい。前記表示については、必要に応じて適宜に配置することができる。
例えば、自律機能検査において心拍数への評定を行うには、トランスジューサ1、2の信号に対するサンプリングレートは、心拍成分が+/−1ms内に、又は、それより細かく解析できるようにしなければならない。さもなければ、ナイキスト基準を満たすサンプリング周波数は十分である。
それぞれのトランスジューサ1、2から得られた信号に対して、解析算法を適用する。特に前記信号に対して、例えばDVT検出のような応用により選ばれた帯域幅を有する所定の周波数を中心にして、血管運動活性を取り調べるための数学的技術である複素復調を行う。複素復調算法による出力は振幅信号と位相信号とからなるが、前記2つの信号を組み合せると、制約された帯域幅を有する振幅及び位相という二者により変調される、いずれも復調周波数を中心とする時変信号が生じることになる。
図3a〜図3cに示す配置と同様に、4チャンネルのシステムによる他の好ましい実施形態は、図5に示すように、膝の裏側に付けられた他の二つのトランスジューサー7、8が備えられているものである。信号はフィルター及びDC除去ステップを含む信号前処理段階を経てから、1組の選ばれた周波数で複素復調が行われる。例えば、1分間につき8〜30サイクルである。スペクトルRFLF(MAPD)が得られるように、周波数のそれぞれに対して、右足(RF)及び左足(LF)からの平均絶対位相差(MAPD、mean absolute phase differences)を算出し、その後、例えば事前トレーニングされた(pre−trained)人工ニューラルネットワークのようなパターン分類器は、前記スペクトルRFLF(MAPD)を利用して、スクリーンに「DVT PRESENT(DVT有り)」又は「DVT NOT PRESENT(DVT無し)」という出力情報を提供する。
図5に示すような4チャンネルのシステムには、図6に示すような6つのMAPDがある。

このようにして、上述した2チャンネルのシステムに対して6倍の診断情報が得られる。
本発明は、DVT検出の他、更に、糖尿病性末梢神経障害、重症虚血肢、自律神経機能、動脈と静脈の疾患などを含む疾患の臨床症状をモニターして評定することができる。
前記症状のそれぞれにおいて、微小循環の血管運動活性は、適切な信号処理算法により抽出されて評定される独特なシグナチェアを持っている。前記算法は、係る臨床症状に決められた適切な周波数帯域に応じて調整される。前記算法は左右の足の間の血管運動の対称性、また血管運動活性の低周波数成分と心拍数変動の低周波数成分との間の類似性を利用するものである。図8に示すように、本発明の装置によれば、血管運動信号から心拍数変動を抽出し、同時に存在している低周波数の心拍数変動と低周波数の血管運動活性の変動を直接的に比較して、糖尿病による交感神経障害に関する情報が提供されるが、この2つの成分の間の相違により糖尿病による交感神経障害が表示される。
図7は、呼吸障害の増大に関わる血管運動活性の変動及び健康者に対する手の握力検査を示している。前記検査は、足裏におけるトランスジューサにより観測されるように、体血圧及び心拍出量に影響を与え、そして、心拍数及び末梢血管運動活性の神経仲介の応答を引き起こすことになる。足における微細血管の更新を行う交感神経繊維のパソロジは血管運動特性に大幅な変化をもたらすので、図7における、静止期と増大した呼吸障害及び手の握力検査との間の信号の変動のいずれも、糖尿病による交感神経障害を表示するものである。

- 疑われたDVT及び糖尿病性末梢神経障害などを含む関連疾患の臨床症状を検出して評定するために、人体の個々の部位における血管運動活性を評定する、光透過及び検出システムを備える装置。
- 人体における一つ又は複数の部位に置かれる、一つ又は複数のウェーブ・トランスジューサと、前記部位において吸収された光を連続的に測定し、前記部位での絶対値に係わる信号を提供するための制御手段とを含む、複数のチャンネルの光透過及び検出システムを有する請求項1に記載の装置。
- 人体における一つ又は複数の部位に置かれる、一つ又は複数のウェーブ・トランスジューサと、前記部位において反射された光を連続的に測定し、前記部位での絶対値に係わる信号を提供するための制御手段とを含む、複数のチャンネルの光透過及び検出システムから構成される請求項1に記載の装置。
