Description
適切な直接的または間接的なカプサイシン受容体アンタゴニストは、肥満ならびに他の関連疾患および障害を治療するために、化合物単独でまたは医薬製剤の形態で許容可能なキャリアと混合して患者に投与することができる。肥満ならびに他の関連疾患および障害を治療する当業者は、そのような治療を必要とするヒトを含む哺乳動物に対して、前記化合物を投与する用量および経路を容易に決定することができる。本発明の医薬組成物に対する用量および望ましい薬物濃度は、想定した特定の使用に依存して変化してよい。動物実験は、ヒトの治療に対して効果的な用量の決定に対する信頼できる手引きを提供する。
カプセル剤
多数の単位カプセルは、100mgの粉末の活性成分、150mgのラクトース、50mgのセルロース、および6mgのステアリン酸マグネシウムで、標準的な2つの硬ゼラチンカプセルを充填することにより調製できる。
ダイズ油、綿実油、およびオリーブオイルのような消化可能な油中における活性成分の混合物を調製し、容積形ポンプを用いてゼラチン中に注入し、活性成分を100mg含む軟ゼラチンカプセルを形成する。前記カプセルは、洗浄し、乾燥するべきである。
錠剤は、通常の手法により、例えば製剤単位が、100mgの活性成分、0.2mgのコロイド状二酸化ケイ素、5mgのステアリン酸マグネシウム、275mgの微結晶性セルロース、11mgのデンプン、および98.8mgのラクトースとなるように調製されてよい。適切なコーティングは、嗜好性または吸収遅延を増大させるために適用されてよい。
注射による投与に適した非経口的な組成物は、例えば1.5重量%の活性成分を10容積%のプロピレングリコールおよび水中で撹拌することにより調製されてよい。前記溶液は、塩化ナトリウムにより等張にし、滅菌するべきである。
水性懸濁液は、経口および/または非経口的な投与のために、例えば100mgの微細な活性成分、20mgのカルボキシメチルセルロースナトリウム、5mgの安息香酸ナトリウム、1.0gのソルビトール溶液、U.S.P.、および0.025mLのバニリンまたは他の味のよいフレーバー剤を含み、各5mLになるように調製することができる。
注射による投与に適した徐放性で非経口的な組成物は、例えば、適切な生物分解性ポリマーを溶媒に溶解し、該ポリマー溶液に活性成分を混和させるように加え、マトリックスから溶媒を除去し、その結果として、マトリックス中に分配した活性成分とポリマーとのマトリックスを形成することにより、調製されてよい。
上記で述べたように使用されるカプサイシン受容体(VR1)アンタゴニストBCTC(上記参照)は、WO 02/08221において述べられているように合成されてよい。
実験動物:オスのカプサイシン受容体(VR1)ノックアウトマウス(n=11)(系統名:B6.129S4-Trpv1tm1Jul、系統番号:003770)およびオスの対照マウス(n=8)をジャクソンラボラトリーズ(Jackson Laboratories), Bar Harbor, ME, U.S.から購入した。前記マウスを、5週齢のときにデンマークへ輸送し、それから29週齢まで、デンマーク、ノボノルディスクA/S社のMalovアニマルユニットで、正常な日光サイクル(午前6時に照明を付け、午後6時に消す)の外界の調節された条件下に保たれた。マウスは、グループに分けられ(ケージごとに1グループ)、標準的なケージ(ユーロスタンダードタイプIV, テクニプラスト(Techniplast), スキャンバー(Scanbur), デンマーク)に入れられ、酸性化(0.4%クエン酸)水道水および標準的な固形飼料(アルトロミン(Altromin), Brogaarden Aps, デンマーク)を自由に利用できるようにした。ケージには、巣作り用の材料として木製のウール(Brogaarden, デンマーク)が供給され、ベッドを作る材料はポプラチップ(Brogaarden, デンマーク)であり、酸性化した水のボトルも同様に、1週間に2回交換した。11週齢の時に、固形飼料を高脂肪食に変えた(Research Diet Prod. No. D12309, Research Diet Inc, New Brunswick, U.S.)。
15週間高脂肪食を与えた後、OGTTを行った。マウスは、OGTTの前18時間は絶食した。空腹時血糖を以下に述べるように測定した後、500 mg/mlの濃度のグルコース(Sygehus apotekerne, デンマーク)を胃管栄養法により経口的に与えた(2 g/kg po)。血糖を測定するための血液試料は、意識のあるマウスの尾の先端の毛細血管から得た。血液をヘパリン処理した5μlのキャピラリーチューブに集め、直ちに250 μlのEBIO緩衝液(EBIO, エッペンドルフ, ドイツ)に懸濁した。前記実験に続いて、グルコースオキシダーゼ法(EBIO, エッペンドルフ, ドイツ)を用いてグルコースを測定した。血糖曲線下面積(AUC)は、台形法によって測定した。2つの群の平均値は、スチューデントのt-検定法を用いて比較した。Pが0.05より小さい場合、統計学的に有意差があると考えた。
高脂肪食を18週間与えた後、グルコースを腹腔内に注射することにより、マウスのインスリン分泌を試験した。マウスは試験前の18時間、絶食させた。500 mg/mlの濃度のグルコース(Sygehus apotekerne, デンマーク)を、注射によって腹腔内に投与した(2 g/kg i.p.)。腹腔内投与のグルコースに対する初期のインスリン反応の評価は、暴露後2分、意識のあるマウスの尾の先端の毛細血管からの血液を約70μl、ヘパリン処理した100μlのキャピラリーチューブに採取ことにより試験される。これらのチューブを遠心分離し(4000 rpm/min/ 4℃)、15μlの血漿を保存し、自家製のELISAを用いた試験まで−20℃で保存した。2つの群の2分における血漿インスリンレベルの平均値は、前記群の分散間の統計学的な有意差によるノンパラメトリックなマン-ホイットニー(Mann-Whitney)U-検定によって比較した。Pが0.05より小さい場合、統計学的に有意差があると考えられる。

6月齢の時、カプサイシン受容体アンタゴニスト(BCTC)が経口投与された後、OGTTがまもなく行われた。ラットはOGTTの前18時間絶食させた。空腹時血糖および血漿インスリンを以下に述べるように測定した。−30分において、15 mg/kgのBCTC(n=10)または溶媒(n=9)を胃管栄養法によって経口的に与えた。ラットは、0時間までケージに残し、血糖をOGTTに従って評価した。簡単に言うと、500 mg/mlの濃度のグルコース(Sygehus apotekerne, デンマーク)を胃管栄養法により経口的に与えた(2 g/kg po)。血糖および血漿インスリンを、グルコース投与後30、60、および120分後に測定した。血糖および血漿インスリンの測定のための血液試料は、意識のあるラットの尾の先端の毛細血管から得た。血糖の測定のための血液は、ヘパリン処理した5μlのキャピラリーチューブに採取し、直ちに250μlのEBIO緩衝液(EBIO, エッペンドルフ, ドイツ)に懸濁した。実験の後、グルコースオキシダーゼ法(EBIO, エッペンドルフ, ドイツ)を用いてグルコースを測定した。
9月齢において、同じラットで腸間膜の炎症メディエータを再び試験した。ラットは18時間絶食し、15 mg/kgのBCTC(n=10)または溶媒(n=9)を胃管栄養法により経口的に与えた。処理後1時間において、ラットをイソフルランで麻酔し、断頭した。腸間膜脂肪組織を素早く除去し、10倍容積の予め冷却したRNAlater(シグマ-アルドリッチ,USA)に移し、−20℃で保管した。遺伝子発現パターンにおける領域間の影響を防ぐために、0.5〜1gの組織をTRIゾール試薬(1 ml/100mg 組織) (TRIzol; インビトロゲン社製)中で均質化した。トータルRNAは、キアゲン(Qiagen)プロトコル(Qiagen RNeasy Mini handbook, 2001)に従って200mlのホモジネートから抽出した。第一の鎖であるcDNAは、製造者のプロトコル(インビトロゲン社)に従って、スーパースクリプトIII逆転写酵素およびランダムヘキサマープライマーを用いて合成した。未知の試料のcDNAは、ヌクレアーゼが存在しない水中で(キアゲン社製)1:12に希釈した。PCR効率の計算のための検量線を作成するために、それぞれのcDNAプールからのサンプルを混合し、1:6、1:12、1:24、1:48、1:96、1:192、および1:384に希釈し、ここでは、100%(±1%)のPCR効率ならびに0.99および1の間のR2のみが許容される。PCR増幅混合物(25μl)には、12.5μlの2×Platinum Quantitative PCR SuperMix-UDG (インビトロゲン社製)、0.625 μlの逆方向プライマー (20 μM)、0.625 μlの順方向プライマー(20 μM)、0.625 μl のプローブ(10 μM) (プローブライブラリー エキシコン(Exiqon) A/S, デンマーク)、および5 μlの希釈したcDNA鋳型が含まれていた。リアルタイム定量的PCRは、MX3000P PCRマシーン(ストラタジーン(Stratagene), USA)を用いて、以下のサイクリングパラメータを用いて行った: 95℃で10分間のポリメラーゼ活性化、95℃で30秒間および60℃で60秒間の40サイクルの増幅。増幅の後、単位複製配列をゲル電気泳動(E-ゲル、インビトロゲン社

BCTC(15 mg/kg)を経口投与された後1時間のiNOS(a)およびF4/80の両方において有意に低いレベルを示した。これは、BCTCが、組織において低い程度の炎症を減少させることにより、経口糖耐性の改善および誘導性インスリン放出の改善を媒介し、これはカプサイシン受容体に拮抗することにより媒介されることを示唆する。

Claims (23)
- カプサイシン受容体の活性を不活化または下方制御することにより哺乳動物における肥満を治療するための方法であって、治療的に有効な量の直接的または間接的なカプサイシン受容体アンタゴニストを、それを必要とする哺乳動物に対して投与することを含んでなる方法。
- 前記カプサイシン受容体アンタゴニストが直接的なカプサイシン受容体アンタゴニストである、請求項1に記載の方法。
- 前記カプサイシン受容体アンタゴニストが間接的なカプサイシン受容体アンタゴニストである、請求項1に記載の方法。
- カプサイシン受容体の活性を不活化または下方制御することにより哺乳動物における肥満に関連する疾患または障害を治療するための方法であって、治療的に有効な量の直接的または間接的なカプサイシン受容体アンタゴニストを、それを必要とする哺乳動物に対して投与することを含んでなる方法。
- 前記カプサイシン受容体アンタゴニストが直接的なカプサイシン受容体アンタゴニストである、請求項4に記載の方法。
- 前記カプサイシン受容体アンタゴニストが間接的なカプサイシン受容体アンタゴニストである、請求項4に記載の方法。
- 請求項4に記載の方法であって、前記肥満に関連する疾患または障害が以下から選択される方法:1型糖尿病、2型糖尿病、耐糖能異常、循環器病、高血圧、インスリン抵抗性、癌、および生殖系の障害。
- 前記肥満に関連する疾患または障害が2型糖尿病である、請求項7に記載の方法。
- 前記肥満に関連する疾患または障害が1型糖尿病である、請求項7に記載の方法。
- 前記肥満に関連する疾患または障害が耐糖能異常である、請求項7に記載の方法。
- 前記肥満に関連する疾患または障害が循環器病である、請求項7に記載の方法。
- 前記肥満に関連する疾患または障害が高血圧である、請求項7に記載の方法。
- 前記肥満に関連する疾患または障害が癌である、請求項7に記載の方法。
- 請求項13に記載の方法であって、前記癌が以下から選択される方法:大腸癌、直腸癌、結腸直腸癌、膵臓癌、食道癌、乳癌、前立腺癌、子宮癌、腎臓癌、子宮体癌、胆嚢癌、甲状腺癌、肝臓癌、子宮頚癌、卵巣癌、胃癌、非ホジキンリンパ腫、および多発性骨髄腫。
- 前記肥満に関連する疾患または障害が生殖系の障害である、請求項7に記載の方法。
- 請求項15に記載の方法であって、前記生殖系の障害が以下から選択される方法:多嚢胞性卵巣症候群(PCO)、不妊症、およびインポテンスまたは勃起機能障害。
- 請求項1〜16のいずれか1項に記載の方法であって、前記直接的または間接的なカプサイシン受容体アンタゴニストが以下から選択される方法:天然物、合成有機化合物、ペプチド、タンパク質、抗体、抗体断片、単鎖抗体、および抗体に基づく構築物。
- 前記哺乳動物がヒトである、請求項1〜17のいずれか1項に記載の方法。
- カプサイシン受容体の活性を不活化または下方制御することにより、治療を必要とする哺乳動物において肥満または肥満に関連する疾患もしくは障害を治療するためのキットであって、直接的または間接的なカプサイシン受容体アンタゴニストを含んでなるキット。
- 前記哺乳動物がヒトである、請求項19に記載のキット。
- 前記肥満に関連する疾患または障害がインスリン抵抗性である、請求項7に記載の方法。
- カプサイシン受容体の活性を不活化または下方制御することにより肥満または肥満に関連する疾患および障害を治療するための方法であって、治療的に有効な量の直接的または間接的なカプサイシン受容体アンタゴニストを、抗糖尿病薬、抗高脂血症薬、抗肥満薬、および肥満または肥満に関連する疾患もしくは障害の結果として生じるまたはそれらに付随する合併症の治療のための薬剤から選択されるさらなる活性薬剤と組み合わせて、それらを必要とする哺乳動物に対して投与することを含んでなる方法。
- 低い程度の炎症が肥満および肥満に関連する障害に付随する状態を治療するための方法であって、治療的に有効な量の直接的または間接的なカプサイシン受容体を、それを必要とする患者に対して投与することを含んでなる方法。
