実施例の要約
原炎症サイトカイン、マクロファージ転移阻害因子(MIF)は、軸となる役割を、幾つかの炎症および自己免疫疾患において演じる。MIFmRNA発現は、非肥満糖尿病マウスにおいてアップレギュレーションされるが、尚僅かしか、1型糖尿病レギュレーターとしての潜在的なポテンシャルについて知られていない。ここで、我々は、MIF蛋白が、多数回低用量ストレプトゾトシンによりマウスにおいて誘発された実験糖尿病の進展の間に、膵島細胞において顕著に上昇されることを示す。MIFに対する中和抗体、もしくは、薬理的MIF阻害剤(S,R)−3−(4−ヒドロキシフェニル)−4,5−ジヒドロ−5−イソキサゾール酢酸メチルエステル(ISO−1)のような例示的なMIF阻害剤を用いたMIF活性の減衰は顕著に、膵島における組織病理学的変化(病変)を抑え、高血糖の進展を抑え、1型糖尿病を患っているかもしくは1型糖尿病のリスクに晒されている哺乳類を処置する本方法の一般的な有用性を示している。観察された有益な効果は、自己反応性リンパ球の増殖および接着の抑制、iNOS発現のダウンレギュレーション、ならびに、膵島および腹腔マクロファージによるNOおよびTNF−αの分泌に帰属され得るが、これらの機序の繋がりの知識は、本発明の教示には必須ではない。本研究は、1型糖尿病の病理におけるMIFに関する決定的な役割を定義し、新たな治療戦略を同定し、自己免疫プロセスを多段階において減衰させる。
MIFが、TNF−α、IL−1β、および酸化窒素(NO)(19)のような、1型糖尿病(DM)の病理に含まれている他の原炎症サイトカインおよび可溶性メディエーターの合成をも刺激するとの観察は、MIF活性の阻害が、疾患の進展を調節するかも知れないとの可能性を喚起した。
マクロファージおよびT細胞両方により仲介される免疫応答におけるMIFの中心的な制御の役割が与えられれば、我々は、多数回低用量ストレプトゾトシン(MLD−STZ)処置されたマウスにおける糖尿病の進行の間のMIF発現、ならびに、MIF活性の中和のこの疾患の進展への影響を研究してきた。これらは、ヒトでのよく認められた1型糖尿病モデルである。この目的では、抗MIFポリクローナル抗体が、(S,R)−3−(4−ヒドロキシフェニル)−4,5−ジヒドロ−5−イソキサゾール酢酸メチルエステル(ISO−1)、MIFの薬理的阻害剤(20)同様、STZを用いた挑戦前に、予防処置としてマウスに与えられた。DMの病理に関する種々の免疫学的パラメーターが求められ、自己反応性細胞増殖および接着ならびに原炎症メディエーター産生も包含していた。我々の知る限り、これが最初の試みであり、in vivoにおけるMIFの中和により、自己免疫糖尿病において干渉させる。
マウス
InbredCBA/Hマウスが、Institute for Biological Research、Belgradeにある我々自体の飼育コロニーから得られた。InbredC57B1/6マウスは元来、Charles River(Calco、Italy)から購入され、次いで、4世代までの間、兄弟姉妹間交配により飼育された。マウスは、標準的な実験室条件下におかれ、食物および水に自由にアクセスできた。該マウスの取り扱いおよび研究のプロトコールは、Local Institutional Animal Care and Use Committeeにより(動物愛護の点から良いものと)認められた。
ストレプトゾトシン(STZ、S−0130)、スルファニラミド、ナフチレンジアミン二塩酸、および非罹患性ウサギIgGが、Sigma(St.Louis、MO)から購入された。抗マウスMIFIgGが、マウスMIFに対して惹起されたウサギ血清から調製され、蛋白A親和性クロマトグラフィーにより、製造元(Pierce、Rockford、IL)の説明書に従って精製された。(S,R)−3−(4−ヒドロキシフェニル)−4,5−ジヒドロ−5−イソキサゾール酢酸メチルエステル(ISO−1)が、前に記載されたように(20)合成された。
糖尿病が、成雄マウスにおいて誘発され、多数回の毒性を下回る用量のストレプトゾトシン(MLD−STZ、40mg/体重kg/日、i.p.投与、5日間連続)を、記載のとおり(21)投与された。抗MIFポリクローナル抗体のインパクト(効果)が、マウス(5〜10匹/処置群)の、5mg/kgの抗マウスMIFウサギIgG抗体での、3、1、2、および5日の日のi.p.投与により、最初のSTZ用量に関して研究された。ISO−1の効果は、当該薬剤のi.p.投与により、用量1mg/マウス/日において、14日間連続研究され、STZの最初の注射3日前に開始された。コントロールの動物は、非免疫IgGもしくは稀釈ISO−1(DMSO/H2O)を与えられた。マウスは、グルコース測定器(Sensimac、登録商標、Imaco、Ludersdorf、Germany)を使用して、血中グルコースレベルの週毎の測定により、糖尿病に関してモニターされた。臨床的な糖尿病は、非肥満の動物における高血糖(血中グルコース11.8mモル/L)により定義された。
定住個体の腹腔細胞(PC)、脾臓単球細胞(SMNC)、および膵島が、個々の、抗MIFIgG処置、ISO−1処置、もしくはコントロールマウスから、STZの最初の注射後15日に、同様に、正常非処置動物から単離された。SMNC(5×106/ウェル)はFicoll勾配遠心分離により調製され、定住個体のPC(2.5×106/ウェル)は冷PBSでの腹腔洗浄により収集され、もしくは、膵島(150〜200膵島/ウェル)は記載されるとおり(22)コラゲナーゼによる消化および密度勾配精製により調製され、24ウェルLimbro培養プレート中においてインキュベートされ、1mLのRPMI−1640培養培地中においてであり、5%の牛胎児血清(FBS)を含有しており、細胞の上清が、48時間後に収集された。培養上清における生物活性なTNF−αの濃度が求められ(以前に記載されたとおり、22)、アクチノマイシンD処理フィブロサルコーマ細胞株L929を用いた細胞溶解バイオアッセイを使用した。NO産生の指標たる亜硝酸(塩)の蓄積が、細胞培養上清中において、Griess反応を使用して求められた。細胞内MIF、IFN−γ、MHC(クラス)II、もしくはiNOSの発現が、細胞主体の僅かに修飾されたELISAプロトコール(23)により求められた。脾臓MNC(5×105/ウェル)もしくはPC(2.5×105/ウェル)が、ポリ−L−リジンにより予備コーティングされた96ウェルマイクロプレートに接着するようにされた。4%パラホルムアルデヒドでの固定化およびPBS中0.1%TritonX−100(T/PBS)での洗浄後、内因性過酸化酵素が、T/PBS中1%H2O2を用いてクウェンチされ、反応が1時間、37℃において、T/PBS中10%FBSを用いてブロックされた。引き続き、細胞が1時間、37℃において、ウサギ抗マウスMIFIgG、ラット抗マウスIFN−γ(Holland Biotechnology、Leiden、オランダ)、マウス抗マウス/ラットMHC(クラス)II(MCA46R、Serotec)、もしくは、ウサギ抗マウスiNOS(シグマ)を用いて、1%BSA含有T/PBS中においてインキュベートされた。洗浄後、これら細胞が1時間、対応している2次抗体(ヤギ抗ウサギIg(H+L)−HRP、ヤギ抗ラットIg(H+L)−HRP、もしくはヤギ抗マウスIg(F(ab’)2−特異的)−HRP)と共にインキュベートされ、再び洗浄され、15分間暗所室温において、0.4mg/mLのOPD(シグマ)、11.8mg/mLのNa2HPO4.2H2O、7.3mg/mLのクエン酸、および0.015%H2O2含有溶液50μLと共にインキュベートされた。該反応は、3NのHClを用いて停止され、その吸光度が、Titertekマイクロプレート読み取り機(Flow)において、492nmにおいて測定された。
SMNCの自然な増殖が、1μCiの3H−チミジン(3H−TdR、ICN)を有する96ウェルマイクロプレート中の、各動物個体からの細胞(5×105/ウェル)のインキュベートにより求められた。取り込まれた放射能が、24時間後、Beckman液体シンチレーションカウンター中において測定された。SMNC(2.5×105/ウェル)の、単層L929線維芽細胞もしくはプラスチックへの自然接着の分析が、クリスタルバイオレットアッセイを前に記載されたように(21)使用して実施された。接着細胞数に対応している吸光度が、570nmにおいて測定された。
脾臓MNC(1×106)が、ラット抗マウスモノクローナル抗体(mAbs)、抗CD11b(MAC−1)フィコエリトリン(PE)、もしくは抗CD25(IL−2受容体α鎖、p55)−ビオチン(PharMingen、San Diego、CA)、次いでストレプタビジン−PE(PharMingen)と共にインキュベートされた。SMNCの各細胞の懸濁は、同一実験群から糖尿病誘発15日後に得られた3〜5匹の動物ならびに正常な非処理マウスからプールしたものであった。細胞表面マーカー発現が、フローサイトメトリー(FACScalibur、Becton Dickinson、Heidelberg、Germany)およびWin MDI 2.8ソフトウェア(Joseph Trotter)を用いて分析された。
膵臓が、中性に緩衝化されたホルマリン中において固定され、次いで、パラフィン中に埋められた。この固定された塊が切開され(7μm厚)、ヘマトキシリンおよびエオシン染色が実施され、炎症変化の発症および程度を査定した。インシュリン炎のスコア化が実施され(以前に記載されたとおり、10、11)、マウスにつき少なくとも15の膵島を検査していき、盲検的に、以降のように格付けされた。

少なくとも15の膵島が、各マウスにつき数えられた。各膵臓に関する平均スコアが、その合計スコアを検査膵島数で割って算出された。インシュリン炎スコア(IS)は、平均値±SDとして表現される。免疫組織学が、パラフィンに埋められたホルマリン固定組織の切片において実施され、以前に記載されたマイクロ波主体の方法(24)を使用した。MIFの免疫染色に関しては、ポリクローナルウサギ抗MIFIgGおよびコントロールウサギIgGが使用された。膵島の検査面積が概算され、MIF+膵島細胞の%が、定量的画像分析システム(Optima6.5、Media Cybernatics、Silver Springs、MD)を使用して測定された。
血中グルコース値が、平均値±SEとして示される。統計解析が、Bonferroni較正およびFisher精密度検定を有するANOVAにより実施された。他の値は、平均±SDとして表現され、データ群が、Student組み合わせt検定を使用して比較された。同様な結果の少なくとも3回の別々の実験の代表例の結果が、表示される。統計的な有意差は、P<0.05に設定された。
MLD−STZ誘発糖尿病マウスにおけるMIF発現の増加
最近の研究が、MIFmRNA発現が、自然発症糖尿病NODマウスにおいてアップレギュレーションされることを明らかにしているが、糖尿病の進行におけるその役割は、知られていない。MIF蛋白発現レベルが、免疫炎症糖尿病の間に代えられるかどうかを決定するために、MLD−STZが、糖尿病感受性CBA/Hマウスに投与された。免疫組織化学が、この疾患の経過の間に、これらのマウスからの膵臓切片における膵島細胞によるMIF蛋白発現の実質的な誘発、ならびに、単核細胞の浸潤を明らかにした(図1B〜D)。同様に、非糖尿病コントロールマウスに比べてより高濃度のMIFが、糖尿病マウスの腹腔細胞(PC)において検出された(図1E)。疾患誘発の間の、抗MIF免疫中和グロブリンでのin vivoでのマウスの処置が顕著に、PCでのMIF発現を抑えた(図1E)。これゆえ、DM誘発の結果として、MIF蛋白含量の増加が、末梢レベルおよび標的組織レベル両方において観察され、MIF蛋白レベルが、抗MIF抗体投与により減弱され得る。
MIF活性阻害が、MLD−STZに晒されたマウスにおける糖尿病を調節するかどうか決定するために、我々は第1に、MIFに対する中和ポリクローナル抗体の効果を、2つのDM感受性同系交配系統マウスにおいて研究した。C57B1/6およびCBA/Hコントロールマウス両方が、PBSもしくは非免疫IgGを用いて処置され、MLD−STZ注射後の2週間に亘り、持続した高血糖を進展させた。MLD−STZは、異なる程度の高血糖を、2系統のマウスにおいて誘発させたが、−3〜+5日の抗MIFAbでのC57B1/6マウス(図2A)もしくはCBA/Hマウス(図2B)の処置は顕著に、MLD−STZ誘発高血糖を阻害した。加えて、膵臓の見本の組織学的検査が、この疾患の15日後に実施され、膵島の浸潤の程度が顕著に、非罹患性IgGを用いて処置された糖尿病コントロールマウスと比べて、抗MIF抗体を用いて処置されたマウスにおけるよりも低かったことを指し示した(図2CおよびD)。我々は、MLD−STZ誘発DMにおけるMIFアンタゴニストISO−1(20)の薬理効果も検査した。従って、我々は、マウスを予防的に14日の期間に亘り、ISO−1を用いて処置し、5回のSTZ注射のうちの最初の3日前に開始した。これらのマウスは、8週の実験期間を通して、正常血糖のままであった(図2B)。MIFの免疫的もしくは薬理的中和の抗糖尿病原性効果は長く継続し、58日のフォローアップ期間全体を通して、変動は限られていた(図2AおよびB)。ISO−1によるMIFのブロックが、膵島の炎症を減弱させた(図2D)。これらのデータは、免疫炎症DMの誘発および進行におけるMIFの決定的な役割、ならびに、リスク集団における1型糖尿病の進展をそらすMIF阻害剤での処置の有用性に関する証拠を与える。
抗MIF処置の細胞への効果を理解するために、CBA/Hマウスからの脾臓単核細胞(SMNC)の機能の特徴のex vivoでの分析が、高血糖15日後の初期高血糖の進行の間に、実施された。糖尿病マウスから単離された自己反応性リンパ球が、健康な動物から単離された細胞に比べて顕著に、ex vivoでの自然増殖の増加を呈することを、以前の研究が示しており、このタイプの細胞が、疾患の進行に寄与するかも知れないことを示唆している(21、22)。MIF阻害剤が、脾臓細胞増殖応答を、ex vivoにおいて調節するかどうか決定するために、糖尿病CBA/Hマウスが、抗MIF抗体もしくはISO−1で処置され、MLD−STZに晒した15日後に安楽死させられ、脾臓細胞が、ex vivoでの分析用に採取された。MLD−STZに晒したマウスの抗MIF抗体での処置は顕著に、糖尿病関連SMNC増殖を阻害した(3H−チミジン取り込みが、糖尿病のコントロールの細胞の63651±10157cpmに対し、29782±3694cpmであった。図3A)。同様に、in vivoでのISO−1でのMIF活性阻害は、脾臓細胞増殖を、コントロールレベル近くにまで逆行させた(非処置のコントロールの細胞の21150±12723cpmに比べ、25637±2018cpm。図3A)。細胞増殖に加え、細胞−細胞接着が、CD11bとICAM−1(CD54)との間の相互作用により仲介されるのであるが、1型DMの免疫プロセスに参画する(21、25)。糖尿病プロセスに参画している細胞間の相互作用への抗MIF処置の可能な結果を決定するために、我々は、SMNCの接着特性を主張し、プラスチックへの細胞の自然接着が、線維芽細胞への接着同様顕著に、in vivoでの抗MIFIgGもしくはISO−1での処置により阻害されたことを見出した(図3B)。接着性のダウンレギュレーションが、CD11b発現の変化と関連しており、フローサイトメトリー分析により明らかにされたとおりであり、CD11bに対して向けられた抗体を使用し、これは、ICAM−1(CD54)への細胞接着を仲介する分子である。これゆえ、コントロールの糖尿病マウス由来のSMNCに比べて、抗MIFIgG処置およびISO−1処置両方が(表1)、CD11b+SMNC頻度および平均抗原密度(平均蛍光強度、MFI)を、非処置正常マウスレベルまで抑え、MIF阻害剤での処置の抗糖尿病原性に対する有益性を指し示していた。
原炎症メディエーターは、齧歯類における1型DMにおいて、重大な役割を演じるので(4〜7、21、22)、我々は、局所および末梢免疫細胞両方からのSTZ関連サイトカイン放出へのMIF阻害効果を、ex vivoにおいて求めた。抗MIFIgGでの内因性MIF蛋白の免疫的中和(図4A)もしくはISO−1でのMIFの薬理的阻害(図4B)が、脾臓MNC、腹腔マクロファージ、および膵島細胞によるTNF−α産生をダウンレギュレーションし、これを、健康な非処置マウスレベルまで低下させた。我々は、細胞内でのiNOSの発現が顕著に、抗MIFもしくはISO−1で処置された動物から単離されたマクロファージにおいて、相対的に高発現のiNOSの糖尿病の動物に比べて抑えられたことをも見出した(図5A)。従って、MIFのブロックが、マクロファージならびに膵島における引き続いてのNO産生を廃止させた(図5B)。MIF阻害剤の効果が、TNF−α、iNOS、およびNOに特異的であったが、これは、脾臓細胞における細胞内IFN−γ発現が、MHCクラスII分子発現同様、コントロールの糖尿病マウスに比べて変わらないままであったからである(図6AおよびB)。
MIFは顕著に、過剰炎症および自己免疫と関連した免疫病理に寄与し、内因性MIFを、中和抗MIF抗体、もしくは、MIF遺伝子の標的とされた破壊で中和しており、数種類の炎症疾患の齧歯類モデルの進展を阻害し、免疫誘発腎疾患(27)、リーシュマニア(28)、グラム陰性およびグラム陽性敗血症(15、29、30)、抗原誘発関節炎(31)、結腸炎(16)、ならびに実験的自己免疫脳脊髄炎(EAE、17)も包含している。最近、自己免疫の仲介によるDMの進展および病理におけるMIFに関する潜在的な役割が、元より糖尿病のNODマウスにおいて示唆されてきており、これは、MIFmRNA発現が顕著に、疾患の進展の間に増加されるからであり、外因性MIF投与が、これらの動物における疾患の発症を増加させる(18)。MIFは構造的に発現され、膵臓β細胞から、インシュリンと一緒に分泌され、オートクリンの因子として振る舞い、インシュリン放出を刺激する(32)。インシュリン分泌の誘発が、β細胞上での発現および免疫細胞に対する抗原提示をしやすくすることにより、機能活性が増強される場合アップレギュレーションされる免疫炎症糖尿病原性経路に寄与すると考えられるので(12)、この<<ホルモン>>特性が、内因性MIFを、β細胞不全および破壊初期の現象に巻き込んでいく更なる重要な因子を代表し得ると考えられる。内因性MIFを標的とするとこれゆえ、1型糖尿病の病理におけるこのサイトカインの役割を解明するのに、ならびに、この病状の治療および/または予防処置に、適切なアプローチであるかも知れない。
実施例1において記載された方法を使用して、糖尿病が、成雄マウスにおいて誘発され、多数回の毒性を下回る用量のストレプトゾトシン(MLD−STZ、40mg/体重kg/日、i.p.投与、5日間連続)を、記載のとおり(21)投与された。糖尿病誘発後のMIF阻害効果が、MIFの薬理的阻害剤(20)たる(S,R)−3−(4−ヒドロキシフェニル)−4,5−ジヒドロ−5−イソキサゾール酢酸メチルエステル(ISO−1)の、用量1mg/マウス/日における14日間連続のi.p.注射により研究され、ストレプトゾトシンの最初の投与7日後に開始された。腹腔血中グルコースが、これらのマウスにおいて、および、ISO−1処理が稀釈ISO−1により置き換えられた場合を除き、同一に処置されたコントロールマウスにおいて、週毎にモニターされた。
更に1型糖尿病におけるMIFの重要性を樹立させるために、MIF−/−C57B1/6マウス(40)がストレプトゾトシン処理され、糖尿病の進展中のMIF+/+マウスと比較された。ストレプトゾトシン処理された場合、MIF−/−マウスは糖尿病を進展させず、一方、MIF+/+マウスは糖尿病を進展させた(図9)。これは更に、例えば、リボザイム、アンチセンス、siRNA等を用いてMIF発現を阻害することに関する方法が、1型糖尿病処置に有用であると期待されることをも、樹立させる。

Claims (25)
- 1型糖尿病を患っているかもしくは1型糖尿病のリスクに晒されている哺乳類を処置する方法であって、該哺乳類に、該哺乳類においてマクロファージ転移阻害因子(MIF)を阻害する物質を含む医薬組成物を投与することを含み、該物質がポリペプチドもしくはポリヌクレオチドである、方法。
- 前記物質が、MIFに特異的に結合する抗体の結合部位を含む、請求項1の方法。
- 前記物質が抗体である、請求項2の方法。
- 前記物質が、MIFに特異的に結合するアプタマーである、請求項1の方法。
- 前記物質が、MIF発現を阻害する、請求項1の方法。
- 前記物質が、前記哺乳類におけるMIFmRNAに特異的なアンチセンス核酸もしくは模倣体である、請求項5の方法。
- 前記物質が、前記哺乳類におけるMIFmRNAに特異的なリボザイム核酸もしくは模倣体である、請求項5の方法。
- 前記物質が、前記哺乳類におけるMIFmRNAに特異的な阻害RNAもしくは模倣体である、請求項5の方法。
- 前記哺乳類が、糖尿病、グルコース不寛容障害、ストレス高血糖、メタボリック・シンドローム、および/またはインシュリン耐性を患っているかもしくは患うリスクに晒されている、請求項1の方法。
- 前記哺乳類が齧歯類である、請求項1の方法。
- 前記哺乳類がヒトである、請求項1の方法。
- 化合物が、1型糖尿病を予防するかもしくは処置するのに有用であるかどうか評価する方法であって:
(a)該化合物がマクロファージ転移阻害因子(MIF)を哺乳類において阻害するかどうか決定し、次いで、もし該化合物がMIFを阻害すれば
(b)該化合物が1型糖尿病の進展を阻害するかどうか決定すること
を含む、方法。 - ステップ(b)が、前記哺乳類における脾臓リンパ球の増殖への前記化合物の効果を評価することにより実施される、請求項14の方法。
- 前記化合物が蛋白である、請求項14の方法。
- 前記蛋白が抗体結合部位を含む、請求項16の方法。
- 前記化合物が核酸もしくは模倣体である、請求項14の方法。
- 前記核酸もしくは模倣体が、アンチセンス、リボザイム、アプタマー、もしくは干渉RNAである、請求項18の方法。
- 前記化合物が1000ダルトン未満の有機分子である、請求項14の方法。
- (a)請求項1においてMIFを阻害するのに使用された物質を含む医薬組成物;ならびに
(b)該組成物を、前記哺乳類に投与するための説明書
を含み、ここで、該哺乳類が1型糖尿病を患っているかもしくは1型糖尿病のリスクに晒されている、キット。 - (a)請求項12においてMIFを阻害するのに使用された物質を含む医薬組成物;ならびに
(b)該組成物を、前記哺乳類に投与するための説明書
を含み、ここで、該哺乳類が1型糖尿病を患っているかもしくは1型糖尿病のリスクに晒されている、キット。 - 1型糖尿病を患っているかもしくは1型糖尿病のリスクに晒されている哺乳類の処置用医薬品の製造のための、請求項1においてMIFを阻害するのに使用された物質の使用。
- 1型糖尿病を患っているかもしくは1型糖尿病のリスクに晒されている哺乳類の処置用医薬品の製造のための、請求項12においてMIFを阻害するのに使用された物質の使用。
