Abstract
【解決手段】生検サンプリングまたは他の目的で少量の内部組織を切断除去する医療装置が開示される。装置の様式は、突き刺す先端部、組織収容開口部および減圧内腔を有する中空プローブと、組織収容開口部を通過する切断エッジを有する、プローブ内のカッターと、カッター駆動機構とを含む。本明細書に開示した様式で使用するカッターは、傾斜した切断先端部と、凹状研磨部を有する切断エッジとを含みうる。装置の様式はまた、減圧内腔に流体接続された吸引器と、吸引器およびカッター駆動機構の整合された作用、および、流体管理システムを含みうる。記載された様式は、対象の組織塊近傍にプローブを一回挿入している間に、組織の複数のサンプルを取り去るのに使用できる。

Description
中空プローブによる吸引を利用する装置は、生検の目的、または、治療用組織除去という目的のような他の目的のために、低侵襲皮下手技で組織のサンプルを除去および/または取得するのに有用である。
本明細書に記載および図示の装置の様式(version)は、生検サンプリングまたは他の目的のために低侵襲手技において組織を取り去るための効率的なシステムおよび方法に向けられている。特に、本明細書の装置の様式は、経皮手技で効率的に組織を引き込み、切断し、取り去るために、収容開口部を備えた中空プローブと、プローブ内にあって切断先端部を有するカッターとを有する装置を対象とする。本明細書の典型的な様式について説明されるように、カッターに傾斜していて丸くなっている切断先端部を設けると、カッターの主に軸方向の動きで、組織を効果的に切断できる。
(1)内部組織を切断、除去するための装置において、
本体と、
前記本体に取り付けられた中空のプローブであって、前記プローブ上の収容開口部、前記プローブに沿って前記収容開口部と流体連通する減圧内腔、および、突き刺し用先端部を上部に有する遠位自由端を含む、プローブと、
前記プローブ内のカッターであって、前記カッター上に切断エッジを含み、前記切断エッジが前記収容開口部の近くに位置し、前記カッターは、前記切断エッジが前記収容開口部を通過できるように前記収容開口部に対して可動である、カッターと、
前記本体によって保持されており、前記減圧内腔との流体接続部を有する吸引器と、
前記本体によって保持されており、前記カッターと機械的に連絡しているカッター駆動機構であって、前記切断エッジが前記収容開口部を横切るように前記カッターを駆動する、カッター駆動機構と、
を備え、
前記装置は、作動するために、外部の減圧源または外部の動力源に接続する必要がない、

前記カッター駆動機構が、前記本体によって保持されたばねをさらに備え、前記ばねは、前記カッターを前記プローブ内で長さ方向に駆動する、装置。

前記吸引器と前記減圧内腔との間で前記流体接続部に沿って設けられた接合点、
をさらに備え、
前記接合点は、少なくとも第1の足部、第2の足部、および第3の足部を有し、
前記吸引器が、前記第1の足部に流体が流れるように接続されており、
前記減圧内腔が、前記第2の足部に流体が流れるように接続されている、装置。
(4)実施態様3記載の装置において、
前記減圧内腔と前記接合点との間において、前記吸引器と前記減圧内腔との間で前記流体接続部に沿って設けられた第1の一方向逆止弁、
をさらに備える、
装置。
(5)実施態様4記載の装置において、
前記接合点の前記第3の足部が、前記装置から流体を排出する、装置。
前記接合点の前記第3の足部と流体が流れるように接続されている第2の一方向逆止弁、
をさらに備える、
装置。
(7)実施態様1記載の装置において、
前記吸引器が、注射器である、装置。
(8)実施態様1記載の装置において、
前記吸引器が、作動部材をさらに備え、
前記装置が、前記吸引器の前記作動部材および前記カッター駆動機構と機械的に連絡しているアクチュエータ、
をさらに備え、
前記アクチュエータの運動は、前記吸引器および前記カッター駆動機構の整合された作動をもたらす、装置。
(9)実施態様8記載の装置において、
前記アクチュエータの運動は、前記吸引器の前記作動部材、および前記カッターの同時の運動をもたらす、装置。
(10)実施態様1記載の装置において、
前記カッターが、カッター管をさらに備え、
前記装置が、前記カッター管内に収集管、
をさらに備え、
前記収集管が、前記カッター管内で長さ方向に移動可能である、装置。
前記収集管内に設けられたイジェクターロッド、
をさらに備え、
前記イジェクターロッドが、前記収集管内で長さ方向に移動可能である、装置。
(12)実施態様1記載の装置において、
前記切断エッジが、凹状研磨部を有し、
前記カッター駆動機構が、前記切断エッジが実質的に前記収容開口部を通過するように、前記プローブ内で前記カッターを長さ方向に駆動し、
前記カッターが、前記プローブ内で実質的に回転しない、装置。
Claims (10)
- 内部組織を切断、除去するための装置において、
本体と、
前記本体に取り付けられた中空のプローブであって、前記プローブ上の収容開口部、前記プローブに沿って前記収容開口部と流体連通する減圧内腔、および、突き刺し用先端部を上部に有する遠位自由端を含む、プローブと、
前記プローブ内のカッターであって、前記カッター上に切断エッジを含み、前記切断エッジが前記収容開口部の近くに位置し、前記カッターは、前記切断エッジが前記収容開口部を通過できるように前記収容開口部に対して可動である、カッターと、
前記本体によって保持されており、前記減圧内腔との流体接続部を有する吸引器と、
前記本体によって保持されており、前記カッターと機械的に連絡しているカッター駆動機構であって、前記切断エッジが前記収容開口部を横切るように前記カッターを駆動する、カッター駆動機構と、
を備え、
前記装置は、作動するために、外部の減圧源または外部の動力源に接続する必要がない、

前記接合点の前記第3の足部が、前記装置から流体を排出する、装置。 - 請求項5記載の装置において、
前記接合点の前記第3の足部と流体が流れるように接続されている第2の一方向逆止弁、
をさらに備える、装置。 - 請求項1記載の装置において、
前記吸引器が、注射器である、装置。 - 請求項1記載の装置において、
前記吸引器が、作動部材をさらに備え、
