Abstract
Description
本発明は、生理監視装置に関し、特に、生理学的測定値に対応する検出された信号の品質を定量的に推定するための処理を含み、かつ信号品質の当該推定に基づいて、臨床医に対して適切なフィードバックを与える監視装置に関する。
本発明は、生理学的パラメータを測定するために用いられる信号の品質を決定するための方法および装置を提供する。本発明の一実施形態は、パルス酸素濃度計に向けられており、測定された生理学的パラメータには、患者の脈拍数と、血液酸素飽和度とが含まれる。測定された生理学的パラメータの精度と信頼性とを示すものである信号品質は、複数の信号品質指標を組み合わせることによって計算され、それぞれの指標は、測定された信号の品質の指標である。信号品質計量値の値は閾値と比較され、この比較に基づいて、様々な決定が医療装置によってなされる。ある決定は、測定された生理学的パラメータを表示するか否かを決定するようにして、正確な測定値のみが表示されることを保証する。別の決定は、フィードバックを与えるようにして、臨床医がセンサの位置をより適切な組織位置に調節するように案内する。
本発明の実施形態は、信号品質を定量的に推定し、信号品質の推定に基づいて、測定された生理的なパラメータを表示するかどうかを決定し、および/またはフィードバックを与えて臨床医にセンサの位置の調節するための案内を行う方法およびシステムを実装する、生理モニタおよびセンサを説明する。信号品質推定は、組み合わせると信号品質の定量的な測定値を提供する複数の信号品質指標の測定に基づく。
上述したように、パルス酸素濃度計は、典型的には、患者の動脈血および心拍におけるヘモグロビンの血液酸素飽和度を含む様々な血流特性を測定するために用いられる。これらの特性の測定は、患者の血液が灌流された組織の一部に光を通過させて、当該組織内の光の吸収および散乱を光電的に感知する非観血的センサを用いることによって達成される。
上記で引用した特許において説明された技法を用いて、血液酸素飽和度および脈拍数の推定値を提供するが、数多くの中間信号が、酸素測定センサによってまず取得され、これらの回路またはアルゴリズムによって最終的に処理される際に、血液酸素飽和度および脈拍数の最終推定値を決定することとなる。これら中間信号の1つの組を、信号品質指標のために用いる。その後、信号品質指標は、組み合わされて処理されて、信号品質計量値を推定する。信号品質計量値の値は閾値と比較され、この比較に基づいて、医療装置によって様々な決定がなされる。ある決定は、測定された生理学的パラメータを表示するか否かを決定するようにする。別の決定は、フィードバックを与えるようにして、信号品質計量値が閾値を下回る場合には、臨床医がセンサの位置をより適切な組織位置に調節するように案内する。
信号品質指標は、信号の信頼性および精度を推定することが可能な、測定されたパラメータである。例えば、血液酸素飽和度をパルス酸素濃度計を用いて測定する場合には、信号品質指標は、血液酸素飽和度の値の推定値が正確なものであるかどうかを間接的に査定することができる。精度についてのこの決定は、測定された大量の値および様々な指標を完全かつ詳細に検討することによって可能となり、どの指標が信号品質を示しているか、および、指標と推定値の精度との間の相関があるとするならばどのようなものであるかを決定する。特定の指標が、ある測定値の精度と高度に相関している可能性があるものの、いくつかの指標の組み合わせが臨床条件のより幅広い範囲に渡って測定値の精度とよく相関すると決定される。これらの指標のうちのいくつかは、「OVERLAP」、「MIN‐MAX‐MIN」、「PATH LENGTH」、および「IR nAv」指標と称される。信号の品質、よって、酸素飽和度の精度を決定するためのこれらおよび他の指標ならびにユーティリティを、それぞれ以下に説明する。
上述のように、信号品質指標は、処理および組み合わされて、一括信号品質(SQ)計量値のための包括値を提供する。一括SQ値に至るために、数多くのアルゴリズムを用いてもよい。そのようなアルゴリズムの一例を以下に示し、「OVERLAP」、「MIN‐MAX‐MIN」、および「IR nAv」指標の組み合わせとして説明されてもよい。
SQ1=100−5*(96−OVERLAP)であって、0から100の間に切り上げ、
SQ2=100−(MIN‐MAX‐MIN−16000)/70であって、0から100の間に切り上げ、
SQ3=100−5*(25−IR nAv)であって、50から100の間に切り上げる。
様々な信号指標を組み合わせることによって信号品質(SQ)値を決定したら、次のタスクである、測定値が正確でパルス酸素濃度計のモニタに表示される価値のあるものかどうかを決定するのに、SQ値を閾値と比較する。上述したように、SQ値は周期的に計算されるので、周期的に変化しうる。この信号品質の変化は、患者の生理的条件の変化および生理的に無関係に生じた変化の両方によって生じうる。信号品質の生理的に無関係な変化は、センサの動き、センサの組織に対する位置決めにおける変化、およびセンサと組織位置との間にある毛によって生じるようなセンサと組織位置との間にある光障害といった要因によって生じうる。このような生理的に無関係な事項に基づく変化がより多く生じるのは、周生期パルス酸素濃度計においてであり、胎児は分娩出産の過程において子宮内で動くので、センサの胎児に対する位置決めが急速に変化しうる。

図2は、本発明に係る、上述の決定アルゴリズムの一実施形態を示すフローチャート200である。最初に、複数の信号品質指標を測定する(ステップ210)。次に、信号品質指標を処理および組み合わせて、SQ計量値を計算する(ステップ212)。上述したように、SQ値は周期的に変化し、また急速に変化してもよい。経時変化するSQ値の将来の変化を滑らかにするために、SQ値はPID処理される(ステップ214)。次に、PID処理されたSQ計量値の値を閾値と比較する(ステップ216)。値が少なくとも閾値に等しい場合には、測定された生理学的値は送出されて、装置のモニタによって表示される(ステップ218)。例えば、医療装置が表示モニタ付のパルス酸素濃度計であり、当該装置が酸素飽和度を測定している場合には、PID処理されたSQ値が閾値を超えて、測定値が信頼性を有し正確であることが示されると、装置は、酸素飽和度の値をその表示モニタに表示すると決定する。測定値(すなわち、酸素飽和度)は、PID処理されたSQ値が閾値を下回るまで、表示されたままとなる。
Claims (39)
- 医療診断装置における生理学的パラメータについての測定値の表示を制御する方法であって、
複数の信号品質指標を測定することと、
前記複数の信号品質指標を処理して、複数の処理された信号品質指標を形成することと、
前記複数の処理された信号品質指標を組み合わせて、前記複数の処理された信号品質指標の組み合わせを形成することと、
前記複数の処理された信号品質指標の前記組み合わせを備える信号品質計量値を計算することと、
信号品質閾値を設定することと、
前記信号品質計量値を前記信号品質閾値と比較することと、
前記信号品質計量値が前記信号品質閾値を超える場合にのみ、前記測定値を表示することとを含む、方法。 - 前記測定値は、酸素飽和度である、請求項1に記載の方法。
- 前記測定値は、脈拍数である、請求項1に記載の方法。
