Abstract
Description
24人のHLA−A*0201陽性ボランティアをこの試験に起用した。ボランティアのHLA多様性は、遺伝的拘束を受けるT細胞応答を集団間で比較できるようにするため、この集団で最も一般的なHLAクラスIサブタイプに限定した。このボランティアらはいずれも、以前にマラリアに接触していなかった。24人の個体のうち、10人が次の上述のPfCSPワクチン臨床治験に参加した。この治験の間、このボランティアらには、1回あたり2500μgとして、4週間の間隔を挟んで合計3回分のPfCSP DNAワクチン(VCL−2510、前述のとおりVical,Inc(米カリフォルニア州サンディエゴ)製(62))を与えた(62)。したがって、この治験で、この10人のボランティアらは、追加免疫RTS,Sワクチンを与える12〜14カ月前にDNAワクチンの最後の投与を受けた。残りの14人のボランティアは、以前にPfCSP DNAワクチンを与えられておらず、したがって非初回刺激対照として使用した。24人のボランティアは全員、PfCSP DNAワクチンとRTS,Sワクチンのどちらによる免疫感作の前にも、PfCSP、HIV、HBVコア抗原、HCV、ワクシニアウイルス、及びdsDNAに対する抗体について陰性であった。DNAによって初回刺激した10人のボランティアのうち6人、及び14人の非初回刺激対照のうち8人が、HBsAgに対する抗体について陽性であった。
上述のように、RTS,Sワクチン単独で免疫感作を施すと、ヒトでは、抗体が誘導され、CD4+T細胞依存的なIFN−γ応答が誘発されるが、抗原特異的なCTLのヒトでの誘発は報告されていない(61)。DNAによって誘導された記憶CTLが、RTS,Sワクチンによる追加免疫によって復活し得るかどうか、並びに追加免疫による応答が、DNA初回刺激によるもとの応答よりも広げられたかどうかを判定するために、異なるボランティアで抗原特異的CTLの細胞障害活性の評価を行った。RTS,Sによる免疫感作の1〜2週間前、並びにRTS,Sを最初及び/又は2回目に投与してから1又は2週間後に、DNAによって初回刺激したボランティア又は初回刺激していないボランティアの血液から末梢血単核細胞(PBMC)を収集した。次いで、これらのPBMCを、抗原提示ターゲット細胞の溶解を検出するクロム放出アッセイで使用した(105)。
インフルエンザマトリックスタンパク質由来のペプチド(残基58〜66、GILGFVFTL、HLA−A2.1;配列番号22)、又は破傷風毒素TヘルパーユニバーサルエピトープP30(残基947〜969、FNNFTVSFWLRVPKVSASHLET、DR及びDP拘束性;配列番号23)を正の対照として使用した(74)。HIVgagタンパク質由来のペプチド(残基77〜85、SLYNTVATL、HLA−A2.1拘束性;配列番号24)、又は熱帯熱マラリア原虫タンパク質由来のペプチドExp−1(残基82〜96、配列AGLLGNVSTVLLGGV、DR拘束性;配列番号25)を負の対照として使用した。
DNAによって初回刺激され、又は初回刺激を受けていない、RTS,Sワクチン投与直前の各ボランティアでは、CTLが検出されなかった。RTS,Sワクチンのみを与えらた14人の非初回刺激ボランティアではいずれも、CTL応答が検出されなかった。抗原特異的かつ遺伝的拘束を受けるCTL応答が、5/10人のDNA初回刺激ボランティアで検出された(図1c)。5人の応答者のうち1人は、最初に投与してから1週間後にCTL(V6)を得、その他の者は、RTS,Sの2回目の投与後にCTLを得た。DNA初回刺激ボランティアでのCTL応答の出現度(7/113アッセイ、6.2%)は、非初回刺激ボランティア(0/125アッセイ、0%)と比べて有意に大きかった(P=0.0047)。CTL応答の出現度は、12〜14カ月前にPfCSP DNAワクチンを3回与えた15人のボランティアにおいて、単独のDNA免疫感作の後に認められたものとほぼ同等であった(30/458アッセイ、6.6%)(107)。
RTS,Sワクチンによって追加免疫した後、CTL応答が陽性であった5人のDNA初回刺激ボランティアのうち、3人(V3、V6、及びV8)は、RTS,S追加免疫後に、DNA初回刺激後には応答がなされなかったエピトープに応答した。詳細には、CTL応答は、両方の負の対照(MHC+対照及び非MHC+ペプチド)のバックグラウンドに対する免疫感作後の特異的溶解率が≧10%である場合に限り陽性とみなした。V3及びV6では、PfCSPワクチンによるDNA初回刺激後、エピトープA2.319に対するCTL応答はなかった(図1a及び図1b)。しかし、RTS,Sによる追加免疫の後、どちらのボランティアも、このエピトープに対するCTL応答を示した(図1c)。V8は、DNA初回刺激の後、エピトープA2.319及びA3.336に対するCTL応答を得なかったが、しかしこの初回刺激を施したボランティアにRTS,Sワクチンで追加免疫を施した後に、これらのエピトープに対するCTL応答が現れた(図1a、1b、及び1c)。
IFN−γ応答の評価は、次のような標準のELISPOTアッセイによって行った。PfCSP特異的なIFN−γ産生細胞の数を、以前に記載されているとおりに、10μg/mlのペプチドの存在下で36時間in vitroで刺激した後、ELISPOTによって測定した(107)。ウェル中のサイトカイン産生細胞に相当するスポットの数(細胞生成スポット;SFC)を、自動化されたスポットカウントシステム(Scanalytics、米バージニア州Fairfax)で数えた。応答は、SFC/106PBMCの平均数として示し、1)実験ペプチドを加えてあるウェル中の平均細胞数が対照ペプチドを加えたウェルよりも有意に大きかった場合(p<0.05、スチューデントのT検定)、2)正味のSFC/ウェル(実験ペプチドウェル中の平均SFC−対照ペプチドウェル中の平均)が≧5SFC/ウェルであった場合、及び3)刺激指数(実験ペプチドウェル中の対照ペプチド中の平均SFC対平均SFCの比)が2.0より大きい場合に有意であるとみなした。さらに、免疫感作の前に得た細胞の、PfCSP特異的ペプチドに対する応答が上で定めたように陽性であった場合、免疫感作後の同じペプチドに対する応答は、陽性であるとみなさなかった。
RTS,Sは、PfCSPの部分及びB型肝炎表面抗原(HBsAg)からなる融合タンパク質である。HBsAgに対する抗体を有する個体では、アジュバント中に含めたRTS,Sによる免疫感作の、T細胞応答の誘発における効率は有意に劣っていた。この結果は、DNA初回刺激ボランティアでは、はるかに目立たなくなった。4種の15〜20アミノ酸ペプチドに対する応答にHBsAg抗体状況が及ぼした影響が顕著であったので、調査を拡大した。最初及び2回目のRTS,S免疫感作後、すべての試験時点で13種のPfCSPペプチド(pPfCSP)プールと19種のHBsAgペプチドプールをPBMC中に同時に用いて上述のようなELISPOTアッセイを行って、PfCSPとHBsAgに対する各IFN−γ応答を比較した。未処置の非DNA初回刺激個体では、HBsAg成分がT細胞応答に免疫優性であったが(HBsAg陰性の非初回刺激ボランティアでは、PfCSPの0/6人の応答者に対してHBsAgは6/6人の応答者、並びにHBsAg陽性の非初回刺激ボランティアでは、PfCSPの1/8人の応答者に対してHBsAgは7/8人の応答者)、PfCSP DNAで初回刺激すると、T細胞応答がPfCSPに向けられて、この免疫優性が釣合いをもったと思われた(表3;HBsAg陰性の初回刺激ボランティアでは、PfCSPの2/4人の応答者に対してHBsAgが2/4人の応答者、並びにHBsAg陽性の初回刺激ボランティアでは、PfCSPの4/6人の応答者に対してHBsAgが6/6人の応答者)。
PfCSP DNAワクチン又はRTS,Sワクチンは、単独でIFN−γ応答を誘発することができるので、RTS,Sワクチンの2回目の投与後、陽性応答者については、両方の群のIFN−γ応答が同等であった(8/10に対して11/14)(表1)。それでもやはり、これまでに報告したように、単独のPfCSP DNAワクチン又はRTS,Sワクチンによって誘発されたIFN−γ応答は、異なるT細胞サブタイプに応じて異なっていた。DNAによる免疫感作は、CD4+細胞及びCD8+T細胞の両方に依存的なIFN−γ応答を誘発し(107)、RTS,Sは、CD4+T細胞依存的な応答だけを誘発する(61)。
DNA初回刺激ボランティアにおけるRTS,Sワクチンでの追加免疫によって、IFN−γ産生T細胞のレパートリーの輪郭が浮かび上がった。ペプチドDR.363(CD8+T細胞エピトープを含んでいない)に対するIFN−γ応答は、単独のPfCSP DNAワクチン又はRTS,Sワクチンで免疫感作したボランティアで、CD4+T細胞にのみ依存的であった。RTS,Sによる追加刺免疫後のDNA初回刺激群の2/3人の応答者(V1及びV5)でも、DR.363に対する同じ型の応答が検出された。驚くべきことに、CD4+T細胞中のIFN−γ mRNAの発現レベルによって測定した応答規模は、DNAのみの3回の免疫感作後では、V1で7.6倍、V5で12.5倍の増加であったのに対し、RTS,Sによる追加免疫の後、ボランティアV1で94.9倍、V5で46.7倍に増大した。図3の「DNA単独」バーを参照されたい。RTS,S追加免疫後の応答の規模は、V1ではDNA免疫感作後の12.5倍、V5では3.7倍であった(図3;「DNA単独」バーと「DNA/RTS,S」バーを比較されたい)。
風乾熱帯熱マラリア原虫スポロゾイトに対する抗体応答の評価を、RTS,Sによる免疫感作前、RTS,Sを最初に投与してから2、4、6及び8週間後、2回目に投与してから1、2、4、及び6週間後に行った。抗体の力価は、これまでに記載されているような間接蛍光抗体試験(IFAT)によって決定した(33)。予想どおり、各群で抗体応答に若干の変動性があったものの、抗体力価は上等であった(図5)。力価は、2回目のRTS,S投与の4週間後に最高点に達し、幾何平均力価は、5120〜20480の範囲であった。しかし、1件を除き、どの時点でも全スポロゾイトに対する抗体力価に統計学的な有意差はなかった。最初に投与してから2週間後において、PfCSPを受けたことがなく、HBsAgに対する抗体を有するボランティア群(DNA−/HB+)の、IFATによる抗体の幾何平均力価3225が、PfCSPを受けたことがあり、HBsAgに対する抗体を有するボランティア(DNA+/HB+)の幾何平均力価718.4よりも大きかった(P=0.02)。これは主に、予定されていたRTS,Sによる2回目の免疫感作のちょうど前の6週間の時点の後に試験を辞退した、DNA−/HB+群の1人のボランティアの高い力価10240によるものであった。2回目のRTS,S投与後、各群間に統計学的有意差は存在しなかった。
熱帯熱マラリア原虫、結核菌(Mycobacterium tuberculosis)、及びHIVのような感染に対する有効で持続性のあるワクチン開発の経過は、予想以上に手間取り、困難であり、かつ複雑であることがわかった。上記の本発明の分析は、PfCSP DNAによる初回刺激とRTS,Sによる追加免疫とによって、免疫系の細胞性及び液性の両部門による応答が誘発されることを示すものである。さらに、HBsAgに対する抗体を有する個体の中で、PfCSP DNAワクチンによる初回刺激を受けた個体は、RTS,Sアジュバント添加ワクチンの投与後、PfCSP DNAを受けたことのないボランティアよりも良好なT細胞応答を生じた。マラリアワクチン又は他のワクチンの大抵の受容者は、免疫感作又は感染によるHBsAgに対する抗体をもつことになるので、このことは、この初回刺激追加免疫による免疫感作戦略の重要な強みとなり得る。

Claims (44)
- マラリアを引き起こす病原体に対するヒトの免疫感作方法であって、
a)少なくとも1種の第1のマラリア抗原をコードしている少なくとも1種のポリヌクレオチドを含む初回刺激ワクチンを、その応答を確立するのに十分な初回刺激量で投与して、ヒトの免疫応答を初回刺激するステップと、
b)続いて、少なくとも1種の第1のマラリア抗原と共通する少なくとも1種のエピトープを有する少なくとも1種の第2のマラリア抗原を含む少なくとも1種のポリペプチドを含む追加免疫ワクチンを、その初回刺激による免疫応答を追加刺激するのに十分な追加刺激量で投与して、前記ヒトの前記の初回刺激による免疫応答を追加刺激するステップとを含み、
前記初回刺激ワクチンの投与がCD8+T細胞を初回刺激し、前記追加免疫ワクチンの投与が、初回刺激によるCD8+T細胞を復活させ、前記初回刺激によるCD8+T細胞の応答を広げ、さらに抗マラリアCD8+T細胞、抗マラリアCD4+T細胞、及び抗マラリア抗体の産生をもたらす方法。 - 前記第2のマラリア抗原が、前記第1のマラリア抗原の全部又は一部を含む、請求項1に記載の方法。
- 前記初回刺激量が、0.01μg〜50mgである、請求項1に記載の方法。
- 前記初回刺激量が2500μgである、請求項3に記載の方法。
- 前記初回刺激量を、前記第2のワクチンの投与前に1回〜5回投与する、請求項3に記載の方法。
- 前記追加刺激量が1μg〜100μgである、請求項1に記載の方法。
- 前記追加刺激量が50μgである、請求項6に記載の方法。
- 前記追加刺激量が25μgである、請求項6に記載の方法。
- 前記初回刺激ワクチンを、IM、IV、ID、皮下、経粘膜、組換え細菌、組換えウイルス、若しくは遺伝子銃、又はこれらの組合せから選択される方法によって投与する、請求項1に記載の方法。
- 前記追加免疫ワクチンを、IM、IV、ID、皮下、経粘膜、組換え細菌、組換えウイルス、若しくは遺伝子銃、又はこれらの組合せから選択される方法によって投与する、請求項1に記載の方法。
- 前記CD8+T細胞が、細胞障害性Tリンパ球を含む、請求項1に記載の方法。
- 前記の第1のマラリア抗原が、スポロゾイト周囲ポリペプチドの少なくとも断片を含む、請求項1に記載の方法。
- 前記の第2のマラリア抗原が、スポロゾイト周囲ポリペプチドの少なくとも断片を含む、請求項1に記載の方法。
- 前記初回刺激ワクチンがPfCSPを含む、請求項1に記載の方法。
- 前記追加免疫ワクチンがRTS,Sを含む、請求項1に記載の方法。
- 前記第1のマラリア抗原が、スポロゾイト周囲タンパク質の実質的に全部を含み、前記第2のマラリア抗原がRTS,Sを含む、請求項1に記載の方法。
- 前記のマラリアを引き起こす病原体が熱帯熱マラリア原虫(P.falciparum)である、請求項1に記載の方法。
- 請求項1に記載の方法で使用するワクチンであって、少なくとも1種の第1のマラリア抗原をコードしている少なくとも1種のポリヌクレオチドを含む初回刺激組成物と、少なくとも1種の第2のマラリア抗原をさらに含む少なくとも1種のポリペプチドからなる追加免疫組成物とを、別々の構成部分として含むワクチン。
- 前記ポリヌクレオチドがスポロゾイト周囲タンパク質の実質的に全部をコードしている、請求項18に記載のワクチン。
- 前記初回刺激組成物がPfCSPを含み、前記追加免疫組成物がRTS,Sを含む、請求項18に記載のワクチン。
- 前記のRTS,Sを含む組成物がさらにアジュバントを含む、請求項18に記載のワクチン。
- マラリアを引き起こす病原体に対するヒトの免疫感作用のキットであって、
a)少なくとも1種の第1のマラリア抗原をコードしている少なくとも1種のポリヌクレオチドを含む初回刺激ワクチンと、
b)前記の少なくとも1種のマラリア抗原と共通する少なくとも1種のエピトープを有する少なくとも1種の第2のマラリア抗原をさらに含む少なくとも1種のポリペプチドからなる追加免疫ワクチンと
を含み、前記初回刺激ワクチンの投与がCD8+T細胞を初回刺激し、前記追加免疫ワクチンの投与が、初回刺激によるCD8+T細胞を復活させ、前記初回刺激によるCD8+T細胞の応答を広げ、さらに抗マラリアCD8+T細胞、抗マラリアCD4+T細胞、及び抗マラリア抗体の産生をもたらすキット。 - 前記初回刺激ワクチンがPfCSPを含む、請求項22に記載のキット。
- 前記追加免疫ワクチンがRTS,Sワクチンを含む、請求項22に記載のキット。
- マラリアを引き起こす病原体に対するヒトの免疫感作用のキットであって、
a)CSタンパク質の実質的に全部又はその断片をコードしている少なくとも1種のポリヌクレオチドを含む初回刺激ワクチンと、
b)CSタンパク質の実質的に全部又はその断片を含む少なくとも1種のポリペプチドを含む追加免疫ワクチンと
を含むキット。 - 前記初回刺激ワクチンがPfCSPを含む、請求項25に記載のキット。
- 前記追加免疫ワクチンが、CSタンパク質のC末端部分の実質的に全部、免疫優性領域の4箇所以上の縦列型反復部分、及びB型肝炎ウイルス由来表面抗原(HbsAg)を含むハイブリッドタンパク質を含む、請求項25に記載のキット。
- 前記追加免疫ワクチンが、RTS,S、及びTh1を誘導するアジュバントを含む、請求項27に記載のキット。
- CSタンパク質の実質的に全部又はその断片をコードしている少なくとも1種のポリヌクレオチドを含む初回免疫組成物と、CSタンパク質の実質的に全部又はその断片を含む少なくとも1種のポリペプチドを含む追加免疫組成物とを、別々の構成部分として含むワクチン。
- 前記初回刺激組成物がPfCSPを含む、請求項29に記載のワクチン。
- 前記追加刺激組成物が、CSタンパク質のC末端部分の実質的に全部、免疫優性領域の4箇所以上の縦列型反復部分、及びB型肝炎ウイルス由来表面抗原(HbsAg)を含むハイブリッドタンパク質を含む、請求項29に記載のワクチン。
- 前記追加免疫組成物が、RTS,S、及びTh1を誘導するアジュバントを含む、請求項31に記載のワクチン。
- マラリアを引き起こす病原体に対するヒトの免疫感作方法であって、
a)CSタンパク質の実質的に全部又はその断片をコードしている少なくとも1種のポリヌクレオチドを含む初回刺激ワクチンを、その免疫応答を確立するのに十分な初回刺激量で投与して、ヒトの免疫応答を初回刺激するステップと、
b)続いて、CSタンパク質の実質的に全部又はその断片を含む少なくとも1種のポリペプチドを含む追加免疫ワクチンを、その初回刺激による免疫応答を追加刺激するのに十分な追加刺激用量で投与して、前記ヒトの前記の初回刺激による免疫応答を追加刺激するステップと
を含む方法。 - 前記初回刺激ワクチンがPfCSPを含む、請求項33に記載の方法。
- 前記追加免疫ワクチンが、前記CSタンパク質のC末端部分の実質的に全部、免疫優性領域の4箇所以上の縦列型反復部分、及びB型肝炎ウイルス由来表面抗原(HbsAg)を含むハイブリッドタンパク質を含む、請求項33に記載の方法。
- 前記追加免疫ワクチンが、RTS,S、及びTh1を誘導するアジュバントを含む、請求項35に記載の方法。
