Abstract
グルタミン合成酵素の阻害に影響されやすい疾患及び症状を治療するためのL−メチオニンS−スルホキシイミンの新規な投与形態が開示されている。新規な投与形態は、1週間の期間にわたって体重に基づいて10mg/kgを超えない、好ましくは8mg/kgを超えない、より好ましくは5mg/kgを超えない、最も好ましくは2.5mg/kgを超えないようにすべきである。
Description
本発明は、霊長類の進行性高アンモニア血症性脳症を治療するためのL−メチオニンS−スルホキシイミンの新規な投与形態に関する。
「The Relative Effect of Methionine Sulfoximine on Different Animal Speicies」と題するGersoffの参考文献には、ラットへのメチオニンスルホキシイミン(以下「MSO」)の腹腔内注射、雑種仔犬へのMSOの皮下注射、及びサルへのMSOの腹腔内注射に関する実験の結果が報告されている。実験から、イヌ、ラット及びサルに対するMSOの最小中毒量が決定された。ラット及びサルは、イヌよりも約100倍MSOに対する耐性があることがさらに明らかになった。MSOで誘発された発作は、メチオニンの投与によって抑制できることも発見された。MSO(発作を引き起こす)に対するメチオニン(発作を防止する)の量の比は、サル及びイヌで決定された。上述のように、Gershoffは、毒性に対する種特異性を示し、イヌは特に敏感であるがサル(霊長類)はそうでないことも示している。50及び100mg/kgの用量のMSOの異性体混合物に耐え得ることも示された。

「Effect of Methionine Sulfoximine in Man」と題するKrakoffの参考文献は、高度進行型の切除不可能な癌に罹患している入院患者へのMSOの投与に関する。MSOは、6又は8時間毎に分割量で投与した。この実験の結果では、肝及び腎機能ならびに血液学的状態に変化がなかった。さらに、腫瘍縮退の証拠は発見されなかった。MSOは、幻覚、失見当識、及び著しい興奮の形で中枢神経系に影響を与えることが指摘された。著者により、症状は累積量よりも1日量のサイズに関連していることが示唆された。報告された実験では、実験を開始する前に、各患者について、それらの肝臓、腎臓、及び血液学的状態の臨床検査評価を行った。これらの評価は、実験の経過中、一定の間隔で続けられ、各対象は、腸性造影的に、又は腫瘤の直接観察によって測定可能な腫瘍退縮の証拠について監視された。腫瘍退縮の証拠は指摘されなかった。
異常な精神状態がそれ自体に、かつメチオニンを追加しないで治ったことも指摘された。この参考文献では、試験対象が痙攣を起こさなかった、可能性が最も高い理由が、痙攣状態に達する前にMSOをやめたことであることも開示された。メチオニン類似体がそれらの毒作用をもたらす正確なメカニズムは知られてないことが述べられていたが、この論文では、MSOがタンパク質への他のアミノ酸の取り込みを抑制するようであり、この抑制がグルタミンによって妨げられることが開示された。この論文では、グルタミン酸からのグルタミンの合成及びグルタミル基の移動がMSOによって抑制されることが続けて開示された。しかし、この文献は、グルタミンに対するMSOの関係がMSOの神経毒性又はグルタミン拮抗薬アザセリン及びDONの腫瘍抑制作用の強化を引き起こすかどうか開示することができなかった。最後に、ヒトでのMSO及びメチオニンの作用が同一ではないことが指摘された。

「Free Glutamine Level in the Rat Brain InVivo After Methionine Sulphoximine Administration」と題するFolbergrovaの参考文献は、脳内の遊離グルタミン合成の抑制がMSI(この文献でのMSOの略語)の神経毒性作用及び発作の発症に関連している可能性があるかどうか研究した。MSIの投与により、てんかんと特徴付けることができる状態が生じることが開示された。しかし、この状態を引き起こすメカニズムは、依然として不明であった。MSIは脳組織内の遊離グルタミンの合成を抑制し、MSI発作時の動物の脳組織ではグルタミンのレベルが著しく低下していることが開示された。MSIを100mg/kg及び50mg/kgの用量で注射した場合、用量が両方とも閾下であるため、ラットは発作性症状を20時間示さず、対照動物と全く異ならないことが指摘された。グルタミンのレベルが、発作量を与えたラット脳と同じ程度まで低下したのは、MSIの投与から20時間後であったことがさらに指摘された。この論文では、発作量を用いた結果に基づき、発作の発症が障害性グルタミン合成に関連している可能性があるようであることが結論付けられた。

「Hyperammonaemia Does Not Impair Brain Function in the Absence of Net Glutamine Synthesis」と題するHawkinsの参考文献では、5〜200mg/kg体重の異なる用量のMSOをラットの腹膜腔内に注射する一連の実験が開示された。ウレアーゼの注射又は門脈大静脈吻合によって引き起こされる高アンモニア血症で見られる物質代謝が最初の2日以内に生じたことが指摘された。MSOを1回腹腔内注射すると、2時間以内に脳のアンモニア濃度が高まることも指摘された。用量範囲に対する応答は、Hawkinsの参考文献の図1に開示された。このデータは、正常率ではなく阻害率として、添付の図3に転載されている。グルタミン合成酵素活性は、用量に比例して低下し、酵素活性の低下は、脳のグルタミン含有量の平行低下を伴うことが指摘された。仮定したように、血漿のアンモニア値は用量に応じて増加し、200mg/kg体重の最大用量時を除いて毒性の証拠はなかった。静脈内に投与すると、より少ない用量のMSOでより多い腹腔内用量と同じ程度に脳のグルタミン合成酵素活性が低下することも開示された。
最後に、この論文でラットに投与したIV薬物は、30mg/kgの投与量であり、その霊長類への毒性は不明であることが認められるはずである。Gershoffの参考文献に見られるように、霊長類への100及び50mg/kgの投与では、試験対象に対する副作用はなかった。しかし、より少ない投与量(30mg/kgなど)における作用はGershoffによって開示されておらず、Hawkinsのラットの試験はこの判断に役立たない。さらに、Hawkinsの参考文献では使用したMSO異性体が開示されていないが、通常の技術の1つにより、使用したLSMSO異性体はHawkinsのデータ及び本発明のデータと適合性があると結論付けることができる。この参考文献は、ラットのLSMSO用量が20mg/kg未満では、グルタミン合成酵素の阻害が劇的に低減することを示す。さらに、Hawkinsが使用したIVプロトコルでは、本発明で開示した投与量より著しく高い30mg/kgのLSMSO(推定)が使用されたことが指摘された。

「Inhibition of Brain Glutamine Accumulation Prevents Cerebral Edema in Hyperammonemic Rats」と題するTakahashiの参考文献では、L−メチオニンS−スルホキシイミンを用いた前処理によるグルタミン合成酵素活性の阻害により、高い血漿アンモニウムレベルにもかかわらず脳のグルタミンレベルの増大も脳の含水量の増加も防止されたことが開示された。高アンモニア血症の期間の脳浮腫は、グルタミンの蓄積に関係することが明らかになった。他と一致して、脳内では、MSO前処理により、グルタミン合成酵素活性の64%が阻害されたことも開示された。グループIの対照ラットと比べてグループIIIの高アンモニア血症ラットでは、皮質のグルタミンレベルが3.3倍増大したことも指摘された。それとは対照的に、グループIVの高アンモニア血症ラットでは、MSO前処理によって皮質のグルタミンレベルは増大しなかった。皮質のグルタミン酸レベルは、MSOで処理していない高アンモニア血症ラットにおいてのみわずかに低下した。さらに、試験対象をMSOで前処理した場合に皮質組織の比重は減少しなかった。加えて、対象をMSOで前処理することによって脳水の増加も防止された。MSO前処理の結果、皮質のグルタミン増加が防止され、脳内でのMSOの作用は、グルタミン合成酵素に対して特異的なようであることが結論付けられた。この論文のH827頁の表1で報告されたデータは、参照によりここに組み込まれている。

脳浮腫、脳幹圧迫及び死は、制御されていない臨床的な高アンモニア血症の避けられない結果である。高アンモニア血症性脳症の病態生理は、脳浮腫からなることが十分に確立されている。脳浮腫は、すべて腫脹性星状細胞からなることを示唆する多くの証拠もある。星状細胞は、高アンモニア血症に誘発されたグルタミン合成の結果膨脹し、蓄積されたグルタミンは、細胞内に水を移動させる細胞内オスモライトとして働くことが分かっている。
1985年に、Brusilowは、高アンモニア血症に伴う脳浮腫が、グルタミン合成酵素によって触媒された星状細胞による脳のグルタミン蓄積の結果であり、その主な脳部位が星状細胞であるという仮説を提案した。したがって、脳のグルタミン合成酵素の阻害により、星状細胞によるグルタミン蓄積が防止され、それによって星状細胞の腫脹及び高アンモニア血症状態における脳浮腫が防止されるはずである。MSOは、7日もの間脳内で持続し得る阻害作用を有する、グルタミン合成酵素のよく知られている強力な阻害剤である(Lamarの638ページ)。しかし、霊長類におけるグルタミン合成酵素に対するL−メチオニンS−スルホキシイミンの作用に関する用量応答研究は公表されていない。
高アンモニア血症によって誘発される脳浮腫は、グルタミン合成を阻害し、それによって星状細胞内でのグルタミン蓄積を防止することによって防止できることが実証されている。この阻害は、グルタミン合成酵素阻害剤であるL−メチオニンS−スルホキシイミンの非経口投与によってもたらすことができる。

MSOの痙攣性作用は、そのGS阻害作用と関連があると考えられているが、2つの作用は無関係であり、MSOは有用な薬物である可能性があることを示唆する説得力のある証拠がある。例えば、ラット脳の平均グルタミンレベル(GS活性の間接測定)は、MSO用量(mg/kg)200、100、及び50で同じ程度まで低下するが、低い方の2用量を与えたラットは、「対照動物と全く異ならなかった」が、最高量を与えた動物は発作を起こした(非特許文献3参照のこと)。
しかし、投与した立体異性体が充分に説明されていないので、これら及び他の多くの研究におけるMSOの正確な投与量は不明である。Sellingerの参考文献でも、L−メチオニンは、MSOの痙攣作用からラットを保護したが、MSOによるGSの阻害に影響を与えなかったことが実証された。さらに、Sellingerは、脳室内に投与した場合のMSOの作用について開示した。MSOの異性体混合物の臨床効果は、2種類の霊長類試験で評価されている。サルは、MSO異性体混合物の50及び100mg/kgの範囲の用量に「観察可能な作用なしで」耐えることが分かっているが、GS活性は測定されなかった(Krakoffを参照のこと)。さらに、MSOの異性体混合物を末期症状の癌患者に1日量で与えた場合、2日後に興奮、失見当識及び幻覚などの著しい副作用が確認された(Krakoffを参照のこと)。この研究の判断は、患者の悲惨な状態及び1日量の潜在的な累積作用によって妨げられた。少なくとも7日間の阻害期間があるグルタミン合成酵素の不可逆的な阻害剤には、レジメンはおそらく必要ではない(Lamar及びFolbergrovaの参考文献を参照のこと)。MSOに対して種特異的応答があるので、霊長類でのGS活性及び臨床症状に対する活性異性体、L−メチオニンS−スルホキシイミンの用量応答評価から、てんかん発作作用なしにGS活性を阻害し、それによって病因に関係なく高アンモニア血症性脳症を治療するのに有用な用量が明らかになるだろう。
高アンモニア血症における脳浮腫は、高アンモニア血症によって誘発される細胞内グルタミン合成の結果である。グルタミンはオスモライト(osmolyte)として働き、細胞に水を浸入させ、脳腫脹(脳浮腫)を引き起こす。
MSOを用いてグルタミン合成を阻害することにより、高アンモニア血症の間にグルタミンが蓄積されないことが知られている。その結果、グルタミンは細胞内有機オスモライトとして働くことができず、それによって脳浮腫が防止される。
上述のように、MSOの1つの残念な副作用は、その既知の痙攣薬としての特性である。十分な量では、この化合物はヒト、ならびに他の動物で痙攣を引き起こすことが証明されている。しかし今回、LSMSOは、脳腫脹を防止し低減させる能力を依然として維持しながら、限られた量で投与できることが発見された。これらの限られた用量の結果、腫脹が低減され、痙攣性の副作用が無くなる。今回特許請求しているMSOの活性LS異性体の投与量は、グルタミン合成酵素の必要な阻害をもたらし、発作などの有害な神経性の徴候を生じない。この作用をもたらすL−メチオニンS−スルホキシイミンの新規な投与量が、本出願の主題である。
本発明は、進行性高アンモニア血症性脳症に罹患している霊長類、特にヒトを治療する方法を開示する。特許請求する方法には、MSOによって生じる痙攣性の副作用なしで脳の腫脹を緩和し低減させるのに十分な10mg/kg以下の投与量でL−メチオニンS−スルホキシイミン(以下「LSMSO」)を投与することが含まれる。
上述のように、MSOの1つのよく知られている残念な副作用は、それが痙攣性であることである。この特性により、脳症の治療に使用しないことになっている。今回、LSMSOを単独で、低用量で投与した場合、発作が生じないことを発見した。これらのより低用量のLSMSOはまた、その投与により脳のグルタミンレベルが低下したので、脳のグルタミン合成酵素を阻害すると考えられる。同様に、これらの発作を引き起こさない用量のLSMSOは、高アンモニア血症状態の対象における脳浮腫を治療又は防止するのに使用することができる。
特許請求する本発明には、様々な段階の進行性高アンモニア血症性脳症の対象を、10mg/kg以下、好ましくは8mg/kg以下、より好ましくは5mg/kg以下、最も好ましくは2.5mg/kg以下の低用量のLSMSOを用いて治療することが含まれる。LSMSOは、対象、例えば、症候性慢性低度高アンモニア血症の患者、血漿のアンモニウムレベルが上昇した対象、さらに高アンモニア血症を発生するリスクが高い対象に投与することができる。この治療は、任意のさらなる脳のグルタミン蓄積を防止し、したがって脳浮腫の任意のさらなる増加を防止すると考えられる。本明細書において報告する進行性高アンモニア血症性脳症の治療で得られた結果から、グルタミン合成酵素の阻害が有益となる他の疾患に罹患している患者にもLSMSOを本明細書で指定した量で投与できることが示唆される。但し、本発明は、高度進行型の切除不可能な癌に罹患している患者への使用を対象としていない。
特許請求する本発明はまた、脳腫脹の低減、発作の最小化又は防止、グルタミン合成酵素の阻害、及び神経症状の最小化又は防止をもたらす。
LSMSOは、対象に経口的、静脈内、又は皮下に投与することができる。IV製剤は、無菌の緩衝生理食塩水として投与できるが、経口製剤は、標準的な薬剤製造技術によって標準的な薬剤として許容される賦形剤を用いて調製することになる。
本発明の化合物を含む薬剤組成物を調製する場合、不活性な薬剤として許容される担体を使用する。薬剤用担体は、固体でも液体でもよい。固体形態の製剤には、例えば、散剤、錠剤、分散性顆粒、カプセル剤、及びカシェ剤がある。
固体担体は、希釈剤、矯味剤、可溶化剤、滑剤、懸濁化剤、結合剤、又は錠剤崩壊剤としても働き得る1種又は複数の物質であってよい。これは封入物質であってもよい。
散剤の場合、担体は一般に、微粉砕した有効成分との混合物である微粉砕した固体である。錠剤の場合、活性化合物を適切な割合で必要な結合特性を有する担体と混合し、所望の形状及びサイズに成形する。
散剤及び錠剤は、有効成分約5%〜約70重量%を含むことが好ましい。適当な担体には、例えば、炭酸マグネシウム、ステアリン酸マグネシウム、タルク、ラクトース、糖、ペクチン、デキストリン、デンプン、トラガカントゴム、メチルセルロース、カルボキシメチルセルロースナトリウム、低融点蝋、ココアバターなどがある。
薬剤組成物には、活性化合物の製剤を、有効成分(他の担体を含んで又は含まずに)が担体で囲まれそれと会合した状態のカプセル剤をもたらす担体としての封入物質と一緒に含めることができる。類似の方法で、カシェ剤も含まれている。錠剤、散剤、カシェ剤、及びカプセル剤は、経口投与に適した固体投与形態として使用することができる。
液体薬剤組成物には、例えば、経口もしくは非経口投与に適した液剤、又は経口投与に適した懸濁剤及び乳剤がある。有効成分の無菌水剤又は水、エタノール、もしくはプロピレングリコールを含む溶媒中の有効成分の無菌液剤は、非経口投与に適した液体組成物の例である。
無菌液剤は、有効成分を所望の溶媒系に溶解し、次いで得られた溶液をメンブレンフィルターに通してそれを滅菌することによって、あるいは、無菌条件下で無菌の化合物をあらかじめ滅菌した溶媒に溶解することによって調製することができる。
経口投与のための水性液剤は、有効成分を水又は他の適切な溶媒に溶解し、要望通りに適当な香料、着色剤、安定剤、及び増粘剤を加えることによって調製することができる。経口使用のための水性懸濁剤は、天然又は合成ゴム、樹脂、メチルセルロース、カルボキシメチルセルロースナトリウムなどの粘性物質、及び製剤技術分野で周知の他の懸濁化剤と一緒に、微粉砕した有効成分を水に分散させることによって作製することができる。
薬剤組成物は、単位投与形態であることが好ましい。こうした形態では、組成物を、適切な量の活性LSMSOを含む1回分に分割する。単位投与形態は、包装した製剤、例えば、小包にした錠剤、カプセル剤、及びバイアル又はアンプルに入れた散剤であってよく、包装には分離量の製剤が含まれる。単位投与形態は、カプセル剤、カシェ剤、又は錠剤自体であってもよく、あるいはこれらの包装した形態のいずれかの適切な個数であってよい。
脳浮腫を治療するための薬剤組成物として、本発明の化合物は一般に、体重約70kgの正常なヒトの成人に対して1日当たり10.0mg/kg以下、好ましくは1日当たり8.0mg/kg以下、より好ましくは1日当たり5.0mg/kg以下、最も好ましくは1日当たり2.5mg/kg以下の投与量レベルで対象に投与できるような薬剤組成物である。こうした正常なヒトの成人の場合、これらの量は、それぞれ700mg、560mg、350mg、及び175mgの投与量に相当する。しかし、使用した特定の投与量は、患者の要求及び治療対象の症状の重症度に応じて異なっていてよい。特定の状況に対する最適投与量の決定は、当技術分野の技術の範囲内である。
実施例1
グルタミン合成酵素の阻害に対する低用量静脈内L−メチオニンS−スルホキシイミン(LSMSO)の作用を、ラット脳及びアカゲザル脳で試験し、Hawkins(1991年)のデータと比較した。屠殺前の24時間、薬物を注射可能にするための短時間の麻酔以外、ラットは覚醒しており無症状であった。LSMSOを5及び10mg/kgの用量で与えたサルは、屠殺前に4時間麻酔をかけた。1匹のサルに、2.5mg/kg用量のMSOを与え、薬物を注射可能にするために短時間麻酔をかけたが屠殺前の24時間覚醒した。このサルの活動性及び食欲は、屠殺前の24時間正常であった。この試験の結果を図1に示す。しかし、霊長類は、低用量のMSOのグルタミン合成酵素阻害作用に対して感受性がより高いことが図1に表される結果から明らかである。これは、MSOのてんかん発作作用の最小化又は防止を可能にする有利な作用であり得る。

Claims (37)
- 霊長類の進行性高アンモニア血症性脳症を治療する方法であって、
治療が必要な霊長類に、腫脹を低減させ、発作を最小限に抑える又は予防するのに有効な量のL−メチオニンS−スルホキシイミンを投与することを含み、前記有効な量が10.0mg/kg以下である方法。 - 霊長類がヒトである請求項1に記載の方法。
- 治療が必要な対象が脳腫脹に罹患している請求項2に記載の方法。
- L−メチオニンS−スルホキシイミンの有効量が、1週間の期間にわたって投与される最大量である請求項2に記載の方法。
- L−メチオニンS−スルホキシイミンの有効な量が8.0mg/kg以下である請求項4に記載の方法。
- L−メチオニンS−スルホキシイミンの有効な量が、5mg/kg以下である請求項4に記載の方法。
- L−メチオニンS−スルホキシイミンの有効な量が、2.5mg/kg以下である請求項4に記載の方法。
- 霊長類対象の進行性高アンモニア血症性脳症を治療する方法であって、
症候性慢性低度高アンモニア血症の対象に、グルタミン合成酵素を阻害し、神経症状を最小限に抑える又は予防するのに有効な量のL−メチオニンS−スルホキシイミンの用量を投与することを含み、前記有効な量が10.0mg/kg以下である方法。 - 霊長類がヒトである請求項8に記載の方法。

- 成人のヒトの進行性高アンモニア血症性脳症を治療するのに適した薬剤単位用量であって、
(1)700mg以下の量のL−メチオニンS−スルホキシイミン、及び、
(2)許容される薬理学的賦形剤、
を含み、腫脹を低減させ、発作を最小限に抑える又は予防するのに有効な薬剤単位用量。 - 560mg以下の量のL−メチオニンS−スルホキシイミンを含む請求項26に記載の単位用量。
- 350mg以下の量のL−メチオニンS−スルホキシイミンを含む請求項26に記載の単位用量。
- 175mg以下の量のL−メチオニンS−スルホキシイミンを含む請求項26に記載の単位用量。
- 腫脹を低減させ、発作を最小限に抑える又は予防するのに有効な量でL−メチオニンS−スルホキシイミンを投与することによって、脳腫脹に罹患している成人のヒトの進行性高アンモニア血症性脳症を治療するための薬剤組成物を調製するためのL−メチオニンS−スルホキシイミンの使用方法であって、
L−メチオニンS−スルホキシイミンの前記有効な量が700mg以下である使用。 - 薬剤組成物が、560mg以下のL−メチオニンS−スルホキシイミンを含有する請求項22に記載の使用方法。
- 薬剤組成物が、350mg以下のL−メチオニンS−スルホキシイミンを含有する請求項22に記載の使用方法。
- 薬剤組成物が、175mg以下のL−メチオニンS−スルホキシイミンを含有する請求項22に記載の使用方法。
- グルタミン合成酵素の阻害に影響されやすい疾患を治療する方法であって、
かかる治療が必要な対象に薬剤有効量のLSMSOを投与することを含み、
LSMSOの前記薬剤有効量が1週間の期間にわたって10mg/kgを超えない方法。 - LSMSOの薬剤有効量が1週間の期間にわたって8mg/kgを超えない請求項43に記載の方法。
