Abstract

【課題】 侵襲の極めて少ない用途での使用に特に適した、高さ調節可能な椎間インプラントを提供する。
【解決手段】 2つの椎骨間に挿入する椎間インプラント(1)は、隣接する接触表面(12,22)を有する第1の部分(10)および第2の部分(20)ならびに誘導装置(15,25)を含む。誘導装置は、第1の部分と第2の部分との接触表面が互いの上で滑動することにより、第1の部分の第1の端部(13)が第2の部分の第1の端部(23)と向かい合うような第1の位置から、第1の部分の第1の端部が第2の部分の第2の端部(24)と向かい合い、かつ、第1の部分の第2の端部(14)が第2の部分の第1の端部(23)と向かい合うような第2の位置まで動くのを導き、椎間インプラントの高さを調節することができる。

Description

本発明は高さ調節可能な椎間インプラントに関する。
椎間インプラントが採用されるのは、主に椎間板が除去された後に、椎骨間の空間を安定化するためである。
この型の椎間インプラントは、EP 0 977 529 B1に記載されており、ほぼ直方体のような形状で、2つの側面壁と、1つの正面壁と、1つの背面壁とを含んで提供される。底面と頂面は開いた状態で設けられる。4つの壁によって囲まれる、椎間インプラントの中空空間は、頂面または底面に面する表面のある少なくとも1つの要素ならびに正面壁および背面壁に設けられて、表面が頂面または底面を超えて突出しない第1の位置と、表面が頂面または底面を超えて突出する第2の位置との間を移動可能になるような調節要素を含む。これにより、椎間インプラントの最終的な高さが調節可能になる。
さらに、1つの部分として作られたバナナ形の椎間インプラントが既知である。しかしながら、この椎間インプラントは高さ調節可能ではない。

したがって、本発明の課題は、侵襲の極めて少ない用途での使用に特に適した、高さ調節可能な椎間インプラントを提供することである。
この課題は請求項1に記載の椎間インプラントにより解決される。本発明のさらなる展開は、従属項に特徴付けられている。
この椎間インプラントにおいては、組立てた状態で、接触表面を互いの上で滑動させて、第1の部分と第2の部分を互いに相対して動かすことによって、インプラントの高さを変えることができる。
第1の部分と第2の部分との互いに相対した滑動を、好ましくはねじ状または螺旋状の運動で行なうことにより、椎間インプラントは侵襲の極めて少ない用途での使用に特に適する。
終端止め部は、好ましくは設けられるが、第1の部分と第2の部分との互いに相対した滑動が終端位置を超えて行われるのを防ぐ。
オートロック機構は、好ましくは設けられるが、第1の部分と第2の部分とが互いに相対して偶発的に動くのを防ぐ。
本発明の付加的特徴および利点は、付随する図面に基づく実施例の説明から明らかである。
本発明の一実施例による椎間インプラントを図1から図6に関連付けて次に示す。
図1および図2から明らかなように、椎間インプラント1は、この特定の実施例では円弧形で設けられる、2つの湾曲した部分10、20からなる。図6に最もよく示すように、2つの部分10、20は厚い端部14、24をそれぞれ有する向かい合ったテーパのある楔部分として設けられ、その端部が、楔のベースおよび厚い端部と反対側のテーパ状の端部13、23を形成する。この配置においては、2つの部分10、20はその楔表面12、22のいずれか一方でそれぞれ互いに接続しているので、接触表面とはここでこれらの楔表面を指す。したがって、側面から見ると、椎間インプラント1はほぼ長方形である。2つの部分10、20を互いに相対してその接触表面12、22に沿って動かすことにより、椎間インプラント1の全体の高さが調節できる。
図3に最もよく示すように、第1の部分10は、第1の接触表面12と、接触表面12の裏側の第1の表面11とを含む。第1の接触表面12はねじ領域または螺旋領域の部分の形で設けられる。断面がアリ形の隆起15が、表面から直角に突出するように第1の接触表面12に設けられる。隆起15はねじ状または螺旋状になるよう設けられる。隆起15は、第1の部分10の、楔のベースを形成する第2の端部14から始まり、第1のテーパ状の端部13より所定の距離の位置まで延在する。
図4に最もよく示すように、第2の部分20は、第2の接触表面22と、接触表面22の裏側の第2の表面21とを含む。第2の表面22はさらにねじ領域または螺旋領域の部分の形で設けられ、第1の接触表面12の同じ領域を補完する。隆起15の断面に対応するアリ形の断面を有する溝25が、第2の接触表面22に設けられる。溝25はねじ状または螺旋状になるよう設けられる。溝25は、第2の部分20の第1のテーパ状の端部23から始まり、楔のベースを形成する第2の端部24より所定の距離の位置まで延在する。
第1の表面および第2の表面(11、21)には、溝形の凹部が設けられるが、この実施例では、平面図で見ると円弧形となるよう設けられる。
椎間インプラント1を所望の高さに調整するために、第1の部分10と第2の部分20とが第1の位置A(図1および5参照)と第2の位置B(図2および6参照)との間を、接触表面12、22に沿って螺旋状の動作で互いに相対して移動し、それにより隆起15が溝25に導かれる。隆起端部16が溝端部26に押し込まれると、第2の位置Bに到達する。溝端部26は、部分が終端位置を超えて押し出されるのを防ぐ。
図5および図6に最もよく示すように、接触表面12、22は表面11、21に対して傾斜しているので、第2の位置Bに押し合わされた後の椎間インプラント1の高さはYとなるが、それは部分が押し合わされる前に第1の位置Aにあったときの高さXを超える。このように、第1の部分10と第2の部分20との互いに相対する滑動により、椎間インプラント1の高さを2つの限界値XおよびYの間で調節することができる。
椎間インプラント1を2つの椎骨間に挿入する際、所望の高さは第1の部分10と第2の部分20とを互いに相対して動かすことで調節し、次いで椎間インプラント1が椎骨間に挿入される。
図7は、椎骨間に挿入した後の、椎間インプラント1に加わる力Fを示す。力Fが、部分10と部分20との互いに相対した偶発的な相対運動をもたらすことを防ぐため、接触表面12、22の傾斜角度α、すなわちねじ軸に対して直交する平面(この特定の実施例においては部分10、20に対応する表面11、12)に対するねじ状または螺旋状の線の勾配を決定する角度が、使用する材料と接触表面12、22の表面性質に依存して適正
に選択されてオートロック機構を作動させるが、このとき、ρ0が摩擦角であり、μ0が静止摩擦係数である適切な条件
α<ρ0(ここでρ0 = arc tanμ0
が本質的に満たされる。
椎間インプラントを作る材料はチタンを使うのが好ましい。代替としてPEEKまたはPTFEのような他の体に優しい金属、合金またはプラスチック材料を使用してもよい。
上述の椎間インプラントの変形を次に記載する。
図8に示す実施例では、第1の部分10および/または第2の部分20の外部表面は、骨材料が椎間インプラント1の中に成長できるようボアホールとして形成された、凹部31を含む。
他の変形した実施例では、隆起15と溝25とが、それぞれ対応する部分10、20の第1の端部13、23から対応する端部14、24まで延在するよう設けられる。この配置により、2つの部分は第2の位置Bを超えて互いに相対して移動できるので、組立てた状態での高さはより大きな範囲で調節できる。
他の変形した実施例では、隆起と溝との接続部がT形、長方形もしくは正方形になるよう設けられるか、または、適合可能な他の形状の接続部として設けられる。
他のさらに変形した実施例では、隆起と溝との接続部はねじ状または螺旋状の線の一部としてでなく、直線状に設けられる。
他のさらに変形した実施例では、第1および/または第2の部分の外形は円弧形では設けられない。隆起と溝との間の接続部がねじ状もしくは螺旋状の線の一部、または直線状に設けられれば十分である。
他のさらに変形した実施例では、第1および/または第2の部分の接触表面は、粗面化され、および/またはミリング加工されて階段状になる。
他のさらに変形した実施例では、第1および/または第2の部分の表面は、溝がなくてもよい。

Claims (12)

  1. 椎間インプラント(1)であって、
    隣接する接触表面(12、22)を有する第1の部分(10)および第2の部分(20)と、
    対応する第1の部分(10)および第2の部分(20)が、第1の部分(10)の第1の端部(13)が第2の部分の第1の端部(23)と向かい合うような第1の位置(A)から、第1の部分(10)の第1の端部(13)が第2の部分の第2の端部(24)と向かい合い、かつ、第1の部分(10)の第2の端部(14)が第2の部分の第1の端部(23)と向かい合うような第2の位置(B)まで、接触表面(12、22)が互いの上を滑動することによって互いに相対して移動するのを導く誘導装置(15、25)とを含む、椎間インプラント(1)。
  2. 第1の部分(10)の第1の端部(13)における、第1の部分(10)の第1の接触表面(12)および第1の接触表面(12)の裏側の第1の表面(11)の間の距離が、第2の端部(14)においてよりも小さく、
    第2の部分(20)の第1の端部(23)における、第2の部分(20)の第2の接触表面(22)および第2の接触表面(22)の裏側の第2の表面(21)の間の距離が、第2の端部(24)においてよりも小さい、請求項1に記載の椎間インプラント(1)。
  3. 接触表面(12、22)および誘導装置(15、25)は、ねじ状または螺旋状の形状で設けられる、請求項1または2に記載の椎間インプラント(1)。
  4. 互いの上で滑動する接触表面(12、22)に終端止め部(16、26)が設けられる、請求項1から3のいずれかに記載の椎間インプラント(1)。
  5. 接触表面(12、22)の傾斜角度αは、使用される材料および2つの接触表面の表面性質に依存して、第1の部分(10)および第2の部分(20)の間のオートロック機構が作動できるように形成される、請求項1から4のいずれかに記載の椎間インプラント(1)。
  6. 誘導装置(15、25)は、隆起および溝接続の形で設けられる、請求項1から5のいずれかに記載の椎間インプラント(1)。
  7. 第1の接触表面(12)、第2の接触表面(22)、隆起(15)、および溝(25)はねじ状または螺旋状の形状で設けられる、請求項6に記載の椎間インプラント(1)。
  8. 溝(25)が、第2の接触表面(22)の第2の端部(24)から所定の距離の位置で終了し、互いの上で滑動する接触表面(12、22)に終端止め部(26)が設けられる、請求項6または7に記載の椎間インプラント(1)。
  9. 隆起(15)の断面がアリ形もしくはT形、または長方形もしくは正方形の形状で設けられ、かつその形状を補完する形状で溝(25)が設けられる、請求項6から8のいずれかに記載の椎間インプラント(1)。
  10. 第1の部分(10)および/または第2の部分(20)はオリフィスを含む、請求項1から9のいずれかに記載の椎間インプラント(1)。
  11. 第1の接触表面(12)および/または第2の接触表面(22)は粗面化され、または階段状に設けられる、請求項1から10のいずれかに記載の椎間インプラント(1)。
  12. 第1の表面(11)および/または第2の表面(21)は溝形の凹部を含む、請求項1から11のいずれかに記載の椎間インプラント(1)。
