本発明はMRI装置に係り、画像のコントラストを最適にするパラメータを短時間に設定する機能を有したMRI装置およびMRI画像撮影方法に関する。

【従来の技術】
磁気共鳴イメージング法(MRI)は、静磁場中に置かれた被検体組織の原子核スピンに対して、そのラーモア周波数をもつ高周波信号で励起し、この励起に伴って発生する磁気共鳴信号から画像を再構成する画像診断法である。

MRI装置は生体内から検出する磁気共鳴信号を用いた画像診断装置であり、解剖学的診断情報のみならず生化学的情報や機能診断情報など多くの診断情報を得ることができるため、今日の画像診断の分野では不可欠なものとなってきている。とくに心臓の造影MRIは最近大きな進歩を遂げ、心筋虚血と心筋梗塞の診断に関しては既に臨床の場で広く用いられるレベルにまで発展した。

MRI装置による虚血性心疾患の診断においては、被検体にMR造影剤(例えばGd−DTPA(gadolinium diethylenetriamine pentaacetic acid)を注入し、所定時間後にIR法(Inversion Recovery法)を用いてT1強調画像を収集することによって梗塞部位の心筋を特定する心筋遅延造影法がある。心筋梗塞では組織浮腫や心筋細胞膜の障害によって、MR造影剤の細胞外液分布容積が正常心筋よりも増大するとされている。

このGd系のMR造影剤は、MRI撮影における縦緩和時間(T1)を短縮させる効果があることが既に知られている。従って、非選択IRパルス(以下IRパルス)を被検体の心臓部位に照射して心筋組織の縦磁化を180度反転させ、所定の待ち時間TI(インバージョンタイム:反転時間)後にRFパルスを照射してT1強調画像を撮影すれば、T1時間の短い造影剤が存在する部位のみを強調して画像化することが可能となる。

ここで、心筋梗塞部と正常心筋とのコントラストを向上させるために上記待ち時間TIは、IRパルスによって反転した心筋梗塞部および正常心筋における核スピンの縦磁化がそれぞれの縦緩和時間T1に基づいてマイナスからプラスに回復する過程で、正常心筋の縦磁化がゼロ近傍になるタイミングで設定することが望ましい。このとき、短いT1を有する造影剤が混入された心筋梗塞部の縦磁化は、既にプラスの値に回復されており、従ってTI後のT1強調画像を収集することによって、心筋梗塞部のみを強調した画像として表示することが可能となる。

虚血性心疾患のMRI診断における本撮影では、各種の高速撮影法が導入されるが、IR―MRIの高速撮影には通常、IRプリパルスを付加したSegmented Fast Field Echo法、またはSegmented Turbo FLASHと呼ばれるk−空間を複数セグメントで分割した高速撮影法が用いられ、このとき、心電波形に同期させて呼吸停止下での撮影が実行される。この心筋遅延造影法を用いた高速撮影MRIは、心筋梗塞病変の分布をきわめて明瞭に描出するため、心筋バイアビリティ診断において、その有用性が期待されている。(佐久間肇 他, “造影MRIによる虚血性心疾患の診断”,INNERVISION(15・13)2000 P.59−66)。

【発明が解決しようとする課題】
しかしながら、上記のように梗塞部位の心筋をコントラストよく表示するためには、TI時間を最適な値に設定することが条件となるが、この値を常に一定に設定することはできない。すなわち、心筋の縦磁化がゼロになるまでの時間は造影剤投与量や投与後の時間や被検体組織などによって変動することが臨床の場で確認されている。このため、正常心筋の信号強度すなわち縦磁化がゼロになる時間をインバージョン時間TIに設定するためには、被検体ごとに、あるいは撮影ごとに上記本撮影前に行うテスト撮影において、TIをパラメータにして得られる画像から最適なTIを求めた後、この最適TIを用いてIR−MRIの本撮影を行っている。
図14は従来の最適TIを求めるためのテスト撮影法を示したものであり、MRI画像を生成する所定のスライス断面を選択し、TI時間がとり得る範囲、例えば200msec〜400msecの範囲を50msecステップで5種類のTIを設定する。この場合の撮影方法は、例えば「IRプリパルスを付加したSegmented Fast Field Echo法」が用いられる。

まず、図14(a)に示すように心電波形のR波から所定時間Tdの後にIRパルスを関心領域全体に印加し、さらにこのIRパルスから最初のTI時間(TI1)として設定された200msecの後にRFパルスを印加して信号を読み取る。但し、このとき、MR信号が受信されるスライス断面の位置は後述する3つの勾配磁場によって決定される。

さらにTI=TI1(200msec)の条件のままで、位相エンコードを変えながら複数回(Nx回)MR信号を読み取り、このとき得られるNxのMR信号を再構成してTI=TI1の場合のMRI画像を得る。但し、このNxのMR信号は1心拍期間でMヶづつN心拍にわたって収集され、このNx(=M*N)のMR信号から1枚のMRI画像を生成する。例えば、M=8、N=16、Nx=128である。なお、被検者は体動の画像への影響を低減するために、上記NxのMR信号の収集期間中は呼吸を停止する。次いで、他のTI時間、TI=TI2(250msec)〜TI5(400msec)の場合においても同様な手順でMRI画像を収集する。異なる5種類のTI時間を設定して得られる5枚の画像において正常心筋の信号強度が最も低い画像を選定し、その時のTI時間を求める。引き続いて、このTI時間を用い、1枚のMRI画像につき上記の位相エンコード数Nxとほぼ同じ位相エンコードデータを収集する本撮影を行う。なお、上記Segmented Fast Field Echo法についての詳細は本発明の実施の形態において説明する。

以上述べたように従来の最適TIの設定法では、1つのTIにつき呼吸停止した状態でそれぞれN心拍期間のMR信号の収集を行ない、さらにこのような収集を5つのTIについて繰り返し行う必要があり、従ってこの方法によればMR信号の収集に多くの時間を要するのみならず、上記5回の呼吸停止は被検者にとって大きな負担となっていた。

とくに、MRIによる虚血性心疾患の検査では、心筋遅延造影は通常心筋パフュージョン撮影やシネ撮影などと組み合わせて行われるため、上記息止めは被験者の負担と疲労を増大させるものであった。

本発明は、この最適TIを求めるために行われるテスト撮影を短時間に行うことにより、心臓検査全体の検査時間の短縮と被検者に与える負担の低減をはかることを目的にしている

【課題を解決するための手段】
上記課題を解決するために、請求項1に係る本発明のMRI装置は、被検者から得られる心電波形に同期したIR法によりMRI画像を得るMRI装置であって、心電波形の1心拍期間において、予め設定される被検体の関心領域の複数スライスに対してそれぞれ異なるTIを設定してMR信号を収集する第1の信号収集手段と、このMR信号に対して再構成を行ってぞれぞれのTIに対応するMRI画像を生成する第1の画像生成手段と、この画像生成手段によって得られる複数のMRI画像に基づいて選択されるTIを所望TIとして設定する所望TI設定手段と、この設定手段によって設定される所望TIを用いて第2のMR信号を得る第2の信号収集手段と、このMR信号に対して再構成処理を行うことによってMRI画像を生成する第2の画像生成手段と、この生成手段によって生成されるMRI画像を表示する画像表示手段とを備えたことを特徴としている。

【発明の実施の形態】
(第1の実施の形態)
図1〜図9において本発明の実施の形態について説明する。図1はMRI装置全体の概略構成を示すブロック図である。

このMRI装置は、磁場を発生させる静磁場発生部1および勾配磁場発生部2と、RFパルス信号を送受信する送受信部3と、システム全体の制御を行う制御部4と、画像再構成と画像の保存を行う高速演算・記憶部5と、被検体11を載せる寝台8と、心電計測部7と、入力部22および表示部21を備える。

静磁場発生部1は、例えば超電導磁石である主磁石13と、この主磁石13に電流を供給する静磁場電源26とを備え、被検体11の周囲に強力な静磁場を形成する。

勾配磁場発生部2は互いに直交するX、Y及びZ軸方向の勾配磁場コイル14と、これらのコイルに電流を供給する勾配磁場電源25を備える。

勾配磁場電源25には、制御部4のシーケンス制御回路24によって勾配磁場制御信号が供給され、被検体11が置かれた空間の符号化が行なわれる。すなわち、この信号に基づいて勾配磁場電源25からX,Y,Z軸勾配磁場コイル14に供給されるパルス電流を制御することにより、X,Y,Z軸方向の勾配磁場は合成され、互いに直交するスライス選択勾配磁場Gs、位相エンコード勾配磁場Ge、および読み出し(周波数エンコード)勾配磁場Grを任意に設定することが可能となる。なお各方向の勾配磁場は静磁場に重畳され被検体11に加えられる。

送受信部3は、被検体11にRFパルスを照射するための照射コイル15およびMR信号を受信し信号検出するための受信コイル16と、これらコイルに接続される送信器17および受信器18が備えられる。ただし照射コイル15と受信コイル16は分離される場合もある。

送信器17は後述のシーケンス制御回路24によって制御される。主磁石13の静磁場強度によって決定される磁気共鳴周波数と同じ周波数をもち、選択励起波形で変調されたRFパルス電流によって照射コイル15を駆動し、被検体11にRFパルスを照射する。受信器18は受信コイル16によってMR信号として受信した信号に対して中間周波変換、位相検波、さらにはフィルタリングなどの信号処理を行った後A/D変換を行う。

制御部4は主制御回路23と、シーケンス制御回路24とを備えている。主制御回路23はCPUおよびメモリを有しており、装置全体を統括して制御する機能を有しているが、とくに入力部22から入力される撮影開始指示信号や撮影方法やパルスシーケンスに関する情報、画像表示フォーマット情報などを一旦記憶する記憶機能を有し、これらの情報に基づいてシーケンス制御回路24にパルスシーケンスの情報(例えば勾配磁場コイル14や照射コイル15に印加するパルス電流の強度、印加時間、印加タイミングなどに関する情報)を送る機能を有している。また、テスト撮影によって求められる複数枚の画像の中から操作者が選択する画像に基づいて、その画像情報から最適TIを読み出し、内部メモリに保存する機能も有している。

シーケンス制御回路24はCPUおよびメモリを備えており、主制御回路23から送られてきたパルスシーケンス情報を記憶し、この情報にしたがって勾配磁場電源25、送信器17、受信器18を制御する。また、心電計測部7からの心電波形からR波を検出し、このR波を基準にしてIRパルスやRF波あるいは勾配磁場の印加タイミングを制御する。

次に、本発明の実施の形態の撮影手順を説明する前に、心筋遅延造影におけるIR法の役割と、IRパルスを付加したFast−Field−Echo法の概要について図2および図3を用いて説明する。

図2はインバージョンリカバリー(IR)について示す。図2(a)の太矢印はIRパルスの印加前における核スピンの磁化方向とその大きさ(Mo)を示しており、t=0においてIRパルスが関心領域に照射された場合、縦磁化は180度反転し−Moの大きさをもつようになる(図2(b))。この縦磁化は組織のもつ緩和時間T1に従って、元の縦磁化(Mo)に戻ろうとする。

図2(b)の曲線41および曲線42はそれぞれ梗塞部位の心筋および正常心筋における縦磁化の戻り曲線を示したものである。梗塞部位の心筋は、造影剤が注入されたことにより正常心筋に対して縦緩和時間T1が短くなっているので、梗塞部位の心筋の縦磁化は曲線41から明らかなように、正常心筋の縦磁化より速く戻る。図2(b)ではt=Tβにおいて正常心筋における縦磁化の大きさはゼロとなり、このとき、梗塞部位の縦磁化はプラスの値をもつ、t>Tβ(例えばTα)では梗塞部位からの信号は正常心筋部位からの信号より大きな信号として検出される。

すなわち、梗塞部位の心筋と正常心筋を最もコントラストよく観察するためには、正常心筋の縦磁化がゼロとなるt=Tβをインバージョン時間(TI)に設定することが望ましい。

次に、上記のIR法を適用した本実施の形態におけるテスト撮影の概要を図3を用いて説明する。図3はIRパルス付加のFast−Field−Echo法を適用したテスト撮影のパルスシーケンスを示しており、1心拍の区間中にTIをパラメータに、複数枚(本実施の形態では5枚)のマルチスライスMRI画像のデータを高速収集する。

すなわち、図3(a)に示した心電波形のR波から所定の遅延時間Td後に、心臓の関心領域に対してIRパルスを照射コイル15によって照射し、このIRパルスの照射からTI1後に第1のスライス断面のMR信号を収集するためのRFパルスを印加する。同様にして、t=TI2〜TI5において第2のスライス断面〜第5のスライス断面のMR信号を収集するためのRFパルスを印加する(図3(b))。

また、図3(c)に示すように、第1〜第5のスライス断面におけるMR信号の収集においては、8種類の位相エンコードに対してのMR信号が間隔TR(繰り返し時間)で順次収集される。図3(d)は、TR区間において行われる1つの位相エンコードに対するMR信号収集時のField−Echo法パルスシーケンスを示しており、(d−1)はIRパルスおよびRFパルスの照射タイミングを、また(d−2)はスライス選択勾配磁場Gs、(d−3)は読み出し(周波数エンコード)勾配磁場Gr、(d−4)は位相エンコード勾配磁場Ge、また(d−5)はMR信号をそれぞれ示している。

例えば、IRパルスの照射(d−1)からTI1後に、スライス選択勾配磁場Gsによって第1のスライス断面が選択され(d−2)、このスライス断面に対してフリップ角α°のRFパルスが照射される(d−1)。このRFパルスの照射が終了したならば、TE(エコー時間)後に第1のスライス断面から受信されるMR信号に対して位置情報を付加するために、第1のスライス断面に対する読み出し方向および位相エンコード方向の勾配磁場GrおよびGeが形成される(d−3、d−4)。但し、field−echo法におけるMR信号の読み出しは、180度RFパルスの印加は行わず、読み出し方向勾配磁場Grを反転することによって行うため短時間でのデータ収集が可能となる。

上記のような撮影原理に基づき、本実施の形態におけるテスト撮影の方法を図4に示す。図3におけるIRパルスとfast−field−echo法の概要説明では説明を簡単にするために位相エンコード方向のデータ数を8としたが、フィールドエコー法におけるデータ収集の繰り返し時間TRを5msecとすれば、8つの位相エンコードデータの収集に要する時間は40msecとなり、後述するTIの増加分50msecを考慮すれば位相エンコードデータ数は8が限界である。これに対して、実際のMRI画像における位相エンコードデータ数は128以上が要求されており、これらのデータを1心拍の間に収集することは不可能となる。このため、本実施の形態ではセグメント分割した撮影方法、すなわちSegmented FastField Echoを用いる。この方法におけるセグメントとは1心拍(心電波形のR−R間隔)の期間を意味し、セグメント分割した撮像法では、1枚の画像を構成するのに必要な位相エンコードデータの収集を複数のセグメントに分割して行う。

すなわち、図4に示すように第1のスライス断面〜第5のスライス断面の各々で要求される128の位相エンコードデータは、各セグメント内で収集可能なデータ数が上記理由により8とすれば16セグメントに分割されて収集される。一方、これらのデータが保存されるMR信号メモリ28の第1スライス断面用メモリ領域K−1も16のセグメントに分割される。これらの位相エンコードデータの収集と保存についての詳細は後述する本実施の形態の撮影手順の中で説明する。

次に、本実施の形態におけるMR信号収集の手順について説明する。なお、この実施の形態におけるMRI撮影は、最適なTI(TI0)を求めるためのテスト撮影と、このテスト撮影によって求められたTI0を設定して行う本撮影の2つのステップからなっている。

最初に、テスト撮影における手順を図1および図4〜図6を用いて説明する。ただし図5はテスト撮影の手順を示すフローチャート、また図6はMR信号メモリ28の構成を示した図である。

テスト撮影において、まず装置の操作者は入力部22よりTI最適化のためのテスト撮影モードの開始コマンドを入力する(ステップS1)。このコマンドが主制御回路23に送られると、主制御回路23は表示部21をテスト撮影用のデータ入力画面に切り換える。操作者はこの画面に対し、入力部22に備えられたマウスやキーボードを使用して、心電波形のR波からIRパルス照射までの遅延時間Tdやテスト画像の枚数(すなわちスライス断面数)、TIの初期値(TI1)と増分(ΔTI)、あるいは撮影方法やパルスシーケンスなど、テスト撮影に必要な情報を入力する(ステップS2)。

最適なTIは種々の条件によって異なることは既に述べたが、その大部分は200msec〜400msecの範囲に含まれる。従って、ここではTIの初期値(TI1)を200msec、TIの増分(ΔTI)を50msec、テスト画像の枚数を5枚に設定する。

次に、操作者は被検体11の体表の所定部位に、心電計測部7のECGセンサ9を装着し、このとき検出される心電波形信号をECGユニット27は受信してデジタル信号に変換する。次に、主制御回路23はデジタル信号に変換された心電波形信号をECGユニット26から読み出し、表示部21に表示する。

操作者は心電波形が正常に得られていることを表示部21のモニタ上で確認した後、テスト撮影開始のコマンドを入力部22にて入力する(ステップS3)。

一方、ECGユニット27はECGセンサ9から送られてくる心電波形をデジタル信号に変換し、主制御回路23を介してシーケンス制御回路24に送る。シーケンス制御回路24は受信した心電波形から第1のR波(セグメント1)を検出し、そのR波を基準にソフトウエアで構成されるタイマによって所定遅延時間Tdを測定する。さらに、R波からTdだけ遅延したIRトリガ信号を生成する。

シーケンス制御回路24は、このIRトリガ信号に基づいて、送受信部3の送信器17に対してIRパルス照射用の制御信号を送り、送信器17は核磁気共鳴を励起させるためのRF電流パルスを照射コイル15に供給する。但しこの場合、被検体11に対してスライス断面の選択を行わず、関心領域全体を励起するためのIRパルスを印加し、このIRパルスによって関心領域の磁化を180度反転させる(ステップS4)。
