(57)【要約】 男性更年期障害は生物学的利用可能なアンドロゲンの血清濃度における有意な低下を特徴とする。男性更年期障害を経験している男性への抗エストロゲン薬の投与は、身体のテストステロン産生を刺激することによって男性における相対的なアンドロゲンの欠乏症を治療することができる。

Description

【発明の詳細な説明】
【０００１】 （技術分野） 本発明は、男性における相対的アンドロゲン欠乏症の治療に関する医薬品を製
造するための抗エストロゲン薬の新規の用途に関する。
【０００２】 （背景技術） 男性においては、加齢と共に、精巣のアンドロゲン産生の低下及び生体のアン
ドロゲン濃度の低下を招く。エストロゲンの産生が比較的短期間で生殖器を除去
した値に低下する女性の状況とは対照的に、男性ではこれには何十年も要し、段
階的に低下して行く。 年配者の群における血清のテストステロン総濃度は、若
年の男性における値に比べて有意に低下している。加齢過程に一致してステロイ
ドホルモン結合グロブリン（ＳＨＢＧ）、さらには遊離の、結合していないグロ
ブリンが増加するために、生物活性のあるテストステロンは低下する。さらに、
エストロゲンは直接変換によりアンドロゲンから産生されるが、エストロゲンの
血清レベルは同じように年齢と相関しては低下しない。その結果、ホルモン環境
は有意に変化する。
【０００３】 男性においては、性ステロイドのホルモン環境は、エストロゲンに対するアン
ドロゲンの優勢を特徴とする。循環しているアンドロゲンの主要成分であるテス
トステロンはｎｍｏｌ／ｌの範囲での単位で血清中に検出されるが、エストロゲ
ン拮抗薬であるエストラジオールはｐｍｏｌ／ｌの範囲内で測定できるにすぎな
い。アンドロゲンのこの無視できない優勢は、基本的に、思春期以降全般にわた
って検出することができるが、年齢と相関して、このアンドロゲンの優位性には
明らかな強度の差異がある。加齢と共に、特に６０歳を越えると、アンドロゲン
の優勢にさほど顕著な強調はなくなる。
【０００４】 年配の男性では、若い頃に検出された以前の値に比べてテストステロンの優勢
に３０〜５０％の相対的な低下がある。
【０００５】 テストステロンの相対的な欠乏はそれ自体、年齢が関係する多数の障害に関与
するとみなすことができる。身体作業能力の限界を伴う筋肉量の低下、骨密度の
低下及び個別の症例では、さらに骨粗鬆症、良性前立腺肥大症と呼ばれる前立腺
サイズの増加、性欲及び性的能力の低下、及び鬱のような精神自律神経系の障害
は、総称的に男性更年期障害と呼ばれることが多く、男性におけるアンドロゲン
の相対的低下によって誘発される。性欲は勃起を得るという欲求であり、一方、
性的能力はその勃起を有する能力である。
【０００６】 若い男性では、男性不妊症を治療するための抗エストロゲン薬で毎日治療する
ことによってもテストステロン値が効果的に上昇することが知られている。

【００１０】 しかしながら、抗エストロゲン薬は男性におけるアンドロゲンの相対的欠乏症
の治療には適さないようだと考えられてきた。従って、例えば、米国特許第５，
８６１，３８９号は、男性におけるアンドロゲンの相対的欠乏症を治療するため
の医薬品の製造のために少なくとも１種のアロマターゼ阻害薬を使用することを
提案している。
【００１１】 （発明の開示） 本発明の目的は、年配の男性におけるアンドロゲンの相対的欠乏症及び／又は
男性更年期障害の関係する具体的障害を抗エストロゲン薬によって治療すること
に関する。
【００１２】 年配の男性におけるアンドロゲンの相対的欠乏症を治療する為に抗エストロゲ
ン薬を使用することによって、驚くべきことにアンドロゲン濃度の長期の上昇を
生じることが注目されている。
【００１３】 テストステロンを産生するように身体を徐々に刺激することによって、抗エス
トロゲン薬は男性におけるテストステロン／エストロゲン比の内因性のバランス
を再度とる結果になる。その結果、アンドロゲンの相対的欠乏症が補正される。
【００１４】 本発明の目的では、抗エストロゲン薬は、エストロゲン受容体結合部位におい
てエストロゲンと競合するあらゆる化合物であり、細胞内エストロゲン受容体の
補充を遅らせうる。従って、抗エストロゲン薬としては、例えば以下のような化
合物はすべて好適である。
【００１５】 トリフェニルエチレン誘導体のトランス異性体であるクエン酸タモキシフェン
。 化学名は、（Ｚ）２−［４−（１，２−ジフェニル−１−ブテニル）フェノ
キシ］−Ｎ，Ｎ−ジメチルエタンアミン ２−ヒドロキシ−１，２，３−プロパ
ントリカルボキシレート（１：１）であり、商標名、ノルバデックスで販売され
ている。；及び
【００１６】 ２［ｐ−（２−クロロ−１，２−ジフェニルビニル）フェノキシ］トリエスラ
ミンシトレート（１：１）であるクエン酸クロミフェン。 分子式 Ｃ２６Ｈ２ ８ ＣｌＮＯ・Ｃ６Ｈ８Ｏ７、分子量５９８．０９であり、商標名クロミッドで販
売されている。
【００１７】 上記の抗エストロゲン薬の列記は限定的ではなく、一定の必要事項を満たすそ
の他の化合物も考慮される。
【００１８】 薬学的に有効な投薬量の抗エストロゲン薬を年配の男性に有効な期間、好まし
くは連続的に投与する。例えば、５〜１０ｍｇを１日量として１日１回又は２回
、タモキシフェンを投与して、テストステロン濃度の基準値の中間値の目標値域
を得る。１０〜２５ｍｇ用量のクロミッドを毎日又は隔日で１００ｍｇまで投与
して基準値の中間値を得る。テストステロン及びエストラジオールの血清濃度を
測定することにより、所望のホルモンバランスが達成されているかどうか及び任
意に用量調整を行うかどうかの早期の指示を与えることができる。
【００１９】 一般に、５〜１０００ｍｇ、好ましくは１０〜１００ｍｇの抗エストロゲン薬
、クエン酸クロミフェン又はクエン酸タモキシフェン又は生物学的に同等に有効
な量の別の抗エストロゲン薬を毎日又は隔日で用いて、男性におけるアンドロゲ
ンの相対的欠乏症を治療する。
【００２０】 例えば、経口で、非経口で、経皮で、例として貼付剤によって、抗エストロゲ
ン薬を投与することができる。
【００２１】 好ましい経口投与については、具体的には、錠剤、コーティング錠、カプセル
、ピル、懸濁剤又は水剤であり、それらは、経口投与に供する抗エストロゲン薬
の製剤に共通して用いられる添加剤及び賦形剤と共に、当業者に一般的によく用
いられている方法によって製造することができる。
【００２２】 本発明に基づいて製造される医薬品は、毎日又は隔日５〜１００ｍｇ量を抗エ
ストロゲン薬の投薬量単位当りの有効成分として含有し、一般的に用いられる添
加剤、賦形剤及び／または希釈剤、または生物学的に等効果の投薬量でその他の
抗エストロゲン薬が加えられている。
【００２３】 抗エストロゲン薬を男性更年期障害を治療するのに用いる場合、エストロゲン
濃度は有効に下げられる。この治療は容易に管理可能であり、抗エストロゲン薬
による治療を特徴付けている。１０ｍｇ用錠剤については、各錠剤は、タモキシ
フェン１０ｍｇに相当する１５．２ｍｇのクエン酸タモキシフェンを含有する。
２０ｍｇ用錠剤については、各錠剤は、タモキシフェン２０ｍｇに相当する３０
．４ｍｇのクエン酸タモキシフェンを含有する。非活性添加物は、カルボキシメ
チルセルロース、カルシウム、ステアリン酸マグネシウム、マンニトール及びデ
ンプンである。
【００２４】 クエン酸クロミフェン錠は、３０％〜５０％の間でシス異性体を含有する、２
つの幾何異性体［シス（ズクロミフェン）及びトランス（エンクロミフェン）］
の混合物である。標準的な市販の錠剤は、５０ｍｇのクエン酸クロミフェン及び
以下の不活性添加物：コーンスターチ、ラクトース、ステアリン酸マグネシウム
、コーンスターチのゼラチン化前駆体、及びスクロースを含有する。 現在の錠
剤は主として女性の不妊症を治療するのに用いられている。本発明に基づいた治
療は、本明細書で指定した低投薬量に合わせるために再度投薬量を熟考している
。
【００２５】 抗エストロゲン薬の組み合わせを投与できること、又は抗エストロゲンとその
他のテストステロン産生薬剤の組み合わせを使用できることも熟考される。
