兼ね備えた椎間円板プロテーゼを提供すること。 【解決手段】 頭側の円板と、この頭側の円板に対して
実質的に平行に軸方向で間隔をおいている尾側の円板
と、これらの円板の間に挿入され、これらの円板を圧縮
弾性的に相互に支持する弾性手段とを備える椎間円板プ
ロテーゼにおいて、前記弾性手段(16,18)が、体
温のときに超弾性特性を有する形状記憶合金でできてい
ることを特徴とする。

Description

【発明の詳細な説明】
【0001】
【発明の属する技術分野】本発明は、上記請求項1の上
位概念に基づく椎間円板プロテーゼに関する。
【0002】
【従来の技術】椎間円板は、脊柱の椎体間における圧縮
弾性的な支持体としての役目をしている。特に変性、磨
減などによる椎間円板の損傷は、運動性の深刻な制約
や、神経学症状、特に痛みや麻痺につながる場合があ
る。このような疾病をもはや保存的に処置できなくなっ
た場合、椎体を相互にブロックすることが公知である。
しかしこのことは、該当する椎骨が相互に動かなくなる
という結果になり、そのために脊柱の硬直が起こってし
まう。
【0003】別案として、欠陥のある椎間円板を、当分
野に属する椎間円板プロテーゼで置き換えることができ
る。このような椎間円板プロテーゼは、上側の頭側の円
板と、下側の尾側の円板とで構成されており、その間
に、これらの円板を圧縮弾性的に相互に支持する弾性手
段が挿入されている。椎間円板プロテーゼは、除去され
た椎間円板の代わりに椎骨と椎骨の間へ挿入され、その
上ないし下に位置している椎骨の椎体にネジで定着され
る。EP0706354B1から公知の椎間円板プロテ
ーゼでは、弾性手段が、弾性的なプラスチッククッショ
ンによって構成されている。FR2734148A1か
ら公知の椎間円板プロテーゼでは、弾性手段が、チタン
または鋼材からなるばねによって構成されており、その
特性曲線は、椎骨の間隔が減るにつれてばね力の指数的
な増加を引き起こす。これらの公知の椎間円板プロテー
ゼは、椎骨の軸方向の運動性とねじり運動性を可能にす
るものである。弾性手段の材料が疲労現象を起すことが
欠点であり、こうした疲労現象は椎間円板プロテーゼの
機能性の低下につながるとともに、金属製のばねの場合
には破損につながる可能性さえある。

【発明が解決しようとする課題】本発明の課題は、椎骨
の優れた運動性と、長期間の機能性とを兼ね備えた椎間
円板プロテーゼを提供することである。

【課題を解決するための手段】この課題は、本発明によ
れば、請求項1の構成要件を備える椎間円板プロテーゼ
によって解決される。本発明の有利な実施形態は、各従
属請求項に記載されている。
【0006】本発明の中心的な思想の要諦は、体温のと
きに超弾性特性を有する形状記憶合金で、椎間円板プロ
テーゼの弾性手段を製作することにある。擬弾性とも呼
ばれる超弾性は、オーステナイトからマルテンサイトへ
の、電圧誘導による変態に依拠するものである。このよ
うな結晶構造の可逆的な変態は、従来の材料や、特に鋼
材、チタンといった従来の金属のフック弾性に比べて、
はるかに大きな弾性変形を可能にする。超弾性変形は結
晶構造の変態に基づいており、従来のフック弾性のよう
に結晶構造の変形に基づいていないので、超弾性変形は
材料の疲労にはつながらない。したがって、移植期間が
長期に及んでいて相応に多数回の荷重変化が生じていて
も、椎間円板プロテーゼの弾性特性が変化をせず、弾性
手段の疲労破損を起す恐れがない。
【0007】弾性手段を製作するのに形状記憶合金を利
用することは、弾性手段の構成に広い自由度を与える。
弾性特性やばね力に関して、弾性手段を細かく段階づけ
することができる。椎骨の軸方向の運動に関してもねじ
り運動に関しても、ばね特性曲線を最善に適合化するこ
とが可能である。
【0008】1つの実施形態では、弾性手段が圧縮コイ
ルばねとして構成されていてよい。このような圧縮コイ
ルばねは、軸方向の運動と、揺動と、ねじり運動とに関
して格別に好都合な椎骨の運動性をもたらす。
【0009】弾性手段を皿ばねによって構成すると、場
合によってはより高い剛性で、優れた軸方向の運動性お
よび揺動の運動性を得ることができるが、この場合には
ねじり運動性は得られないか、もしくはわずかにしか得
られない。
【0010】別の実施形態では、円環体の形態で円板と
円板の間に介在するコイルばねによって弾性手段が構成
されていてよい。このようなコイルばねは、コイルばね
がその軸に対して垂直に圧縮されることで、軸方向の運
動性を可能にする。コイルばねの巻回が軸に対して傾け
ば、ねじり運動が可能となる。そして最後に、弾性手段
が1つまたは複数の板ばねとして構成されていてもよ
い。このような板ばねによって、特に、柔軟な軸方向の
緩衝を実現することができる。ねじり運動は、板ばねの
幅に応じて、程度の差こそあれ可能である。
【0011】このような椎間円板プロテーゼは、格別に
高い運動性が必要とされる脊柱の領域、すなわち、特に
腰椎の領域や下側の胸椎の領域に使用するのが好まし
い。このとき弾性手段の構成や寸法は、椎間円板プロテ
ーゼの用途に応じて選択することができ、特に軸方向の
所要の弾性や、所望のねじり運動性に応じて選択するこ
とができる。
【0012】形状記憶合金からなる弾性手段は、弾性手
段のばね特性を損なう恐れのある組織の内方成長を防ぐ
ために、柔軟な保護外套で包むのが好ましい。保護外套
は、生体適合的な永久弾性プラスチックでできているの
が好ましい。

【発明の実施の形態】次に、図面に描かれている実施例
を参照しながら、本発明について詳しく説明する。椎間
円板プロテーゼは、上側の頭側の円板10と、下側の尾
側の円板12とを有している。円板10および12は、
剛性の高い生体適合的な材料で製作されており、有利に
はチタンで製作されている。円板10および12の横断
面形状は、実質的に、自然の椎間円板ないし椎体の形状
に一致している。円板10および12の外側表面には、
2つの椎骨の間に椎間円板プロテーゼを挿入したときに
円板10および12を椎体の当接する面に定着させる役
目をする爪4が一体成形されている。円板10および1
2は軸方向に間隔をおいており、圧縮弾性的な弾性手段
によって相互に支持されている。
【0014】この弾性手段は、図1の実施例では圧縮コ
イルばね16として構成されており、図2の実施例では
皿ばね18のパッケージとして構成されている。圧縮コ
イルばね16ないし皿ばね18は、円板10および12
の間へ軸方向中心に挿入されている。このようにして円
板10および12は、圧縮コイルばね16ないし皿ばね
18の圧力に抗して、軸方向へ相互に圧縮されたり相互
に傾くことができる。さらに図1の実施例の圧縮コイル
ばね16は、円板10および12のある程度の相互のね
じれを許容する。皿ばね18の場合、このようなねじれ
は不可能か、もしくは非常にわずかな程度にしか可能で
ない。
【0015】図3の実施例では、円環体の形態で円板1
0および12の円周に沿って設置されたコイルばね20
として、弾性手段が構成されている。コイルばね20の
巻回が変形することで、円板10および12は軸方向へ
相互に圧縮されたり相互に傾くことができる。コイルば
ね20の巻回がその軸に対して傾くことによって、円板
10および12のねじり運動も可能である。
【0016】図4の実施形態では、弾性手段が板ばね2
2によって構成されている。この板ばね22は、一方の
端部が上側の円板10に取り付けられ、他方の端部が下
側の円板12に取り付けられたストライプの形態を有し
ている。板ばね22はU字型に湾曲しており、図4の左
側に示すように外方に向かって湾曲していてよく、ある
いは図4の右側に示すように内方に向かって湾曲してい
てもよい。このような板ばね22が少なくとも3つ、た
だし有利には6つまたはそれ以上、円板10および12
の円周に沿って等しい角度間隔で配置される。板ばね2
2は格別に柔軟な軸方向の緩衝を可能にし、それが特に
円板10および12の傾動も可能にする。ねじり運動
は、板ばね22が(円板10および12の円周方向に)
わずかな幅を有している場合にのみ可能である。
【0017】図4に図示しているようなU字型に湾曲し
た個々の板ばね22に代えて、円板10および12の間
で軸方向中央に配置され、円周全体にわたって等しい角
度間隔で分散されて半径方向外側に向かうアームを有し
ており、これらのアームが上側の円板10と下側の円板
12に交互に取り付けられている、個々の板ばねが設け
られていてもよい。
【0018】弾性手段16,18,20,22は、体温
(35℃から40℃)の範囲で超弾性特性を有する形状
記憶合金で製造されている。このような形状記憶合金
は、たとえばニッケル・チタン合金であってよい。
【0019】椎間円板プロテーゼの中へ組織が内方成長
するのを防ぐため、保護外套24が設けられている。こ
の保護外套24は、椎間円板プロテーゼの運動性を妨げ
ないように、十分に柔軟で永久弾性的な生体適合プラス
チックでできている。
【0020】図示した実施例では、保護外套24は薄壁
の円筒状のスリーブの形態を有しており、その開いた端
部がそれぞれ頭側の円板10ないし尾側の円板12に張
り渡され、適当なやり方で密閉されて取り付けられてい
る。

  1. 【請求項1】 頭側の円板と、この頭側の円板に対して
    実質的に平行に軸方向で間隔をおいている尾側の円板
    と、これらの円板の間に挿入され、これらの円板を圧縮
    弾性的に相互に支持する弾性手段とを備える椎間円板プ
    ロテーゼにおいて、前記弾性手段(16,18)が、体
    温のときに超弾性特性を有する形状記憶合金でできてい
    ることを特徴とする椎間円板プロテーゼ。
  2. 【請求項2】 前記弾性手段(16,18)が超弾性の
    ニッケル・チタン合金でできている、請求項1に記載の
    椎間円板プロテーゼ。
  3. 【請求項3】 前記弾性手段が、円板(10,12)の
    間で軸方向中心に配置された圧縮コイルばね(16)で
    構成されている、請求項1または2に記載の椎間円板プ
    ロテーゼ。
  4. 【請求項4】 前記弾性手段が皿ばね(18)で構成さ
    れている、請求項1または2に記載の椎間円板プロテー
    ゼ。
  5. 【請求項5】 前記弾性手段が、円環体の形態で円板
    (10,12)の間に配置されたコイルばね(20)で
    構成されている、請求項1または2に記載の椎間円板プ
    ロテーゼ。
  6. 【請求項6】 前記弾性手段が1つまたは複数の湾曲し
    た板ばね(22)で構成されている、請求項1または2
    に記載の椎間円板プロテーゼ。
  7. 【請求項7】 前記弾性手段(16,18,20,2
    2)が保護外套(24)によって包囲されている、請求
    項1から6までのいずれか1項に記載の椎間円板プロテ
    ーゼ。
