り、さらに体内の通路の内腔を拡張するべく、外向きの
半径方向に拡張力が加わる際の拡張および変形した状態
において、第二の直径を獲得することが可能であること
を特徴とする、拡張可能な腔内エンドプロテーゼに関す
る。本発明は特に、疾患により狭められるかまたは閉塞
された血管の修復または復興に特に有用な、拡張可能な
腔内脈管エンドプロテーゼに関する。一般に、この種の
医療装置は、脈管ステントまたはグラフトと呼ばれる。

拡張性ステントであり、一般にバルーンカテーテルのバ
ルーンを、それが装備されることによって受け取ること
ができる管状部材を含む。バルーン拡張性ステントを血
管に植え込むための一般的な方法は、適当なデリバリー
システムのバルーンカテーテルの上に、拡張していない
収縮状態のステントを装置することを含む。次にカテー
テルを、血管壁の切口にとおし、血管の疾患部位または
狭まった部位を埋めるべく位置するまでの血管の長さ分
を滑り降ろす。次いでバルーンカテーテルの助けによ
り、血管内壁に向かってステントを拡張する。これは、
血管をあらかじめ広げておき、ステントが必要であると
決定された後に行なってもよい。別法として、血管をス
テント自体により広げ、後者をバルーンにより拡張して
もよい。どちらの場合でも、関与した血管に永続的な支
持を与えるため、一旦バルーンが空になり、カテーテル
が再び引っ込まれても、ステントはその配備された、拡
張された形態を維持するだろう。
テントの広範囲におよぶ概説は、Rotterdam Thoraxcent
re Interventional Cardiology Group の、Patrick W.
Serruysらによる、Handbook of Coronary Stentsに掲載
されている。この概説では21頁以降に、いわゆるPalm
atz-Schatz TMステントについて、ステントの分野での
金本位制として記載している。このステントは、1以上
のブリッジにより互いに連結されたステンレス鋼の多数
の連続した溝つきチューブに関係する。このステントは
最も広く使用され、実地に検査されており、世界中で6
00000人を超える患者に移植されているが、まだ数
多くの欠点に悩まされている。主な欠点は、ステント対
血管比の一定不変性および、収縮ならびに配備される柔
軟性に関係がある。ステント対血管比は、拡張した状態
で血管がステントに支えられる度合に影響するものであ
り、高くあるべきのみならず、ステントの長さをとおし
て一定であることが好ましい。しかしながら Paknatz-S
chatz TMステントでは、隣接するチューブ間にブリッジ
が不可避であるため、一旦配備されたステントでは、隣
接するセグメント間にむきだしの領域が生じることにな
り、このような場所は、減少しかつ実際に不十分なステ
ント対血管比を生じる。他の欠点は、収縮および配備さ
れた状態における、ステントセグメントのどちらかとい
えば高い厳格性に関係している。結果的に、ステントは
限られた柔軟性しかもたないことになり、それは体内の
意図した部位へのステントのデリバリーを妨げる。この
ステントの配備状態での柔軟性の不足は、典型的には2
cmより長いセグメントにわたって血管を直線化し、ス
テント処理をした領域の後期の再狭窄の主要な原因とな
るらしい。典型的には、このことはステント移植の約6
か月後に起こることがある。

は、同じ参考文献の63頁以降に記載されており、Cord
is Coronary Stentに関するものである。この装置は、
一本のタンタル(Ta)の針金片からなる。針金は連続
した正弦波を形成するべく巻かれ、長手軸に沿ってらせ
ん状に回旋する。針金の両端は溶接されて終わる。同様
の装置がRadiologicalSociety of North America(RSN
A)11/95のアニュアルシンポジウムにおいて提示され
た。この末梢用ステントの態様は、中間に溶接を取り入
れており、ステント全体にパターンが描かれている。こ
の装置は、隣接する針金のらせん状の回転を含み、それ
は隣り合う位置で溶接にて合一され、装置の全長をとお
して高度に規則的な針金の分布を呈する。クリンプされ
た外形やステント対血管比などの特性は、収縮および拡
張した状態において全長にわたり一定している。しか
し、その構造のため、特定の機能性を加えたりある欠点
を取り除いたりするべくデザインを変える段になると、
この装置にはデザインの自由度がほとんどない。またこ
の装置の内部の応力は、一旦それが傷つけられると、隣
り合う回転間の確かな溶接の供給を妨げる。さらに、こ
のような溶接ならびに針金の末端に対する溶接は、特に
装置の拡張時に弱点を残す。

による同時係属欧州特許出願98201446.6に記載されてい
る。この装置は、管壁から分離された、互い違いに配さ
れた波動の実質的に連続した構造を特色とする。前記実
質的に連続した構造は、前記管体の長手軸に沿って実質
的にらせん状に前進する少なくとも一つのパターンを含
み、隣接する波動を連結する連結要素を含む。この連結
要素は、それらにより連結される波動の複合的な伸長で
ある。
張された両状態における柔軟性については特に優れた特
性を提供するが、外からの荷重を受けるとまだ破壊され
やすいかもしれない。そのため抹消血管および動脈にお
ける適用、たとえば人体の四肢や腹部領域など骨格によ
る保護を実質的に欠く領域におけるそれらへの適用は、
あまり適切ではないかもしれない。本発明の目的は、冒
頭のパラグラフで言及したような、外力に耐える改良さ
れた能力をもち、前記抹消血管への適用に適するように
した拡張可能な腔内エンドプロテーゼを提供することで
ある。
パラグラフに記載された型の拡張可能な腔内エンドプロ
テーゼは、本発明によれば、前記管状部材の前記壁の少
なくとも一部が、前記管状部材の長手軸に沿って実質的
にらせん状に前進している互い違いに配された一次波動
の、少なくとも一つの実質的に連続した回旋を含み、第
一の一次波動が第一の連結要素により、連携する第二の
一次波動に連結されており、前記第一の一次波動に続く
第三の一次波動が、第二の連結要素によって、前記第二
の一次動に続く連携する第四の一次波動に連結されてお
り、さらに前記第一および第二の連結要素が、前記一次
波動間に間置されている連結支柱により、少なくとも前
記第一の非拡張状態において、互いに連結していること
を特徴とする。当該連結支柱により加えられる付加的な
要素は、装備された状態の装置にさらなる剛性を付与す
る。このことは当該エンドプロテーゼの好ましい態様に
おいて特に顕著であり、それは本発明によれば、前記少
なくとも一つの回旋のらせん状の回転上に規則的に分布
している多数の連結要素により、一次波動が互いに相互
連結されていること、および、前記回転内の各連結要素
から前記回転内の次の連結要素までの連結支柱が存在す
ることが特徴づけられる。ある意味では、連結要素間の

る同じ下部構造にはめ込むことにより、実質的に連続し
た第二の骨組み構造が、一次波動の間に間置されてこの
装置内に実現する。
は、前記管状部材の前記壁の少なくとも一部は、前記管
状部材の長手軸に沿って互いに実質的に平行に前進して
いる、互い違いに配置された一次波動の少なくとも二つ
の実質的に連続した回旋を含むこと、第一の回旋が前記
第一および第三の波動を含むこと、第二の回旋が前記第
二および第四の波動を含むこと、当該連結支柱が前記の
両回旋の間に間置されていることを特徴とする。この態
様は、前記の両回旋によって提供される、間置された連
結要素ならびに連結支柱をもつ二重らせん構造を特色と
する。この装置の拡張に際しては、間置された下部構造
が、装置の長手軸に対して再配位され、連結要素は前記
軸に対して実質的に横向きに回転するであろうし、一方
連結支柱は前記軸に対してさらに平行な方向をとるであ
ろう。結果として、配備された状態の装置においては、
連結要素の対が、両方の巻きを相互連結している、本体
の軸に実質的に横向きの不連続な環様の要素を形成する
であろう。これらの環様の要素は、実質的に長手方向に
向けられた連結支柱により、それら自身が相互に連絡さ
れている。このような結合は、装置が一旦拡張された状
態に変形されると、一緒になってすぐれた骨組み特性お
よびステント対血管比を生じる一方で、収縮および拡張
した両状態において、構造には優れた柔軟性が残され
る。結果として構造全体に、その収縮および拡張柔軟性
を損なうことなく配備された状態におけるすぐれたフー
プ強度ならびにステント対血管比が特徴づけられる。
い態様においては、前記構造が管壁から分離された実質
的に連続したフィラメントを含むこと、前記連結要素
が、それらが相互に連絡している一次波動の統合された
伸長であること、および連結支柱がそれによって連結す
る連結要素の統合された伸長であることを特徴とする。
フィラメントはコンピュータに制御されたレーザー切断
か、または他の精度の高い技術を中実の管に適用するこ
とにより形成してもよい。従って、当該装置は構造に協
調を導入しないいかなる構成でも形成してよい。
例および添付の図面を参考にしてさらに説明する:図1
は、本発明のエンドプロテーゼの態様の中央部分を示す
等角図であり、図2は、図1の装置を切断し、折れを開
いた場合の平面図である。この図は単に説明のために描
いたものであり、どこも縮尺では描かれていない。その
かわりに、ある寸法は明瞭さのために誇張されているか
もしれない。類似した要素にはできるだけ同じ参照数字
をつけてある。
ゆるステントは、図の端の向こうに、第一および第二の
末端を有する実質的に管状の部材を含む。壁面1は、図
1にその中央のモジュール部分が描かれている部材のこ
れらの末端の間に配置されている。図の範囲の外側で
は、繰り返し要素であるこの中央のモジュールが両側の
介在部分において繰り返されており、この介在部分は装
置の相対する側の末端モジュールから中央モジュールを
分離する。異なる部分の間の基本的な差異は、装置のら
せん状に前進するパターンの隣接する波動間の連結要素
の数にあり、すべては本文中に参考文献として取り入れ
た、出願人による同時係属出願に記載されたとおりであ
る。
は、ニチノール(NiTi)か、または出願人による同
時係属出願に記載されたような他の適切な生物適合性物
質の管である中実管の壁の外で、切断されるかまたは他
の方法で解放されている。ステントの壁面は、体内の通
路、特に血管の内腔への当該部材の腔内デリバリーを可
能にする第一の収縮または非拡張状態ならびに、第二
の、体内の通路の内腔を拡張させるべく外向きの半径方
向に拡張力が加えられる際の、拡張または配備された状
態を獲得することができる。示した状態は、製造された
時の、第一の収縮した状態に相当する。外向きの半径方
向の拡張力は、当該部材に受け入れられた後に膨張させ
られるいわゆるバルーンカテーテルを用いて装置にかけ
てもよい。別法として、前記力は装置に内在するばねの
力であってもよく、それは最初装置を収縮状態にとどめ
るべくそれを被っていたスリーブを装置から除去するこ
とにより放出される。第一の型のステントは、一般にバ
ルーン拡張性ステントと呼ばれ、もう一方のステントは
通常、自己拡張性と呼ばれる。本態様は第一の型のステ
ントに焦点を合わせているが、本発明が自己拡張ステン
トにも適用することができることが理解されよう。
少なくとも描かれている装置の部分では基本的に、互い
違いに配された一次波動11〜14の実質的に連続した
回旋10であって、当該管状部材の長手軸に沿って実質
的にらせん状に前進している回旋からなる。前記回旋1
0の次の回転からの隣接する一次波動11〜14は、各
々の回転間の隙間を埋める連結要素21、22、23に
よって互いに相互連結されている。連結要素21、2
2、23は、各々S字状にカーブした屈曲20を含み、
それは一旦装置が収縮状態から配備された状態に変形す
ると、装置の半径方向の拡張性を大いに亢進する。本文
に示したような連結要素21、22、23の代りに、前
述の同時係属出願に記載されているように、特別の要求
と装置の所望の特徴とに適合させるべく、他の工夫され
た連結要素を用いてもよい。
後の平面図にて示している図2を用いて、当該構造内の
相互連結についてさらに詳細に説明する。図からわかる
ように、第一の一次波動11が第一の連結要素21によ
って関連する第二の一次波動12に連結される時には、
一方で第一の一次波動11に続く第三の一次波動13
が、第二の連結要素22によって、第二の一次波動12
に続く第四の一次波動14に連結される。装置全体のス
テント対血管比、およびさらに重要なことにはそのフー
プ強度を亢進するために、第一および第二の連結要素2
1、22は、一次波動11〜14の回転間に間置されて
いる連結支柱30によりそれら自身が相互に連結されて
いる。当該連結支柱30は、互い違いに配された二次波
動31の下部構造を含み、それは装置の半径方向への十
分な拡張性を可能にする。
結支柱30により、次のさらなる連結要素23に連結さ
れ、連結要素21、22、23のS字状の湾曲20が各
々、二次波動31の前記下部構造に埋め込まれかつ統合
された一種の三次波動を形成するようにする。このパタ
ーンは一次波動11〜14の回旋10によって位置づけ
られたらせん状の経路に沿って連続しており、一次波動
11〜14の間において、高度に規則的な下部構造が二
次波動31および三次波動20に関して得られるように
する。この場合、一次波動11〜14のらせん状の回転
あたり、三つの連結要素21、22、23が適用されて
いる。結果として、三つの回転A、B、Cが一次波動1
1〜14の回旋10に対し、反らせん状に前進する構造
として識別されるだろう。このようならせん形の回転
A、B、Cは、一次波動11〜14の回旋10と共に二
重らせん構造を創るため、一旦装置が配備されるとすぐ
れたフープ強度を生じる結果となる。一次波動11〜1
4の回旋10は、当該構造内において主要な骨組みを引
き受けており、一方回転A、B、Cは別のらせん方向に
おけるより小さい骨組みを提供する。両構造は、連結要
素21、22により相互に連結されている一次波動11
〜14において交差している。
は、前記下部構造にうまく適応させるべく、二次および
三次波動20、31における下部構造の波動のピッチs
2の整数倍にほぼ等しい。また、二次波動31の振幅
は、一次波動11、14の回転の長手方向のピッチpの
約半分である。この振幅は、前記回転の間にある隙間が
二次波動の下部構造によって定義されるパターンにより
ほぼ完全に埋められるため、装置の最適なステント対血
管比を導く。
の技術により管壁より切り離されるため、当該構造は事
実上、連続し、合着したフィラメントを含む。当該連結
要素は、それらが相互連結する一次波動の、統合された
伸長部であり、同様に、当該連結支柱もそれらにより相
互連結される連結要素の統合された伸長部である。前記
フィラメントのパターン、幅および形は、コンピュータ
ー支援による技術を用いて厳密に調製しかつ実行しても
よい。したがって設計者は、構造に強調や機械的折衷案
を導入することなく、所望の特性に装置を適合させる極
めて大きい自由度をもつことになる。それゆえ、連結支
柱30には、収縮および拡張した両状態において柔軟性
を獲得するべく、一次波動11〜14より小さいフィラ
メント幅が与えられている。この例では、これらの幅は
各々約0.15mmおよび0.20mmである。連結要
素21、22、23は、中間の約0.18mmの幅を有
する。
ルーン上に配備され、処置されるべき狭窄へ、体腔をと
おしてカテーテルによりガイドされる。この場所におい
てバルーンは、血管内に十分な血液の通路を再び確立す
るために必要な直径までステント装置を拡張するべく膨
張させられる。この拡張された直径は、血管をとおる血
流を傷つけることなく、ステント本体を被う天然の組織
の成長を可能にするために、血管の天然の直径よりもい
くぶん大きくてよい。ステントの拡張後、移植されたス
テントを所定の場所に残したままカテーテルを引っ込め
る。前記組織の成長のため、当該ステントは最終的に
は、血管壁を連続的に支持しつつ、その中に実質的に埋
め込まれることになる。
が全くないこと、および構造内の多数の波動のため、収
縮ならびに拡張した両状態において、装置全体は極めて
柔軟性がある。さらに、本発明の装置は処置されるべき
血管の天然の進路に非常に相似しており、したがって平
等に分布した支持を提供するべくうまく適合する。連結
支柱は構造にさらなる凝集性を付与し、その結果極めて
良好なフープ強度を生じ、この装置を突然の外からの荷
重を比較的受けやすい末梢血管の処置に特に適したもの
にする。
述したが、この発明がこの特別の実例に決して限定され
ないことが理解されよう。それどころか、この発明の範
囲内において、他の多くの態様ならびに変異が熟練した
従事者には実行可能である。それゆえ、彼はこの装置を
特別の適用に適合するべく、異なる振幅、ピッチ、およ
びパターンを用いてもよい。また、一次波動のらせん状
の回旋の数は、一つだけより多くてもよい。特に、装置
の一以上の部分は、たとえば、前記管状部材の長手軸に
沿って互いに実質的に平行して前進している、互い違い
に配された一次波動の二つの実質的に連続した回旋を含
んでもよい。それゆえこれらの回旋の最初の一つが前記
第一および第三の波動を含み、他方が第二および第四の
波動を含んでもよい。この場合、連結要素は前記回旋の
間に規則的に分布しており、すべては連結支柱によって
互いに相互連結されており、結果として生じる拡張され
た構造は、らせん状に前進している一次波動によって長
手方向につなぎ合わされた、連結支柱によって形成され
る輪様の要素を特徴とするだろう。この構造は、半径方
向の大きい不撓性またはフープ強度を有していながら、
天然の血管の進路に対する高い順応性が可能である。

Claims (9)
- 【請求項1】 第一および第二の末端と、前記第一およ
び第二の末端の間に配置された壁面とを有する管状部材
を含み、当該壁が第一の非拡張状態において体内の通路
の内腔、特に血管内への前記部材の腔内デリバリーを可
能にする第一の直径を有しており、さらに体内の通路の
内腔を拡張するべく、外向きの半径方向に拡張力が加わ
る際の拡張および変形した状態において、第二の直径を
獲得することが可能な拡張可能な腔内エンドプロテーゼ
であって、前記管状部材の前記壁の少なくとも一部が、
前記管状部材の長手軸に沿って実質的にらせん状に前進
している互い違いに配された一次波動の、少なくとも一
つの実質的に連続した回旋を含み、第一の一次波動が第
一の連結要素により、連携する第二の一次波動に連結さ
れており、前記第一の一次波動に続く第三の一次波動
が、第二の連結要素により前記第二の一次動に続く連携
する第四の一次波動に連結されており、さらに前記第一
および第二の連結要素が、少なくとも前記第一の非拡張
状態において、前記一次波動間に間置されている連結支
柱により互いに連結していることを特徴とするエンドプ
ロテーゼ。 - 【請求項2】 一次波動が、前記少なくとも一つの回旋
のらせん状の回転上に規則的に分布している多数の連結
要素により、互いに相互連結されていることおよび、前
記回転内の各連結要素から前記回転内の次の連結要素ま
での連結支柱が存在することを特徴とする、請求項1に
記載のエンドプロテーゼ。 - 【請求項3】 前記連結支柱が、少なくとも収縮した状
態の装置において、次の一次波動の回転の間に存在して
いる、互い違いに配された二次波動の下部構造を含むこ
とを特徴とする、請求項2に記載のエンドプロテーゼ。 - 【請求項4】 前記一次波動が、少なくとも局部的には
前記二次波動の共通ピッチの実質的に整数倍の共通ピッ
チを有することを特徴とする、請求項3に記載のエンド
プロテーゼ。 - 【請求項5】 当該二次波動が、実質的に前記回転の長
手方向のピッチの半分に等しい振幅を有することを特徴
とする、請求項4に記載のエンドプロテーゼ。 - 【請求項6】 当該連結要素が、少なくとも前記第一
の、非拡張状態の装置において、前記連結支柱の互い違
いに配された二次波動の前記下部構造に埋め込まれて存
在する、少なくとも一つの三次波動を含むことを特徴と
する、請求項3、4、または5のいずれか一項に記載の
エンドプロテーゼ。 - 【請求項7】 前記管状部材の前記壁の少なくとも一部
が、前記管状部材の長手軸に沿って互いに実質的に平行
に前進している、互い違いに配された一次波動の実質的
に連続した少なくとも二つの旋回を含むことと、第一の
旋回が前記第一および第三の波動を含むことと、第二の
旋回が前記第二および第四の波動を含むこと、および当
該連結支柱が、前記両旋回の間に間置されていることを
特徴とする、先行する請求項のいずれか一項に記載のエ
ンドプロテーゼ。 - 【請求項8】 前記構造が、管壁から分離されている実
質的に連続したフィラメントを含むことと、前記連結要
素が、それらが相互連結している一次波動の統合的な伸
長であること、および当該連結支柱が、それにより連結
している連結要素の統合的な伸長であることを特徴とす
る、先行する請求項のいずれか一項に記載のエンドプロ
テーゼ。 - 【請求項9】 前記連結支柱が、当該一次波動よりも小
さいフィラメント幅を示すことを特徴とする、請求項8
に記載のエンドプロテーゼ。
