Abstract

(57)【要約】 【課題】 中心位置決め部材の中にステムが沈下するこ
とへの対応策を取ることなく広範囲な人工大腿骨ステム
に適合させることが可能な中心位置決め部材を提供す
る。 【解決手段】 中空の本体と等間隔で配置された複数の
アームとを有する中心位置決め部材が提供される。アー
ムは中空の本体からカップの握りのような形状で延びて
いる。各アームは該アームの長手方向に沿って円形から
次第に卵形へと変化していく断面形状を有する。

Description

【発明の詳細な説明】
【０００１】
【発明の属する技術分野】この発明は、人工補綴物イン
プラントのための中心位置決め部材、特に股間接部置換
術において患者の大腿部に埋植される人工大腿骨ステム
に関する。
【０００２】
【従来の技術および発明が解決しようとする課題】中心
位置決め部材（centralisers）またはスペーサ（spacer
s）は、股関節部置換ステムが挿入される骨小腔の内面
に埋植されたステムが触れないように該ステムの遠位端
に取り付けられる。骨小腔に接着されたステムの場合、
該ステムと骨小腔の内面との間に骨用セメントが置かれ
る隙間が存在する。取り囲んでいる骨用セメントマント
ル内にステムの位置を規制することは、置換された関節
の長期残存率にとって極めて重要である。セメントを骨
小腔に沈着させ、次に中心位置決め部材が取り付けられ
たステムを挿入することができる。均一かつ損傷の無い
セメントマントルがステムの周囲に得られるように試行
することが重要である。
【０００３】既知の中心位置決め部材は、ステムの遠位
端に嵌合するキャップ状の形態を有するものであり、ま
たのステムの穴あけされた端部の内側に固定される中心
位置決め部材である。例えば米国特許第４，６５８，３
５１号に開示されているように、中心位置決め部材が置
かれる大腿骨ステムの遠位端のテーパ面に合うテーパ状
の内面を有することが可能な環状の中心位置決め部材も
知られている。
【０００４】米国特許第３，７９３，６５０号は、骨小
腔の壁面に接触させるためにステムから延伸したスプリ
ング部材を有する中心位置決め部材またはスペーサを開
示している。
【０００５】欧州特許公報第ＥＰ０４２７４４４号は、
股間接部ステムの端部と嵌合するキャップ状の中心位置
決め部材またはスペーサを開示している。このキャップ
は、該キャップの外側方向に延びるフィンまたはウィン
グを有する。このようなフィンまたはウィングはキャッ
プの本体部分の円周方向かつ内側方向に沿ってキャップ
の本体部分に向けて折り畳まれるようになっている。

【課題を解決するための手段】本発明は、骨小腔に挿入
される人工的な構成要素に使用される中心位置決め部材
であって、該中心位置決め部材は複数のスペーサー手段
が突き出た中空の本体からなり、またスペーサー手段は
中空の本体から外側方向に延びるアームの形を有するも
のであり、さらにアームの断面形状が該アームの長手方
向に沿って変化する中心位置決め部材を提供する。
【０００７】中空の本体は、１つの開口端部を有し、か
つキャップ状であってもよい。あるいは、中空の本体
は、２つの開口端部を有し、かつ長く延びた環形を有す
るものであってもよい。中空の本体は、切頭円柱形のも
のであってもよい。
【０００８】好ましくは、各々のアームは中空の本体に
取り付けられた第１の端部を有する。また、各々のアー
ムの第２の端部もまた中空の本体に取り付けられていて
もよい。
【０００９】好ましくは、アームは該アームの長手方向
に沿って円形から次第に卵形へと変化していく断面形状
を有するものであってもよい。
【００１０】好ましくは、アームは前記中空の本体の周
りに円周等分に離間して設けられている。等間隔で離れ
た３本のアームを有するものであってもよい。
【００１１】好ましくは、アームは中空の本体を通る中
心軸に対して平行に延びるようにして中空の本体に取り
付けられている。

【発明の実施の形態】以下、本発明に基づく中心位置決
め部材の１実施形態例についてあくまでも１例として図
面を参照しながら説明する。
【００１３】中心位置決め部材１は、中空の本体２と該
本体２から外側方向に延びる複数のアーム４とを有す
る。
【００１４】本体２は、長く延びた円環形をなすもの
で、上端が切り取られた中空状の円柱形（切頭円柱形）
であってもよい。また、本体２は内面２０と外面２２と
を有する。内面２０および外面２２は本体２の上縁端部
６および下縁端部１０で接している。本体２の上縁端部
６および下縁端部１０はどちらも一般に円形をなしてい
るが、上縁端部６は下縁端部１０よりも径が大きい。
【００１５】この実施例では、中心位置決め部材１は本
体２の周りに等間隔で離間した３本のアーム４を有す
る。各アーム４は本体２の上縁端部６の近傍に結合した
第一の端部２４を有する。一方、各アーム４の第２の端
部２６は本体２の下縁端部１０の近傍に結合している。
各アーム４は本体２から外側方向に湾曲してＤ字状をな
している。さらに、これらのアーム４は本体２を通る中
心軸に対して平行に延びている。したがって、アーム４
は本体２の周囲に等間隔で設けられたカップの握りの部
分のようにみえる。
【００１６】各アーム４は、第１の端部２４から第２の
端部２６にかけて変化する断面形状を有する。各アーム
４の最上部１４は、該アーム４に曲げ剛性を与える概略
円形状の断面を有する。各アーム４の中間部分１６では
卵形の度合いを増した断面形状となり、各アーム４の下
側の部分１８は細い卵形の断面形状を有する。アーム４
に沿う断面形状の変化によって、該アーム４の第２の端
部２６に向けてアーム４の可とう性が増大する。
【００１７】本発明の目的は、中心位置決め部材の中に
ステムが沈下することへの対応策を取ることなく広範囲
な人工大腿骨ステムに適合する中心位置決め部材を提供
することである。中心位置決め部材１は、可塑性ポリメ
タクリル酸メチル（ＰＭＭＡ）物質からなるもので、周
囲を取り囲むＰＭＭＡセメントマントルに接着すること
ができる。
【００１８】本実施形態例では、直径が１０ｍｍ乃至２
０ｍｍの骨小腔に対して２ｍｍ刻みで適合するように中
心位置決め部材１が設計されている。中心位置決め部材
１の寸法は、ステムの寸法や、接着されたステムととも
に使用される遠位のリーマーに関連した外部形態に関連
づけられている。
【００１９】中心位置決め部材１は、大腿骨ステムの遠
位端を固定する。この際、大腿骨ステムは該ステムの遠
位端から約１０ｍｍ乃至１５ｍｍのところで中心位置決
め部材１の内面２０と接する。
【００２０】中心位置決め部材１は、ステムの寸法プラ
ス２ｍｍセメントマントルからステムの寸法プラス４ｍ
ｍセメントマントルまでの範囲の骨小腔に取り付けられ
ることができる。このような寸法のばらつきは、準備さ
れた骨小腔に対してステムの寸法を標準よりも小さくす
ることを外科医が決定する場合に生ずる。中心位置決め
部材１は、可とう性のアーム４を持つことによって様々
な寸法の骨小腔に適合する。アーム４の可とう性を制限
することによって、セメントマントルが標準よりも小さ
くなることを防ぐことができる。言い換えれば、セメン
トマントルの最小限の大きさを、遠位端で２ｍｍとすべ
きである。
【００２１】本発明の中心位置決め部材１は、アーム４
に沿って変化する断面形状によって屈曲を規制する複数
のアーム４を有することによって上記の要件を達成す
る。
【００２２】アーム４は両端２４，２６で規制されてい
るので、このことによって中心位置決め部材１は、求め
られていない小径の骨髄腔への挿入を阻むことが保証さ
れる。このことによって、外科医による管およびステム
の寸法の選択を管理することが保証され、それによって
ステムの寸法と管の寸法とが一致する。しかし、外科医
は所定の骨髄腔に対するステムの寸法を標準よりも大き
くすることが求められたとしても、中心位置決め部材１
の望まれないアーム４への抗力を克服することによって
そうすることができる。
【００２３】以上、本発明に基づく中央位置決め部材の
一実施形態例を説明したが、本発明の範囲から離れるこ
となく上記実施例に対する改良および修飾を加えること
が可能であることは言うまでもない。
【００２４】好ましい実施態様は以下の通りである。 （１）前記中空の本体は、１つの開口端部を有し、かつ
キャップの形状を有する請求項１に記載の中心位置決め
部材。 （２）前記中空の本体は、２つの開口端部を有し、かつ
長く延びた環状である請求項１に記載の中心位置決め部
材。 （３）前記中空の本体は、切頭円柱形である実施態様
（２）に記載の中心位置決め部材。 （４）前記アームの各々は、前記中空の本体に取り付け
られた第１の端部を有する請求項１あるいは実施態様
（１）乃至実施態様（３）のいずれか１項に記載の中心
位置決め部材。 （５）前記アームの各々は、前記中空の本体に取り付け
られた第２の端部を有する実施態様（４）に記載の中心
位置決め部材。
【００２５】（６）前記アームは、前記アームの長手方
向に沿って円形から次第に卵形へと変化していく断面形
状を有する請求項１または実施態様（１）乃至実施態様
（５）のいずれか１項に記載の中心位置決め部材。 （７）前記複数のアームは前記中空の本体の周りに円周
等分に離間して設けられている請求項１または実施態様
（１）乃至実施態様（６）のいずれか１項に記載の中心
位置決め部材。 （８）前記複数のアームは、等間隔で離れた３本のアー
ムからなることを請求項１または実施態様（１）乃至実
施態様（７）のいずれか１項に記載の中心位置決め部
材。 （９）前記複数のアームは、前記中空の本体を通る中心
軸に対して平行に延びるようにして前記中空の本体に取
り付けられている請求項１または実施態様（１）乃至実
施態様（８）のいずれか１項に記載の中心位置決め部
材。 （１０）図面に関連して実質的に上述した構成からなる
中心位置決め部材。

【発明の効果】以上述べたように、本発明によれば、中
心位置決め部材の中にステムが沈下することへの対応策
を取ることなく広範囲な人工大腿骨ステムに適合させる
ことが可能な中心位置決め部材を提供できるという効果
がある。
