【発明の詳細な説明】 【産業上の利用分野】
本発明は、骨への固定装置を備えた股関節用ソケット
補綴具に関する。
【従来技術およびその課題】
例えば、金属皿（ドイツ公開公報第2,950,536号）の
据え付け部の外周に形成された螺子のような、ソケット
補綴具をセメントを用いないで骨内に固定するための従
来の周知装置は、その補綴具の外周面全面を隙間なく覆
って該補綴具の周囲に存在する骨組織内へ直接に噛み込
めることが要求される。万一、再手術するような場合に
は、ソケットの挿入面がもはや解剖学的に円形ではなく
楕円形となるように、ソケットの頭部が置き換えられる
べき位置へ置き換えられることによって寛骨臼の開口部
が拡大されてしまうので、本来の寛骨臼の凹状球面形状
は、くぼんだ形状が楕円形に変形されたソケットの挿入
面を下方へ広がるように変化させ、新規な寛骨臼ソケッ
トを安定して装着するには、どんな場合にも問題が生じ
る。そして円形が修復されるまで寛骨臼の外側へさらに
ドリリングを行うことにより、半球状凹部が楕円状拡張
の最も大きな寸法にまで適応させなければならず、さら
にはこの半球状凹部がその窪んだ形状をも含まなければ
ならない。その長軸方向で、ソケットの挿入面の最も大
きな拡張方向へその窪んだ形状を含むことは、この方向
には必要な骨の実体が欠落しているので、最も不可能な
ことである。正常な腸骨翼はその必要な拡張を備えてお
らず、特に腹面の方向には、そのような処置は腹面の方
向の有効な領域を越えて行き、完全な腸骨翼を尾部の方
から頭部の方へ傷付けて、それを駄目にしてしまう。極
端により多くの骨を失うことは、そのような処置におい
てより多くの不利益になると見なされる。その腸骨翼
は、それによって安定性をもっと失い、同様に、将来に
必要とされる寸法もそれによってかなり制限される。 そのような場合には、度々の処置が空洞部分も含めて
楕円形状のソケットの「欠き」を骨セメントで満たし、
且つ人造ヒップソケットをもう一度埋め込む。そのよう
な処置において骨セメントは、骨の復元を妨げるばかり
ではなく、実際に証明されているように、さらに破壊へ
導く。同時に、達成され得る安定性がまさに制限される
ことになる。さらに周知の処置は、寛骨臼の欠けている
領域内へ各腸する人造ヒップソケットを受け入れるため
の半身の貝殻のような凹形状を呈するとともに、尾部方
向および頭部方向の縁に金属製クリップが取り付けら
れ、且つ骨螺子によって腸骨翼に固定される金属製の埋
め込み物を適用することである。可能な固定における安
定性は、この場合においても問題である。その中で骨の
復元と同時に起こる人工ヒップソケットの新しい接合を
達成するために、空洞部分も含めて楕円形状の「欠き」
を自質もしくは同質の骨材でで満たすような試みも時々
行われる。 勿論、この処置に伴って、達成し得る元来の安定性は
極めて不確かなものとなり、たとえ患者が長期間動かな
くとも、残っている成功の可能性は極めて疑わしい。
【発明の目的】
本発明は上述のごとき従来技術の課題に鑑み、これを
有効に解決すべく創案されたものである。したがって本
発明の目的は、十分な固定を確かなものにするととも
に、骨の復元を阻害しない股関節用ソケット補綴具を提
供することにある。
【発明の開示】
本発明に係る股関節用ソケット補綴具は、上述のごと
き従来技術の課題を解決し、その目的を達成するために
以下のような構成を備えている。 即ち、骨の中で該補綴具を支持するために、ソケット
の表面から突出し、且つ空洞部分を完全に満たしてしま
うことなく、ソケットと骨の組織との間のスペースを橋
渡し、その結果、その中での骨の自然な復元が徐々に許
容され、且つその骨の組織自体もその補綴具の支持に関
与し得るような、少なくとも一つの支持部材を備えてい
る。 支持部材は、ソケットへ挿入された後に、ソケットの
内側方向からの操作によってソケットの後方（外側）か
ら突出し、それ以降はもはや動かせなくするのに好都合
なようにデザインされている。この目的のために、ソケ
ットおよび支持部材は互いに協働して保持装置を構成し
なければならず、この保持装置は、その後の支持部材の
転移と固定力の変換を許容する。例えばバヨネット保持
装置のような、他の構成に加えて、螺子付き穴としての
補綴具側の保持装置のデザインは、上記螺子付き穴に対
応する螺子が少なくともソケット側端部に形成されたピ
ンとしてデザインされた支持部材による連接を有する本
発明においては特に好適である。支持部材は、既にソケ
ットに挿入されて保持装置内に存在していてもよい。保
持装置もしくはソケット内の螺子付き穴が多数設けられ
ていることは更に好都合であって、ソケットの挿入後
に、保持装置は支持のために必要な最も好ましい位置を
選択できる。そのような多数の保持装置は以下のような
利点もある。即ち、一般的には１本の支持部材では不十
分であるが、それを幾本か用いることによって有効に作
用させ得る。 ソケットの内側から保持装置の開口に挿通されるピン
としての支持部材のデザインは、その断面の大きさを制
限し、そのようにして骨の表面上で有効な支持面積の大
きさを制限する。もし大きな支持面積が必要な場合に
は、ソケットの埋め込みの前にでも、骨の表面の適当な
箇所に、若しくは支持部材が進行して行ってその後方で
骨の表面に接触するように、保持装置がそのように用い
られて有効な領域内にあるソケットの外側の支持部材の
骨側端部に、それに固定される支持プレートを取り付け
ることも可能である。 支持部材のできるだけ大きな支持面積を得るために、
その後方は骨側端部において中実にデザインされてもよ
い。併し乍らそれは、骨螺子が通るのをガイドするため
に、中空の支持部材を用いるのに有利である。 ソケットの挿入後、手術する外科医は利用する保持装
置のそれぞれを決定し、深さゲージを用いて骨の表面ま
での距離、選択すべき支持部材の長さを決定する。もし
これを回避する必要がある場合には、それぞれが互いに
テレスコピックに螺合される各スリーブ部材よりなる、
本発明に係る支持部材を用いることも可能である。これ
らテレスコピックに螺合された各スリーブ部材の他の特
徴によれば、縮められた状態では、螺子付き穴を備える
ソケット部分の厚さ寸法よりも実質的には短い共通の長
さ寸法となり、そのことによってソケットの挿入時に既
にその中に含んでおき、ソケットと骨との間の各距離に
応じてこれらを用いることも可能である。
【実施例】
以下に本発明の好適な一実施例について、第１図ない
し第11図を参照して説明する。第１図および第２図に示
す股関節の部分でスマッジングを施して示した領域にお
いて、図示の補綴具部分１は、図示しないプラスチック
挿入物によって補われたソケット補綴具の金属製据え付
け部を形成し、その表面一面には螺子として形成された
係止突起部２が配置されているが、再手術により新たに
装着されるソケット補綴具が、領域３および４の部分で
上記補綴具部分１が横方向へ骨組織まで直接に達する範
囲内で、患者の頭蓋の方へ移動することによって、寛骨
臼が大略楕円状に広げられており、このことは、セメン
トを用いない固定の場合には不適当である。 上述のことは第２図および第３図に総て認められ、据
え付け部には、種々の法則に基づいて表面一面に規則的
に配置された多数の螺子穴５が形成されている。この螺
子穴は、ソケット軸に関して共通な平面内に位置して、
その中心軸が関節のほぼ中間点に向かうような角度に配
置されている。ソケット軸に含まれるその角度は、開口
に近い方の列では約50゜から90゜であり、開口から遠い
方の列では約35゜から50゜である。どんな場合でも６か
ら10の、例えば８個の穴がその表面一面の各列に配置さ
れている。全体的には、支持部材の角度はソケット軸に
対して35゜から90゜の範囲に設定されるのが良い。 支持部材としては頭なしのピン部材が与えられてお
り、第４図から第７図にかけて示すようにこれらのピン
部材は中空にも中実にもでき、種々の長さにされてその
一端には溝６や、あるいは回転道具のアタッチメントの
ための６角形のソケットもしくはその種の他のものを備
えている。第２図に示すように、三つの支持部材７は、
どんな場合にも補綴具の上下へ用いられ、一方では、そ
れらが骨の表面８にまで突出してその後ろ側へ僅かに押
されるように、且つ他方では、内側へ突出はするがその
挿入物を受け入れるために備えられた表面９を超えない
ように、その支持部材の長さは選定される。 第２図に示すように、支持部材は骨の表面に対してそ
の他の補助なしに凭れ掛かってそこに固定されており、
第１図に見られる支持部材のうちの一つにあっては、中
空支持部材の内部を通って骨螺子10が設けられている。
他の二つの支持部材にあっては、骨に作用する面圧を小
さくするために支持プレート11が設けられており、その
支持プレートは第８図および第９図に示されたタイプの
ものであって、骨の表面８と支持部材７の間に単に挟ま
れることによって保持されている。これらに替わって、
支持部材への接続を確実にするために特別な接続部材を
用いることも可能である。 補綴具と骨の表面８との間の空洞部12は、この領域に
おける骨組織の復元を促すための骨の材質で裏打ちされ
ており、この骨組織は、成長の速度に依存するが、数週
間から数カ月後の補綴具に対する強度への交換に関係し
ている。支持部材は、その時までに本来の機能を確かな
ものにし、且つその安定性を高める。 テレスコピックに構成された支持部材は、第10図およ
び第11図に示すように、数個のスリーブ部材13,14,15,1
6から構成され、そのうちの最も長いスリーブ部材13は
ソケットの螺子付き穴に螺合する雄螺子と、次に長いス
リーブ部材14の雄螺子に螺合する雌螺子を備えており、
このようにして最も短いスリーブ部材16まで順次接続さ
れる。最終のスリーブ部材16は、雌螺子は持っていない
が、骨螺子を、そして回転工具の６角形ソケットをも受
け入れるための穴を備えている。三つの大きなスリーブ
部材は、それぞれの内側で、第10図中右側の端部に雌螺
子付きカラー17を有する一方、他方端の外側で雄螺子付
きカラー18を有しており、それぞれ次に大きなスリーブ
部材からそれよりも外側へはさらに螺進できないよう
に、互いのカラーが協働するように構成されている。 最初の段階で、全スリーブ部材は、これらを受け入れ
る補綴具の穴５の長さよりも十分に短く、図示の各スリ
ーブ部材の長さに相当する全長分だけの長さとなるよう
に、他のひとつの内側に配置される。そして、適当な工
具を用いることによって、スリーブ部材16が回転させら
れ、骨の表面８に到達するまで矢印（第10図）の方向へ
螺進する。 ボルト７およびテレスピック支持部材13〜16のために
適当な周知の装置によって、これらを回転可能な位置に
到達させて固定できる。 補綴具部分１の固定後、関節の表面を形成する挿入部
材は挿入される。 支持部材のための保持装置５を形成する補綴具部分
は、好ましいどんなタイプにもできる。併し乍ら、本発
明の典型的な実施例から見られるように、本発明は、特
に保持装置および特別な挿入物を含んだ堅い材質の据え
付け部分１を備えたソケット補綴具に用いられるのに好
都合である。 実施例において支持部材は、補綴具の表面と骨の表面
との間の空間を橋渡しするように用いられている。ま
た、それら支持部材はその空間が存在しない場合でも用
いることができる。即ち、骨の中により良い固定を行う
のに簡単であり、この接続において支持部材は、その端
部を鋭く尖らせることができるのであるが、骨螺子とい
うかたちで張力に変換させることなく、骨の表面に僅か
に入り込んで用いるられる。
【効果】
本発明によれば、以下のような優れた効果が発揮され
る。 即ち、骨の復元を阻害することなく、股関節用ソケッ
ト補綴具を確実に固定できる。

Claims (13)

    (57)【特許請求の範囲】
  1. 【請求項１】寛骨臼内に支持される股関節用ソケット補
    綴具において、 寛骨の上記寛骨臼を形成している部分に面する外表面
    と、人工関節挿入部を受け入れる内表面（９）とを有
    し、さらに上記寛骨に固定するための手段（２）を有し
    ているソケット部材（１）と、 上記ソケット部材に連結されるように、上記外表面から
    外方へ且つ上記寛骨臼内へ可変の長さで延びる形状にさ
    れた少なくとも一つの細長い支持部材（7,13〜16）とを
    備え、 上記支持部材は、ソケット部材が寛骨に固定された後に
    長さを可変調整できるように、ソケット部材の内表面か
    らアクセス可能であり、そのことによって、ソケット部
    材の外表面と、寛骨の上記寛骨臼を形成している部分と
    の間に広がっているいかなる空間も橋渡しする、股関節
    用ソケット補綴具。
  2. 【請求項２】上記ソケット部材は、複数の螺子穴（５）
    を有しており、各支持部材（7,13〜16）が、少なくとも
    部分的に螺子の切られているピンの形状にされている、
    請求項１記載の股関節用ソケット補綴具。
  3. 【請求項３】上記支持部材（7,13〜16）は、基部から遠
    隔した外側端部を有しており、その外側端部に支持板
    （11）が設けられている、請求項１記載の股関節用ソケ
    ット補綴具。
  4. 【請求項４】上記支持部材（7,13〜16）は、骨螺子（1
    0）を受け入れるべく中空にされている、請求項１記載
    の股関節用ソケット補綴具。
  5. 【請求項５】上記支持部材は、互いに入れ子式に螺合さ
    れる複数のスリーブ（13〜16）を備える、請求項１記載
    の股関節用ソケット補綴具。
  6. 【請求項６】上記ソケット部材は、厚さを有し、且つ少
    なくとも一つの螺子穴と入れ子スリーブ（13〜16）とを
    含んでおり、引き込まれた状態では、螺子穴を含んでい
    るソケット部材の厚さよりも長くない共通の長さを有し
    ている、請求項５記載の股関節用ソケット補綴具。
  7. 【請求項７】寛骨臼を有する寛骨部分に対して固定され
    る股関節用ソケット補綴具において、 上記寛骨部分を形成する骨の数箇所（3,4）に対して、
    直接且つ剛に噛合する噛合手段（２）を含んでいると同
    時に、上記寛骨臼（12）に対して露出され、且つ該骨
    （８）から隔てられている外表面と、挿入物を受け入れ
    るためのソケットを規定する内表面とを有する堅い基部
    （１）と、 上記ソケット部材に連結される少なくとも一つの支持部
    材（7,13〜16）とを備え、 上記支持部材は、上記外表面から外方へ且つ寛骨臼内へ
    可変の長さで延びる形状にされ、且つソケット部材の内
    表面からアクセス可能であり、 上記可変の長さは、ソケット部材が上記寛骨に固定され
    た後に選択的に調整することができる、股関節用ソケッ
    ト補綴具。
  8. 【請求項８】上記基部は、少なくとも一つの螺子穴
    （５）を有しており、且つ支持部材の一つが、該螺子穴
    に螺合する螺刻部分を有するピン（7,13〜16）の形状に
    されている、請求項７記載の股関節用ソケット補綴具。
  9. 【請求項９】上記基部は、複数の螺子穴を有している、
    請求項８記載の股関節用ソケット補綴具。
  10. 【請求項１０】少なくとも一つの支持部材が、基部から
    寛骨部分に通される骨螺子（10）を受け入れるべく中空
    にされている、請求項７記載の股関節用ソケット補綴
    具。
  11. 【請求項１１】上記支持部材は、上記骨の部分に凭れ掛
    かるべく、横断方向に向けられたプレート部分（11）を
    含んでいる、請求項７記載の股関節用ソケット補綴具。
  12. 【請求項１２】上記支持部材は、互いに入れ子式に螺合
    される複数のスリーブ（13〜16）を備える、請求項７記
    載の股関節用ソケット補綴具。
  13. 【請求項１３】上記基部は、厚さを有し、且つ支持部材
    が連結される穴を含んでおり、支持部材のスリーブが、
    入れ子式に引き込まれた状態を有し、該状態において、
    該支持部材は上記基部の厚さよりも大きくない全長を有
    する、請求項７記載の股関節用ソケット補綴具。
