Descrizione di una invenzione avente titolo:
"NUOVA COMPOSIZIONE PER IL TRATTAMENTO DELL'ACNE"
RIASSUNTO
Nuova composizione farmaceutica in forma di pomata contenente spironolattone per il trattamento dell'acne.
DESCRIZIONE
La presente invenzione si riferisce a una nuova composizione per il trattamento dell'acne.
L'acne ? una affezione flogistica cronica che virtualmente si manifesta in tutti gli adolescenti. Clinicamente ? caratterizzata da comedoni, papule, noduli e cisti. Sebbene non vi sia differenza di sesso, i maschi presentano lesioni pi? gravi ? sebbene colpisca tutte le razze, l'acne ? lieve in quelle orientali.
L'acne degli adolescenti ? considerata come uno stato fisiologico. D'altra parte essa pu? essere indotta o esacerbata da farmaci, agenti chimici (olii, idrocarburi clorurati, catrame), e condizioni che impediscono la traspirazione come vestiari troppo pesanti in climi tropicali. I farmaci che favoriscono l'acne in individui predisposti sono l'ACTH, i corticosteroidi, il testosterone, le gonadotropine, i contracettivi orali, il trimetadone, gli ioduri ed i bromuri. Talvolta intere famiglie appaiono particolarmente colpite dall'acne, e questo rilievo ha suggerito un fattore ereditario. Fattori dietetici sono stati sopravalutati nel passato.
Le lesioni dell'acne vengono divise in flogistiche e non flogistiche. Esse sono caratteristicamente distribuite sul viso (escluse le aree periorbitali), dorso, torace e spalle. Le lesioni non flogistiche includono comedoni aperti.e chiusi ("punti neri" e "punti bianchi"). Un comedone aperto consiste in una piccola area piatta o sollevata, con un foro centrale contenente cheratina e lipidi; il comedone in realt? ? un follicolo pilosebaceo contenente un tappo di cheratina. L'apice del tappo ? scuro poich? ? ossidato (non sporco). Un poro comedonico chiuso ? sormontato da epitelio; questa ? la lesione che si manifesta come papula a meno che l'epidermide sovrastante non venga asportata. Poich? nel comedone chiuso si ha intrappolamento della cheratina, esso ? una sorgente potenziale di lesioni flogistiche costituite da papule eritematose, noduli e cisti.
E1 stato ora trovato che l'acne pu? essere efficacemente curata per trattamento percutaneo con una preparazione farmaceutica contenent? spironolattone. Detto trattamento percu taneo consiste nell'applicazione, sull'epidermide di una persona affetta da acne, di una preparazione farmaceutica in forma di pomata, per un periodo variabile da 20 a 90 giorni .
Il contenuto di spironolattone nella pomata pu? variare entro ampi limiti, per esempio in una percentuale da 0,1 a 10 percento di spironolattone sul peso di veicolo usato per la preparazione della pomata. Ovviamente la quantit? di pomata che sar? usata dipender? largamente dalla concentrazio ne di ingrediente attivo spironolattone.
Come veicolo potr? essere usato qualsiasi veicolo adatto per la preparazione di pomate come per esempio vaselina, lanolina, gliceridi saturi poliossietilenati, esteri di acidi grassi con polialcoli ed altri, con o senza l'aggiunta di conservanti, antifermentativi, antibatterici, emollienti, aromatizzanti, ecc.
Il dosaggio di applicazione della pomata, come si ? detto varia con la concentrazione di spironolattone. Esso ? preferibilmente regolato in modo da ottenersi la somministrazione giornaliera di circa 10 fino a circa 250 mg di spironolattone attraverso la cute del viso, e di circa 25 fino a 1000 mg di spironolattone sul dorso, ossia sulle due pi? frequenti localizzazioni dell'acne. Queste dosi giornaliere possono essere applicate sulla cute una sola volta al giorno, o preferibilmente suddivise in dosi minori ad intervalli di 4-12 ore, cos? da assicurare un assorbimento regelare nel tempo dell'ingrediente attivo.
I seguenti esempi illustrano l'invenzione senza limitarla.
ESEMPIO 1
Una pomata per applicazione cutanea ? preparata dagli ingredienti seguenti con tecniche usuali.
ESEMPIO 2
Sessantacinque pazienti di et? compresa fra 15 e 25 anni, di cui 30 donne e 35 uomini, sofferenti al viso e/o al dorso per acne di 2?, 3? o 4? grado secondo la classificazione di Pillsbury et al. A Manual of Cutaneous Medicine, WB Saunders Co. Philadelphia, Pa. 1961, pag. 273-276, furono trattati con una pomata al 5 percento in peso di spironolattone in lanolina, in quantit? tale da aversi una somministrazione giornaliera di circa 75 mg di sostanza attiva sul viso e circa 130 mg di sostanza attiva sul dorso suddivisa in due applicazioni al giorno. Il trattamento fu continuato per 4-0 giorni .
L'osservazione dei pazienti fu fatta dallo stesso osservatore. prima del trattamento ed a giorni 20 e 40 di somministrazione; le aree interessate della cute furono fotografate prima dell 'inizio del trattamento ed ai giorni 20? e 40? del trattamento.
La risposta clinica fu altamente positiva in 36 pazienti entro 20 giorni; in 12 di questi le lesioni da acne erano completamente scomparse dopo 40 giorni di trattamento. In altri 24 pazienti si osserv? miglioramento clinico soltanto dopo 40 giorni. In 5 soli casi non si osserv? miglioramento apprezzabile.
5 pazienti presentarono eritema facciale e sensazione di bruciore dopo applicazione della pomata; questi sintomi scomparivano spontaneamente anche senza interrompere il trattamento. In nessun caso si notarono effetti secondari indesiderabili quali si notano per somministrazione orale di spironolattone.
Questi risultati dimostrano l'utilit? del trattamento percutaneo di soggetti affetti da acne in cui ? controindicato

1) Uso dello spironolattone per 11 trattamento terapeutico dell'acne.
2) Una preparazione farmaceutica per il trattamento dell'acne, contenente come ingrediente attivo spironolattone.
3) Una preparazione farmaceutica per il trattamento dell'acne, contenente come ingrediente attivo spironolattone incorporato in un veicolo per pomata.
A) Una preparazione farmaceutica secondo la rivendicazione 3, in cui lo spironolattone ? presente in quantit? da 0,1 a 10 percento sul peso totale della preparazione.
