Abstract

Tige fémorale pour prothèse de hanche, qui comprend un élément métaphysaire (1) traversé par un canal (5, 6) s'étendant sensiblement suivant son axe longitudinal, un élément diaphysaire (2) et des moyens pour réaliser l'accouplement de ces deux éléments dans ledit canal, ces moyens étant complémentaires et l'un (11) étant prévu à l'extrémité proximale de l'élément diaphysaire (2) et l'autre (14a, 15a) sur un organe rapporté (14, 15) venant en appui sur l'élément métaphysaire, caractérisée en ce que le moyen d'accouplement (11) prévu sur l'élément diaphysaire comporte deux parties cylindriques successives (12, 13) portant respectivement des filetages en sens inverse, ces deux parties cylindriques ayant des diamètres différents et en ce que l'autre moyen d'accouplement (14a, 15a) est complémentaire de l'une ou l'autre desdites parties cylindriques (12, 13).

La présente invention concerne une tige fémorale pour prothèse de hanche, du type comprenant un élément métaphysaire traversé par un canal s'étendant sensiblement suivant son axe longitudinal, un élément diaphysaire et des moyens pour réaliser l'accouplement de ces deux éléments dans ledit canal, ces moyens étant complémentaires et l'un étant prévu à l'extrémité proximale de l'élément diaphysaire et l'autre sur un organe rapporté venant en appui sur l'élément métaphysaire.
Dans les tiges fémorales connues du type susvisé, qu'il s'agisse de tiges de première intention ou de tiges de révision, comportant un élément métaphysaire et un élément diaphysaire indépendants à solidariser de façon définitive une fois l'orientation relative de ces élément définie, ce sont les moyens d'accouplement desdits éléments qui constituent, du point de vue de la résistance mécanique, la partie la plus faible de ces tiges.
En effet, elles sont soumises à des sollicitations en flexion et en torsion et, lors du cycle de la marche, les efforts répétés et les vibrations auxquels sont ainsi soumises lesdites tiges, ont un effet mécanique sur les moyens d'accouplement desdits éléments.
On notera par ailleurs qu'une telle tige, selon qu'elle est implantée du côté gauche ou du côté droit de la hanche, subit des efforts directement opposés.
Or, les tiges fémorales actuellement commercialisées sont toutes équipées de moyens d'accouplement constitués par des vis standard dites à filetage à droite. Il s'ensuit que de telles vis subissent du côté gauche du patient des efforts tendant à conduire à un vissage. Au contraire, du côté droit du patient, elles subissent des efforts opposés tendant à conduire à un dévissage et, partant, à la désolidarisation de l'élément métaphysaire et de l'élément diaphysaire ; un tel dévissage peut aller jusqu'à libérer des particules d'usure métalliques extrêmement nocives pour l'os au niveau de la zone d'accouplement.
Le but de la présente invention est de remédier à ces inconvénients et pour ce faire, elle propose une tige fémorale telle que définie au premier paragraphe de cette description,

cette tige étant caractérisée en ce que le moyen d'accouplement prévu sur l'élément diaphysaire comporte deux parties cylindriques successives portant respectivement des filetages en sens inverse, ces deux parties cylindriques ayant des diamètres différents et en ce que l'autre moyen d'accouplement comporte une partie complémentaire de l'une ou l'autre desdites parties cylindriques.
On dispose ainsi d'une tige fémorale utilisable aussi bien du côté droit que du côté gauche d'un patient, cette tige pouvant comporter un organe rapporté comprenant une partie cylindrique pourvue soit d'un filetage à droite soit d'un filetage à gauche selon le côté opéré du patient.
Plus précisément, dans le cas où la tige selon l'invention est une tige pour prothèse de hanche droite, le moyen d'accouplement prévu sur l'organe rapporté de l'élément métaphysaire est complémentaire de la partie cylindrique de l'autre moyen d'accouplement, portant un filetage à gauche ; et dans le cas où la tige selon l'invention est une tige pour prothèse de hanche gauche, le moyen d'accouplement prévu sur l'organe rapporté de l'élément métaphysaire est complémentaire de la partie cylindrique de l'autre moyen d'accouplement, portant un filetage à droite.
Selon un mode de réalisation de l'invention, le moyen d'accouplement comportant les deux parties cylindriques est une tige filetée prolongeant axialement l'extrémité proximale de l'élément diaphysaire et dont la partie cylindrique de plus grand diamètre est la plus proche de l'extrémité distale de l'élément diaphysaire.
Selon un autre mode de réalisation, le moyen d'accouplement comportant les deux parties cylindriques est un orifice taraudé dont la partie cylindrique de plus grand

diamètre est la plus éloignée de l'extrémité distale de l'élément diaphysaire.
L'invention est exposée ci-après plus en détails, en regard des dessins annexés dans lesquels : - la figure 1 est une vue de côté d'un mode de réalisation de la tige selon l'invention, après accouplement de l'élément métaphysaire et de l'élément diaphysaire, - la figure 2 est une vue de face de l'élément diaphysaire de la tige selon la figure 1, - la figure 3 est une vue de face de la tige selon la figure 1, l'élément métaphysaire étant représenté en coupe partielle longitudinale, avant mise en place de l'organe rapporté dudit élément métaphysaire, - la figure 4 est une vue de face en coupe partielle longitudinale de l'élément diaphysaire d'un autre mode de réalisation de la tige selon l'invention, et - la figure 5 est une vue de face en coupe partielle longitudinale de la tige selon l'invention, comportant l'élément diaphysaire selon la figure 4 et avant mise en place de l'organe rapporté de l'élément métaphysaire.
Si l'on se reporte à la figure 1, on peut voir que la tige fémorale selon l'invention comprend de manière connue en soi un élément métaphysaire 1 et un élément diaphysaire 2 indépendants, pouvant être solidarisés par des moyens d'accouplement (non représentés sur la figure 1).
L'élément métaphysaire 1 comporte à sa partie haute un col prothétique 3 terminé par une partie tronconique 4 destinée à recevoir une tête prothétique (non représentée).
L'élément 1 est par ailleurs percé longitudinalement et axialement par un canal divisé en plusieurs zones, à savoir, comme le montrent les figures 3 et 5, essentiellement une zone inférieure tronconique 5 dont le diamètre le plus grand est situé à l'extrémité inférieure dudit canal, cette zone 5 s'évasant à sa partie haute pour former une zone supérieure tronconique 6.
Comme le montrent les figures 2 et 4, l'élément diaphysaire 2 comporte une partie supérieure cylindrique 7 qui se prolonge, au

niveau de sa partie proximale, par une partie tronconique 8 complémentaire de la zone tronconique 5 susmentionnée.
L'élément diaphysaire 2 comporte par ailleurs une partie inférieure 9 destinée à être introduite dans le canal médullaire du fémur et qui présente une courbure dont le rayon correspond sensiblement au rayon de courbure moyen du fémur.
Cette partie inférieure 9 peut en outre comporter un ou plusieurs perçages transversaux 10 pour la mise en place d'une ou plusieurs clavettes de solidarisation de ladite partie 9 au fémur.
La partie tronconique 8 et la zone tronconique 5 forment un système à emmanchement conique permettant une libre rotation de l'élément métaphysaire 1 par rapport à l'élément diaphysaire 2, avant leur solidarisation par mise en oeuvre de moyens d'accouplement.
On ajoutera que cette partie tronconique 8 et cette zone tronconique 5 pourraient en variante être respectivement remplacées par une partie cylindrique et une zone cylindrique complémentaires, ladite partie cylindrique ayant alors un diamètre légèrement inférieur à celui de la partie cylindrique 7.
Conformément à la présente invention, les moyens d'accouplement peuvent, comme montré par les figures 2 et 3, comprendre une tige 11 prolongeant axialement l'extrémité supérieure de la partie tronconique 8. Cette tige porte une première partie cylindrique filetée 12 et une deuxième partie cylindrique filetée 13.
La première partie 12 est la plus proche de l'extrémité supérieure de la partie tronconique 8, son diamètre est supérieur à celui de la deuxième partie 13 et son filetage est inverse de celui de la deuxième partie 13.
Dans le mode de réalisation représenté, la partie 12 comporte un filetage à gauche et la partie 13 un filetage à droite, l'inverse étant évidemment possible.
Lesdits moyens d'accouplement comprennent par ailleurs un écrou 14 ou 15 comportant respectivement une partie 14a pourvue d'un filetage à droite (écrou 14) ou une partie 15a pourvue d'un filetage à gauche (écrou 15) et complémentaire respectivement de

la partie 12 ou 13 de la tige filetée 11. Cet écrou est conformé pour pouvoir être reçu dans le canal de l'élément métaphysaire 1 ; par ailleurs, l'écrou 14 comporte une surface extérieure tronconique 16 et l'écrou 15 une surface extérieure tronconique 17, ces surfaces étant complémentaires de la zone tronconique 6 dudit canal de l'élément 1.
Comme le montre la figure 3, l'assemblage de la tige fémorale selon l'invention est réalisé en introduisant la partie haute de l'élément diaphysaire 2, par le bas, dans le canal percé dans l'élément métaphysaire 1 et emmanchement de la partie tronconique 8 dans la zone tronconique 5. Après réglage de la position relative de ces deux éléments, soit in situ, soit sur la table d'opération, l'écrou approprié (c'est-à-dire écrou 14 pour une tige fémorale destinée à une jambe gauche ou écrou 15 pour une tige fémorale destinée à une jambe droite) est vissé sur la partie cylindrique correspondante de la tige filetée 11. Il en résulte un serrage de l'élément 1 sur l'élément 2 et donc solidarisation de ces deux éléments.
Selon la variante représentée par les figures 4 et 5, les moyens d'accouplement comprennent non pas une tige filetée 11, mais un orifice taraudé 18 prévu à l'extrémité supérieure de la partie tronconique 8. Cet orifice comporte deux parties successives 19,20 de diamètre différent, celle (19) de diamètre le plus faible étant la plus proche de la partie distale de l'élément 2. Par ailleurs, ces deux parties 19,20 comportent respectivement des filetages inverses. Dans l'exemple représenté sur les figures 4 et 5, la partie 19 comporte un filetage à gauche et la partie 20 un filetage à droite.
Lesdits moyens d'accouplement comprennent en outre une vis 21 ou 22 dont la partie filetée 21a ou 22a est complémentaire respectivement de la partie 19 ou 20 de l'élément 2. Plus précisément, la vis 21 est à filetage à gauche et complémentaire de la partie cylindrique 19 et elle est mise en oeuvre dans une tige destinée à une jambe droite ; par ailleurs, la vis 22 est à filetage à droite et complémentaire de la partie cylindrique 20 et elle est mise en oeuvre dans une tige destinée à une jambe gauche.

La tête des vis 21,22 comprend une partie tronconique 23, 24 complémentaire de la zone tronconique 6 du canal de l'élément métaphysaire.
L'opération d'assemblage de la tige fémorale selon cette variante est tout à fait analogue à celle à mettre en oeuvre pour assembler la tige fémorale selon le mode de réalisation décrit précédemment.
Les tiges fémorales de la présente invention sont livrées avec l'écrou 14 ou 15 ou la vis 21 ou 22 prémonté (e) ce qui évite à l'opérateur d'être dans l'obligation de choisir l'écrou ou la vis approprié (e) à la hanche opérée, ce qui peut être source d'erreur.

Claims (5)

REVENDICATIONS
1. Tige fémorale pour prothèse de hanche, qui comprend un élément métaphysaire (1) traversé par un canal (5,6) s'étendant sensiblement suivant son axe longitudinal, un élément diaphysaire (2) et des moyens pour réaliser l'accouplement de ces deux éléments dans ledit canal, ces moyens étant complémentaires et l'un (11 ; 18) étant prévu à l'extrémité proximale de l'élément diaphysaire (2) et l'autre (14a, 15a ; 21a, 22a) sur un organe rapporté (14, 15 ; 21,22) venant en appui sur l'élément métaphysaire, caractérisée en ce que le moyen d'accouplement (11 ; 18) prévu sur l'élément diaphysaire comporte deux parties cylindriques successives (12,13 ; 19,20) portant respectivement des filetages en sens inverse, ces deux parties cylindriques ayant des diamètres différents et en ce que l'autre moyen d'accouplement (14a, 15a ; 21a, 22a) est complémentaire de l'une ou l'autre desdites parties cylindriques.
2. Tige fémorale selon la revendication 1, caractérisée en ce que le moyen d'accouplement comportant les deux parties cylindriques (12,13) est une tige filetée (11) prolongeant axialement l'extrémité proximale de l'élément diaphysaire et dont la partie cylindrique (12) de plus grand diamètre est la plus proche de l'extrémité distale de l'élément diaphysaire.
3. Tige fémorale selon la revendication 1, caractérisée en ce que le moyen d'accouplement comportant les deux parties cylindriques (19,20) est un orifice taraudé (18) dont la partie cylindrique (20) de plus grand diamètre est la plus éloignée de l'extrémité distale de l'élément diaphysaire.

4. Tige fémorale selon la revendication 1,2 ou 3, pour prothèse de hanche droite, caractérisée en ce que le moyen d'accouplement (15a ; 21a) prévu sur l'organe rapporté (15 ; 21) de l'élément métaphysaire est complémentaire de la partie cylindrique (12 ; 19) de l'autre moyen d'accouplement, portant un filetage à gauche.
5. Tige fémorale selon la revendication 1,2 ou 3, pour prothèse de hanche gauche, caractérisée en ce que le moyen d'accouplement (14a ; 22a) prévu sur l'organe rapporté (14 ; 22) de l'élément métaphysaire est complémentaire de la partie cylindrique (13 ; 20) de l'autre moyen d'accouplement, portant un filetage à droite.
