Abstract

Cette cupule (2) d'implant cotyloïdien présente au moins un perçage (4) obturé par un bouchon (9).Selon l'invention, le bouchon (9) est relié à la cupule (2) par au moins une liaison ruptible, et présente sur sa face orientée vers l'intérieur de la cupule (2) des moyens permettant sa désolidarisation de la cupule (2).

La présente invention concerne une cupule d'implant cotyloïdien de prothèse articulaire de hanche.
Une technique classique de remplacement d'une articulation de hanche consiste à placer une cupule dans la cavité cotyloïdienne de l'os iliaque.
La cupule de forme sensiblement hémisphérique reçoit la tête sphérique d'une prothèse fémorale placée dans le canal médullaire de l'os fémoral.
Un insert en matériau synthétique, notamment en polyéthylène, est disposé dans la cupule pour assurer la réception de la tête sphérique de la prothèse fémorale. Cet insert favorise le glissement de la tête fémorale par rapport à la cupule
La mise en place de la cupule cotyloïdienne peut se faire de manière avantageuse par impactage dans la cavité cotyloïde de l'os iliaque.
Le maintien de la cupule est réalisé par la complémentarité de forme de la cupule et de la cavité cotyloïdienne de l'os iliaque.
Ce maintien peut être complété par des saillies s'étendant depuis la face de la cupule en contact avec l'os. Ce dernier, en repoussant autour de ces saillies, immobilise la cupule.
Il peut également être nécessaire de maintenir la cupule par des vis qui traversent cette dernière pour venir pénétrer dans l'os.
A cet effet, les cupules sont pourvues d'une pluralité de perçages permettant à un chirurgien de placer une ou plusieurs vis en fonction de la configuration particulière de l'os sur lequel la cupule doit être placée.
En pratique, deux ou trois vis sont nécessaires pour assurer l'immobilisation de la cupule par rapport à l'os, si bien que plusieurs perçages ne sont pas traversés par des vis et sont donc laissés ouverts.
Or, l'existence de ces perçages dans la paroi de la cupule n'est pas sans conséquences.
En effet, au cours des sollicitations subies par la prothèse, le polyéthylène formant la coque insérée dans la cupule a tendance à fluer travers les trous et, par conséquent, entre en contact avec l'os.
En outre, il se produit des microdéplacements entre la cupule et l'insert de polyéthylène. Le frottement entre l'insert et les arêtes vives de la

cupule conduit à la formation de débris de polyéthylène. Ces débris se détachent de l'insert et entrent en contact avec l'os par des perçages.
Or, le polyéthylène a une action d'ostéolyse sur les tissus osseux.
Il a donc été envisagé d'obturer les perçages par des bouchons.
Les bouchons actuels qui, pour la plupart, sont emboîtés dans les perçages, sont toutefois très insatisfaisants sur de nombreux points.
D'une part, leur retrait est peu aisé pour le chirurgien et, d'autre part, ils présentent une tenue du bouchon présentant une faible fiabilité dans le temps.
Un but de l'invention est de proposer une cupule pour implant cotyloïde présentant des perçages obturés par des bouchons, ceux-ci pouvant être retirés de façon particulièrement aisée, tout en présentant un haut degré de fiabilité, sans pour autant engendrer un important surcoût économique.
De manière connue en soi, la cupule cotyloïdienne présente au moins un perçage obturé par un bouchon.
Selon l'invention, le bouchon est relié à la cupule par une liaison ruptible et présente sur sa face orientée vers l'intérieur de la cupule des moyens permettant la désolidarisation de la cupule.
La cupule selon l'invention permet donc, d'une part, de réaliser une liaison sûre entre le bouchon et la cupule et, d'autre part, peut être aisément rompue par un chirurgien qui souhaiterait utiliser un perçage pour faire passer une vis de fixation.
Avantageusement, le bouchon est relié à la cupule par au moins un point de soudure susceptible d'être rompu par la mise en rotation du bouchon.
La liaison par soudure par points, tout en assurant une bonne tenue du bouchon à la cupule est susceptible d'être facilement rompue par cisaillement
De préférence, les points de soudure sont réalisés sur la face extérieure de la cupule.
En outre, trois points de soudure disposés à 1 200 les uns des autres sur la face extérieure de la cupule relient le bouchon à la cupule.
Selon une caractéristique particulièrement avantageuse, le bouchon comprend au moins deux évidements susceptibles de recevoir au

moins deux griffes d'un outil dont la rotation entraîne la rupture des points de soudure.
Grâce à cet outil, le bouchon peut être mis en rotation de manière aisée, cette rotation produisant la rupture des points de soudure.
En outre, les évidements sont disposés à la périphérie du bouchon et sont diamétralement opposés.
Selon une autre caractéristique, chaque bouchon présente une partie cylindrique adjacente à une partie tronconique complémentaire de chaque perçage ayant une paroi cylindrique débouchant dans la face intérieure de la cupule et une paroi tronconique débouchant dans la face extérieure de la cupule.
Pour sa bonne compréhension, l'invention est décrite ci-après en référence au dessin annexé représentant, à titre d'exemple, une cupule cotyloïdienne selon celle-ci.

La figure 3 est une vue en coupe selon ttt-ttt de la figure 2 ;
La figure 4 est une vue à échelle agrandie de la cupule au niveau d'un bouchon d'obturation.
Comme le montrent les figures, la cupule 2 comprend un corps hémisphérique métallique.
Des perçages 4 sont pratiqués dans la paroi de la cupule 2. Ces perçages 4 permettent le passage de vis destinées à pénétrer dans le tissu osseux.
Les perçages 4 présentent une paroi cylindrique 6 du côté intérieur de la cupule et une paroi tronconique 7 allant en se rétrécissant ers le côté extérieur de la cupule 2.
Des bouchons 9 présentant une partie cylindrique 10 et une partie tronconique 11 complémentaires des perçages 4, sont soudés par soudage électrique par points 14 à la cupule 2.
Les points de soudage 14 reliant les bouchons 9 à la cupule 2 sont visibles sur la figure 2. Ces points de soudure sont réalisés sur la face extérieure de la cupule 2.
Chaque bouchon 9 est relié à la cupule par trois points de soudures 14 disposés à 1200 les uns des autres.

Sur sa face orientée vers l'intérieure de la cupule 2, les bouchons 9 présentent deux évidements 15 périphériques diamétralement opposés.
Comme cela apparaît sur la figure 4, l'extraction d'un bouchon 9 afin d'ouvrir l'accès à un perçage 4 par lequel un chirurgien souhaiterait faire passer une vis est effectuée grâce à un outil 16.
Cet outil présente deux griffes 18 séparées d'une distance égale à celle séparant les évidements 15 ménagés dans le bouchon 9.
L'outil 16 est amené en contact du bouchon 9, les griffes 18 venant s'insérer dans les évidements 15. Le chirurgien exerce alors un couple de torsion sur l'outil 16 qui est transmis au bouchon 9. Les points de soudure 14 qui présentent une résistance au cisaillement faible sont alors rompus, désolidarisant le bouchon 9 de la cupule 2.
L'invention fournit ainsi une cupule ayant les nombreux avantages indiqués plus haut. Cette cupule permet notamment d'offrir à un chirurgien la possibilité de retirer de manière très aisée un bouchon obturant un perçage. En outre, la liaison par soudage par points, du bouchon à la cupule, assure une liaison sûre entre ces derniers.
Il va de soi que l'invention n'est pas limitée à la forme d'exécution décrite ci-dessus mais qu'elle en embrasse au contraire toutes les variantes de réalisation.
Ainsi, la face du bouchon orientée vers l'intérieur où la cupule pourrait présenter une rainure diamétrale ou une empreinte creuse à six pans permettant à un outil d'exercer un couple de rotation sur le bouchon pour le désolidariser de la cupule.

Claims (7)

REVENDICATIONS
1. Cupule (2) d'implant cotyloïdien dont la paroi présente au moins un perçage (4) obturé par un bouchon (9), caractérisé en ce que le bouchon (9) est relié à la cupule (2) par au moins une liaison ruptible, et présente sur sa face orientée vers l'intérieur de la cupule (2) des moyens permettant sa désolidarisation de la cupule (2).
2. Cupule selon la revendication 1, caractérisée en ce que le bouchon (9) est relié à la cupule (2) par au moins un point de soudure (14) susceptible d'être rompu par la mise en rotation du bouchon (9).
3. Cupule selon la revendication 2, caractérisée en ce que les points de soudure (14) sont réalisés sur la face extérieure de la cupule (2).
4. Cupule selon la revendication 3, caractérisée en ce que trois points de soudures (14) disposés à 1 200 les uns des autres sur la face extérieure de la cupule (2) relient le bouchon (9) à la cupule (2).
5. Cupule selon l'une des revendications 1 à 4, caractérisée en ce que le bouchon (9) comprend au moins deux évidements (15) susceptibles de recevoir au moins deux griffes (18) d'un outil (16) dont la rotation entraîne la rupture des points de soudure
6. Cupule selon la revendication 5, caractérisée en ce que les évidements (15) sont disposés à la périphérie du bouchon (9) et sont diamétralement opposés.
7. Cupule selon l'une des revendications 1 à 6, chaque perçage (4) ayant une paroi cylindrique (6) débouchant dans la face intérieure de la cupule (2) et une paroi tronconique (7) débouchant dans la face extérieure de la cupule, complémentaire d'un bouchon (9) présentant une partie cylindrique (10) et une partie tronconique (10).
