Abstract

La présente invention concerne une prothèse antiluxation, et notamment une prothèse de hanche, comprenant une cupule cotyloïdienne (1) creusée d'une cavité semi-sphérique (9) dans laquelle est montée, à rotation, une tête fémorale (11) sphérique, de même diamètre, solidaire d'une queue fémorale (13) fixée dans le fémur (15) d'un patient. Cette prothèse est caractérisée en ce que la cupule cotyloïdienne (1) comporte un bossage circulaire (17) creusé d'une rainure circulaire ouverte vers l'intérieur, destinée à recevoir une butée-clavette (21) constituée d'une portion de couronne circulaire montée à coulissement dans ladite rainure, le rayon interne de ladite portion de couronne étant inférieur au rayon de la tête fémorale (11), de façon à assurer le maintien de celle-ci.

Description

La présente invention concerne une prothèse et plus spécialement une prothèse de hanche anti-luxation.
De façon habituelle, une prothèse de hanche est essentiellement constituée, notamment, d'une cupule cotyloïdienne, destinée à être fixée dans la cavité de l'os cotyloïde par des moyens de scellement appropriés, et une queue fémorale métallique, destinée à être fixée dans le fémur, qui se termine par une tête fémorale sphérique qui vient s'ajuster dans une cavité semi-sphérique de la cupule cotyloïdienne, de façon que le fémur puisse pivoter autour de cette dernière.
Afin d'éviter les problèmes de luxation, on empêche la tête fémorale de sortir de la cupule cotyloïdienne en la maintenant en place au moyen d'une butée. Cette butée est habituellement constituée d'une demi-couronne, dont l'évidement central est de plus petit diamètre que celui de la tête fémorale, que l'on fixe à l'aide de vis sur la cupule cotyloïdienne.
Un tel dispositif présente de notables inconvénients, à la fois en ce qui concerne sa mise en oeuvre, et sa tenue ultérieure. En effet, l'utilisation de vis impose le perçage préalable de trous dans la cupule, l'écartement de ceux-ci étant déterminé au moyen d'un gabarit. On conçoit aisément qu'une telle opération de perçage soit particulièrement difficile à mettre en oeuvre. Par ailleurs, on sait que les cupules cotyloïdiennes et les butées de maintien de la tête fémorale sont habituellement constituées de matière plastique, la plupart du temps du cestylène, si bien que la fixation par vis qui s'effectue dans ces matériaux n'est parfois pas suffisante pour résister au couple d'arrachement important exercé par le fémur sur la butée et qui se traduisent par des arrachements des vis, et/ou une déformation des butées, qui entraînent des contraintes post-opératoires importantes, ou conduisent le chirurgien à intervenir de nouveau sur la prothèse. Cet arrachement est encore aggravé par le fait que l'effort exercé par la tête fémorale sur la butée se produit dans la direction de mise en place des vis de maintien.
La présente invention a pour but d'éviter ces inconvénients en proposant une prothèse anti-luxation pourvue d'une butée de maintien de la tête fémorale dans la cavité de la cupule cotyloïdienne, qui se met en place sans nécessiter l'utilisation d'outils, et dans laquelle l'effort exercé par la tête fémorale ne s'effectue pas suivant la direction de mise en place de la butée.
La présente invention a ainsi pour objet une prothèse anti-luxation, et notamment une prothèse de hanche, comprenant une cupule cotyloïdienne creusée d'une cavité semi-sphérique dans laquelle est montée, à rotation, une tête fémorale sphérique, de même diamètre, solidaire d'une queue fémorale fixée dans le fémur d'un patient, caractérisée en ce que la cupule cotyloïdienne comporte un bossage circulaire creusé d'une rainure circulaire ouverte vers l'intérieur, destinée à recevoir une butée-clavette constituée d'une portion de couronne circulaire montée à coulissement dans ladite rainure, le rayon interne de ladite portion de couronne étant inférieur au moyen de la tête fémorale, de façon à assurer le maintien de celle-ci.
Dans un mode de mise en oeuvre de l'invention, la face interne de la butée est de forme sphérique, et prolonge la cavité sphérique prévue dans la cupule cotyloïdienne, ce qui améliore le maintien de la tete fémorale.
Dans un autre mode de mise en oeuvre de l'invention, le bossage de la cupule cotyloïdienne est percé d'un trou recevant un pion de maintien, apte à être enfoncé dans la butée semi-circulaire afin d'immobiliser celle-ci en rotation.
La figure 1 est une vue partielle en perspective d'une prothèse de hanche suivant l'invention qui est fixée sur l'os iliaque.
La figure 2 est une vue en perspective d'un mode de mise en oeuvre de la prothèse de hanche suivant l'invention.
La figure 3 est une vue en coupe suivant la ligne III

La figure 6 est une vue en coupe suivant la ligne VI-VI de la prothèse représentée sur la figure 4.
La prothèse de hanche représentée sur les figures 1 à 3 est essentiellement constituée d'une cupule cotyloïdienne 1 de forme globale semi-sphérique, d'une enveloppe métallique 3 fixée dans l'os iliaque 5 et d'un noyau 7 en matière plastique solidaire de l'enveloppe 3 et qui est creusé en son centre d'une cavité semi-sphérique 9 qui reçoit une tête fémorale sphérique 11 liée à une queue fémorale 13 solidarisée du fémur 15 d'un patient.
Le noyau 7 est pourvu, dans sa partie diamétrale, d'un bossage circulaire 17, qui s'étend sur le tiers environ de la périphérie du noyau 7, et qui forme un angle au centre a d'environ 1200. Le bossage circulaire 17 est creusé d'une rainure circulaire interne 19 qui s'ouvre vers l'intérieur et qui est destinée à recevoir une butée-clavette de verrouillage 21.
La butée-clavette 21 a la forme d'un secteur de couronne circulaire, dont le diamètre externe est identique à celui de la rainure 19 et dont l'épaisseur e est voisine de celle-ci, de façon à pouvoir prendre place, à frottement, à l'intérieur de la rainure 19. Le rayon interne r de la butéeclavette 21 est inférieur au rayon R de la tête fémorale 11 de façon que, lorsque celle-ci est en place dans la cavité centrale 9 du noyau 7, elle empêche la tête 11 de sortir de ladite cavité 9, tout en lui permettant tout mouvement de pivotement.
De préférence, comme on peut le voir sur la figure 3, le bord interne 22 de la butée circulaire 21 est de forme sphérique, ce qui permet de minimiser le frottement de la tete fémorale 11 avec la butée-clavette 21, tout en améliorant le guidage de ladite tête 11.
La mise en place de la butée-clavette 21 est particulièrement simple puisque, après avoir mis en place la tete fémorale 11 dans la cavité 9, il suffit d'introduire celle-ci dans la rainure 19 puis de la faire tourner autour de la te te fémorale 11 par un mouvement circulaire représenté par la flèche F.
Une fois la butée-clavette 21 mise en place, on constate que les différents efforts qui s'exercent sur la butée n'engendrent aucun effet de rotation propre à faire sortir celle-ci de la rainure 19, si bien qu'elle peut être maintenue en position par le simple jeu des frottements. Bien entendu, il est possible de prévoir des moyens pour assurer la fixation en rotation de la butée-clavette 21 sur la cupule cotyloïdienne.
Comme représenté sur la figure 3a, ces moyens peuvent etre constitués d'un pion 25 en forme de harpon qui prend place dans un trou 26 prévu dans l'épaisseur du bossage 17 et qui, une fois la butée-clavette 21 en place, est enfoncé à force dans le noyau 7.
Dans une variante de mise en oeuvre de l'invention représentée sur les figures 4 à 6, la butée 21 est respectivement pourvue, à sa partie supérieure et à sa partie inférieure, de deux bossages 32 et 36 qui permettent d'augmenter son épaisseur, ce qui a pour effet d'améliorer d'une part sa résistance mécanique et d'autre part d'augmenter la portée de la partie sphérique 22' en contact avec la tête fémorale 11 ce qui améliore le guidage de celle-ci.
Par ailleurs dans cette forme de mise en oeuvre de l'invention, la partie supérieure de la cavité semi-sphérique 9 prévue dans le noyau 7 de la cupule cotyloïdienne 1 comporte un chanfrein circulaire 38, sensiblement à 450, qui permet, comme représenté sur la figure 6, d'une part de mettre en portée la queue fémorale 13 sur une ligne plutôt qu'en un point et, d'autre part, d'augmenter l'angle b formé par l'axe yy' de la queue fémorale 13 avec l'axe de symétrie zz' de la cupule 1.

Claims (4)

REVENDICATIONS
1.- Prothèse anti-luxation, et notamment prothèse de hanche, comprenant une cupule cotyloïdienne (1) creusée d'une cavité semi-sphérique (9) dans laquelle est montée, à rotation, une tete fémorale (11) sphérique, de même diamètre, solidaire d'une queue fémorale (13) fixée dans le fémur (15) d'un patient, caractérisée en ce que la cupule cotyloïdienne (1) comporte un bossage circulaire (17) creusé d'une rainure circulaire (19) ouverte vers l'intérieur, destinée à recevoir une butée-clavette (21) constituée d'une portion de couronne circulaire montée à coulissement dans ladite rainure (19), le rayon interne (r) de ladite portion de couronne étant inférieur au rayon (R) de la tete fémorale (11), de façon à assurer le maintien de celle-ci.
2.- Prothèse suivant la revendication 1 caractérisée en ce que la face interne de la butée-clavette (21) est de forme sphérique, et prolonge la cavité sphérique (9) prévue dans la cupule cotyloïdienne (1).
3.- Prothèse suivant l'une des revendications 1 ou 2 caractérisée en ce que le bossage (17) de la cupule cotyloïdienne (1) est percé d'un trou (26) recevant un pion de maintien (25), apte à etre enfoncé dans la butée-clavette (21) afin d'assurer son verrouillage en rotation.
4.- Prothèse suivant l'une quelconque des revendications précédentes caractérisée en ce que la partie supérieure de la cavité semi-sphérique (9) prévue dans la cupule cotyloïdienne (1) comporte un chanfrein circulaire (38) destinée à recevoir en appui la queue fémorale (13).
