Abstract
L'invention combine avantageusement deux appareils ou parties d'appareil. L'utilisation d'au moins une diode laser à semi-conducteur 1 permet d'obtenir une configuration d'appareil qui autorise, avec un seul module laser 6, aussi bien des interventions microchirurgicales au moyen d'un rayonnement impulsionnel, que des traitements de coagulation par un rayonnement continu. Application aux traitements de l'oeil.
Description
L'invention concerne un appareil ophtalmo-
logique pour effectuer des traitements de coagulation
au moyen d'un rayonnement laser continu et des inter-
ventions microchirurgicales au moyen d'un rayonnement laser déclenché. En ophtalmologie, on utilise essentiellement deux types de lasers, à savoir des lasers à gaz Ar/Kr pour la coagulation, que l'on fait fonctionner en continu (en régime continu) avec une puissance de quelques watts,
ainsi que des lasers Nd-YAG déclenchés pour des interven-
tions microchirurgicales, que l'on fait fonctionner avec des énergies par impulsion de l'ordre du mJ. Etant donné que les deux types de laser peuvent seulement fonctionner chacun dans un mode, continu ou pulsé, leur application
dans l'ophtalmologie conduit à des configurations d'ap-
pareils propres, plus ou moins complexes. Selon l'état de la technique, des appareils combinés, convenant à la fois pour des traitements de coagulation et pour des
interventions microchirurgicales, sont réalisables seu-
lement par la réunion des appareils lasers mentionnés sur un appareil ophtalmologique, une lampe à fente par exemple, et elles sont techniquement compliquées donc coûteuses.
Le but de l'invention est de créer une con-
figuration d'appareil autorisant, avec seulement un mo-
dule laser, aussi bien des interventions microchirurgi-
cales par rayonnement impulsionnel que des traitements de coagulation par irradiation continue et qui, en plus, possède un faible encombrement et soit techniquement moins complexe que les appareils connus jusqu'à présent pour
cette application.
Conformément à l'invention, on obtient ces
résultats par l'utilisation d'un seul module laser con-
tenant au moins une diode laser à semi-conducteur pour
les deux domaines d'application.
On peut faire fonctionner les diodes lasers à semi-conducteur pour émettre un rayonnement aussi bien en régime impulsionnel qu'en régime continu. La longueur d'onde émise, de 770-810 nm, convient bien à des fins de
coagulation sur le fond de l'oeil. Des puissances en ré-
gime continu de 200-500 mW sont réalisables sans que cela demande des dispositions techniques particulières et la prévision de moyens auxiliaires supplémentaires permet d'atteindre une puissance de 2 watts. On dispose ainsi
de suffisamment de puissance pour la coagulation. En ré-
gime impulsionnel, des puissances moyennes de 1 watt sont habituelles avec les diodes lasers à semi-conducteur; pour une fréquence de répétition des impulsions de 10-20 Hz
et les durées d'impulsion recherchées de 10 ns, cela cor-
respond à des énergies par impulsion allant jusqu'à 50 mJ, ce qui suffit pour des applications à la microchirurgie dans l'oeil. On peut faire fonctionner une diode laser tant en régime continu qu'en régime impulsionnel par un appareil d'alimentation à partir du secteur. Une optique variable permet de concentrer le rayonnement laser dans
les milieux antérieurs de l'oeil ou sur le fond de l'oeil.
Selon un exemple avantageux de l'invention,
le module laser est placé au-dessus du microscope d'ob-
servation d'une lampe à fente et le rayonnement laser
est dirigé sur l'oeil à traiter par un miroir manipu-
lateur.
Les avantages obtenus par l'invention con-
sistent surtout en ce que, grâce à l'emploi d'une unique source laser à la fois pour le rayonnement continu et pour le rayonnement impulsionnel, on obtient une structure
d'ensemble particulièrement favorable et simple de l'ap-
pareil. Comme la source laser ne demande pas de pièces qui s'usent, comme des lampes à éclairs ou des tubes à décharge, il devient possible de réaliser des systèmes
d'ensemble ne demandant pas d'entretien.
D'autres caractéristiques et avantages de
l'invention ressortiront plus clairement de la description
qui va suivre d'un exemple de réalisation non limitatif, ainsi que de la figure unique du dessin annexé, montrant la disposition selon l'invention d'un module laser sur
une lampe à fente.
Sur le dessin, la référence 6 désigne un module laser contenant des diodes lasers à semi-conducteur 1. L'appareil d'alimentation secteur, nécessaire pour faire fonctionner les diodes lasers à semi-conducteur, n'est pas représenté sur le dessin. On peut utiliser un appareil d'alimentation secteur adéquat du commerce. Une diode laser à semi- conducteur 1 est représentée au point le plus haut dans le module laser 6. Des lentilles 7a-7d font diverger le rayonnement laser, lequel est dirigé sur un miroir manipulateur 5 qui est placé dans le faisceau d'éclairage 8 d'une lampe à fente 3 conventionnelle. La référence 9 désigne la manette pour manoeuvrer le miroir manipulateur 5 et 2 désigne le microscope d'observation
de la lampe à fente. La référence 10 indique un appui-
tête pour le patient. Le faisceau laser 4 et le faisceau d'éclairage de la lampe à fente sont focalisés au point
11 oh se trouve l'oeil du patient.
Claims (2)
1. Appareil ophtalmologique pour effectuer des traitements de coagulation au moyen d'un rayonnement laser continu et des interventions microchirurgicales au moyen d'un rayonnement laser impulsionnel, caractérisé par l'utilisation d'un seul module laser (6) contenant au moins une diode à semi-conducteur (1) pour les deux
domaines d'application.
2. Appareil ophtalmologique selon la revendi-
cation 1, caractérisé en ce que le module laser (6) est placé au-dessus du microscope d'observation (2) d'une lampe à fente (3) et que le rayonnement laser (4) est
dirigé par un miroir manipulateur (5) sur l'oeil à traiter.
