Il s'agit donc de pratiquer une incision de grande profondeur ( une dizaine de centimetres) après quoi ,#le-chirurgien utilise un ou plusieurs écarteurs , pour déplacer les masses musculaires et libérer l'accès de ses instruments au nerf sciatique . Au début de l'opération , le chirurgien pratique sur le dos 14 du patient , une incision longitudinale 15 , orientée le long d'une apophyse épineuse 16 du rachis . En profondeur , cette incision 15 s'étend jusqu'au niveau des apophyses articulaires 17 , si bien que , sur son trajet , les masses musculaires 18 doivent être progressivement détachées de la paroi latérale 19 de l'apophyse epineuse à laquelle elles adhéraient
Au fur et--à mesure que l'incision progresse en profondeur , le chirurgien y introduit la spatule 5 de l'écrteur 1 . Arrivé à la profondeur désirée , il déplace latéralement ( figure 4 , flèche 20) , la spatule 5 pour suivre le contour osseux . Ce mouvement latéral prend fin lorsque l'extrémité de la languette 7 peut être enfin engagée derrière l'apophyse articulaire 21 correspondante (figure 4)
A partir de ce moment , l'apophyse articulaire 21 constitue , pour la spatule 5 , un point d'appui pour l'écrteur 1 , qui joue alors le rôle d'un levier , sous l'effet de la traction due au poids 13 tirant sur la chaînette 12 . Le poids 13 pend ainsi latéralement , contre la jonction entre le flanc et le dos du patient .Son poids est suffisant pour maintenir l'écarteur 1 à la position illustrée sur la figure 4 , ou les masses musculaires 18 sont éloignées de la paroi latérale osseuse 19
Le raccord 9 étant branché , l'extrémité lU de la fibre optique 8 constitue une source de lumière froide qui éclaire le fond du champ opératoire , comme indiqué sur la figure 4 , par la référence 22
On voit sur la figure 4 , que 1 'écar- teur 1 selon l'invention dégage de lui-même, le champ operatoire , tout en l'éclairant , si bien que le chirurgien peut désormais commodément intervenir dans la zone 22 , malgré la faible longueur de incision 15
