Description
La présente invention concerne un endoscope,notam-
ment un laparoscope,comportant un fût dans lequel s'étend un canal pour le passage d'instruments,un guide de lumière à fibres et un système optique,dont la partie proximale avec un oculaire sort latéralement en étant une ou plusieurs fois coudée, à l'extérieur de la partie proximale renforcée du fût, le diamètre intérieur du fût étant égal à au moins 9,7mm
avec cependant un maximum de 10,4 mm.
Les fûts des laparoscopes connus précités ont des dimensions extérieures correspondant à un diamètre de 10 à ,8 mm,de manière que les incisions abdominales puissent être maintenues très courtes pour éviter autant que possible des lésions ou dommages à des vaisseaux,qui peuvent survenir lorsque le diamètre de l'endoscope est augmenté rendant nécessaires des incisions abdominales plus longues.Les fûts connus ayant les dimensions précitées comportent un canal longitudinal dont le diamètre intérieur est compris entre 3,0 et 5,0 mm et dont le système optique qui s'y étend a un diamètre d'environ 4,0 à 5,0 mm. Tous les instruments à faire passer dans l'endoscope,tels que sondes,pinces de biopsie,pinces de préhension,sondes de coagulation ou autres, sont adaptées à ces diamètres du canal. Dans les canaux actuels pour instruments,il n'était cependant pas possible de faire passer des appareils pour la mise en place de clips sur des trompes ou appareils pour saisir les trompes en produisant un pli très nettement marqué de manière à pouvoir faire glisser sur la trompe pliée une bague élastique de pinçage de cette trompe,de sorte que,pour de tels instruments, le médecin devait utiliser des laparoscopes ayant un fût de diamètre agrandi et un canal à instruments également de plus grand diamètre,ce qui conduisait cependant encore à des
incisions abdominales plus longues.
L'invention se propose donc de permettre,tout en conservant les diamètres usuels des fûts de laparoscopes,de faire passer dans le canal du fût du laparoscope tous les instruments existants actuellement utilisés,et, en outre, des instruments de diamètre plus grand,par exemple pour le

pinçage des trompes au moyen d'un clips ou bien d'une
bague élastique.
Ce problème est résolu selon l'invention en ce que,en utilisant des fûts de laparoscopes du type précité,le diamètre du système optique passant dans le fût est égal à environ 20 à 25 pour cent du diamètre intérieur du fût et que le diamètre intérieur du canal à instruments est d'environ 75 à 70 pour
cent du diamètre intérieur du fût.
Cet élargissement du canal à instruments avec une forte diminution du diamètre du système optique,est suffisant pour permettre l'utilisation de tous les instruments en cause
ayant des diamètres différents pour des opérations ou des trai-
tements à effectuer dans la cavité abdominale,de sorte que le praticien est en mesure de conserver le Jeu d'instruments existants et qu'il ne lui est plus nécessaire d'utiliser des
laparoscopes de diamètres différents.
On a décrit ci-après des réalisations de l'invention en se référant aux dessins annexés dans lesquels: Figs.l à 3 représentent en vue latérale schématique des fûts de laparoscopes classiques, et
Figs.4 et 5 représentent deux coupes à plus grande échel-
le suivant Il-II des figures 1 à 3,pour différents diamètres du
système optique et du canal à instruments.
Des laparoscopes,généralement introduits dans la cavité abdominale au moyen d'un fourreau trocart, se composent d'un fût 1 dans lequel s'étend en longueur un canal à instruments 2,
un système optique 3 et un guide de lumière à fibres 4. Le gui-
de de lumière à fibres 4 remplit le volume situé au-dessus et/ou en dessous du canal 2 et du système optique 3 et il est regroupé dans un manchon raccord 5 placé du côté proximal auquel est relié un câble guide de lumière 6(représenté en trait interrompu)partant d'une source de lumière. Le système optique 1 sort latéralement de l'extrémité proximale renforcée la du fût 1 en direction de l'oculaire 7,par exemple avec un double coude (figure l),avec un simple coude (figure 3) ou en
oblique par rapport à l'axe de tige (figure 2).
Pour résoudre le problème posé,selon la présente invention,le canal à instruments 2 est augmenté en diamètre intérieur,dans le cas de figure 4 Jusqu'à 70% du diamètre intérieur de fût,c'est-à-dire Jusqu'à environ 7 mm et, dans le cas de figure 5,Jusqu'à 75 pour cent du diamètre intérieur de tige,c'est-à-dire à peu près Jusqu'à 77 mm. En correspondance le système optique est diminué en diamètre Jusqu'à environ pour cent,c'est-à- dire jusqu'à 2,4 mm ou bien jusqu'à 20 pour cent,c'est-à-dire Jusqu'à environ 2,3 mm,de sorte qu'il
devient maintenant possible,dans un fût d'une pièce ou homo-
gène,de passer dans le canal 3 tous les instruments existant dans une clinique ou disponibles pour un praticien ainsi que des instruments de plus grands diamétres,par exemple pour la
mise en place d'agrafes ou de bagues élastiques pour l'obtu-
ration des trompes.

Claims (1)
- REVENDICATION-. Endoscope,notamment laparoscope,comportant un fût dans lequel sont disposés un canal de passage d'instruments, un guide de lumière à fibres et.un système optique,dont lapartie proximale munie d'un oculaire sort,en faisant -un ou plusieurs coudes,latéralement de la partie proximale renforcée du fût,le diamètre intérieur du fût s'élevant aumoins à 9,7 mm mais cependant au maximum à 10,4.mm,caractéri-sé en ce que le diamètre du système optique (3)passant dans le fût(l) s'élève à environ 20 à 25 pour cent du diamètre intérieur donné du fût,et en ce que le diamètre intérieur du canal à instruments s'élève à environ 75 àpour cent dudit diamètre intérieur donné du fût.
