Lors du remplacement d'une articulation de la han-
che par une endoprothèse de cette articulation, on opère en général, d'une part en remplaçant dans le bassin du sujet la cavité articulaire sphérique par une pièce creuse en matière plastique, en céramique ou en métal et, d'autre part, la tête sphérique du fémur par une pièce sphérique en métal, en céramique ou en une matière analogue et on met en prise ces deux pièces. Pour les endoprothèses de la hanche connues, la tête sphérique de la prothèse de l'articulation forme un ensemble rigide au contact direct de la cavité articulaire mise en place, de sorte que les charges verticales prenant naissance
lorsque le sujet marche ou court sont absorbées relative-
ment rigidement et directement par la prothèse de l'arti-
culation, ce qui doit conduire naturellement à une usure de l'articulation de remplacement et, par ailleurs, fait
subir des secousses au patient.
L'invention a pour but de supprimer ou tout au moins
de réduire au minimum l'usure à prévoir actuellement.
L'invention a donc pour objet une cavité articu-
laire pour la tête sphérique d'une endoprothèse de l'ar-
ticulati.on de la hanche, caractérisée en ce qu'elle com-
porte une monture cylindrique en métal ou en céramique destinée à être fixée au bassin du sujet, dans laquelle est montée déplaçable axialement de façon limitée contre un tampon élastique, une garniture en matière plastique résistant à l'usure, rigide, recevant dans un logement
sphérique la tête sphérique d'une prothèse de l'articu-
lation de la hanche.
Grâce à la solution selon l'invention, on arrive à ce que les chocs qui se produisent lorsqu'un sujet
marche sur le sol sont absorbés élastiquement par la pro-
thèse de l'articulation de la hanche remplaçant l'arti-
culation naturelle car, désormais la tête sphérique, peut, lors de chaque pas du sujet, appuyer avec une déformation
élastique la cavité articulaire ou la garniture de mati-
-2- ère plastique contre le tampon et comprimer élastiquement celui-ci, chaque pas du sujet étant absorbé pratiquement élastiqueme'nt dans l'articulation et par conséquent les surfaces de contact de la prothèse de l'articulation de la hanche ne subissent qu'une usure très réduite, de sor- te qu'on peut prévoir une durée utile sensiblement accrue des prothèses de cette articulation. Outre cela, chaque pas est absorbé élastiquement, en souplesse par le sujet, et cela facilite également de façon décisive la marche
et la course.
L'invention sera décrite plus en détail en regard du dessin annexé à titre d'exemple nullement limitatif et sur lequel: la figure unique représente une coupe axiale d'une cavité
articulaire d'une prothèse de l'articulation de la hanche.
D'après cet exemple de forme de réalisation, la cavité articulaire destinée à la tête sphérique d'une prothèse d'articulation hanche-cuisse est constituée par un cylindre creux 1 métallique, compatible avec le corps humain, à fixer au bassin et fermé par un fond 2.-Pour réaliser une fixation parfaite, le cylindre 1 comporte à l'extérieur des rugosités, des sillons ou des filets
hélicoïdaux. Une garniture 3 en matière plastique com-
portant un évidement 4 à peu près hémisphérique est embol-
tée de façon à pouvoir se déplacer axialement de façon limitée dans cette monture cylindrique,en soi connueen raison du fait que la garniture 3 essentiellement rigide est placée contre un tampon S constitué par une matière
se déformant élastiquement, de préférence unesilicone.

tance qui correspond à la compressibilité du tampon élas-
- 3 - tique S ou à la mobilité axiale de la garniture 3 dans
la monture cylindrique 1, 2.
Afin que la garniture 3 ne se sépare pas de la monture 1,2 et par exemple ne se perde pas au cours d'un transport, elle comporte sur son périmètre extérieur de petites saillies séparées réparties sur ledit périmètre
ou un anneau légèrement saillant 8 ou une bride 8 péri-
phériques, lesquels pénètrent dans une gorge 9 ménagée dans la face intérieure du cylindre 1. La gorge 9 a alors une largeur au moins égale au déplacement axial possible de la garniture 3 dans le cylindre 1. Enfin, des tiges de guidage 10 parallèles à l'axe pouvent être incorporées dans les brides 6 et 7 pour le guidage axial des pièces 1 et 3 l'une dans l'autre et sont fixées dans la bride 6 ou la bride 7. Il est également possible de placer les pièces de guidage 10 orientées vers l'intérieur, comme
cela est représenté en traits interrompus.
La cavité articulaire décrite est mise en place et
fixée dans un creux de l'os du bassin et la tête sphéri-
que pénètre dans l'évidement 4 en forme de sphère d'une prothèse d'articulation jambe-cuisse. Le porteur de cette prothèse peut marcher d'un pas élastique grâce à cette cavité articulaire car, à chaque pas sur le sol, le tampon 5 est comprimé élastiquement par le poids du corps et reprend ensuite son épaisseur initiale, ce qui rend par ailleurs vraisemblable un accroissement de la
durée de vie utile de la cavité articulaire ou de l'en-
semble de la prothèse de l'articulation de la hanche.
Il va de soi que la présente invention n'a été décrite qu'à titre indicatif et non limitatif et qu'elle est susceptible de diverses variantes sans sortir de son cadre. - 4 -

Claims (3)

REVENDICATIONS
1. - Cavité articulaire pour la tête sphérique d'une endoprothèse de l'articulation de la hanche,
caractérisée en ce qu'elle comporte une monture cylindri-
que (1, 2) en métal ou en céramique destinée à être fixée au bassin du sujet,dans laquelle est montée déplaçable axialement de façon limitée contre un tampon élastique (5), une garniture (3) en matière plastique résistant à l'usure, rigide, recevant dans un logement sphérique (4) la tête sphérique d'une prothèse de l'articulation
de la hanche.
2. - Cavité articulaire selon la revendication 1, caraatêrisée en ce qu'un tampon élastique (5) en forme de disque cylindrique est mis en place dans la monture cylindrique (1, 2) entre son fond (2) et la garniture (3) et en ce que l'arête extérieure du périmètre de la garniture est munie d'une bride radiale (6) qui se trouve à une certaine distance, comptée suivant l'axe, du bord
de l'ouverture de la monture cylindrique (1).
3. - Cavité articulaire selon l'une des revendica-
tions 1 et 2, caractérisee en ce que le périmètre de la garniture (3) en matière plastique est pourvu de saillies séparées ou est entouré d'lun anneau qui pénètre dans une gorge ménagée dans la paroi intérieure du cylindre
(1) et dont la largeur est au moins égale à la compres-
sibilité du tampon (5) ou à la course axiale maximale
de la garniture dans la monture cylindrique.
