Abstract
Implante de articulación de la rodilla modular que incluye un componente de implante de la articulación prostética básica que se puede implantar estructurado para funcionar como un primer componente de implante de articulación prostética completa (5, 6, 10 a 13, 15, 20 a 23) y un subcomponente de articulación modular estructural que se puede añadir (27, 30, 33, 35 a 37, 50, 70) en el que el implante de la articulación de rodilla modular incluye un componente femoral que presenta una superficie condilar lisa (10) de geometría generalmente convexa que incluye, en una pestaña condilar distal, un condilo medial inferior (11) y un condilo lateral inferior (12); en una pestaña posterior (5), un condilo medial posterior (13) y un condilo lateral posterior; en una pestaña anterior (6), un condilo medial anterior (15) y un condilo lateral anterior; con una superficie opcional en un lado superficial de la pestaña anterior (6), en la que generalmente se puede desplazar una tróclea; siendo dicha superficie condilar (10) de tal modo que existe una muesca intercondilar entre los condilos medial y lateral; a través de la cual, cuando se añade el sub-componente estructural que se puede añadir al primer componente del implante de la articulación que se puede implantar, se forma un segundo implante de articulación prostética completa sin que se haya de extraer el implante de la articulación del lugar en el que se había implantado inicialmente, caracterizado porque el primer componente del implante de la articulación prostética completa es un implante de articulación prostética completa básica (5, 6, 10 a 13, 15, 20 a 23) que comprende una parte estructural de recepción (1 a 4, 2C, 3F, 3P, 4H, 27C, 27H), que se forma de manera integral con el implante de la articulación prostética completa básica en una sola pieza y que se puede adaptar para recibir dicho subcomponente de la articulación modular estructural que se puede añadir (27, 30, 33, 35 a 37, 50, 70), y el componente femoral es un componente femoral artificial que incluye una estructura de componente femoral a modo de parte estructural de recepción, que incluye dos paredes laterales (2) unidas a una pared delantera (4), la misma que se puede unir en las direcciones proximal y de profundidad a dichas pestañas condilares distales posterior y anterior (5, 6), con la citada pestaña distal unida a las mencionadas pestañas posterior y anterior (5, 6), y la dicha superficie condilar lisa (10) de geométrica generalmente convexa, que está unida a la estructura del componente femoral en las direcciones distal y superficial.
Description
Prótesis de articulación modular.
La presente invención se refiere a una
articulación modular. En particular a una realización particular de
ésta, haciendo referencia a una articulación de rodilla prostética
o a los componentes de ésta.
Son conocidas varias articulaciones artificiales
y partes de las mismas.
Mediante Goodman y otros, el documento de
patente US 5 766 257 (del 16 de junio de 1998) se da a conocer una
articulación artificial particularmente muy útil. La misma presenta
una transferencia de carga natural.
Un cierto nivel de modularidad existe entre
algunas de las articulaciones conocidas. Por ejemplo, con prótesis
de cadera totalmente convencionales, se pueden disponer de
cabezales de cerámica de diversos tamaños para la fijación a un
componente del tallo femoral. En el campo particular de las
prótesis de rodilla, es conocido disponer unas espigas
intramedulares, largas, que se pueden insertar o roscar y
adaptaciones de éstas, tanto en los componentes femorales
(adaptación que da a la cara proximal) como tibiales (adaptación
que da a la cara distal); y espigas femorales, cortas, que dan la
espalda al condilo. Sin embargo, estos tipos, en general, no son
intercambiables después de la implantación, a menos que, por
ejemplo, la prótesis se tenga que sacar y modificar a continuación
de su extracción, lo que puede presentar un impacto profundo,
serio y negativo para el paciente.
Siempre es deseable minimizar la extensión de la
invasión quirúrgica y sus complicaciones potenciales que pueden
surgir de ésta. Véase, por ejemplo, Goodman y otros.
También se conoce un implante de articulación
prostética a partir del documento EP 0 724 868 A en el que una
primera pieza prostética (1) presenta una pieza de anclaje (18) y
por lo menos una conexión que puede girar (15). Una segunda pieza
prostética (2) presenta una pieza de anclaje (4) y una superficie
deslizante plana o curvada (6). Se coloca una pieza intermedia (3)
entre la conexión que puede girar y la superficie deslizante. La
pieza intermedia y la superficie deslizante presentan cada una una
pieza de guiado (7, 11a) y la segunda pieza prostética presenta
una pieza de guiado (98) que sobresale en una dirección proxímal.
Un acoplamiento (12) conectado a la primera pieza prostética
coopera con la pieza de guiado (8) y fija un eje pivotante en
parte para ambas partes de la prótesis. Sin embargo, la
articulación se implanta con todos sus componentes característicos
y no es posible cambiar el implante, por ejemplo para alargar la
espiga intramedular, sin la extracción de la articulación del lugar
del
implante.

Otro implante de articulación modular es
conocido a partir del documento DE 4 434 806 en el que se establece
una conexión que se puede añadir a una articulación de rodilla
prostética durante la operación de cirugía, o la articulación de
rodilla prostética se puede implantar sola.
La presente invención tiene como objetivo
proporcionar una articulación modular que permita añadir
componentes a una articulación implantada sin sacar la articulación
del lugar en el que ha sido implantada.
De acuerdo con la presente invención se
proporciona un implante de articulación de la rodilla modular que
incluye un componente de implante de la articulación prostética
básica que se puede implantar estructurado para funcionar como un
primer componente del implante de la articulación prostética
completa y un subcomponente de articulación modular estructural
que se puede añadir en donde el implante de la articulación de
rodilla modular incluye un componente femoral que presenta una
superficie condilar lisa de geometría generalmente convexa que
incluye una pestaña condilar distal, un condilo medial inferior y
un condilo lateral inferior; en una pestaña posterior, un condilo
medial posterior y un condilo lateral posterior; en una pestaña
anterior, un condilo medial anterior y un condilo lateral
anterior; con una superficie opcional en un lado superficial de la
pestaña anterior, en la que generalmente se puede desplazar una
tróclea; siendo dicha superficie condilar de tal modo que hay
presente una muesca intercondilar entre los condilos medial y
lateral; a través de la cual, cuando se añade el subcomponente
estructural que se puede añadir al primer componente del implante
de la articulación que se puede implantar, se forma un segundo
implante de articulación prostética completa sin que se haya de
extraer el implante de la articulación del lugar en el que se había
implantado inicialmente, caracterizado porque el primer componente
del implante de la articulación prostética completa es un implante
de articulación prostética completa básica que incluye una parte
estructural de recepción, que se forma de manera integral con el
implante de la articulación prostética completa básica como una
sola pieza y que se puede adaptar para recibir dicho subcomponente
de la articulación modular estructural que se puede añadir y el
componente femoral es un componente femoral artificial que incluye
una estructura de componente femoral a modo de parte estructural de
recepción, que incluye dos paredes laterales unidas a una pared
delantera, la misma que se puede unir en las direcciones proximal y
de profundidad a las citadas pestañas condilares distales posterior
y anterior, con dicha pestaña distal unida a las mencionadas
pestañas posterior y anterior; y la mencionada superficie condilar
lisa de geometría generalmente convexa, que está unida a la
estructura del componente femoral en las direcciones distal y
superficial.
Otras características de la invención se pondrán
de manifiesto a partir de las reivindicaciones dependientes.
La invención es útil en artroplastia.
De un modo significativo, mediante la presente
invención, se proporciona una articulación modular adaptable y
versátil. Incluso después de la implantación de la articulación
básica, se pueden añadir los componentes que deben añadirse sin la
extracción de la articulación básica, tal como puede ser requerido
por el estado del paciente. Así, la invasión quirúrgica a partir de
una futura revisión se puede mantener en un mínimo, aunque el
médico esté provisto con una serie de opciones progresivas en
tratamiento artroplástico. Numerosas ventajas adicionales están
presentes en la invención.

necesariamente dibujos a escala, se tiene que señalar brevemente lo

La invención se puede además entender a través
del presente detalle, que se debe leer a la vista de los dibujos.
Esto se tiene que tomar en un sentido ilustrativo, y no de un modo
necesariamente limitativo.
Se tiene que tener presente la anteriormente
mencionada patente de Goodman y otros.
En referencia a los presentes dibujos, la
articulación modular de la presente invención se representa para
los fines de ilustración como una prótesis de la rodilla izquierda
para un ser humano, o a modo de componentes que se pueden añadir a
la prótesis. Una articulación que corresponda a la rodilla humana
derecha puede ser una imagen especular de la de la izquierda. Otras
prótesis, especialmente otras articulaciones de tipo ginglimoide,
por ejemplo, los codos o nudillos pueden estar provistos mediante
analogía apropiada a las rodillas en la práctica de la invención.
Las articulaciones modulares de la invención también se pueden
disponer, con las modificaciones adecuadas, para la implantación en
animales. Cada paciente es diferente, y de este modo, cada uno debe
de ser ajustado a medida. Las articulaciones de la invención
también se pueden disponer perfectamente para aplicaciones
mecánicas o de robots. En consecuencia se pueden emplear varios
diseños, formas y tamaños de articulaciones, y de materiales en la
práctica de la invención.
Con una especial atención a los presentes
dibujos, un componente de la articulación modular puede ser,
digamos, en la forma de una "caja" e incluir un alojamiento de
recepción 1 que puede presentar paredes (laterales) opuestas 2,
superior 3, y otra pared (frontal) que limita la inserción 4, que
se puede oponer o no a otra pared. Se puede(n) disponer
orificio(s) en las paredes, por ejemplo, orificio superior
3H y/o orificio de alojamiento del elemento de estabilización
posterior 3P, que se puede soportar con la pestaña superior
proximal 3F y la pestaña de pared lateral 2F, respectivamente
(véase la figura 13) o no estar soportadas más allá de lo que
simplemente proporciona la pared superior 3 (véase la figura 14); y
un orificio del tornillo que asegura el módulo que se puede añadir
4H (véanse las figuras 4, 5 y 15) que por lo demás puede no estar
roscado, digamos, para la inserción de una espiga en lugar de un
tornillo de ajuste 36. Como otra opción posible, se pueden disponer
canales o ranuras tales como el canal 2C en paredes opuestas 2, y
un eje o varilla, por ejemplo, varilla de estabilización posterior
35, se puede guiar en un lugar interior cerca de la parte superior
3 del alojamiento 1 y se puede asegurar allí mediante la inserción
de tornillos o clavijas, por ejemplo, tornillo de ajuste 36, en el
interior del orificio 2H (véase la figura 4). De modo conocido en
el actual estado de la técnica, un orificio o un canal se pueden
enfundar con un cierre adecuado (que no se ilustra) tal como un
capuchón de polietileno cuando no se están utilizando.
Preferentemente, las paredes son cónicas, por ejemplo, para un huso
Browne & Sharpe, digamos, con un ángulo 2X de entre 1,5 y 2
grados. Los módulos que se pueden añadir 30 (35, 37, 50)
provechosamente corresponden a la forma de alojamiento 1 Aunque se
han representado un alojamiento 1 en forma de caja y módulos que se
pueden añadir en forma de caja 30, se pueden emplear otras formas.
Por ejemplo, la "caja" modular que se puede añadir puede ser
redonda, digamos, con un lado plano para mantener la "caja" a
partir del desplazamiento. Una "caja" que se puede añadir de
este tipo se inserta ventajosamente en el interior de un
receptáculo conformado en correspondencia, digamos, en un
componente femoral de un implante para la rodilla. Volviendo a los
dibujos actuales, el alojamiento 1 puede presentar paredes
sustancialmente planas (véanse las figuras 1 a 3) o presentar una o
más paredes con partes planas unidas en un ángulo tal como con la
pared 4 (véanse las figuras 4 y 5) que presente una parte inferior
4L y una parte superior 4U. Los módulos que se pueden añadir 30
que corresponden se pueden seleccionar para ajustarse a estos
alojamientos 1. Comparar las figuras 1, 3, 6, 7, 13, y 15 con las
figuras 4, 5 y 10 a 12. Un componente que se puede añadir se añade
después de que la articulación básica se ha implantado en el
paciente -sin que se haya de extraer la articulación básica. Entre
tales tipos, de un modo conveniente, el{los) componente(s)
que se puede(n) añadir es(son) capaz(ces) de
ser añadidos de modo íntercondilar, lo que, por ejemplo, puede
incluir el dispositivo que gira que se puede insertar desde abajo
con una pieza de tipo macho que sobresale inferiormente y, que
puede oscilar 50 (véanse las figuras 1 a 3, 6, 7 y 10 a 12); espiga
femoral intramedular 37 (véase la figura 14) y/o tope de
estabilización posterior 35 (véase la figura 15). En el caso de este
último, tal como se conoce en el actual estado de la técnica, el
componente tibial está provisto con una púa, clavija, poste, u otra
estructura posterior, por ejemplo, como parte de una cuña de la
bandeja tibial, para ser un empuje hacia arriba entre los condilos
de tal modo que, cuando el tope 35 entra en contacto con el
elemento tibial que se encuentra encima, se impide la subluxación
anterior o dislocación durante la flexión de la rodilla. Un
componente que se puede añadir, después de la cirugía inicial,
también puede incluir el simple recurso de inserción de una varilla
(que no se ilustra) a través de una abertura disponible tal como
un orificio 3H (véanse las figuras 13 y 14) o un orificio abierto
27H (véase la figura 16) en una articulación básica implantada para
ayudar a fijar una fractura postoperatoria, o empleo de una espiga
intramedular que se puede enroscar, que se puede insertar desde por
encima 27 (véase la figura 16).
En general, el componente femoral básico en
adición a la(s) adaptación(es) de componentes que se
pueden añadir pueden incluir una pestaña posterior 5, una pestaña
anterior 6, una superficie articular/condilar 10 que generalmente
incluye el condilo medial inferior 11; el condilo lateral inferior
12; el condilo medial posterior 13; el condilo lateral posterior
(que no se ilustra en los presentes dibujos); el condilo medial
anterior 15; el condilo lateral anterior (que no se ilustra en los
presentes dibujos); y así sucesivamente. Se puede proporcionar un
conjunto de espigas femorales 7 condilares de apoyo y/o de capa
porosa 9 de tipo tal como de metal de capa porosa.
El componente tibial básico adicionalmente a
la(s) adaptación(es) de componentes que se pueden
añadir puede incluir una superficie de articulación que proporciona
una sustancia (cuña de la bandeja tibial) 20 tal como de
polietileno de peso molecular muy alto (UHMWPE); que
articula/empareja las superficies condilares 21 (medial) y 22
(lateral); bandeja tibial 23; tope/sujeción de la cuña de la
bandeja tibial 24; receptáculo o orificio 25 que recibe el
dispositivo que gira/la espiga axial. Se puede disponer una
articulación metal sobre metal para que haga la componente tibial y
su superficie de articulación 21 completamente de metal.
Opcionalmente, el componente tibial básico también puede contener
el orificio 27C (véanse las figuras 8 y 9) ó 27H (véase las figura
16) que recibe la espiga tibial; pestañas de soporte 28 y/o la capa
porosa 29 (véase la figura 16). También, el componente tibial
básico puede contener un conjunto de orificio(s) a través de
las bandeja tibial 23 a través de los que se puede(n) pasar
el(los) tornillo(s) del hueso (que no se ilustra).
Tal como se ha aludido previamente, el tipo
preferible de componentes que se pueden añadir 30 puede ser una
espiga 33/37 (véase la figura 14) o un módulo que contiene un
elemento de limitación posterior tal como la caja (véase la figura
15) que presenta una barra de limitación posterior trasera 35
incluida con éste. Se puede hacer y emplear una combinación de
espiga y módulo que contiene una limitación posterior, o se puede
hacer y emplear una combinación de espiga y módulo que contiene un
elemento que gira. Generalmente, la presencia de una limitación
posterior obvia el uso efectivo del elemento que puede girar.
Adicionalmente, el componente que se puede
añadir 30, de un dispositivo que gira que se puede insertar por
abajo con una pieza de tipo macho que sobresale inferiormente y,
que puede oscilar 50, en general, puede incluir un alojamiento 31
que presenta paredes laterales 32; pared superior 33 (véanse las
figuras 1 a 3, 6 y 7), que se pueden eliminar en algunas
realizaciones (véanse las figuras 10 a 12); y una pared frontal 34.
De este modo, la parte superior 33 de una "caja" modular es
opcional. De hecho, una realización preferida (véanse las figuras
10 a 12) es sin parte superior. Sin embargo, una parte superior
puede transmitir estabilidad estructural, si se requiere, y/o
proporcionar otra ubicación para sujetar otro componente (por
ejemplo, véase la figura 14, espiga intramedular femoral) o
proporcionar un lugar para la fijación de la "caja" en su
correspondiente receptáculo. Los orificios 52 en las paredes
laterales 32 alojan una espiga de articulación (eje) 55. El bloque
pivotante (elemento que gira) 51 presenta un orificio 52A, que
continua a lo largo de la dirección de los orificios 52; la
cubierta de la espiga cónica 53; y el orificio de la espiga
perforadora 54. El eje 55 pasa a través de los orificios 52 y 52A,
que proporcionan un pivote para el elemento que gira 51. La cubierta
de la espiga cónica 53 puede ser un tapiado liso, cónico, digamos,
con una conicidad Morse (véanse las figuras 3 y 7), o estar
provisto con roscas 56 (véanse las figuras 1, 2 y 6). La espiga
cónica 61 se inserta en el interior de la cubierta 53 y se puede
sujetar en su lugar con una espiga perforadora 65 (véanse las
figuras 3 y 7) o con roscas 66 (véanse las figuras 1, 2 y 6).
Preferentemente, sin embargo, la parte 50 es construida de una sola
pieza 70 con la parte elemento que gira superior como un bloque 71
que está hecho a partir de la misma pieza que la parte media de la
pierna 73 y la parte inferior del poste de limitación 70, siendo
los dos últimos en general cilíndricos, con el poste de limitación
estrechamente conformado en forma al receptáculo 25 del componente
tibial. El orificio del eje 72 puede estar dispuesto en la parte
superior 71 para alojar el paso del eje 55. Alternativamente, la
parte del elemento que gira se puede hacer totalmente construida en
una sola pieza con un elemento de eje construido en el mismo que
sale como un poste que se opone en cualquiera de los lados del
elemento 71, y el mismo se fija en el interior de un alojamiento 31
mediante el forzado del alojamiento alrededor para fijarlo.
También, el alojamiento 31 puede incluir paneles frontales
conformados 34L (inferior) y 34U (superior). El orificio 34H se
puede emplear para sujetar la unidad 50 en el componente femoral a
través de un tornillo o espiga (pasado en el interior del orificio
4H). Para proporcionar a la limitación cónica, por ejemplo, la
anteriormente mencionada conicidad Browne & Sharpe, los lados
32 son apropiadamente inclinados, digamos, en alrededor de un
ángulo 32X de entre 1,6 y 2 grados (véase la figura 11). Los
principios funcionales básicos de un elemento módulo que gira son
paralelos a los de la articulación artificial de la mencionada
patente de Goodman y otros.
La capacidad se puede disponer módulo a módulo.
Por ejemplo, la presente invención se puede alojar en una capacidad
caja a caja. Por ejemplo, el módulo espiga 30/37 (véase la figura
14) se puede hacer para que sea capaz de recibir un módulo
adicional tal como un módulo elemento de estabilización posterior
tal como el módulo 35/50 o recibir un dispositivo que gira que se
puede insertar por debajo con una pieza de tipo macho que sobresale
inferiormente y, que puede oscilar tal como el módulo 30/50 ó
30/70.
Alternativamente, se puede extraer un módulo
instalado previamente antes de la instalación de otro módulo. En
esta conexión, se puede disponer una ranura de apalancamiento (que
no se ilustra) al módulo del componente que se puede añadir para
ayudar a su extracción.
Se pueden disponer otras adaptaciones para otros
componentes modulares que se pueden insertar para tal como, por
ejemplo, una varilla de estabilización posterior separada 35, que
se puede insertar en el interior del orificio 3P (véase la figura
13); una espiga intramedular femoral que se puede colocar desde
arriba 37 en el interior de un orificio 3H, cuya espiga presenta
roscas internas 38 de tal modo que se puede sujetar con tornillo 39;
o una espiga tibial que se puede colocar desde abajo 27Q, cuya
espiga presenta roscas externas para que enrosque en el interior de
un orificio tapado 27C (véanse las figuras 8 y 9). Sin embargo,
estas otras adaptaciones y componentes modulares que se pueden
insertar puede que no se puedan insertar fácilmente después de que
se haya implantado la articulación básica.
Se pueden añadir el(los)
componente(s) que se puede(n) añadir según se ha
indicado para un paciente en especial.
Dimensiones adicionales a modo de ejemplo pueden
incluir éstas que siguen a continuación:

Se pueden emplear materiales y métodos adecuados
para hacer la articulación modular de la presente invención, y la
articulación básica se puede crear según un cimiento anatómico.
Véase, por ejemplo, la especificación de la patente de Goodman y
otros. De este modo, se pueden emplear en la práctica de la
presente invención metales, cerámicas, y plásticos, incluyendo
plásticos de diseño. Sin embargo, preferentemente, se utilizan
sustancialmente metal en todas partes, excepto, por ejemplo, para
una cuña de la bandeja tibial, digamos otra vez, que se puede
hacer de UHMWPE (polietileno de muy alta densidad), o para cierres,
y otra vez decimos, que se pueden hacer de material de polietileno
adecuado. Más preferentemente, en artroplastias de articulación
totales, se utiliza el mismo metal para ambos componentes de la
articulación, por ejemplo, componentes femoral y tibial de la
rodilla. Se prefiere el cobalto como metal o aleación base,
especialmente cuando se dispone según las bien conocidas normas
ASTM F-799 o ASTM F-1537.
De un modo beneficioso, la articulación modular
de la presente invención está construida a lo largo de una
cimentación de la articulación básica que pueda proporcionar una
transferencia de carga natural. Véanse, por ejemplo, las presentes
figuras 1 a 16. Véase también, la descripción de la patente de
Goodman y otros.
En consecuencia, entre otras realizaciones en la
práctica de la presente invención, se resalta lo que sigue a
continuación: en un articulación artificial, que generalmente
presenta una capacidad de transferencia de carga natural, que
incluye un primer componente que incluye una primera superficie
articular y un dispositivo que gira, en donde el dispositivo que
gira incluye una pieza de tipo macho que sobresale inferiormente y,
que puede oscilar; y un segundo componente que incluye una segunda
superficie articular para que se empareje con la primera superficie
articular, y un receptáculo del dispositivo que gira, en donde el
receptáculo del dispositivo que gira incluye una pieza de tipo
hembra - con el primer componente que se puede emparejar con el
segundo componente a través de una cooperación
macho-hembra del dispositivo que gira y el
receptáculo del dispositivo que gira, y en donde el primer
componente puede cooperar con el segundo componente en contacto con
la primera y segunda superficies articulares y articulados en la
articulación cuando el primer componente está emparejado con el
segundo componente, en donde la primera y segunda superficies
articulares son de tal modo que pueden presentar contacto de
deslizamiento anatómico la una contra la otra durante la rotación
anatómica adicionalmente a la flexión y la extensión anatómicas -
la mejora puede incluir una adaptación tal como que por lo menos un
componente que se puede añadir se puede añadir a la articulación
básica sin la extracción de la articulación del lugar en el que se
puede implantar inicialmente. También se debe resaltar a lo largo
de las líneas que preceden la realización que sigue de la presente
invención: en un componente femoral para una rodilla, que incluye
lo que sigue a continuación:
- una estructura de componente femoral, que incluye dos paredes laterales conectadas a una pared frontal, siendo conectable la misma que se puede conectar en direcciones proximal y en profundidad a las pestañas distal condilar, posterior y anterior, con la pestaña distal conectada a las pestañas posterior y anterior;
- una superficie condilar lisa de geometría generalmente convexa conectada a la estructura del componente femoral en direcciones distal y superficial, que incluye un condilo medial inferior; un condilo lateral inferior; un condilo medial posterior; un condilo lateral posterior; un condilo medial anterior; y un condilo lateral anterior; más una superficie troclear en un lado superficial de la pestaña anterior, en la que generalmente se puede desplazar una tróclea; siendo la superficie condilar de tal modo que existe una ranura intercondilar entre los condilos medial y lateral; y
- un dispositivo que gira conectado a las estructura del componente femoral, que incluye lo que sigue a continuación:
- un elemento de rotación angular que presenta un orificio del elemento de rotación en una extremidad proximal a éste, cuyo orificio abarca el elemento de rotación en una dirección medial a lateral, cuyo elemento de rotación alrededor de una extremidad distal de ésta se puede insertar en el interior de un receptáculo correspondiente en una bandeja tibial; y
- un eje que pasa a través del orificio que abarca el elemento de rotación y que está conectado a una estructura mediante orificios en las paredes de ésta;
- la mejora puede incluir a modo de dispositivo que gira, un dispositivo modular de rotación que se inserta desde abajo, que se puede insertar en el interior y se puede extraer de un receptáculo correspondiente de la estructura del componente femoral, en donde el elemento de rotación angular está conectado al eje que está conectado a las paredes del dispositivo modular de rotación que se inserta desde abajo mediante orificios en las paredes a través de las que pasa el eje.
La presente invención se dispone de este modo.
Se pueden emplear varias características, partes, subcombinaciones
y combinaciones con y sin referencia a otras características,
partes, subcombinaciones y combinaciones en la práctica de la
invención, y se pueden efectuar numerosas adaptaciones y
modificaciones dentro de las limitaciones de las
reivindicaciones.
Claims (11)
1. Implante de articulación de la rodilla
modular que incluye un componente de implante de la articulación
prostética básica que se puede implantar estructurado para
funcionar como un primer componente de implante de articulación
prostética completa (5, 6, 10 a 13, 15, 20 a 23) y un subcomponente
de articulación modular estructural que se puede añadir (27, 30,
33, 35 a 37, 50, 70) en el que el implante de la articulación de
rodilla modular incluye un componente femoral que presenta una
superficie condilar lisa (10) de geometría generalmente convexa
que incluye, en una pestaña condilar distal, un condilo medial
inferior (11) y un condilo lateral inferior (12); en una pestaña
posterior (5), un condilo medial posterior (13) y un condilo
lateral posterior; en una pestaña anterior (6), un condilo medial
anterior (15) y un condilo lateral anterior; con una superficie
opcional en un lado superficial de la pestaña anterior (6), en la
que generalmente se puede desplazar una tróclea; siendo dicha
superficie condilar (10) de tal modo que existe una muesca
intercondilar entre los condilos medial y lateral; a través de la
cual, cuando se añade el sub-componente estructural
que se puede añadir al primer componente del implante de la
articulación que se puede implantar, se forma un segundo implante de
articulación prostética completa sin que se haya de extraer el
implante de la articulación del lugar en el que se había implantado
inicialmente, caracterizado porque el primer componente del
implante de la articulación prostética completa es un implante de
articulación prostética completa básica (5, 6, 10 a 13, 15, 20 a
23) que comprende una parte estructural de recepción (1 a 4, 2C,
3F, 3P, 4H, 27C, 27H), que se forma de manera integral con el
implante de la articulación prostética completa básica en una sola
pieza y que se puede adaptar para recibir dicho subcomponente de la
articulación modular estructural que se puede añadir (27, 30, 33,
35 a 37, 50, 70), y el componente femoral es un componente femoral
artificial que incluye una estructura de componente femoral a modo
de parte estructural de recepción, que incluye dos paredes
laterales (2) unidas a una pared delantera (4), la misma que se
puede unir en las direcciones proximal y de profundidad a dichas
pestañas condilares distales posterior y anterior (5, 6), con la
citada pestaña distal unida a las mencionadas pestañas posterior y
anterior (5, 6), y la dicha superficie condilar lisa (10) de
geométrica generalmente convexa, que está unida a la estructura
del componente femoral en las direcciones distal y superficial.
2. Articulación modular según la reivindicación
1, en la que un dispositivo que gira (30, 50, 70) está unido a la
estructura del componente femoral, que incluye lo que sigue a
continuación:
- un elemento de rotación angular (31, 51) que presenta un orificio para el elemento de rotación (52, 52A) en un extremo proximal de éste, orificio que atraviesa el elemento de rotación en una dirección medial hacia el lateral, cuyo elemento de rotación alrededor de un extremo distal del mismo se puede insertar en el receptáculo correspondiente (25) en una bandeja tibial (23); y
- un eje (55) que pasa a través el orificio (52, 52A) que atraviesa el elemento de rotación y que está unido a una estructura por medio de los orificios (52, 52A) en las paredes laterales (32) de éste; y porque
- el dispositivo que gira (30, 50, 70) comprende un dispositivo modular que gira que se puede insertar desde abajo, que se puede insertar en y se puede retirar de un receptáculo correspondiente de la estructura del componente femoral.
3. Articulación modular según la reivindicación
1 ó 2, en la que el alojamiento de recepción (1) está previsto en
la estructura del componente femoral y generalmente entre los
condilos medial y lateral (11, 12), siendo capaz de recibir dicho
alojamiento de recepción una pieza insertada del componente modular
desde una dirección inferior.
4. Articulación modular según la reivindicación
3, en la que el alojamiento de recepción (1) presenta la forma de
una caja que tiene las paredes laterales opuestas (2), una pared
superior (3) y una pared delantera de limitación de la pieza
insertada (4).
5. Articulación modular según la reivindicación
4, en la que las paredes laterales (2) están inclinadas la una con
relación a la otra.
6. Articulación modular según la reivindicación
3 6 4, que presenta por lo menos un orificio en por lo menos una de
dichas paredes lateral, superior y delantera (2, 3, 4).
7. Articulación modular según la reivindicación
1, en la que el por lo menos un componente que se puede añadir
presenta la forma de un primer módulo (30/32-34/37)
en el que se puede insertar un segundo módulo
(30/35-36, 30/50, 30/70) sin la retirada del primer
módulo del implante de la articulación.
8. Articulación modular según la reivindicación
7, en la que el primer módulo incluye una forma de caja
(30/32-34) que está adaptada para recibir el
segundo módulo que incluye una forma de caja
(50/32-34) que se puede alojar en la caja del primer
módulo.
9. Articulación modular según la reivindicación
1, en la que el por lo menos un subcomponente que se puede añadir
se selecciona a partir del grupo constituido por un dispositivo que
gira que se puede insertar (50, 70) con una pieza de tipo macho que
sobresale inferiormente y que puede oscilar (50-52,
52A, 55, 61, 70-74), una espiga intramedular (37) y
un tope de estabilización posterior (35).
10. Articulación modular según la reivindicación
9, en la que el subcomponente estructural que se puede añadir
incluye un componente tibial (20-25) y presenta una
articulación de metal sobre metal siendo de metal las superficies
de la articulación (10) de dicho componente femoral (5, 6, 10) y
las superficies de la articulación (21, 22) de dicho componente
tibial (20-25).
11. Articulación modular según la reivindicación
1, en la que la articulación presenta una capacidad de
transferencia de carga natural e incluye un primer componente que
incluye una primera superficie de articulación (10) y un
dispositivo que gira (50, 70) en la que el dispositivo que gira
incluye una pieza de tipo macho que sobresale inferiormente y que
puede oscilar (50-52, 52A, 55, 61,
70-74); y un segundo componente (20, 33) que
incluye una segunda superficie de articulación (21, 22) destinada a
acoplarse a la primera superficie de articulación (10), y un
receptáculo del dispositivo que gira (25), en el que el receptáculo
del dispositivo que gira incluye una pieza de tipo hembra (25),
pudiéndose acoplar dicho primer componente (1) a dicho segundo
componente por medio de una cooperación macho-hembra
de un poste de tipo macho (61, 70, 71, 73, 74) del dispositivo que
gira (50, 70) y del receptáculo del dispositivo que gira (25) en la
que dicho primer componente (1) puede cooperar con dicho segundo
componente (20, 23) mediante un contacto de la primera y segunda
superficies de articulación (10, 21, 22) y en la juntura de la
articulación cuando dicho primer componente (1) se acopla con dicho
segundo componente (20, 23) y en la que dichas superficies primera
y segunda de articulación (10, 21, 22) son tales que pueden
presentar un contacto de deslizamiento anatómico la una contra la
otra durante una rotación anatómica además de una flexión y de una
extensión anatómicas.
