Prótesis modular de cadera.
La presente invención se refiere en general al
campo de la artroplastia total de cadera, y, más especialmente, a un
vástago de cadera modular de tres segmentos que permite la
intercambiabilidad completa de tamaños entre las partes componentes,
y sin embargo proporciona una resistencia superior al
desacoplamiento de los componentes durante la utiliza-
ción.

La modularidad en el diseño de la artroplastia
total de cadera es un concepto en evolución que se cita de modo
creciente en la literatura clínica. Las ventajas de estos sistemas
incluyen su flexibilidad para el llenado del canal próximo y distal
según la necesidad del usuario, la adaptación a situaciones
difíciles de pérdidas de hueso y deformidades proximales. Estos
diseños, sin embargo, provocan inquietudes que se refieren al
aspecto estructural de las interconexiones
metal-metal debido a esfuerzos y desacoplamiento de
los componentes.
El documento WO 95/13757 da a conocer una
prótesis modular de cadera, que comprende un segmento proximal que
incluye un cuello encajable de forma bloqueable con un componente de
cabeza femoral, incluyendo además el segmento proximal una parte
macho ahusada que se extiende distalmente desde dicho cuello, un
segmento distal que tiene un extremo proximal y una punta distal,
incluyendo el segmento distal una parte macho ahusada adyacente al
extremo proximal del mismo, y un segmento metafisial que tiene un
extremo proximal y un extremo distal, incluyendo además el segmento
metafisial un orificio axial a lo largo del mismo. El orificio axial
incluye partes ahusadas hembra primera y segunda, la primera parte
ahusada hembra situada adyacente al extremo proximal del segmento
metafisial y configurada dimensionalmente para encajar de forma
bloqueable en la parte ahusada macho del segmento proximal, y la
segunda parte ahusada hembra situada adyacente a dicho extremo
distal del segmento metafisial y configurado dimensionalmente para
encajar de forma bloqueable con la parte ahusada macho del segmento
distal.
Según la presente invención, en dicha prótesis
modular de cadera, la parte ahusada macho del segmento distal y la
segunda parte ahusada hembra del segmento metafisial, y/o la parte
ahusada macho del segmento proximal y la primera parte ahusada
hembra del segmento metafisial, comprenden cada una de ellas una
sección ahusada cónica que se une con una sección de forma general
parabólica.
La geometría de ahusado cónico/ahusado parabólico
combinada de cada parte ahusada asegura suficiente zona de contacto
ahusada, y disminuye los esfuerzos de contacto interfacial y
esfuerzos del cuerpo interno bajo cargas de flexión de la unión
ahusada macho/hembra. Cada una de las secciones ahusadas cónicas
tiene ángulos de ahusado en el intervalo de aproximadamente 1º a
2,5º para proporcionar resistencia a la torsión aumentada en las
uniones ahusadas. El segmento proximal se puede encajar de forma
bloqueable con el extremo proximal del segmento metafisial para
alinear los orificios axiales formados a través de los segmentos
proximal y metafisial. El extremo proximal del segmento distal puede
encajarse de forma bloqueable con el extremo distal del segmento
metafisial para alinear los orificios axiales formados a través de
los segmentos distal y metafisial.
Opcionalmente, el segmento proximal está formado
con un orificio pasante, y el segmento distal se forma con un
orificio roscado adyacente a su extremo proximal. Estos orificios
son alineables con el orificio axial del segmento metafisial. Un
tornillo, configurado dimensionalmente para pasar por los orificios
alineados, se encaja en forma de rosca con el orificio roscado
formado en el segmento distal para aumentar además, si se desea, el
acoplamiento bloqueado de los componentes de la prótesis.
La presente invención proporciona las siguientes
ventajas: (a) resistencia superior al desmontaje de los componentes
por incremento de la zona de contacto de forma ahusada y reducción
de los esfuerzos de contacto debido a cargas de flexión y torsión en
las uniones ahusadas; (b) flexibilidad intraoperativa por su
modularidad; (c) completa intercambiabilidad de cualquier segmento
con cualquier otro segmento; (d) ajustabilidad de cada segmento para
anteversión y retroversión independiente de la posición de otros
segmentos, permitiendo así un diseño universal para aplicaciones
derecha e izquierda de cadera; (e) selección independiente de
longitud de patas y desplazamiento de la prótesis; (f) aplicación
primaria y de revisión con el mismo sistema; (g) permite al cirujano
confeccionar el dispositivo a medida de la anatomía del paciente
incluso frente a una cirugía de revisión que podría llevar a un
déficit de hueso; y (h) la utilización de todos los estilos y
tamaños de componentes de cabeza femoral.

detallada y realizaciones preferentes de la invención, y junto con
la descripción detallada, sirven para exponer los principios de la
invención. Debe entenderse, sin embargo, que tanto los dibujos como
la descripción son solamente explicativos y no son limitativos de la
invención.

Haciendo referencia a continuación a las figuras
1-8, en las que se utilizan números de referencia
iguales para identificar componentes iguales a través de diversas
vistas, se muestra en general con el numeral (10) una primera
realización de la prótesis modular de cadera de la invención. Como
se muestra en la figura 1, la prótesis de cadera (10) incluye de
manera general: (a) un segmento proximal (12); (b) un segmento
metafisial (14); y (c) un segmento distal (16). Un tornillo roscado
(18) puede utilizarse opcionalmente para aumentar el acoplamiento de
bloqueo de los segmentos (12), (14), y (16) como se describe más
adelante. Según esta realización, el segmento proximal (12), el
segmento metafisial (14), y el segmento distal (16) se fabrican cada
uno como partes separadas. Como resultado de ello, los segmentos
pueden ser dimensionados cada uno independientemente de cualquier
otro. Esta capacidad de dimensionado independiente proporciona
modularidad a la prótesis, es decir, proporciona al cirujano una
amplia selección de configuraciones de prótesis para adaptarse
virtualmente a cualquier situación anatómica encontrada durante la
cirugía. Ventajosamente, la prótesis modular (10) de la invención
puede ser implantada utilizando técnicas de implantación de cemento
a hueso bien conocidas o, alternativamente, puede ser implantada en
modalidad sin cemento, utilizando aplicaciones de superficie para
encajar con el hueso bien conocidas por las personas expertas en la
materia.
Haciendo referencia a continuación a la figura 2,
el segmento proximal (12) incluye un cuello (20) formado con: (a) un
brazo desplazado angularmente (21) que termina en una columna (22)
ahusada macho; (b) un componente de prolongación (24) que se
extiende distalmente del cuello (20) formado con una parte (25)
ahusada macho, y terminando en un tetón cilíndrico (26) y (c) un
orificio segmentado (27) formado a través del cuello (20),
componente de extensión (24) y tetón (26). Preferentemente, el
segmento proximal (12) está fabricado a partir de una aleación
biocompatible de titanio de elevada resistencia. Sin embargo, el
segmento proximal (12) puede fabricarse de otros materiales
biocompatibles tales como una aleación de cobalto y cromo, acero
inoxidable, y materiales compuestos. La superficie acabada exterior
del segmento proximal (12) está preferentemente pulida, con una
superficie de rugosidad promedio de 32 micropulgadas o menos, tal
como se ha determinado por rugosimetría. La superficie exterior
acabada puede también ser mate o mecanizada lisa, utilizando
técnicas de preparación de superficie bien conocidas por los
expertos.
La columna ahusada (22) del segmento proximal
(12) está configurada dimensionalmente para un acoplamiento de
bloqueo con la parte ahusada complementaria hembra de un componente
de cabeza femoral (no representado). Un experto en la materia
reconocerá fácilmente que el segmento proximal (12) puede estar
fabricado para adaptarse a todos los estilos y materiales de
componentes de cabeza femoral. Una hendidura (23) está formada en el
brazo (21) y la columna (22) en cada lado del segmento proximal (12)
para incrementar el intervalo de desplazamiento entre el cuello (20)
y el componente acetabular (no representado) de un sistema de
sustitución de una unión total de cadera, y para facilitar el
acoplamiento de una herramienta (no mostrada) de separación de la
cabeza femoral cuando es necesario para desencajar la cabeza femoral
del segmento proximal (12) durante la reparación o revisión de la
prótesis de cadera (10).
Como realización preferente, la parte ahusada
(25) del componente de extensión (24) comprende una sección (25a)
ahusada cónica macho combinada con una sección (25b) ahusada macho
conformada en forma generalmente parabólica que tiene un radio de
unión (R2) de unos 0,65 cm (0,25 pulgadas) (ver figuras 2 y 8). La
geometría parabólica de la sección ahusada (25b) disminuye los
esfuerzos de contacto interfacial y esfuerzos del cuerpo interno
bajo cargas de flexión entre la parte ahusada (25) y la parte (33)
ahusada complementaria hembra del segmento metafisial (14) (descrito
más adelante). Como realización preferente, la sección ahusada
cónica (25a) tiene un ángulo de huso en el intervalo de
aproximadamente 1º a 2,5º para proporcionar una resistencia a la
torsión aumentada en la unión ahusada proximal/metafisial. En la
realización ilustrativa de la invención mostrada en la figura 8, la
sección ahusada cónica (25a) tiene una longitud aproximada de 0,97
cm (0,43 pulgadas) y la sección ahusada parabólica (25b) tiene una
longitud aproximada de 0,23 cm (0,09 pulgadas). Por estas longitudes
ahusadas ilustrativas, la proporción de la longitud ahusada
parabólica con respecto a la longitud ahusada cónica es de un 21%
aproximadamente. Como realización preferente, la proporción de la
longitud ahusada parabólica/ahusado cónico debería estar en el
intervalo de aproximadamente 5% a aproximadamente 30%. Este
intervalo asegura suficiente zona de contacto ahusada, y minimiza la
presencia de esquinas agudas en la sección ahusada parabólica (25b)
que puedan llevar a esfuerzos elevados de punto de contacto en la
unión ahusada proximal/metafisial cuando la prótesis está sujeta a
esfuerzos de flexión. Como realización preferente, la sección
ahusada cónica (25a) tiene un radio de unión (R1) de unos 0,23 cm
(0,09 pulgadas) (ver figura 8). La sección complementaria ahusada
cónica (33a) del segmento ahusado hembra (33) tiene un radio de
unión (R3) de unos 0,13 cm (0,05 pulgadas). Estos radios que
difieren crean una situación de esfuerzo reducido en la unión
ahusada proximal/metafisial en la vecindad del intersticio (G) (ver
figura 7) que se crea cuando se unen los segmentos proximal y
metafisial. Ventajosamente, las mismas geometrías y radios para las
partes ahusadas (25) y (33) pueden utilizarse para todos los tamaños
de segmento proximal (12) y segmento metafisial (14), aumentando de
este modo la intercambiabilidad de tamaños, y de este modo, la
modularidad, entre los segmentos proximal y metafisial.
Como realización preferente, el tetón (26) tiene
una longitud de unos 0,46 cm (0,18 pulgadas) para aumentar el brazo
del momento del componente de extensión (24) (ver figuras 2 y 8) y
de este modo, ayuda en la descarga de la unión ahusada
proximal/metafisial en la inducción de esfuerzos de flexión en la
prótesis. Como con las geometrías de ahusado y radios de unión
descritos anteriormente, la misma longitud para el tetón (26) puede
utilizarse para todos los tamaños de segmento proximal (12). El
tetón (26) está configurado dimensionalmente menor que el diámetro
de las secciones (32a), (32b) y (32c) del orificio pasante (32) en
el segmento metafisial (14) (que se describe más adelante) de modo
que, cuando el componente de extensión (24) del segmento proximal
(12) se recibe en forma deslizante en el orificio pasante (32) en el
montaje de los componentes de la prótesis (ver figura 7 y su
explicación más adelante), el tetón (26) no encajará inicialmente en
la pared lateral del orificio (32). A la aplicación de carga
suficiente a la cabeza de femoral de la prótesis (no representada),
el tetón (26) contactará con la pared lateral del orificio
intermedio del segmento (32b) del orificio (32), y de esta manera,
transfiere una parte de los esfuerzos inducidos de flexión fuera de
la unión ahusada proximal/metafisial.
Haciendo referencia nuevamente a la figura 2, el
orificio segmentado (27) del segmento proximal (12) incluye una
primera sección recta (27a), una sección intermedia ahusada (27b), y
una segunda sección recta (27c). Como realización preferente, la
sección (27b) se ahusa hacia adentro hacia el orificio de la sección
(27c), en un ángulo de unos 60º. Los orificios de las secciones
(27a), (27b) y (27c) están configurados dimensionalmente para
permitir al tornillo (18) pasar a través del segmento proximal (12).
El orificio de la sección (27a) también actúa como un avellanado
para la cabeza del tornillo (18), y debería ser dimensionado
suficientemente grande para recibir cómodamente un accionador
mecánico tal como un destornillador o una broca para encajar de
forma roscada el tornillo (18) con el orificio roscado (42) formado
en el segmento distal (16) (descrito de forma más completa más
adelante) cuando el tornillo (18) se utiliza como parte del montaje
de la prótesis (10).
Haciendo referencia a continuación a la figura 3,
el segmento metafisial (14) tiene un extremo proximal (14a), un
extremo distal (14b) y está configurado con una sección trapecial de
pirámide truncada (30), integrada con una sección cónica (31). Como
se muestra en la figura 4, este perfil se presenta a sí mismo en una
sección transversal como una sección en general trapecial (36),
desplazada de una sección en general circular (35).
Alternativamente, la sección piramidal (30) puede estar construida
de modo que el segmento metafisial (14) tiene una sección
transversal rectangular en general desplazada de una sección
transversal circular en general. El segmento metafisial (14) se
fabrica preferentemente de una aleación de titanio de elevada
resistencia, biocompatible, pero también puede fabricarse a partir
de otros materiales biocompatibles tales como la aleación de cobalto
y cromo, acero inoxidable, y materiales compuestos.
El segmento metafisial (14) también incluye un
orificio (32) que comprende una sección de orificio proximal (32a),
una sección de orificio intermedio (32b), y una sección de orificio
distal (32c). Haciendo referencia a las figuras 3 y 8, el segmento
de orificio (32a) está formado de una parte ahusada hembra (33) que
comprende una sección ahusada cónica (33a) que se une a la sección
(33b) de forma generalmente parabólica ahusada. Las secciones
ahusadas hembra (33a) y (33b) son complementarias de las secciones
ahusadas macho (25a) y (25b), respectivamente, de la sección
cilíndrica (24). Tal como se ha realizado en este caso, la sección
ahusada cónica (33a) tiene un ángulo de ahusado en el intervalo de
1º aproximadamente a 2,5º aproximadamente, una longitud de unas 0,5
pulgadas, y un radio de unión (R3) (al que se hace referencia
anteriormente) de unos 0,13 cm (0,05 pulgadas). La sección ahusada
parabólica (33b) tiene una longitud de aproximadamente 0,23 cm (0,09
pulgadas) y un radio de unión (R4) de unos 0,65 cm (0,25 pulgadas)
(ver figura 8). Para las anteriores longitudes ilustrativas de
ahusado, la proporción de la longitud de ahusado parabólico con
respecto a la longitud de ahusado cónico es aproximadamente de un
18%. Las secciones ahusadas (33a) y (33b) están configuradas
dimensionalmente para encajar de forma bloqueable con las secciones
ahusadas (25a) y (25b), respectivamente, en el momento de la
inserción de la sección cilíndrica (24) en el orificio (32). Como
con las secciones ahusadas (25a) y (25b) de la sección cilíndrica
(24), la proporción de la longitud ahusada parabólica/ahusada cónica
para las secciones ahusadas (33a) y (33b) debería estar en el
intervalo del 5% aproximadamente al 30% aproximadamente para
asegurar esfuerzos de contacto y esfuerzos internos reducidos en la
zona de la unión ahusada proximal/metafisial. También, tal como se
explicó anteriormente con respecto al segmento proximal (12), se
pueden utilizar las mismas geometrías de ahusado y radios de unión
para las secciones ahusadas (33a) y (33b), para todos los tamaños
del segmento metafisial (14) para incrementar la intercambiabilidad
de los componentes proximal y metafisial, y de este modo, la
modularidad de la prótesis (10).
Haciendo nuevamente referencia a las figuras 3 y
8, el orificio del segmento (32c) del orificio metafisial (32) está
formado de la parte ahusada (34), que comprende una sección ahusada
cónica (34a) y una sección ahusada de forma parabólica general
(34b). Las secciones ahusadas (34a) y (34b) están configuradas
dimensionalmente para encajar de forma bloqueable en las
correspondientes secciones ahusadas macho (43a) y (43b) del segmento
distal (16), respectivamente, en el momento de la inserción del
extremo proximal (16a) del segmento distal (16) en el orificio (32)
del segmento metafisial (14) (como se explica más adelante de forma
más completa). Tal como se ha realizado en este caso, la sección
ahusada cónica (34a) tiene una longitud de aproximadamente 1,30 cm
(0,51 pulgadas), un intervalo de ángulo de ahusado de
aproximadamente 1º a 2,5º aproximadamente, y un radio de unión (R5)
de unos 1,27 cm (0,50 pulgadas). La sección ahusada parabólica (34b)
tiene una longitud de aproximadamente 0,23 cm (0,09 pulgadas) y un
radio de unión (R6) de unos 0,65 cm (0,25 pulgadas) (ver figura 8).
Para las anteriores longitudes ahusadas ilustrativas, la proporción
de la longitud ahusada parabólica con respecto a la longitud ahusada
cónica es aproximadamente del 18%. Tal como con las otras partes
ahusadas de la prótesis (10) explicadas anteriormente, la proporción
de la longitud de ahusado parabólico/ahusado cónico debería estar en
el intervalo de aproximadamente 5% a aproximadamente 30% para
asegurar suficiente zona de contacto ahusado y minimizar los
esfuerzos de contacto de puntos elevados en la unión ahusada
proximal/metafisial. Asimismo, tal como con las otras partes
ahusadas descritas anteriormente, se pueden utilizar las mismas
geometrías de ahusado y radios de unión para las secciones ahusadas
(34a) y (34b) para todos los tamaños del segmento metafisial (14)
para aumentar la intercambiabilidad de los componentes, y de este
modo, la modularidad de la prótesis (10).
La geometría del segmento metafisial (14)
incrementa la estabilidad de torsión del componente durante su
utilización en el cuerpo, y proporciona mejor llenado del canal
intramedular proximal. La superficie exterior acabada del segmento
metafisial (14) puede ser pulida, con una rugosidad superficial
promedio de unos 0,81 mm (32 micropulgadas) o menos determinado por
rugosimetría. La superficie exterior acabada puede también ser mate
o mecanizada lisa, utilizando técnicas de preparación de superficie
bien conocidas por los técnicos en la materia. Como realización
preferente, la superficie exterior del segmento metafisial (14)
contiene un revestimiento superficial para encajar con el hueso, tal
como, por ejemplo, superficie chorreada con arena, revestimiento con
rociado de plasma, revestimiento de gotas de metal sinterizado,
revestimiento de hidroxilapatita u otros revestimientos bioactivos
tales como cerámica de biocristal, hueso desmineralizado y portador,
y factor y portador de crecimiento. La aplicación de tales
revestimientos a superficies de implantes metálicos es bien conocida
en la técnica. Opcionalmente, el segmento metafisial (14) puede ser
fabricado con un anillo distal (37). El anillo distal (37) es una
zona de material elevado igual en espesor al espesor mínimo del
revestimiento para acoplamiento con el hueso aplicado a la
superficie exterior del segmento metafisial. El anillo distal (37)
incrementa el espesor de la pared de la sección cónica (31) del
segmento metafisial (14). Esto a su vez incrementará la resistencia
a la fatiga de la sección cónica (31) por incrementar el espesor
local de la pared y protegiéndola de hendiduras que puedan resultar
del proceso del revestimiento poroso. Como realización preferente,
el anillo distal (37) debería ser usado en tamaños pequeños del
segmento metafisial (14), en los que la pared lateral de la sección
cónica (31) en la proximidad del extremo distal (14b) puede ser
relativamente delgada. Las personas expertas en la materia pueden
determinar fácilmente los niveles de esfuerzos locales en la sección
cónica (31), que pueden necesitar la utilización del anillo distal
(37) para un tamaño particular del segmento metafisial (14).
Haciendo referencia a continuación a la figura 5,
el segmento distal (16) está formado de un extremo proximal (16a),
una punta distal (16b), e incluye una serie de estrías
longitudinales afiladas (40) formadas a lo largo de una longitud
incremental de su superficie exterior. Los bordes afilados de las
estrías (40) entran dentro de la pared cortical del hueso del canal
intramedular para incrementar la estabilidad a la torsión del
segmento distal (16) durante el uso de la prótesis en el cuerpo. El
segmento distal (16) está también formado opcionalmente por una
ranura coronal (41) que empieza en la punta distal (16b), y prosigue
de forma proximal en una longitud incremental de la misma. La ranura
coronal (41) aumenta la flexibilidad del segmento distal (16). Esta
flexibilidad aumentada inhibe la concentración de esfuerzos en la
punta distal (16b) cuando se carga la prótesis, y permite a la
prótesis adaptarse mejor a la curvatura del canal intramedular. Las
personas expertas en la materia reconocerán que la longitud de las
estrías longitudinales (40) puede fácilmente ajustarse tal como se
desee, a la luz del esquema completo de diseño de la prótesis, para
facilitar la resistencia a cargas a la torsión sobre la prótesis. En
la realización ilustrativa del segmento distal (16) mostrado en las
figuras, la longitud de las estrías longitudinales (40) es
aproximadamente del 80% de la longitud completa del segmento distal
(16). Ventajosamente, la misma relación de longitud de estría a
longitud de segmento distal puede utilizarse para todos los tamaños
de segmento distal (16). Los expertos en la materia reconocerán que
la longitud de la ranura coronal (41) puede ser fácilmente ajustada
para proporcionar el grado deseado de flexibilidad en el segmento
distal (16) sin comprometer indebidamente la resistencia a la fatiga
del segmento distal.
Como realización preferente, la punta distal
(16b) tiene una sección transversal axial, parabólica en general, la
cual sirve también para reducir los esfuerzos de contacto entre el
segmento distal (16) y el hueso en la proximidad de la punta distal.
Tal como se muestra en la figura 6, el segmento distal (16) tiene
una sección transversal redonda en general, pero puede fabricarse
con otras geometrías de sección transversal tales como, por ejemplo,
hexagonal u oval. Opcionalmente, el segmento distal (16) puede estar
formado de canales longitudinales en vez de estrías longitudinales
afiladas para facilitar tanto la flexibilidad incrementada del
vástago como el acoplamiento con el hueso cortical en el canal
intramedular. Aunque el segmento distal (16) mostrado en las figuras
tiene un perfil recto, puede también ser curvado para emparejar
mejor con la curvatura natural del canal intramedular del paciente.
El segmento distal (16) está fabricado preferentemente a partir de
una aleación de titanio de elevada resistencia, biocompatible, pero
también puede ser fabricado de otros materiales biocompatibles tales
como aleación de cobalto y cromo, acero inoxidable, y otros
materiales compuestos. Además, el segmento distal (16) está provisto
preferentemente de una superficie exterior de acabado pulido que
tiene una rugosidad superficial promedio de 32 micropulgadas o
menos, determinadas por rugosimetría. El segmento distal puede estar
dotado también de una superficie exterior de acabado mate o
mecanizada lisa utilizando técnicas de preparación de superficie
bien conocidas por los expertos en la materia. Para facilitar la
fijación del segmento distal (16) a la pared cortical del hueso del
canal intramedular, si se desea, el segmento distal (16) puede estar
fabricado también sin estrías longitudinales, y dotado en vez de
ello de un hueso poroso que acople en el revestimiento de la
superficie, tal como, por ejemplo, superficie por ataque con arena,
revestimiento rociado de plasma, revestimiento de gotas de metal
sinterizado, revestimiento de hidroxilapatite, u otros
revestimientos bioactivos tales como la cerámica de biocristal,
hueso desmineralizado y portador, y factor y portador de
crecimiento.
Haciendo referencia a continuación a las figuras
5 y 8, el segmento distal (16) está formado también con un orificio
roscado (42) adyacente a su extremo proximal (16a). El orificio (42)
está configurado dimensionalmente para encajar por roscado el
tornillo (18) a la inserción a través de los orificios alineados del
segmento proximal (12), segmento metafisial (14), y segmento distal
(16) (ver la explicación más adelante). El segmento distal (16) está
formado también con una parte macho ahusada (43) adyacente al
extremo proximal (16a). El segmento ahusado (43) comprende una
sección ahusada cónica (43a) y una sección ahusada de forma
parabólica general (43b). Las secciones ahusadas macho (43a) y (43b)
están configuradas dimensionalmente para encajar de forma bloqueable
con las correspondientes secciones ahusadas hembra (34a) y (34b) del
segmento metafisial (14), respectivamente, a la inserción del
extremo proximal (16a) del segmento distal (16) dentro del orificio
(32) del segmento metafisial (14). Como realización en este caso, la
sección ahusada cónica (43) tiene una longitud de unos 1,22 cm (0,48
pulgadas), un ángulo de ahusado en el intervalo de aproximadamente
1º a 2,5º, y un radio de unión (R7) de unos 0,23 cm (0,09 pulgadas).
La sección ahusada parabólica (43b) tiene una longitud de unos 0,23
cm (0,09 pulgadas) y un radio de unión (R8) de aproximadamente 0,65
cm (0,25 pulgadas) (ver figura 8). Para las anteriores longitudes
ahusadas ilustrativas, la proporción de la longitud ahusada
parabólica con respecto a longitud ahusada cónica es de 19%
aproximadamente. La proporción de longitud ahusado parabólico/cónico
debería estar en el intervalo de aproximadamente 5% a 30% para
asegurar suficiente zona de contacto ahusada y minimizar los
esfuerzos elevados de puntos de contacto en la unión ahusada
metafisial/distal. Asimismo, tal como con las otras partes ahusadas
descritas anteriormente, pueden ser utilizadas las mismas geometrías
de ahusado y radios de unión para las secciones ahusadas (43a) y
(43b), para todos los tamaños del segmento distal (16), para
aumentar la intercambiabilidad de los componentes distal y
metafisial, y, de este modo, la modularidad de la prótesis (10).
Refiriéndonos a continuación a las figuras 7 y 8,
se muestran las vistas en sección transversal del segmento proximal
(12), segmento metafisial (14), y segmento distal (16) para ilustrar
más claramente la relación interna entre estos componentes al
montarlos. Tal como se muestra en las figuras, el componente de
extensión (24) del segmento proximal (12) se recibe en relación de
deslizamiento de ajuste ceñido en el orificio de la sección (32a)
del segmento metafisial (14), con secciones ahusadas (25a) y (25b)
de la extensión (24) encajando de forma bloqueable en las secciones
ahusadas (33a) y (33b) del orificio de la sección (32a),
respectivamente. Similarmente, el extremo proximal (16a) del
segmento distal (16) se recibe en relación de deslizamiento de
ajuste ceñido en el orificio del segmento (32c) del segmento
metafisial (14), con secciones ahusadas (43a) y (43b) del segmento
distal (16) que encaja de forma bloqueable las secciones ahusadas
(34a) y (34b) del orificio del segmento (32c), respectivamente.
Antes de que se establezca una relación de bloqueo ahusado entre el
segmento proximal (12) y el segmento metafisial (14), se establece
la orientación angular del brazo (21) y columna (22) del segmento
proximal (12) para colocar la columna (22) en la posición deseada
para recibir un componente de cabeza femoral convencional (no
representado). Al acoplamiento de bloqueo de las partes ahusadas
complementarias de los segmentos proximal, metafisial y distal, los
orificios (27), (32) y (42) estarán en alineación axial.
Seguidamente, se inserta el tornillo (18) a través de los orificios
alineados para acoplamiento roscado con la sección complementaria
roscada del orificio (42). El tornillo (18) tiene una cabeza
avellanada (19) que puede alojarse en el avellanado (28) formado en
la sección (27a) del orificio metafisial (27). El tornillo (18), si
se desea, se asegura apretadamente para un acoplamiento de bloqueo
más fuerte de los segmentos proximal, metafisial y distal.
La presente invención puede realizarse en otras
formas distintas a la descrita en la descripción detallada de la
invención, sin salirse de las características esenciales de la
invención. Por consiguiente, las realizaciones descritas de la
invención han de considerarse en todos los aspectos como
ilustrativas y no restrictivas. El alcance de la presente invención
es, por consiguiente, el indicado por las reivindicaciones expuestas
a continuación, y no por la descripción anterior de la invención.
Todas las modificaciones que caen dentro del significado y rango de
equivalencia de la materia objeto de lo reivindicado han de quedar
comprendidas en el ámbito de las reivindicaciones.
Claims (22)
1. Prótesis modular de cadera (10), que
comprende:
un segmento proximal (12), incluyendo dicho
segmento proximal (12) un cuello (20) encajable de forma bloqueable
con un componente de cabeza femoral, incluyendo además dicho
segmento proximal (12) una parte ahusada macho (25) que se extiende
distalmente desde dicho cuello (20);
un segmento distal (16) que tiene un extremo
proximal (16a) y una punta distal (16b), incluyendo dicho segmento
distal (16) una parte ahusada macho (43) adyacente a su extremo
proximal (16a);
un segmento metafisial (14) que tiene un extremo
proximal (14a) y un extremo distal (14b), incluyendo además dicho
segmento metafisial (14) un orificio axial (32) a través del mismo,
incluyendo dicho orificio axial (32) partes ahusadas hembra primera
y segunda (33, 34), situada dicha parte ahusada hembra primera (33)
adyacente a dicho extremo proximal (14a) de dicho segmento
metafisial (14) y configurado dimensionalmente para encajar de forma
bloqueable dicha parte ahusada macho (25) de dicho segmento proximal
(12), situada dicha parte ahusada hembra segunda (34) adyacente a
dicho extremo distal (14b) de dicho segmento metafisial (14) y
configurado dimensionalmente para encajar de forma bloqueable con
dicha parte ahusada macho (43) de dicho segmento distal (16), en el
que uno de los segmentos distales (16) y el segmento metafisial (14)
incluyen una parte de superficie exterior para encajar en el
hueso;
caracterizada porque dicha parte ahusada
macho (43) de dicho segmento distal (16) y dicha segunda parte
ahusada hembra (34) de dicho segmento metafisial (14), y/o dicha
parte ahusada macho (25) de dicho segmento proximal (12) y dicha
primera parte ahusada hembra (33) de dicho segmento metafisial (14),
comprende cada una una sección ahusada cónica (25a; 33a; 34a; 43a)
unida a una sección parabólica general (25b; 33b; 34b; 43b).
2. Prótesis modular de cadera, según la
reivindicación 1, en la que dicho segmento proximal (12) incluye
además una parte de superficie exterior que encaja en el hueso y un
orificio axial (27) a través del mismo, encajable dicho segmento
proximal (12) con dicho extremo proximal (14a) de dicho segmento
metafisial (14) para alinear dichos orificios axiales (27, 32)
formados a través de dichos segmentos proximal y metafisial (12,
14), incluyendo además dicho segmento distal (16) un orificio axial
roscado (42) adyacente a su extremo proximal (16a), siendo encajable
dicho extremo proximal (16a) de dicho segmento distal (16) con dicho
extremo distal (14b) de dicho segmento metafisial para alinear
dichos orificios axiales (32, 42) formados a través de dichos
segmentos distal y metafisial (16, 14), comprendiendo además dicha
prótesis modular de cadera (10) un tornillo (18) configurado
dimensionalmente para pasar a través de dichos orificios alineados
(27, 32, 42) de dichos segmentos proximal, metafisial y distal (12,
14, 16) y en acoplamiento roscado con dicho orificio axial roscado
(42) de dicho segmento distal (16).
3. Prótesis modular de cadera, según la
reivindicación 1, en la que dicho segmento distal (16) incluye una
parte de superficie exterior para encajar con el hueso.
4. Prótesis modular de cadera, según la
reivindicación 3, en la que dicha superficie de acoplamiento con el
hueso de dicho segmento distal (16) comprende estrías longitudinales
(40) formadas a lo largo de una longitud incremental de la
misma.
5. Prótesis modular de cadera, según la
reivindicación 1, en la que dicho segmento distal (16) comprende
además una ranura coronal (41) a lo largo de una longitud
incremental del mismo.
6. Prótesis modular de cadera, según la
reivindicación 3, en la que dicha superficie de acoplamiento con el
hueso de dicho segmento distal (16) se selecciona del grupo que
consiste en una superficie por ataque con arena, revestimiento de
gotas de metal sinterizado, revestimiento de hidroxilapatita,
revestimiento con rociado de plasma, revestimiento de cerámica de

7. Prótesis modular de cadera, según la
reivindicación 1, en la que dicho segmento distal (16) presenta una
sección transversal en general circular.
8. Prótesis modular de cadera, según la
reivindicación 1, en la que la punta distal (16b) de dicho segmento
distal (16) presenta una sección transversal en general
parabólica.
9. Prótesis modular de cadera, según la
reivindicación 1, en la que dicho segmento distal (16) está
fabricado de un material seleccionado del grupo que consiste en una
aleación de metal titanio, aleación de cobalto y cromo, y acero
inoxidable.
10. Prótesis modular de cadera, según la
reivindicación 1, en la que la proporción de la longitud ahusada
parabólica con respecto a la longitud ahusada cónica asociada con
dicha parte ahusada macho (43) de dicho segmento distal (16) y dicha
segunda parte ahusada hembra (34) de dicho segmento metafisial (14)
está en el intervalo de aproximadamente 5% a aproximadamente
30%.
11. Prótesis modular de cadera, según la
reivindicación 10, en la que el ángulo de ahusado de dicha sección
ahusada cónica (43a) de dicho segmento distal (16) y dicha segunda
parte ahusada hembra (34) de dicho segmento metafisial (14) están en
el intervalo de aproximadamente 1º a 2,5º.
12. Prótesis modular de cadera, según la
reivindicación 1, en la que la sección ahusada macho (25) de dicho
segmento proximal (12) está formada por un componente de extensión
(24) que se extiende distalmente desde dicho cuello (20).
13. Prótesis modular de cadera, según la
reivindicación 12, en la que dicho componente de extensión (24)
incluye un componente tetón (26) que se extiende distalmente desde
el mismo.
14. Prótesis modular de cadera, según la
reivindicación 1, en la que la proporción de la longitud ahusada
parabólica con respecto a la longitud ahusada cónica asociada con la
parte ahusada macho (25) de dicho segmento proximal (12) y dicha
primera parte ahusada hembra (33) de dicho segmento metafisial (14)
está en el intervalo de aproximadamente 5% a aproximadamente
30%.
15. Prótesis modular de cadera, según la
reivindicación 14, en la que el ángulo de ahusado de dicha sección
ahusada cónica (25a) de dicho segmento proximal (12) y dicha primera
parte hembra ahusada (33) de dicho segmento metafisial (14) están en
el intervalo de aproximadamente 1º a aproximadamente 2,5º.
16. Prótesis modular de cadera, según la
reivindicación 1, en la que dicho segmento proximal (12) está
fabricado de un material seleccionado del grupo que consiste en
aleación de metal titanio, aleación de cobalto y cromo, y acero
inoxidable.
17. Prótesis modular de cadera, según la
reivindicación 1, en la que el segmento metafisial (14) incluye una
superficie de acoplamiento con el hueso seleccionada del grupo que
consiste en un acabado por ataque de arena, revestimiento de gotas
de metal sinterizado, revestimiento de hidroxilopatita,
revestimiento con rociado de plasma, revestimiento de cerámica de
biocristal, hueso y portador desmineralizado, y factor y portador de
crecimiento.
18. Prótesis modular de cadera, según la
reivindicación 1, en la que dicho segmento metafisial (14) está
fabricado a partir de un material seleccionado de un grupo que
consiste en aleación de metal titanio, de aleación de cobalto y
cromo, y de acero inoxidable.
19. Prótesis modular de cadera, según la
reivindicación 1, en la que dicho segmento metafisial (14) presenta
una sección trapecial de pirámide truncada integrada con una sección
cónica en general.
20. Prótesis modular de cadera, según la
reivindicación 19, en la que dicho segmento metafisial (14) tiene
una sección transversal trapecial en general, desplazada de una
sección transversal circular en general.
21. Prótesis modular de cadera, según la
reivindicación 19, en la que dicho segmento metafisial (14) incluye
un anillo exterior (37) formado alrededor, como mínimo, de una parte
de dicha sección en general cónica.
22. Prótesis modular de cadera, según la
reivindicación 1, en la que dicho segmento proximal (12) puede
encajar con dicho extremo proximal (14a) de dicho segmento
metafisial (14) para alinear los orificios axiales (27, 32) formados
a través de dichos segmentos proximal y metafisial (12, 14), siendo
encajable dicho extremo proximal (16a) de dicho segmento distal (16)
con dicho extremo distal (14b) de dicho segmento metafisial (14)
para alinear los orificios axiales (32, 42) formados a través de
dichos segmentos distal y metafisial (16, 14); y
dicha prótesis modular de cadera (10) comprende
además un tornillo (18) configurado dimensionalmente para pasar a
través de dichos orificios alineados (27, 32, 42) de dichos
segmentos proximal, metafisial y distal (12, 14, 16) y en
acoplamiento roscado con dicho orificio axial roscado (42) de dicho
segmento distal (16).
