Description
Configuraciones de stents mejoradas.
La presente invención se refiere a stents con una
configuración mejorada.
Los stents son endoprótesis expansibles
radialmente que constituyen implantes típicamente intravasculares
capaces de ser implantados transluminalmente y de aumentar de tamaño
radialmente tras ser introducidos percutáneamente. También se han
implantado en tractos urinarios y conductos biliares. Se usan para
reforzar los vasos del organismo y para prevenir la restenosis tras
una angioplastia en el sistema vascular. Pueden ser
auto-expansibles o expandirse mediante una fuerza
radial interna, como cuando se montan sobre un globo.
En el pasado, los stents han tenido generalmente
una forma tubular pero han estado compuestos por muchas
configuraciones y se han hecho a partir de muchos materiales,
metales y plástico incluidos. Se han usado metales corrientes como
acero inoxidable así como metales con memoria de forma tales como
Nitinol y similares. También se han hecho stents a partir de
materiales plásticos biodegradables. Tales stents se han formado a
partir de alambre, material para la fabricación de tubos, etc.
Algunos stents son auto-expansibles y otros se
expanden mediante una fuerza radial interna.
En el documento EP 0734698 se describe un stent
que tiene unas aberturas que se repiten tanto en dirección
longitudinal como en la dirección periférica del stent y permiten la
expansión del stent. Cada abertura tiene al menos una parte
dispuesta oblicuamente al eje longitudinal del stent, tanto en un
estado comprimido como en un estado expandido del stent.
La presente invención proporciona nuevas
configuraciones de los segmentos que forman los stents que pueden
adaptarse a todos los diversos tipos de stents de la técnica
anterior anteriormente descritos y/o conocidos de antemano en la
técnica. Las nuevas configuraciones tienen numerosas ventajas. Por
ejemplo, las configuraciones de la invención limitan el retroceso y
añaden resistencia a la compresión para un stent expandido, entre
otras cosas. Además, los stents de la presente invención son
flexibles longitudinalmente.
Los stents de la invención comprenden una
pluralidad de elementos anulares alineados para formar un stent de
cuerpo cilíndrico. Cada elemento anular, a su vez, está compuesto
por segmentos abiertos y, generalmente, con forma de bumerán. Los
segmentos están conectados entre sí de arriba abajo alrededor de
cada uno de los elementos anulares. Los elementos anulares contiguos
están conectados por uno o más elementos de interconexión. Cada
elemento de interconexión se prolonga desde un extremo de un
segmento de un elemento anular con forma de bumerán hasta un extremo
de un segmento con forma de bumerán de un elemento anular contiguo.
En una forma de realización preferida, el elemento de interconexión
se prolonga desde cada segmento con forma de bumerán de un elemento
anular a un segmento adyacente con forma de bumerán de un elemento
anular contiguo.
Es deseable que los elementos de conexión que
unen los elementos anulares contiguos tengan forma de "U" o
"zigzag", aunque se pueden usar otros elementos de conexión
curvilíneos y rectilíneos.
Los elementos con forma de bumerán contiguos de
un elemento anular se pueden interconectar a través de un enlace que
se prolonga desde la parte superior hasta la parte inferior de un
segmento contiguo. Los enlaces pueden variar en el diseño, desde un
conector corto y recto hasta cualquiera de las formas descritas a
continuación para conectar elementos entre sí.
La figura 1 muestra esquemáticamente formas de
bumerán;
la figura 2 es una vista plana en planta de una
forma de realización de una configuración de stent de la invención
en su estado no expandido;
la figura 3 es una vista longitudinal del stent
de la figura 2 en su estado tubular normal no expandido;
la figura 4 es una vista plana en planta de una
forma de realización de una configuración de stent de la invención
en su estado no expandido;
la figura 5 es una vista longitudinal del stent
de la figura 3 en su estado tubular expandido.

se puede usar para unir elementos anulares contiguos en otra forma
de realización de la invención; y
la figura 7 es una vista del stent de la figura 3
tras ser doblado.
Para los fines de la presente invención, el
término "bumerán" se usa para describir la forma de
determinados segmentos de stent y se usa en el sentido que se
describe en el Websters New Collegiate Dictionary en
referencia a la figura 1 del presente documento:
"Bumerán 1": "Palo arrojadizo curvado o
anguloso que puede lanzarse de forma que regrese a un punto cercano
al de partida".
En el 110 de la fig. 2 o la fig. 4 se ilustra una
vista plana de una forma de realización de un stent generalmente
cilíndrico según la invención. El stent puede estar formado por un
tubo de un metal tal como nitinol, o acero inoxidable
preferiblemente, que se haya atacado con ácidos o, preferiblemente,
cortado con láser para obtener la configuración que se muestra en la
vista plana en planta de las figuras 2 ó 4. La configuración puede
estar formada por una lámina plana y enrollada en forma de cilindro
con una línea de unión soldada o similar que une los bordes 112 y
114, o la configuración puede estar formada directamente en forma de
un pequeño tubo tal como un hipotubo. En el 210 de la figura 3 se
muestra generalmente una forma tubular del stent.
Las configuraciones que se muestran en las figs.
2 a 4 están constituidas por una pluralidad de elementos anulares
alineados 114, alineados según se muestra para proporcionar un stent
de cuerpo generalmente cilíndrico. Cada elemento anular 114 está
compuesto por una serie de segmentos con forma de cilindro
generalmente en 118 (ver el segmento sombreado en las figuras para
mayor claridad) que tiene una estructura abierta unida de arriba 120
abajo 122 en la unión de los segmentos 124. Cada segmento bumerán
comprende una primera parte superior 119 por lo general dispuesta
longitudinalmente y una segunda parte inferior 121 por lo general
dispuesta longitudinalmente con una abertura entre las mismas. Los
segmentos 118 están dispuestos o conectados en malla, tal como se
muestra en las figuras, con los extremos 126 de segmentos adyacentes
de elementos anulares contiguos unidos por elementos de
interconexión 128. En las figs. 2 a 4, el elemento de interconexión
es un elemento en forma de U que es una curva parcialmente abierta.
Otros elementos de interconexión incluyen el elemento en zigzag 228
tal como se muestra en la fig. 6, que se puede usar en lugar del
elemento en forma de U 128 para unir los elementos anulares
contiguos 114.
Las configuraciones de las figs. 2 y 4 son
sustancialmente similares entre sí, diferenciándose principalmente
por la presencia de una concavidad 130 en cada parte inferior 122 de
cada segmento 118 en la configuración de la fig. 4. Sin quedar
limitados por una teoría en particular, se cree que la presencia de
la concavidad limita hasta que punto el stent se curva hacia fuera
del plano al expandirse.
Es deseable que los segmentos con forma de
bumerán sean al menos básicamente simétricos respecto a una línea
central 138 que se prolonga desde la parte superior 120 del segmento
hasta la parte inferior 122 del segmento. La línea central 138 está
situada a medio camino entre los extremos 126 del segmento.
Cuando el stent de la fig. 2 se expande, tal como
se muestra generalmente en el 310 de la fig. 5, por ejemplo en un
globo, los segmentos con forma de bumerán 118 del stent no expandido
adoptan una nueva configuración. Los segmentos 318 adoptan la forma
de triángulos redondeados con partes inferiores protuberantes
322.
Es deseable que los elementos de interconexión
tengan forma de U tal como se muestra en las figs. 2 a 4, o de
zigzag tal como se muestra en la fig. 6. Sin embargo, en un sentido
más general, la invención contempla el uso de elementos de
interconexión curvilíneos, así como rectilíneos, elementos rectos
incluidos. En los documentos WO 97/32543 de Divysio Solutions LTD. y
WO 97/40780 de David C. Jang se describen ejemplos de otros
conectores apropiados. Por supuesto, los segmentos con forma de
bumerán contiguos se pueden unir por los lados con un área de
superposición entre segmentos contiguos con forma de bumerán.
También es deseable que los elementos de
interconexión sean flexibles a fin de adaptarse a la curvatura del
stent sin que los segmentos con forma de bumerán sufran una
distorsión considerable. La figura 7 muestra un stent de la figura 3
que ha sido curvado. Tal como se muestra en la fig. 7, al curvar el
stent, los elementos de interconexión en tensión se abren, mientras
que los elementos de interconexión en compresión se cierran para
adaptarse a la curvatura del stent.
Aunque, tal como se muestra en las figuras, el
elemento de interconexión se prolonga desde cada segmento con forma
de bumerán de un elemento anular hasta el segmento con forma de
bumerán más cercano de un elemento anular contiguo, la invención
contempla además la posibilidad de que el elemento de interconexión
se prolongue desde cada segmento con forma de bumerán de un elemento
anular hasta el segmento con forma de bumerán adyacente, de un
elemento anular contiguo, siguiente al más cercano. En este último
caso, los extremos primero 140 y segundo 144 de cada elemento de
interconexión 128 estarían descentrados circunferencialmente a lo
largo del stent.
En un sentido más general, la invención contempla
además un stent en el que cada elemento anular contiguo se
interconecta mediante uno o más elementos de interconexión y cada
elemento de interconexión se prolonga desde un extremo de un
segmento con forma de bumerán de un elemento anular hasta un extremo
de un segmento con forma de bumerán de un elemento anular contiguo.
Así, no es necesario que el elemento de interconexión se prolongue
desde cada segmento con forma de bumerán. Un ejemplo de esto es un
stent en que los elementos de interconexión se prolongan desde cada
segundo o tercer segmento con forma de bumerán de un elemento
anular.
La invención contempla además la posibilidad de
alterar la orientación de algunos de los elementos anulares. En una
de tales formas de realización, los elementos anulares contiguos del
patrón plano están girados 180º unos en relación con otros a fin de
que los elementos anulares contiguos apunten en direcciones
opuestas.
Aunque los extremos de los segmentos adyacentes
más cercanos de los elementos anulares contiguos se muestran en las
figuras alineados entre sí a lo largo de la circunferencia del
stent, la invención contempla además formas de realización del stent
en las que los segmentos adyacentes más cercanos de los elementos
anulares contiguos están desplazados circunferencialmente unos
respecto a otros.
En otra serie más de formas de realización, los
elementos anulares contiguos (o no contiguos) pueden estar formados
por elementos con forma de bumerán de diferentes tamaños. Así, los
elementos anulares contiguos (o no contiguos) pueden abarcar
diferentes longitudes. Subsidiariamente, los elementos anulares
contiguos (o no contiguos), pueden comprender un número diferente de
segmentos con forma de bumerán.
Aunque en la forma de realización de las figs. 2
a 4, se muestra la unión de los segmentos 124 como un enlace pequeño
y recto que se prolonga desde la parte superior de un segmento hasta
la parte inferior de un segmento contiguo, la invención contempla
además la posibilidad de que los elementos anulares contiguos de
dentro de un elemento anular estén conectados mediante enlaces con
forma de U, enlaces en zigzag o de cualquiera de las formas
descritas anteriormente para los elementos de interconexión. Además,
entre los segmentos con forma de bumerán se pueden intercalar
segmentos con otras formas.
El stent de la invención puede ser
auto-expansible o expansible mecánicamente, por
ejemplo, mediante un globo. El stent puede estar hecho de diversos
materiales biocompatibles apropiados entre los que se incluyen
metal, plástico y cualquier otro material capaz de funcionar como un
stent expansible. Por ejemplo, el stent puede estar hecho de hilo o
cinta de un metal tal como tantalio, acero inoxidable o similar, o
de una lámina metálica o tubo metálico. Puede ser un tubo delgado.
Puede ser de una aleación con memoria de forma tal como, por
ejemplo, Nitinol o similares.
La finalidad de los ejemplos y la descripción
anteriores es ilustrativa y no exhaustiva. Estos ejemplos y esta
descripción sugerirán al experto en esta materia muchas variaciones
y posibilidades. Se pretende que todas estas posibilidades y
variaciones queden incluidas en el alcance de las reivindicaciones
adjuntas. Aquellos que estén familiarizados con la técnica pueden
reconocer otros equivalentes a las formas de realización específicas
descritas en el presente documento, y también se pretende que estos
equivalentes queden incluidos en las reivindicaciones adjuntas al
presente documento.
Claims (15)
1. Stent básicamente cilíndrico, expansible
radialmente (110) en estado no expandido compuesto por
una pluralidad de elementos anulares (114)
alineados para formar un stent de cuerpo cilíndrico,
y cada elemento anular (114) está compuesto por
una pluralidad de segmentos con forma de bumerán (118), y cada
segmento con forma de bumerán tiene una primera parte superior (119)
dispuesta, por lo general, longitudinalmente y una segunda parte
inferior (121) dispuesta, por lo general, longitudinalmente, con una
abertura con forma de bumerán entre las mismas, los segmentos con
forma de bumerán contiguos están interconectados, la parte superior
con la parte inferior, alrededor de cada uno de los elementos
anulares,
elementos anulares contiguos interconectados en
uno o más lugares.
2. El stent de la reivindicación 1 en el que los
elementos anulares contiguos (114) se interconectan mediante uno o
más elementos de interconexión (128), y cada elemento de
interconexión se prolonga desde un extremo de un segmento con forma
de bumerán de un elemento anular hasta un extremo de un segmento con
forma de bumerán de un elemento anular contiguo.
3. El stent de la reivindicación 1 en el que un
elemento de interconexión (128) se prolonga desde cada segmento con
forma de bumerán (118) de un elemento anular (114) hasta un segmento
con forma de bumerán de un elemento anular contiguo.
4. El stent de la reivindicación 3 en el que un
elemento de interconexión (128) se prolonga desde cada segmento con
forma de bumerán (114) de un elemento anular hasta un segmento con
forma de bumerán adyacente de un elemento anular contiguo.
5. El stent de la reivindicación 1 en el que cada
elemento de interconexión (128) tiene uno o más pliegues en el
mismo.
6. El stent de la reivindicación 5 en el que cada
elemento de interconexión (128) es un segmento con forma de U.

interconectados mediante un enlace con forma de U o en zigzag.
