Abstract
Dispositivo de sujeción quirúrgico, siendo el dispositivo de sujeción una grapa para sujetar un tendón (26) de injerto a un hueso, comprendiendo la grapa (60) quirúrgica dos patillas (61) sustancialmente paralelas interconectadas mediante un elemento (62) de cabeza transversal, siendo cada patilla sustancialmente de la misma longitud y caracterizadas por tener una parte (63) de extremo libre conectada sustancialmente en ángulo (65) recto a la parte (64) de cabeza, de tal manera que el elemento de cabeza se desplace lateralmente desde las partes (63) de extremo libre, y una arandela (66) dispuesta alrededor de las partes (63) de extremo libre próxima a la conexión (65) en ángulo recto con las partes (64) de cabeza.
Description
Dispositivo de sujeción quirúrgico.
La presente invención se refiere generalmente a
un dispositivo de sujeción para sujetar tendones a huesos y, en
particular, a un tornillo quirúrgico que tiene una arandela para
inmovilizar tendones dentro de orificios formados previamente en el
hueso.
Se sabe que los ligamentos cruzados anteriores
lesionados se pueden reparar mediante el injerto de una parte del
tendón rotuliano, que se ha cultivado con bloques de hueso
fisiológicamente unido en cada extremo, entre el fémur y la tibia
en la articulación de la rodilla. Los bloques cultivados de hueso
se colocan normalmente en orificios perforados previamente en el
fémur y la tibia.
Este método de reparación de ligamentos a menudo
es una operación difícil y que lleva mucho tiempo con las
consiguientes preocupaciones por la comodidad y seguridad del
paciente. El propio cultivo de tendón rotuliano puede también
producir problemas para el paciente. En muchos pacientes, el tendón
de injerto a menudo no está en un estado fisiológico adecuado para
su uso. Por tanto, es deseable cultivar tendón sin hueso unido y
luego colocar el tendón en el sitio requerido, de tal manera que el
crecimiento óseo fije el tendón al hueso.
Un requisito principal para la fijación es que el
tendón de injerto se fije de manera segura sin movimiento ni
movimiento axial como un émbolo (arriba y abajo, "pistoning")
dentro de un orificio del hueso.
Los métodos actuales para la fijación de un
tendón incluyen una grapa, alrededor de la que se coloca el tendón
antes de que la grapa se sitúe dentro del hueso. Este método sufre
dos desventajas principales. En primer lugar, en el lado femoral,
la grapa se aplica a la superficie externa del fémur y, por tanto,
se requiere de una incisión extra. En segundo lugar, debido a que
la fijación está en el exterior del fémur y el tendón se encuentra
en un túnel del hueso de aproximadamente 40 mm de longitud, la
elasticidad del tendón permite el movimiento axial como un émbolo
bajo carga y así, la cicatrización del tendón de injerto al hueso
puede fallar o ser una cicatrización fibrosa, débil que permite la
posterior laxitud.

Los dispositivos de sujeción quirúrgicos típicos
consisten en dispositivos para sujetar un extremo de un tendón de
injerto a un hueso, teniendo el hueso un orificio formado en el
mismo para alojar el dispositivo de sujeción quirúrgico, siendo el
dispositivo de sujeción quirúrgico un tornillo que tiene una
cabeza, un cuello y un vástago con un extremo distal a la cabeza,
teniendo el vástago una rosca formada a lo largo de al menos una
parte de su longitud, siendo la cabeza generalmente semiesférica y
de un diámetro superior al del vástago, y estando dispuesta una
arandela anular alrededor del cuello del tornillo intermedio entre
la cabeza y el vástago, en el que la arandela está dispuesta de tal
manera que en funcionamiento, el extremo del tendón se pueda
sujetar a la misma e inmovilizarse sustancialmente entre la cabeza
y el interior del orificio.
La rosca en sección longitudinal a lo largo de la
totalidad de la rosca preferiblemente carece de una línea de corte
exterior. En una realización, el tornillo tiene un vástago que
tiene una rosca que es preferiblemente aproximada o completamente
sinusoidal en sección longitudinal de modo que presenta una rosca
suave carente de una línea de corte exterior.
Preferiblemente, el tornillo tiene un casquillo
adaptador concéntrico con la cabeza y una canulación longitudinal
concéntrica al vástago.
En otros dispositivos de sujeción, la arandela
anular se coloca adyacente a la cabeza de un tornillo y

anular, los cuatro orificios localizados preferiblemente en una
mitad de la parte anular.
En otros dispositivos de sujeción, está dispuesto
un tapón anular hecho de hueso alrededor del cuello del tornillo,
entre la arandela anular y la cabeza. El cuello del tornillo en
estos dispositivos de sujeción está adaptado preferiblemente de tal
manera que el tapón de hueso se comprima lateralmente cuando el
tornillo está en uso.
Los dispositivos de sujeción pueden sujetar un
extremo de un tendón a un hueso, comprendiendo las etapas de:
(a) formar un orificio en el hueso, teniendo el
orificio un primer extremo y un segundo extremo y comprendiendo dos
partes, la primera parte adyacente al primer extremo, que tiene un
diámetro mayor que el diámetro de la segunda parte, distal al
primer extremo, de tal manera que hay formado un tope anular en el
orificio;
(b) sujetar el extremo del tendón a la
arandela;
(c) introducir la arandela con el tendón sujeto
en el orificio de tal manera que la arandela se sitúe próxima al
tope anular; y
(d) roscar el tornillo quirúrgico en el orificio
de tal manera que la arandela esté contigua al tope anular y el
tendón se inmovilice con la cabeza del tornillo forzando al tendón
a que esté contiguo al hueso dentro de la primera parte del
orificio.
Preferiblemente, el tendón tiene suturas sujetas
en su extremo, cosiéndose preferiblemente las suturas a uno o más
orificios de la parte anular de la arandela. Las suturas se cosen
más preferiblemente a cuatro orificios de la arandela,
localizándose preferiblemente los cuatro orificios en una mitad de
la parte anular.
El tornillo quirúrgico es preferiblemente de una
longitud tal que un extremo del vástago alcance la corteza externa
del hueso cuando se rosca el tornillo en el hueso. Preferiblemente,
el tornillo se inserta utilizando el casquillo adaptador.
Preferiblemente, el hueso es un fémur y el
orificio en el hueso se forma preferiblemente en el cóndilo femoral
mediante una broca.
Preferiblemente, el tornillo es canulado de modo
que acepte alambres de Kirschner de diámetro mayor o similares que
permitan el guiado exacto del tornillo artroscópicamente.
Los dispositivos de sujeción quirúrgicos pueden
sujetar un extremo de un tendón a un hueso, comprendiendo las
etapas de:
(a) formar un orificio en el hueso;
(b) sujetar el extremo del tendón a la arandela
\hbox{anular;}
(c) colocar la arandela concéntricamente y
próxima a un extremo del tapón de hueso anular;
(d) insertar la arandela y el tapón de hueso en
el orificio utilizando un medio de inserción, de tal manera que la
arandela sea distal a la entrada del orificio; y
(e) roscar el tornillo quirúrgico en el orificio,
de tal manera que el tapón de hueso se comprima lateralmente,
inmovilizándose así el tendón entre la pared del orificio y una
parte sustancial de la longitud del tapón de hueso.
Cuando se utilizan tales dispositivos de
sujeción, se prefiere que el tapón de hueso anular, antes de la
inserción en el orificio del hueso, se monte en un soporte,
teniendo el soporte un primer y segundo extremos y rodeando
preferiblemente al tapón de hueso. En otro dispositivo de sujeción,
la arandela y el tapón de hueso se montan ambos en el soporte, de
tal manera que el tendón sujeto a la arandela se encuentra
sustancial y longitudinalmente a lo largo de la longitud del tapón
de hueso. Preferiblemente, el soporte tiene una protuberancia
próxima a la arandela y al tapón de hueso para alojar al
tendón.
Preferiblemente, el medio de inserción comprende
un émbolo situado en el segundo extremo del soporte que está
adaptado para expulsar el tapón de hueso y la arandela desde el
soporte al orificio del hueso.
La arandela anular, el tapón de hueso, el soporte
y el tornillo quirúrgico se colocan preferiblemente sobre un
alambre guía situado en el centro dentro del orificio.
La presente invención consiste en un dispositivo
de sujeción quirúrgico, siendo el dispositivo de sujeción una
grapa para sujetar un tendón de injerto a un hueso, comprendiendo
la grapa quirúrgica dos patillas sustancialmente paralelas
interconectadas mediante un elemento de cabeza transversal, siendo
cada patilla sustancialmente de la misma longitud y caracterizadas
por tener una parte de extremo libre conectada sustancialmente en
ángulo recto a una parte de cabeza, de tal manera que el elemento
de cabeza se desplaza lateralmente desde las partes de extremo
libre, y una arandela dispuesta alrededor de las partes de extremo
libre próxima a la conexión en ángulo recto a las partes de
cabeza.
Preferiblemente, la grapa quirúrgica tiene dos
patillas paralelas, cada patilla de la misma longitud y teniendo
una parte de extremo libre conectada en ángulo recto a una parte
de cabeza.
El elemento de cabeza transversal que
interconecta las dos partes de cabeza preferiblemente se inclina
hacia abajo desde el plano de las partes de cabeza.
Preferiblemente, la grapa quirúrgica está
compuesta por un elemento lineal doblado hasta la forma requerida.
Preferiblemente, el elemento lineal es un alambre con las partes de
extremo libre teniendo preferiblemente una punta de flecha aguda
para facilitar la inserción de la grapa en el hueso. Las partes del
alambre que es más probable que entren en contacto con el tendón
están son redondeadas preferiblemente, para evitar daños al tendón.
Preferiblemente, el alambre es de un metal inerte
biológicamente.
Preferiblemente, la arandela es de forma oval con
bordes redondeados para evitar daños al tendón de injerto.
Preferiblemente, existen dos orificios desplazados lateralmente en
la arandela para cada una de las patillas. Preferiblemente, la
arandela está hecha de un metal inerte biológicamente.
En una realización adicional, la presente
invención comprende un método para sujetar un asa de tendón a un
hueso utilizando el dispositivo de sujeción quirúrgico según el
tercer aspecto de la invención, que comprende las etapas de:
- (a) colocar la arandela a través del asa del tendón;
- (b) insertar las partes de extremo libre de las patillas de la grapa a través de la arandela de modo que se coloque el asa de tendón dentro de la grapa; e
- (c) insertar la grapa en el hueso de tal manera que el tendón se sujete firmemente al hueso tanto por debajo de la arandela como por debajo del elemento de cabeza desplazado lateralmente, de tal manera que cualquier inclinación de la grapa bajo carga servirá para presionar el elemento de cabeza más firmemente sobre el tendón, mejorando así la laxitud del tendón y la grapa del hueso.
Preferiblemente, la grapa se inserta en el hueso
en el sitio requerido utilizando una herramienta apropiada de tipo
martillo.
Sólo a modo de ejemplo, se describirán ejemplos
de dispositivos de sujeción quirúrgicos con referencia a los
dibujos adjuntos, en los que:
La figura 1 es una vista en sección longitudinal
de un dispositivo de sujeción quirúrgico;
la figura 2 es una vista en planta de una
arandela anular;
la figura 3 es una vista en sección longitudinal
del dispositivo quirúrgico y la arandela de la figura 1 y la
figura 2, respectivamente, tras la inserción en un hueso;
la figura 4 es una vista en sección longitudinal
de un dispositivo de sujeción quirúrgico;
la figura 5 es una vista en sección longitudinal
del tornillo quirúrgico de la figura 4, tras la inserción en un
hueso;
la figura 6 es una vista en sección longitudinal
de un soporte de tapón de hueso;
la figura 7 es una vista en sección longitudinal
de otro soporte de tapón de hueso;
la figura 8 es una vista en alzado posterior del
soporte de la figura 7; y
la figura 9 es una vista en perspectiva de la
presente invención.
En la figura 1, generalmente se muestra como 10
un tornillo quirúrgico para sujetar un tendón de injerto a un
hueso.
El tornillo 10 quirúrgico incluye una cabeza 11
generalmente semiesférica, un cuello 12 y un vástago 13. El vástago
13 tiene una rosca 14 que es aproximadamente sinusoidal en sección
longitudinal de modo que presenta una rosca suave carente de una
línea de corte exterior. Un casquillo 15 adaptador hexagonal se
coloca concéntricamente dentro de la cabeza 11. El tornillo 10
también tiene una canulación 16 central a lo largo de la longitud
del tornillo 10.
Una arandela 17 anular está dispuesta alrededor
del cuello 12 del tornillo 10. Tal como se representa más
claramente en la figura 2, la arandela 17 anular tiene cuatro
orificios 18 en su parte 19 anular. Los orificios 18 se localizan
en una mitad de la parte 19 anular.
La posición del tornillo 10 quirúrgico cuando
sujeta un tendón a un hueso se representa generalmente en la figura
3.
Con el fin de sujetar un tendón utilizando el
tornillo 10 y la arandela 17 de la figura 1 y la figura 2, en
primer lugar, se perfora un orificio 21, compuesto por dos partes
22 y 23, en el hueso 24. La primera parte 22 tiene un diámetro
mayor que el de la segunda parte 23, de tal manera que se forma un
tope 25 anular en el orificio 21.
Entonces, se cose un tendón 26 que tiene suturas
sujetas a un extremo (no representado) a la arandela 17 utilizando
los orificios 18. La arandela 17 con el tendón 26 sujeto se
introduce entonces en el orificio 21 alrededor del alambre 27 guía,
de tal manera que la arandela 17 se coloque próxima al tope 25
anular.
El tornillo 10 quirúrgico se rosca entonces en el
orificio 21, guiado por el alambre 27 guía central, de tal manera
que la arandela 17 se fuerza a estar contigua al tope 25 anular. El
tendón 26 también se inmoviliza contra la pared de la primera parte
22 del orificio 21 mediante la cabeza 11 del tornillo 10.
Una segunda realización de un tornillo quirúrgico
se muestra generalmente como 30 en la figura 4.
El tornillo 30 quirúrgico incluye una cabeza 31,
un cuello 32 afilado y un vástago 33. El vástago 33 tiene una
rosca 34 que es aproximadamente sinusoidal en sección longitudinal,
de modo que presenta una rosca suave carente de línea de corte
exterior. Un casquillo 35 adaptador hexagonal está situado
concéntricamente dentro de la cabeza 31. El tornillo 30 también
tiene una canulación 36 central a lo largo de la longitud del
tornillo 30.
Una arandela 37 anular está dispuesta alrededor
del cuello 32 del tornillo 30. Un tapón 38 de hueso anular está
dispuesto alrededor del cuello 32 entre la arandela 37 y la cabeza
31.
La posición del tornillo 30 quirúrgico cuando
sujeta un tendón a un hueso se representa generalmente en la figura
5.
En esta realización, cuando el tornillo 30 se
rosca en el orificio 39 en el hueso 41, el tapón 38 de hueso se
comprime lateralmente, inmovilizándose así el tendón 26 que está
sujeto a la arandela 37 contra la pared del orificio 39. La
inmovilización del tendón 26 entre el tapón 38 de hueso y la pared
del orificio 39 garantiza la buena fijación del tendón 26 al hueso
41, reduciéndose así el movimiento axial como un émbolo del tendón
y el tiempo de cicatrización.
Con el fin de insertar el tapón 38 de hueso y la
arandela 37 en el orificio 39, se emplea un aparato mostrado
generalmente como 50 en la figura 6.
Antes de la inserción del tapón 38 de hueso y la
arandela 37 en el orificio 39, se coloca un alambre 51 guía en el
centro dentro del orificio 39. Entonces, se sitúa un soporte 52
sobre el alambre 51 guía. El soporte es un manguito cilíndrico en
el que se inserta el tapón 38 de hueso. El tapón 38 de hueso está
canulado y, por tanto, se desliza a lo largo del alambre 51 guía
dentro del soporte 52.
Entonces, una arandela 37 a la que se sujeta un
tendón 26, se desliza sobre el alambre 51 guía y se sitúa próxima a
un extremo 53 del soporte 52. Un medio de inserción que comprende
un émbolo 54 montado en el otro extremo 55 del soporte 52 expulsa
el tapón 38 de hueso desde el soporte 52 al orificio 39 a lo largo
del alambre 51 guía. La arandela 37 a la que se sujeta el tendón
26, se coloca antes que el tapón de hueso y, por tanto, se inserta
en el orificio 39 delante del tapón 38 de hueso.
Una vez que la arandela 37 y el tapón 38 de hueso
se han situado en el orificio 39, se retrae el émbolo 54 y el
soporte 52 se retira del alambre 51 guía.
Tras retirar el soporte 52, el tornillo 30
quirúrgico se desliza entonces sobre el alambre 51 guía y se rosca
en el orificio 39. El cuello 32 del tornillo 30 comprime el tapón
38 de hueso, inmovilizándose así el tendón 26 entre el tapón 38 de
hueso y la pared del orificio 39.
Otra realización del soporte 52 se representa en
la. figura 7 y en la figura 8. En esta realización del soporte 52,
se proporciona una protuberancia 55 en un lado del manguito del
soporte 52. Utilizando esta realización del soporte 52, el tapón 38
de hueso y la arandela 37 a la que se sujeta el tendón 26 se
insertan ambos en el soporte 52. El tendón se sitúa sustancial y
longitudinalmente a lo largo del tapón 38 de hueso y se aloja
dentro de la protuberancia 55 del soporte 52.
Una realización de la presente invención se
muestra generalmente como 60 en la figura 9.
La grapa 60 quirúrgica incluye dos patillas 61
paralelas interconectadas mediante un elemento 62 de cabeza
transversal redondeada. Cada una de las patillas 61 comprende una
parte 63 de extremo libre y una parte 64 de cabeza conectada en
ángulos 65 rectos, de tal manera que el elemento 62 de cabeza se
desplaza lateralmente desde la parte 63 de extremo libre de las
patillas 61. La grapa 60 quirúrgica también incluye una arandela 66
dispuesta alrededor de las partes 63 de extremo libre próxima a los
ángulos 65 rectos en las patillas 61.
Las partes de extremo libre tienen una punta 67
en forma de flecha para facilitar la inserción de la grapa 60 en el
hueso.
La arandela 66 es de forma oval con bordes 68
redondeados para evitar daños al tendón 26 que se pasa alrededor de
la arandela 66.
Con el fin de sujetar el tendón 26 a un hueso, la
grapa 60 se clava en un hueso, con el tendón 26 sujeto tanto al
sitio de la arandela 60 como al elemento 62 de cabeza
transversal.
Cualquier laxitud de la grapa 60 bajo carga sirve
para presionar adicionalmente el elemento 62 de cabeza transversal
más firmemente sobre el tendón 26, mejorando así cualquier laxitud
del tendón 26 del hueso.
Se apreciará por los expertos en la técnica que
pueden. realizarse numerosas variaciones y/o modificaciones a la
invención, tal como se muestra en las realizaciones, sin apartarse
del espíritu o alcance de la invención, tal como se describe
ampliamente. Por tanto, las presentes realizaciones han de
considerarse en todos los aspectos como ilustrativas y no
restrictivas.
Claims (6)
1. Dispositivo de sujeción quirúrgico, siendo el
dispositivo de sujeción una grapa para sujetar un tendón (26) de
injerto a un hueso, comprendiendo la grapa (60) quirúrgica dos
patillas (61) sustancialmente paralelas interconectadas mediante un
elemento (62) de cabeza transversal, siendo cada patilla
sustancialmente de la misma longitud y caracterizadas por
tener una parte (63) de extremo libre conectada sustancialmente en
ángulo (65) recto a la parte (64) de cabeza, de tal manera que el
elemento de cabeza se desplace lateralmente desde las partes (63)
de extremo libre, y una arandela (66) dispuesta alrededor de las
partes (63) de extremo libre próxima a la conexión (65) en ángulo
recto con las partes (64) de cabeza.
2. Dispositivo de sujeción quirúrgico según la
reivindicación 1, en el que la grapa (60) quirúrgica tiene dos
patillas (61) paralelas, cada patilla de la misma longitud y
teniendo una parte (63) de extremo libre conectada en ángulo (65)
recto a una parte (64) de
cabeza.

3. Dispositivo de sujeción quirúrgico según las
reivindicaciones 1 ó 2, en el que el elemento (62) de cabeza
transversal que interconecta dos partes de cabeza se inclina hacia
abajo desde el plano de las partes de cabeza.
4. Dispositivo de sujeción quirúrgico según una
cualquiera de las reivindicaciones 1-3, en el que la
grapa quirúrgica está compuesta por un elemento lineal doblado
hasta la forma (60) requerida.
5. Dispositivo de sujeción quirúrgico según la
reivindicación 4, en el que el elemento lineal es un alambre con
las partes de extremo libre que tienen una punta (67) de flecha
aguda para facilitar la inserción de la grapa en el hueso y las
partes del alambre que son más probables que entren en contacto con
el tendón son redondeadas para evitar daños al tendón.
6. Dispositivo de sujeción quirúrgico según una.
cualquiera de las reivindicaciones 1-5, en el que la
arandela. (66) es de forma oval con bordes (68) redondeados para
evitar daños al tendón de injerto y tiene dos orificios desplazados
lateralmente para cada una de las patillas.
