Abstract
Um ein medizinisches System umfassend mindestens zwei sich unterscheidende Implantate und mindestens zwei sich unterscheidende Instrumente, wobei jedes der mindestens zwei sich unterscheidenden Instrumente ausgebildet und dazu bestimmt ist, mit einem ihm zugeordneten kompatiblen Implantat der mindestens zwei sich unterscheidenden Implantate zusammenzuwirken so zu verbessern, dass es sicher eingesetzt werden kann, wird vorgeschlagen, dass das medizinische System ein Mikroskop, welches ein Mikroskopsichtfeld definiert, und eine Kompatibilitätsprüfungsvorrichtung umfasst zum automatischen Prüfen einer Kompatibilität eines der mindestens zwei sich unterscheidenden Instrumente und eines der mindestens zwei sich unterscheidenden Implantate, die gemeinsam im Mikroskopsichtfeld positioniert sind. Ferner wird ein Verfahren zum Prüfen einer Kompatibilität von Implantaten und Instrumenten eines medizinischen Systems vorgeschlagen.
Description
Medizinisches System und Verfahren zum Prüfen einer Kompatibilität von Implantaten und Instrumenten eines medizinischen Systems
Die vorliegende Erfindung betrifft ein medizinisches System umfassend min destens zwei sich unterscheidende Implantate und mindestens zwei sich un terscheidende Instrumente, wobei jedes der mindestens zwei sich unterschei denden Instrumente ausgebildet und dazu bestimmt ist, mit einem ihm zuge ordneten kompatiblen Implantat der mindestens zwei sich unterscheidenden Implantate zusammenzuwirken.
Ferner betrifft die vorliegende Erfindung ein Verfahren zum automatischen Prüfen einer Kompatibilität von Implantaten und Instrumenten eines medizini schen Systems, welches mindestens zwei sich unterscheidende Implantate und mindestens zwei sich unterscheidende Instrumente umfasst, wobei jedes der mindestens zwei sich unterscheidenden Instrumente ausgebildet und dazu be stimmt ist, mit einem ihm zugeordneten kompatiblen Implantat der mindes tens zwei sich unterscheidenden Implantate zusammenzuwirken.
Medizinische Systeme der eingangs beschriebenen Art kommen insbesondere in der Neurochirurgie zum Einsatz. Sie umfassen eine Mehrzahl unterschiedli cher medizinischer Clips in Form von Aneurysmenclips. Für unterschiedliche Anwendungsfälle sind die Clips insbesondere unterschiedlich geformt, weisen unterschiedliche Schließkräfte auf und aus unterschiedlichen Materialien gefer tigt, beispielsweise aus Titan oder Phynox. Da die Clips empfindlich sind und für eine zuverlässige Implantation nicht Undefiniert mit einem beliebigen In strument, beispielsweise einer Pinzette oder Zange, appliziert werden können, werden insbesondere unterschiedliche Instrumente eingesetzt, die in Abhän gigkeit der wie beschrieben teilweise sehr unterschiedlichen Clips mit diesen zusammenwirkend angewendet werden. Dabei ist es wichtig, dass Instrument und Clip zueinander kompatibel sind, um eine Beschädigung des jeweiligen Clips durch das Instrument beim Zusammenwirken zu vermeiden.
Es ist daher eine Aufgabe der vorliegenden Erfindung, ein medizinisches Sys tem der eingangs beschriebenen Art so zu verbessern, dass es sicher einge setzt werden kann.
Diese Aufgabe wird bei einem medizinischen System wird bei einem medizini schen System der eingangs beschriebenen Art erfindungsgemäß dadurch ge löst, dass das medizinische System ein Mikroskop, welches ein Mikroskopsicht feld definiert, und eine Kompatibilitätsprüfungsvorrichtung umfasst zum auto matischen Prüfen einer Kompatibilität eines der mindestens zwei sich unter scheidenden Instrumente und eines der mindestens zwei sich unterscheiden den Implantate, die gemeinsam im Mikroskopsichtfeld positioniert sind.
Die vorgeschlagene Weiterbildung eines bekannten medizinischen Systems er möglicht es insbesondere einem Anwender auf einfache Weise, zuverlässig zu prüfen, ob das von ihm zur Implantation eingesetzte Instrument und das für die Implantation bestimmte Implantat zueinander kompatibel sind. Die Prü fung führt das System automatisch durch. Hierfür müssen Instrument und Im plantat nur gemeinsam im Mikroskopsichtfeld positioniert werden. Die Kompa tibilitätsprüfungsvorrichtung kann dann Implantat und Instrument erkennen, beispielsweise an ihrer jeweiligen Form und/oder Größe oder einer das Implan tat beziehungsweise das Instrument charakterisierenden Kennzeichnung, bei spielsweise einer Codierung. Hier ist insbesondere zu beachten, dass bei medi zinischen Systemen, bei denen Implantate unter Zuhilfenahme eines Mikro skops implantiert werden, ein Operateur ein Instrument häufig bereits ange reicht bekommt, welches mit dem zu implantierenden Implantat in Eingriff steht. Da er beim Eingriff lediglich das Mikroskopbild betrachtet, also sein Blick auf dem Operationssitus verweilt, hat er keine Möglichkeit, um selbst durch ei nen direkten Blick auf die ihm angereichte Einheit zu prüfen, ob das Instru ment und das in diesem gehaltene Implantat kompatibel sind oder nicht. Das vorgeschlagene System ermöglicht es ihm insbesondere, wenn er Implantat und Instrument ins Mikroskopsichtfeld bringt, dies zu prüfen, und zwar in au tomatischer Weise durch das System. So kann vor einer endgültigen Implanta tion des jeweiligen Implantats insbesondere sichergestellt werden, dass von
einer dem Operateur assistierenden Person angereichte Kombination von In strument und Implantat auch für ein Zusammenwirken bestimmt sind oder nicht.
Günstig ist es, wenn das Mikroskop in Form eines optischen Mikroskops oder eines digitalen Mikroskops ausgebildet ist. Letztlich spielt es keine Rolle, wel cher Art das Mikroskop ist, um die Kompatibilitätsprüfung durchzuführen. Die se erfolgt automatisch mit der Kompatibilitätsprüfungsvorrichtung des Sys tems. So lassen sich beispielsweise medizinische Systeme, die bereits im Ein satz sind, auf einfache Weise nachrüsten.
Vorteilhaft ist es, wenn das Mikroskop einen optischen Mikroskopbildsensor umfasst. Insbesondere kann dieser in Form eines CCD-Sensors oder eines CMOS-Sensors ausgebildet sein. Derartige Mikroskopbildsensoren können ins besondere bei digitalen Mikroskopen zum Einsatz kommen. Sie können auch bei optischen Mikroskopen genutzt werden, bei denen eine digitale Kamera an geschlossen werden kann. So lassen sich beispielsweise rein optische Mikro skope nachrüsten. Derartige optische Mikroskopbildsensoren gestatten es, ein digitales Bild des Mikroskopsichtfelds zu erzeugen, zu speichern und weiterzu verarbeiten. Dieses kann dann gegebenenfalls genutzt werden, um Daten aus zulesen, beispielsweise Beschriftungen und Codierungen an Implantat oder In strument, die im Mikroskopsichtfeld sichtbar sind.
Gemäß einer weiteren bevorzugten Ausführungsform kann vorgesehen sein, dass die Kompatibilitätsprüfungsvorrichtung eine optische Detektionseinrich tung umfasst zum Detektieren einer optischen Codierung, dass die Detektions einrichtung ein Detektionssichtfeld definiert und dass das Mikroskopsichtfeld und das Detektionssichtfeld mindestens teilweise überlappen. Die optische De tektionseinrichtung ermöglicht es insbesondere, ein Bild des Detektionssicht felds zu erzeugen. Überlappen sich das Detektionssichtfeld und das Mikroskop sichtfeld, kann so zumindest auch ein Teil des Mikroskopsichtfelds mit der De tektionseinrichtung erfasst werden. Wird beispielsweise ein optisches Mikro-
skop eingesetzt, welches keinen Mikroskopbildsensor, also keine digitale Bil derfassungseinrichtung umfasst, kann mit der optischen Detektionseinrichtung ein Bild des Detektionssichtfelds und aufgrund der vorgeschlagenen Anord nung auch mindestens eines Teils des Mikroskopsichtfelds aufgenommen wer den. So ist es möglich, herkömmliche medizinische Systeme aufzurüsten, bei spielsweise optische Mikroskope mit einer Kompatibilitätsprüfungsvorrichtung auszustatten.
Um die Kompatibilität von Instrument und Implantat zuverlässig prüfen zu können, ist es vorteilhaft, wenn ein Überlapp des Mikroskopsichtfelds und des Detektionssichtfelds in einem Bereich von etwa 50% bis 100% liegt. Insbeson dere kann der Überlapp in einem Bereich von etwa 80% bis 100% liegen. Vor zugsweise liegt der Überlapp in einem Bereich von etwa 95% bis 100%. Je größer der Überlapp von Mikroskopsichtfeld und Detektionssichtfeld ist, um so zuverlässiger können alle Gegenstände im Mikroskopsichtfeld erfasst und auf eine Kompatibilität geprüft werden. Idealerweise ist der Überlapp so groß, dass zuverlässig alle Teile, die sich im Mikroskopsichtfeld befinden, auch im Detektionssichtfeld erkennbar sind.
Um ein Bild des Detektionssichtfelds schnell und einfach auswerten zu können, ist es vorteilhaft, wenn die optische Detektionseinrichtung mindestens einen optischen Bildsensor umfasst. Insbesondere kann es sich beim optischen Bildsensor um einen CCD-Sensor oder einen CMOS-Sensor handeln. Der opti sche Bildsensor kann beispielsweise von einer Kamera umfasst sein, die an ein optisches Mikroskop angeschlossen wird. Wie erwähnt können so optische Mik roskope auf einfache Weise nachgerüstet werden. Zudem lassen sich Digital aufnahmen auf einfache Weise auswerten, beispielsweise lassen sich Gegen stände oder Codierungen darin auf einfache Weise identifizieren.
Ein besonders kompaktes medizinisches System kann bereitgestellt werden, wenn der Mikroskopbildsensor den optischen Bildsensor definiert oder bildet. Auf diese Weise kann insbesondere erreicht werden, dass das Detektionssicht feld und das Mikroskopsichtfeld vollständig, also zu 100 %, überlappen. Mit
anderen Worten nutzt die Kompatibilitätsprüfungsvorrichtung in diesem Fall den Mikroskopbildsensor zum Erzeugen des Detektionssichtfelds, welches mit dem Mikroskopsichtfeld übereinstimmt.
Günstig ist es, wenn die mindestens zwei sich unterscheidenden Implantate und die mindestens zwei sich unterscheidenden Instrumente jeweils mindes tens eine optische Codierung umfassen, dass sich die optische Codierung der mindestens zwei sich unterscheidenden Implantate unterscheidet und dass sich die optische Codierung der mindestens zwei sich unterscheidenden Instru mente unterscheidet. Implantate und Instrumente in der beschriebenen Weise zu codieren ermöglicht es einem Anwender, diese gezielt zu unterscheiden. So kann insbesondere durch die optische Codierung Größe oder beispielsweise Maulteilgeometrie eines Instruments codiert werden. Bei Implantaten, bei spielsweise medizinischen Clips, können so deren Größe und beispielsweise auch Schließkräfte sowie die Materialien, aus denen Sie ausgebildet sind, co diert werden. Durch optische Codierungen lässt sich zudem auch eine einfache und gegebenenfalls redundante Kompatibilitätsprüfung auf Sicht durchführen, beispielsweise wenn die optischen Codierungen einfache Farbcodierungen sind. So kann eine beispielsweise einem Operateur assistierende Person direkt er kennen, ob Instrument und Implantat kompatibel sind, beispielsweise durch Vergleich einer Farbcodierung von Instrument und Implantat.
Günstig ist es, wenn die mindestens eine optische Codierung von Implantaten und Instrumenten, die zueinander kompatibel und für ein gemeinsames Zu sammenwirken bestimmt sind, identisch ist. So kann, unabhängig von der au tomatischen Kompatibilitätsprüfungsvorrichtung durch das System, auch ein Anwender rein optisch, durch Betrachten von Instrument und Implantat, eine einfache Sichtprüfung durchführen und so direkt erkennen, ob die Kombination von Instrument und Implantat überhaupt kompatibel, beispielsweise von ei nem Hersteller zugelassen ist. Allerdings ist es nicht zwingend erforderlich, dass zueinander kompatible Instrumente und Implantate eine identische Co-
dierung aufweisen. Eine Kompatibilität kann sich insbesondere auch bei unter schiedlichen Codierungen ergeben und durch Vergleich dieser Codierungen er folgen, deren Kompatibilität in entsprechenden Tabellen hinterlegt ist.
Auf einfache Weise können Instrumente und Implantate automatisch erkannt werden, wenn die mindestens eine optische Codierung in Form eines Barcodes, eines QR-Codes, eines Formcodes oder eines Farbcodes ausgebildet ist. So lassen sich beispielsweise aus digitalen Aufnahmen des Detektionssichtfelds Barcodes, eines DataMatrix-Codes, eines QR-Codes, eines Formcodes oder ei nes Farbcodes auf einfache Weise erkennen. Formcodes können insbesondere vorgegeben werden durch eine Form der Implantate beziehungsweise Instru mente selbst oder zumindest eines Teils derselben. In diesem Fall ist dann keine zusätzliche Codierung zwingend erforderlich. Die Codierung wird vorge geben durch Implantat und Instrument selbst. Jedes Implantat beziehungs weise jedes Instrument kann auch zwei oder mehr optische Codierungen um fassen. Dabei kann es sich um die Kombination insbesondere der genannten Codierungsformen handeln. Die mindestens eine optische Codierung, insbe sondere ein DataMatrix-Code beziehungsweise die anderen oben genannten Alternativen von optischer Codierungen, kann auch als Marker für Navigations anwendungen beziehungsweise für Augmented Reality Anwendungen beim Implantieren des Implantats, insbesondere beim Applizieren eines medizini schen Clips, verwendet werden. Insbesondere können nicht sichtbare Struktu ren dargestellt werden, zum Beispiel das Implantat und das zugehörige Instru ment. Insbesondere können der medizinische Clip, ein zugehöriges Clipanlege- instrument und eine Gefäßstruktur, an die der Clip appliziert werden soll, dar gestellt werden. Ferner ist es insbesondere möglich, zusammenwirkende Ele mente, insbesondere von aneinander anliegenden Flächenbereichen, beispiels weise von einem medizinischen Clip und dem Clipanlegeinstrument hinsichtlich eines Sitzes des Clips in einer Clipaufnahme, auch als Maulteil des Clipanlege- instruments bezeichnet, über eine Formerkennung zu beurteilen.
Vorzugsweise umfasst die Kompatibilitätsprüfungsvorrichtung eine Bildverar beitungseinrichtung zum automatischen Bestimmen von Instrumenten
und/oder Implantaten im Mikroskopsichtfeld. Die Bildverarbeitungseinrichtung kann insbesondere auch ausgebildet sein zum automatischen Bestimmen von Instrumenten und/oder Implantaten im Detektionssichtfeld. Die Bildverarbei tungseinrichtung ermöglicht es auf einfache Weise, insbesondere optische Co dierungen in einem aufgenommenen Bild des Mikroskopsichtfelds beziehungs weise des Detektionssichtfelds auszuwerten. Dies bietet insbesondere Möglich keiten für eine Prozessoptimierung, zum Beispiel im Hinblick auf eine Doku mentation eine chirurgischen Eingriffs sowie eine Nachverfolgung von einge setzten Implantaten und Instrumenten, auch als Tracking bezeichnet.
Günstig ist es, wenn die Bildverarbeitungseinrichtung ausgebildet ist zum Aus lesen der mindestens einen Codierung von Instrumenten und/oder Implanta ten aus einem Bild des Detektionssichtfelds und/oder des Mikroskopsichtfelds. Eine solche Bildverarbeitungseinrichtung ermöglicht es insbesondere, Instru mente sowie Implantate aufgrund ihrer Codierung zu erkennen und gegebe nenfalls auch einem Nutzer anzuzeigen, um welches Instrument beziehungs weise Implantat es sich handelt. Codierungen können auch einfache Weise durch Vergleich mit hinterlegten Codierungen aus einem Bild extrahiert wer den.
Vorzugsweise umfasst die Kompatibilitätsprüfungsvorrichtung eine Anzeigeein richtung zum Anzeigen eines Ergebnisses der Kompatibilitätsprüfung. Hierbei kann es sich beispielsweise um einen Bildschirm handeln. Die Anzeigeeinrich tung kann insbesondere auch in Form einer akustischen Anzeigeeinrichtung ausgebildet sein, um einem Anwender akustisch mitzuteilen, ob Implantat und Instrument kompatibel sind oder nicht. Die Anzeigeeinrichtung kann insbeson dere in Form eines Displays des Mikroskops oder in Form eines externen, vom Mikroskop unabhängigen Displays ausgebildet sein. Zum Beispiel kann die An zeigeeinrichtung in einem Hybrid-Operationssaal von einer Datenbrille umfasst sein oder eine Sprachausgabe umfassen.
Vorteilhaft ist es, wenn das Mikroskop eine Mikroskopanzeigeeinrichtung um fasst zum Anzeigen eines Bildes des Mikroskopsichtfelds und wenn die Mikro skopanzeigeeinrichtung die Anzeigeeinrichtung umfasst. So kann das medizini sche System besonders kompakt ausgebildet werden. Beispielsweise kann die Anzeigeeinrichtung in Form eines Fensters oder Teilbereichs der Mikroskopan zeigeeinrichtung ausgebildet werden. Mithin lassen sich also bei Bedarf Ergeb nisse von Kompatibilitätsprüfungen in ein auf der Mikroskopanzeigeeinrichtung angezeigtes Mikroskopbild einblenden. So kann ein Operateur, welcher bei spielsweise auf der Mikroskopanzeigeeinrichtung ein Bild des Mikroskopsicht felds sieht, also beispielsweise den Operationssitus, direkt eingeblendet be kommen, ob das dargestellte Instrument und und das mit diesem gehaltene Implantat kompatibel sind.
Gemäß einer weiteren bevorzugten Ausführungsform kann vorgesehen sein, dass die mindestens zwei Implantate in Form medizinischer Clips ausgebildet sind und dass die mindestens zwei Instrumente in Form von Clipanlegeinstru- menten ausgebildet sind. Insbesondere können die medizinischen Clips in Form von Aneurysmenclips ausgebildet sein. Die Clipeinlegeinstrumente, auch als Applikatoren bezeichnet, wirken mit den medizinischen Clips zusammen, um diese beispielsweise zur Behandlung von Aussackungen an Hohlorganen im Körper eines Menschen oder eines Tieres zu applizieren.
Günstig ist es, wenn die mindestens zwei Instrumente ein mit einem der min destens zwei Implantate in Eingriff bringbares Instrumentenende umfassen und wenn die mindestens eine optische Codierung am oder im Bereich des In strumentenendes angeordnet oder ausgebildet ist. Steht das Instrumenten ende des Instruments, welches im Mikroskopsichtfeld positioniert ist, mit ei nem Implantat in Eingriff, dann können so die Codierungen sowohl am Im plantat als auch am Instrument in großer räumlicher Nähe zueinander positio niert und dadurch auf einfache Weise erfasst werden.
Die eingangs gestellte Aufgabe wird ferner bei einem Verfahren der eingangs beschriebenen Art erfindungsgemäß dadurch gelöst, dass die die Kompatibili tät von einem Implantat und einem Instrument des medizinischen Systems automatisch geprüft wird, wenn das Implantat und das Instrument gemeinsam im Mikroskopsichtfeld eines Mikroskops positioniert sind.
Wie bereits oben eingehend erläutert, kann so eine zusätzliche Sicherheit beim Einsatz von Instrumenten und Implantaten erreicht werden, denn ob Instru ment und Implantat zueinander kompatibel sind, lässt sich für einen Anwender auf den ersten Blick nicht in allen Fällen unbedingt erkennen. Eine solches Ver fahren hilft daher, die Implantation bestimmter Implantate mit nicht dafür be stimmten Instrumenten zu vermeiden.
Günstig ist es, wenn die Implantate und Instrumente des Systems eine opti sche Codierung aufweisen und wenn die Kompatibilität geprüft wird durch Ver gleich der optischen Codierung des Implantats und der optischen Codierung des Instruments, die gemeinsam im Mikroskopsichtfeld positioniert sind. Eine solche Vorgehensweise erfordert nicht zwingend die Bereitstellung von Codie rungstabellen. Insbesondere können so auch neue Instrumente und Implan tate auf Kompatibilität geprüft werden, die nicht vorab in einer Codierungsta belle hinterlegt wurden. Eine Kompatibilität kann insbesondere dann vorliegen, wenn die optischen Codierungen von Instrument und Implantat identisch sind.
Für eine sichere Anwendung des Verfahrens ist es vorteilhaft, wenn das Ergeb nis des Vergleichs ausgegeben wird. Insbesondere kann dies optisch und/oder akustisch erfolgen. Beispielsweise kann eine Inkompatibilität einem Operateur durch einen Warnton oder eine entsprechende Ansage mitgeteilt werden.
Des Weiteren wird die Verwendung eines der oben beschriebenen Systeme zur Durchführung eines der oben beschriebenen Verfahren vorgeschlagen.
Die nachfolgende Beschreibung bevorzugter Ausführungsformen der Erfindung dient im Zusammenhang mit den Zeichnungen der näheren Erläuterung. Es zeigen:

Ein Ausführungsbeispiel eines medizinischen Systems 10 ist schematisch in Fi gur 1 dargestellt.
Es umfasst ein Mikroskop 12, welches ein Mikroskopsichtfeld 14 definiert. Das System 10 umfasst ferner zwei oder mehr sich unterscheidende Implantate 16 und zwei oder mehr sich unterscheidende Instrumente 18. Die Instrumente 18 und Implantate 16 sind ausgebildet und dazu bestimmt, im Falle der Kompati bilität bestimmungsgemäß miteinander zusammenzuwirken.
In Figur 1 ist schematisch ein Beispiel eines Instruments 18 in Form eines Cli papplikators 20 dargestellt, welcher an einem distalen Instrumentenende 22
ein aufgenommenes Implantat 16 in Form eines medizinischen Clips 24 hält.
Mit dem Clipapplikator 20 kann der medizinische Clip 24 geöffnet und bei spielsweise an einer Aussackung 26, auch als Aneurysma bezeichnet, eines Hohlorgans, insbesondere in Form eines Blutgefäßes 28, appliziert werden.
Wie in Figur 1 schematisch dargestellt, ist das Mikroskop 12 in Form eines di gitalen Mikroskops ausgebildet. Es umfasst eine Mikroskopanzeigeeinrichtung 30 in Form eines Bildschirms 32, auf welchem ein Bild des Mikroskopsichtfelds 14 dargestellt ist. Ein Operateur 34, welcher das Instrument 18 handhabt, überwacht die Applikation des Implantats 16 auf dem Bildschirm 32.
Figur 2 zeigt schematisch ein Ausführungsbeispiel eines medizinischen Sys tems 10, welches eine Kompatibilitätsprüfungsvorrichtung 36 umfasst. Sie dient dem Zweck, automatisch eine Kompatibilität von Instrument 18 und Im plantat 16 zu prüfen, die gemeinsam im Mikroskopsichtfeld 14 positioniert sind.
Die Kompatibilitätsprüfungsvorrichtung 36 umfasst eine optische Detektions einrichtung 38. Sie definiert ein Detektionssichtfeld 40. Die bei diesem Ausfüh rungsbeispiel unabhängig vom Mikroskop 12 bereitgestellte Detektionseinrich tung 38 ist derart angeordnet, dass das Mikroskopsichtfeld 14 und das Detek tionssichtfeld 40 sich mindestens zur Hälfte überlappen. Mithin liegt also ein Überlapp von Mikroskopsichtfeld 14 und Detektionssichtfeld 40 in einem Be reich von mindestens etwa 50%.
Das in Figur 2 schematisch dargestellte Mikroskop 12 umfasst einen optischen Mikroskopbildsensor 42. Dieser ist in Form eines CCD-Sensors oder eines CMOS-Sensors ausgebildet.
Auch die optische Detektionseinrichtung 38 umfasst einen optischen Bildsensor 44. Dieser ist als CCD-Sensor oder CMOS-Sensor ausgebildet.
Die sich unterscheidenden Implantate 16 und die sich unterscheidenden In strumente 18 des medizinischen Systems 10 umfassen jeweils eine optische Codierung 46. Sie ist in Form eines Farbcodes 48 ausgebildet. Bei dem Aus führungsbeispiel der Figur 2 ist das Implantat 16 eingefärbt. Am Instrument 18 ist ein im Bereich des Instrumentenendes 22 angeordnetes Element des In struments 18 mit einem Farbcode 48 gekennzeichnet. Bei dem Ausführungs beispiel der Figur 2 handelt es sich bei dem Element um eine Schlussschraube 50 des als Clipapplikator 20 ausgebildeten Instruments 18.
Die Detektionseinrichtung 38 ist ausgebildet zum Detektieren der optischen Codierung 46. Dies wird dadurch ermöglicht, dass die Detektionseinrichtung 38 ein Bild des Detektionssichtfelds 40 mit dem Bildsensor 44 aufnimmt. Die Kompatibilitätsprüfungsvorrichtung 36 umfasst eine Bildverarbeitungseinrich tung 52. Mit dieser können Instrumente 18 und Implantate 16 im Detektions sichtfeld 40 sowie in demjenigen Bereich des Mikroskopsichtfelds 14 erfasst werden, welcher mit dem Detektionssichtfeld 40 überlappt.
Bei dem Ausführungsbeispiel der Figur 2 ist die Bildverarbeitungseinrichtung 52 ausgebildet zum Auslesen der optischen Codierung 46 des Instruments 18 sowie des Implantats 16 aus dem Bild des Detektionssichtfelds 40. Mithin ist die Bildverarbeitungseinrichtung 52 ausgebildet, um den Farbcode 48 am In strument 18 und Implantat 16 zu erkennen.
Zum Anzeigen eines Ergebnisses der Kompatibilitätsprüfung umfasst die Kom patibilitätsprüfungsvorrichtung 36 eine Anzeigeeinrichtung 54. Bei dem in Fi gur 2 dargestellten Ausführungsbeispiel des medizinischen Systems 10 um fasst die Mikroskopanzeigeeinrichtung 30 die Anzeigeeinrichtung 54. Diese ist in Form eines eingeblendeten Fensters in dem auf dem Bildschirm 32 ange zeigten Bild des Mikroskopsichtfelds 14 realisiert. Auf der Anzeigeeinrichtung 54 wird in nicht dargestellter Weise in Textform oder durch beispielsweise eine Ampeldarstellung angezeigt, ob die Farbcodes 48 von Instrument 18 und Im plantat 16 übereinstimmen. Bei dem in Figur 2 dargestellten Ausführungsbei spiel ist dies der Fall.
Das mit dem Mikroskopbildsensor 42 erzeugte Bild des Mikroskopsichtfelds 14 wird mit einer Recheneinrichtung 56 aufbereitet und auf der Mikroskopanzei geeinrichtung 30 dargestellt.
Ein weiteres Ausführungsbeispiel eines medizinischen Systems 10 ist schema tisch in Figur 3 dargestellt. Bei diesem Ausführungsbeispiel ist die optische De tektionseinrichtung 38 vom Mikroskop 12 umfasst. Der Mikroskopbildsensor 42 bildet den optischen Bildsensor 44.
Die Kompatibilitätsprüfungsvorrichtung 36 umfasst insbesondere das Mikro skop 12 und die von der Mikroskopanzeigeeinrichtung 30 umfasste Anzeige einrichtung 54. Das mit dem Mikroskopbildsensor 42 aufgenommene Bild des Mikroskopsichtfelds 14 wird mit einer von der Recheneinrichtung 56 umfassten Bildverarbeitungseinrichtung 52 verarbeitet und auf dem Bildschirm 32 darge stellt. Das Ergebnis der Kompatibilitätsprüfung wird auf dem Bildschirms 32 angezeigt, insbesondere wie oben im Zusammenhang mit dem Ausführungs beispiel der Figur 2 beschrieben in Form eines beschreibenden Textes und/oder in Form einer Ampeldarstellung.
Bei dem in Figur 3 dargestellten schematischen Ausführungsbeispiel ist am In strument 18 und Implantat 16 eine optische Codierung 46 vorgesehen, und zwar in Form eines DataMatrix-Codes 58.
Die Bildverarbeitungseinrichtung 52 ist ausgebildet, um aus dem mit dem Mik roskopbildsensor 42 aufgenommenen Bild des Mikroskopsichtfelds 14 die opti schen Codierungen 46 von Instrument 18 und Implantat 16 zu extrahieren und miteinander zu vergleichen.
Sowohl das Ausführungsbeispiel der Figur 2 als auch das Ausführungsbeispiel der Figur 3 des medizinischen Systems sind optional ausgebildet, um einen Formcode aus dem Bild des Mikroskopsichtfelds 14 beziehungsweise des De tektionssichtfelds 40 zu extrahieren. Der Formcode 60 wird definiert durch das Implantat 16 an sich sowie das Instrument 18 an sich. Insbesondere können
beide Ausführungsbeispiele auch unterschiedliche Codierungen 46 an einem Implantat 16 sowie einem Instrument 18 erkennen. Dies lässt noch eine siche rere Prüfung von Implantat 16 und Instrument 18 zu, da quasi bei der Prüfung von zwei optischen Codierungen 46 sowohl am Instrument 18 als auch am Im plantat 16 eine redundante Kompatibilitätsinformation erhalten werden kann.
Die optische Codierung 46 ist beim Ausführungsbeispiel der Figur 3 distalseitig der Schlussschraube 50 am Instrument 18 angeordnet, mithin also im Bereich des Instrumentenendes 22.
Figur 4 zeigt schematisch das Überlappen des Mikroskopsichtfelds 14 und des Detektionssichtfelds 40. Um die Kompatibilitätsprüfung durchführen zu kön nen, müssen das Implantat 16 und das Instrument 18 so im Mikroskopsicht feld 14 und gleichzeitig im Detektionssichtfeld 40 positioniert sein, dass beide optischen Codierungen 46, also sowohl am Implantat 16 als auch am Instru ment 18, gleichzeitig erfasst werden können. Dies ermöglicht es, mit der Kom patibilitätsprüfungsvorrichtung 36 die beiden erfassten optischen Codierungen 46 zu vergleichen und automatisch eine Kompatibilität von Implantat 16 und Instrument 18 zu bestätigen, falls die beiden optischen Codierungen 46 iden tisch sind, oder eine Warnung auszugeben, wenn sich die beiden optischen Co dierungen 46 unterscheiden.
In den Figuren 5 und 6 sind diese beiden Alternativen, also Kompatibilität und Inkompatibilität von Implantat 16 und Instrument 18 schematisch dargestellt.
In Figur 5 sind sowohl am Instrument 18 als auch am Implantat 16 jeweils 2 optische Codierungen 46 vorgesehen, nämlich jeweils einmal als Farbcode 48 und einmal als DataMatrix-Code 58. Zusätzlich kann optional auch die Form von Instrument 18 und Implantat 16 erfasst und auf Kompatibilität geprüft werden.
Bei dem Beispiel der Figur 5 stimmen beide optischen Codierungen 46 überein. Beim Beispiel der Figur 6 unterscheiden sich sowohl die Farbcodes 48 als auch
die DataMatrix-Codes 58. Mithin kann also bei der Situation gemäß Figur 5 die Kompatibilität von Instrument 18 und Implantat 16 bestätigt werden, beim Beispiel der Figur 6 ist sie zu verneinen. Es kann dann beispielsweise eine Warnung ausgegeben werden
Bei alternativen, nicht dargestellten Ausführungsbeispielen können weitere op tische Codierungen in Form von Barcodes oder QR-Codes alternativ oder zu sätzlich vorgesehen sein.
In der optischen Codierung 46 kann insbesondere eine Größe des Implantats, beispielsweise drei unterschiedliche Größenfamilien, nämlich "Standard",
"Mini" und "Lang" differenziert werden. Zu diesen drei Größenfamilien korres pondieren drei zugehörige Instrumente 18, die kompatibel sind. Beispielsweise können die drei Größen über einen geeigneten Farbcode, beispielsweise blau für "Standard", rot für "Mini" und grün für "Lang" codiert werden.
Eine farbliche Codierung kann insbesondere genutzt werden, um dauerhaft im Körper verbleibende Implantate, beispielsweise permanente Clips eines Aneu rysmenclipsystems, oder temporäre Implantate, beispielsweise temporäre Clips eines solchen Aneurysmenclipsystems, voneinander zu unterscheiden. Insbesondere kann zusätzlich zur farblichen Codierung der Größe eine farbli che Codierung der Information permanent oder temporär ergänzt werden.
Mit dem DataMatrix-Code können unterschiedliche Informationen von Instru ment 18 und Implantat 16 codiert und direkt am Instrument 18 beziehungs weise am Implantat 16 hinterlegt werden, beispielsweise Herstellungsdatum, Schließkraft eines Clips oder Material, aus dem der Clip hergestellt wurde. Die im DataMatrix-Code codierte Information kann insbesondere auch zur Nach verfolgung und Dokumentation eines operativen Eingriffs genutzt werden. Mit hin können mit den beschriebenen Ausführungsbeispielen medizinischer Sys teme 10 also nicht nur Kompatibilitäten von Instrumenten 18 und Implantaten 16 überprüft werden, sondern die ermittelten Informationen auch zur Prozess-
Optimierung genutzt werden. So ist es beispielsweise möglich, automatisch ei nen Nachbestellvorgang auszulösen, wenn ein bestimmtes Implantat 16 im plantiert wurde. Zudem lassen sich implantierte Implantate auch direkt einem Patientenausweis zuweisen. Ferner kann ermittelt werden, wie oft welcher Typ von Implantat 16 permanent implantiert wurde und welche temporären Im plantate 16 eingesetzt wurden.
Implantate 16 und Instrumente lassen sich mit den beschriebenen Ausfüh rungsbeispielen medizinischer Systeme 10 insbesondere auch intraoperativ so wie im Aufbereitungsprozess nachverfolgen. Dies stellt einen Service für den Anwender dar, bietet eine Datensammlung für den Hersteller und ermöglicht eine Erfassung von Registerdaten und die Verwendung in klinischen Studien.
Bei dem Ausführungsbeispiel der Figur 2 ist die Detektionseinrichtung 38 sepa rat vom Mikroskop 12 ausgebildet. Sie kann in Form einer beliebigen Digitalka mera realisiert werden. Beispielsweise können in einem Operationssaal hierfür eine Navigationskamera oder auch eine Datenbrille mit integrierter Kamera genutzt werden.
Alternativ zur Integration der Anzeigeeinrichtung 54 in die Mikroskopanzeige einrichtung 30 kann ein zusätzlicher Bildschirm vorgesehen werden. Das Er gebnis der Kompatibilitätsprüfung kann auch in einer Datenbrille, die ein Ope rateur trägt, angezeigt werden, oder akustisch über eine Sprachausgabe erfol gen.
Die beschriebenen Ausführungsbeispiele medizinischer Systeme 10 ermögli chen auf einfache Weise eine automatische Prüfung, ob ein Instrument 18 und ein Implantat 16, die gemeinsam genutzt werden sollen, miteinander kompati bel sind oder nicht. Diese Information ist von großer Tragweite, da so insbe sondere eine Beschädigung von Implantaten 16 durch den Einsatz eines in kompatiblen Instruments 18 vermieden werden kann.

Claims
1. Medizinisches System (10) umfassend mindestens zwei sich unterschei dende Implantate (16) und mindestens zwei sich unterscheidende In strumente (18), wobei jedes der mindestens zwei sich unterscheidenden Instrumente (18) ausgebildet und dazu bestimmt ist, mit einem ihm zu geordneten kompatiblen Implantat (16) der mindestens zwei sich unter scheidenden Implantate (16) zusammenzuwirken, dadurch gekennzeich net, dass das medizinische System (10) ein Mikroskop (12), welches ein Mikroskopsichtfeld (14) definiert, und eine Kompatibilitätsprüfungsvor richtung (36) umfasst zum automatischen Prüfen einer Kompatibilität ei nes der mindestens zwei sich unterscheidenden Instrumente (18) und ei nes der mindestens zwei sich unterscheidenden Implantate (16), die ge meinsam im Mikroskopsichtfeld (14) positioniert sind.
2. Medizinisches System nach Anspruch 1, dadurch gekennzeichnet, dass das Mikroskop (12) in Form eines optischen Mikroskops (12) oder eines digitalen Mikroskops (12) ausgebildet ist.
3. Medizinisches System nach Anspruch 2, dadurch gekennzeichnet, dass das Mikroskop (12) einen optischen Mikroskopbildsensor (42) umfasst, insbesondere einen CCD-Sensor oder einen CMOS-Sensor.
4. Medizinisches System nach einem der voranstehenden Ansprüche, da durch gekennzeichnet, dass die Kompatibilitätsprüfungsvorrichtung (36) eine optische Detektionseinrichtung (38) umfasst zum Detektieren einer optischen Codierung (46), dass die Detektionseinrichtung (38) ein De tektionssichtfeld (40) definiert und dass das Mikroskopsichtfeld (14) und das Detektionssichtfeld (40) mindestens teilweise überlappen.
5. Medizinisches System nach Anspruch 3, dadurch gekennzeichnet, dass ein Überlapp des Mikroskopsichtfelds (14) und des Detektionssichtfelds
(40) in einem Bereich von etwa 50% bis 100% liegt, insbesondere in ei nem Bereich von etwa 80% bis 100%, vorzugsweise in einem Bereich von etwa 95% bis 100%.
6. Medizinisches System nach Anspruch 4 oder 5, dadurch gekennzeichnet, dass die optische Detektionseinrichtung (38) mindestens einen optischen Bildsensor (44) umfasst, insbesondere einen CCD-Sensor oder einen CMOS-Sensor.
7. Medizinisches System nach Anspruch 6, dadurch gekennzeichnet, dass der Mikroskopbildsensor (42) den optischen Bildsensor (44) definiert oder bildet.
8. Medizinisches System nach einem der voranstehenden Ansprüche, da durch gekennzeichnet, dass die mindestens zwei sich unterscheidenden Implantate (16) und die mindestens zwei sich unterscheidenden Instru mente (18) jeweils mindestens eine optische Codierung (46) umfassen, dass sich die optische Codierung (46) der mindestens zwei sich unter scheidenden Implantate unterscheidet und dass sich die Codierung (46) der mindestens zwei sich unterscheidenden Instrumente (18) unterschei det.
9. Medizinisches System nach Anspruch 8, dadurch gekennzeichnet, dass die mindestens eine optische Codierung (46) von Implantaten (16) und Instrumenten (18), die zueinander kompatibel und für ein gemeinsames Zusammenwirken bestimmt sind, identisch ist.
10. Medizinisches System nach Anspruch 8 oder 9, dadurch gekennzeichnet, dass die mindestens eine optische Codierung (46) in Form eines Bar codes, eines DataMatrix-Codes (58), eines QR-Codes, eines Formcodes (60) oder eines Farbcodes (48) ausgebildet ist.
11. Medizinisches System nach einem der voranstehenden Ansprüche, da durch gekennzeichnet, dass die Kompatibilitätsprüfungsvorrichtung (36) eine Bildverarbeitungseinrichtung (52) umfasst zum automatischen Be stimmen von Instrumenten (18) und/oder Implantaten (16) im Mikros kopsichtfeld (14).
12. Medizinisches System nach Anspruch 10, dadurch gekennzeichnet, dass die Bildverarbeitungseinrichtung (52) ausgebildet ist zum Auslesen der mindestens einen Codierung (46) von Instrumenten (18) und/oder Im plantaten (16) aus einem Bild des Detektionssichtfelds (40) und/oder des Mikroskopsichtfelds (14).
13. Medizinisches System nach einem der voranstehenden Ansprüche, da durch gekennzeichnet, dass die Kompatibilitätsprüfungsvorrichtung (36) eine Anzeigeeinrichtung (54) umfasst zum Anzeigen eines Ergebnisses der Kompatibilitätsprüfung, wobei insbesondere die Anzeigeeinrichtung in Form eines Displays des Mikroskops oder in Form eines externen Displays ausgebildet ist. Z.B in einem Hybrid OP über eine Datenbrille oder eine Sprachausgabe
14. Medizinisches System nach Anspruch 13, dadurch gekennzeichnet, dass das Mikroskop (12) eine Mikroskopanzeigeeinrichtung (30) umfasst zum Anzeigen eines Bildes des Mikroskopsichtfelds (14) und dass die Mikros kopanzeigeeinrichtung (30) die Anzeigeeinrichtung (54) umfasst.
15. Medizinisches System nach einem der voranstehenden Ansprüche, da durch gekennzeichnet, dass die mindestens zwei Implantate (16) in Form medizinischer Clips (60) ausgebildet sind und dass die mindestens zwei Instrumente (18) in Form von Clipanlegeinstrumenten (20) ausge bildet sind.
16. Medizinisches System nach einem der Ansprüche 8 bis 14, dadurch ge kennzeichnet, dass die mindestens zwei Instrumente (18) ein mit einem
der mindestens zwei Implantate (16) in Eingriff bringbares Instrumen tenende (22) umfassen und dass die mindestens eine optische Codierung (46) am oder im Bereich des Instrumentenendes (22) angeordnet oder ausgebildet ist.
17. Verfahren zum Prüfen einer Kompatibilität von Implantaten (16) und In strumenten (18) eines medizinischen Systems (10), welches mindestens zwei sich unterscheidende Implantate (16) und mindestens zwei sich un terscheidende Instrumente (18) umfasst, wobei jedes der mindestens zwei sich unterscheidenden Instrumente (18) ausgebildet und dazu be stimmt ist, mit einem ihm zugeordneten kompatiblen Implantat (16) der mindestens zwei sich unterscheidenden Implantate (18) zusammenzu wirken, dadurch gekennzeichnet, dass die Kompatibilität von einem Im plantat (16) und einem Instrument (18) des medizinischen Systems (10) automatisch geprüft wird, wenn das Implantat (16) und das Instrument (18) gemeinsam im Mikroskopsichtfeld (14) eines Mikroskops (12) positi oniert sind.
18. Verfahren nach Anspruch 17, dadurch gekennzeichnet, dass die Implan tate (16) und Instrumente (18) des Systems (10) eine optische Codie rung (46) aufweisen und dass die Kompatibilität geprüft wird durch Ver gleich der optischen Codierung (46) des Implantats (16) und der opti schen Codierung des Instruments (18), die gemeinsam im Mikroskopsi chtfeld (14) positioniert sind.
19. Verfahren nach Anspruch 18, dadurch gekennzeichnet, dass das Ergebnis des Vergleichs ausgegeben wird, insbesondere optisch und/oder akus tisch.
20. Verwendung eines Systems nach einem der Ansprüche 1 bis 16 zur Durchführung eines Verfahrens nach einem der Ansprüche 17 bis 19.
