Le premier compartiment 2 comporte une source d'énergie sous la forme d'une pile bouton 4 (pouvant être remplacée par une batterie rechargeable), qui prend place à côté d'une carte à circuit imprimé 5 comportant les composants électroniques de gestion du dispositif. Une poussée même limitée dans cette zone aboutit à un déplacement d'amplitude bien supérieure au niveau de l'extrémité libre 13 de la lame ressort 10, déplacement qui comporte une composante horizontale et une composante verticale, laquelle est celle qui nous intéresse principalement dans le cadre de l'invention. Le déplacement d'allure verticale de ladite extrémité libre 13 est exploité pour réaliser la pression sous la plante des pieds en vue de la stimulation de la pompe veineuse.
Le fonctionnement de ce dispositif de remontée veineuse est en réalité cyclique :
Le cycle consiste en la mise en route par le patient, avec une télécommande (typiquement son téléphone portable), qui envoie un signal de commande initial à la carte électronique 5. La première instruction émanant de l'unité de contrôle électronique a pour fonction de faire tourner le moteur 6 électrique rotatif dans un premier sens, après avoir contrôlé que la crémaillère 8 se trouve dans une position de repos neutre. En bout de l'axe moteur, une roue dentée 7 tourne sur et actionne la crémaillère 8, qui se déplace par conséquent avec le poussoir 9 en direction de la lame ressort 10. Ce poussoir 9, en avançant, appuie sur une portion intérieure de la lame ressort 10, dont l'extrémité libre 13 monte, sensiblement perpendiculairement à la direction du déplacement de la crémaillère 8 et du poussoir 9, d'une hauteur prédéterminée, par exemple et de préférence d'environ 14 mm.
La lame ressort 10, en montant, exerce une pression vers le haut sur le pied, via la semelle supérieure de la chaussure, à l'endroit précis des pompes naturelles, qui se situent sous la voûte plantaire. L'exercice de cette pression a pour effet de faire remonter le sang vers le coeur.
Toujours dans ce cycle de fonctionnement, après une durée prédéterminée de rotation du moteur 6 suffisante pour que la lame ressort 10 soit dans une position garantissant que son arête libre 13 soit à la hauteur voulue, le moteur 6 est stoppé par un signal de la carte électronique qui contrôle et commande la réalisation du cycle. La mise à l'arrêt du moteur 6 dure par exemple environ 1 seconde, durée pendant laquelle la pression est maximale sous la voûte plantaire. Ensuite, l'unité électronique fait redémarrer le moteur 6 selon une rotation de sens opposé, la roue dentée 7 actionnant alors la crémaillère 8 dans le sens opposé au premier sens du déplacement. La lame ressort 10 n'est plus contrainte par le poussoir 9, et peut libérer son énergie et reprendre progressivement sa forme initiale, repoussant le poussoir 9 à mesure que la crémaillère 8 s'éloigne sous l'effet de la rotation inverse du moteur 6. Cela aboutit à stopper la pression sur la voûte plantaire, et à laisser libre la circulation du sang, qui reprend son cycle naturel.
La lame ressort 10 étant redescendue, un signal émanant de la carte électronique arrête le moteur 6 et met fin au cycle de base. Ensuite, après une pause qui dure par exemple de 12 à 14 secondes, l'unité électronique envoie une information pour faire redémarrer le moteur en vue d'entamer un nouveau cycle qui obéit exactement aux mêmes logique et cinématique que le précédent, et ainsi de suite. Les cycles se succèdent l'un après l'autre, leur répétition étant stoppée par le patient quand il le souhaite, à l'aide de la télécommande.
Les figures 4 et 5 montrent une variante dans laquelle le dispositif 1 est directement intégré dans la partie basse de la chaussure comportant un talon et une semelle 20 dont dépasse un doigt de poussée 21 traversant ladite semelle 20 (voir en figure 6). Un servo-moteur 6 comporte en sortie d'arbre moteur une portion cannelée 23 ajustée dans un orifice cannelé localisé dans un moyeu 26 à une première extrémité d'un levier 22 rotatif dont l'extrémité opposée, libre, est prévue pour actionner le doigt de poussée 21.
Un détecteur 24 de la présence d'un pied sur la semelle 20 est par ailleurs également connecté à la carte électronique 5, et relié et contrôlé par le module électronique de cette carte 5. Ce détecteur 24 est de préférence basé sur une technologie optique, par exemple une cellule photo-électrique qui envoie un signal différencié selon que la lumière lui parvient ou est cachée par le pied sur la semelle 20. Le connecteur, par exemple de recharge 25 de la batterie 4 située dans le compartiment arrière, se trouve à l'arrière de la semelle 20, au niveau du talon.
Les exemples de fonctionnement et la configuration du dispositif tel qu'illustrés dans les figures ne sont pas exhaustifs de l'invention, qui peut au contraire englober des variantes de forme et de cinématique. Ainsi, la commande peut être non plus sans fil mais au contraire filaire, si un connecteur équipant la carte électronique est prévu et accessible de l'extérieur de la semelle. Il en va de même pour le remplacement de la pile par une batterie, qui nécessite également des moyens de connexion accessibles de l'extérieur de la semelle en vue de sa recharge.
Enfin, la conception des moyens mécaniques, en particulier de la chaîne crémaillère 8/poussoir 9/lame ressort 10, ou la chaîne arbre cannelé 23/levier 22 ne sont que deux parmi de nombreuses possibilités de réaliser la pression sous la voûte plantaire à partir des moyens moteurs 6 commandés en cycles successifs.

The operation of this venous lift device is actually cyclic:
The cycle consists in the start-up by the patient, with a remote control (typically his mobile phone), which sends an initial command signal to the electronic card 5. The first instruction emanating from the electronic control unit has the function of rotating the rotary electric motor 6 in a first direction, after having checked that the rack 8 is in a neutral rest position. At the end of the motor shaft, a toothed wheel 7 rotates and actuates the rack 8, which consequently moves with the pusher 9 in the direction of the leaf spring 10. This pusher 9, while advancing, presses on an inner portion of the leaf spring 10, the free end 13 rises, substantially perpendicular to the direction of movement of the rack 8 and the pusher 9, a predetermined height, for example and preferably about 14 mm.
The spring blade 10, while rising, exerts upward pressure on the foot, via the upper sole of the shoe, at the precise location of the natural pumps, which are located under the arch. The exercise of this pressure has the effect of raising blood to the heart.
Still in this operating cycle, after a predetermined duration of rotation of the motor 6 sufficient for the leaf spring 10 to be in a position ensuring that its free edge 13 is at the desired height, the motor 6 is stopped by a signal from the card which controls and controls the realization of the cycle. The shutdown of the engine 6 lasts for example about 1 second, during which time the maximum pressure is under the arch. Then, the electronic unit restarts the motor 6 in a rotation in opposite direction, the toothed wheel 7 then actuating the rack 8 in the opposite direction to the first direction of movement. The leaf spring 10 is no longer constrained by the pusher 9, and can release its energy and resume progressively its initial shape, pushing the pusher 9 as the rack 8 moves away under the effect of the reverse rotation of the motor 6. This results in stopping the pressure on the arch, and leave free blood circulation, which resumes its natural cycle.
With the spring blade 10 down, a signal emanating from the electronic card stops the motor 6 and terminates the basic cycle. Then, after a pause that lasts for example 12 to 14 seconds, the electronic unit sends information to restart the engine to start a new cycle that obeys exactly the same logic and kinematics as the previous one, and so to after. The cycles follow one after the other, their repetition being stopped by the patient when he wishes, using the remote control.
The Figures 4 and 5 show a variant in which the device 1 is directly integrated in the lower part of the shoe having a heel and a sole 20 which exceeds a thrust finger 21 through said sole 20 (see Figure 6). A servo-motor 6 comprises at the output of the motor shaft a corrugated portion 23 fitted in a grooved orifice located in a hub 26 at a first end of a rotary lever 22 whose opposite end, free, is provided to actuate the finger thrust 21.
A detector 24 of the presence of a foot on the soleplate 20 is furthermore also connected to the electronic card 5, and connected and controlled by the electronic module of this card 5.
