Definitions
- the invention relates to a method for operating a hearing aid device, in particular a tinnitus therapy device, with a microphone and with an ear canal which can be inserted into an auditory canal of a user or at least partially arranged in the auditory canal.
- the invention further relates to a hearing aid device which can be operated according to such a method.
- Hearing aid devices are portable hearing aids that are used to care for the hearing impaired or hearing impaired.
- different types of hearing aid devices such as behind-the-ear hearing aids (BTE) and hearing aids with an external receiver (RIC: receiver in the canal) and in-the-ear hearing aids (IdO), for
- BTE behind-the-ear hearing aids
- RIC receiver in the canal
- IdO in-the-ear hearing aids
- Concha hearing aids or channel hearing aids ITE: In-The-Ear
- CIC Completely-In-Channel
- IIC Invisible-In-The-Channel
- the hearing aids listed by way of example are worn on the outer ear or in the auditory canal of a hearing aid user.
- bone conduction hearing aids, implantable or vibrotactile hearing aids are also available on the market. The stimulation of the damaged hearing takes place either mechanically or electrically.
- Such hearing aids have in principle as essential components an input transducer, an amplifier and an output transducer.
- the input transducer is typically an acousto-electrical transducer, such as a microphone, and / or an electromagnetic receiver, such as an induction coil or (radio frequency, RF) antenna.
- the output transducer is usually called an electro-acoustic transducer, for example as a miniature speaker (earphone), or as an electromechanical transducer, such as a bone conduction earpiece.
- the amplifier is usually integrated in a signal processing device.
- the power is usually supplied by a battery or a rechargeable battery.
- Severe hearing loss rather than hearing impairment, often triggers neuroplastic reorganization of the central auditory system in the affected person's brain, and is therefore often a trigger and cause of onset (chronic) tinnitus.
- Tinnitus or tinnitus is generally understood to mean all types of head or ear noises that are not caused by auditory signals of the environment conducted in the ear.
- a distinction is made between a so-called “subjective tinnitus” and a so-called “objective tinnitus”.
- An objective or physical tinnitus is caused by a sound source present in the body, in particular in the inner ear, of the person concerned.
- the acoustic emissions (emissions) of this sound source into the auditory canal can be measured as spontaneous otoacoustic emissions (SOAE).
- SOAE spontaneous otoacoustic emissions
- Subjective tinnitus In subjective or non-physical tinnitus, there is no such sound source and thus can not be measured. Subjective tinnitus is perceptible only to the subject, and is most often caused by misdirected nerve activity in auditory and other parts of the brain, such as is caused by the neuroplastic reorganization of the central auditory system described above.
- tinnitus Although tinnitus is harmless, it puts a lot of strain on many sufferers. In particular, chronic tinnitus often leads to serious psychological problems and thus sometimes has a negative effect on the professional and social life of the affected person. Furthermore, for example, the concentration is disturbed and causes sleep problems.
- noise devices tinnitus noisers, audio stimulators, tinnitus control instruments, tinnitus maskers
- tinnitus noisers the patient with the help of a hearing aid device similar tinnitus therapy device, a so-called noiser or masker, a quiet little disturbing noise is offered as an audible signal.
- tinnitus therapy devices are usually integrated as an additional function in hearing aids or hearing aid devices.
- the acoustic signal is generated, for example, with a signal frequency which corresponds approximately to the perceived tinnitus frequency and thus to mask ("mask”) the tinnitus.
- notched Environmental Sounds A New Hearing Aid Supported Tinnitus Treatment Evaluated in 20 Patients
- the hearing aid device or tinnitus therapy device receives an acoustic ambient signal by means of a microphone and filters out the or each tinnitus frequency from the signal in the course of a signal processing.
- the filtered signal is output as an acoustic output signal by means of a handset to the user.
- a hearing aid in which an acoustic output signal is generated in which one or more tinnitus frequencies of the user are suppressed.
- This makes possible a neuroplastic reorganization of the affected person's central auditory system, which reverses the tinnitus-causing maladaptive neuroplastic reorganization of the affected person's central auditory system.
- a tinnitus masking signal is generated in which the tinnitus frequency (s) is filtered out (suppressed) by means of the notch filter or bandpass filter.
- a hearing aid device is described as a tinnitus therapy device.
- the known hearing aid device is designed here for a therapy of an objective tinnitus.
- the generated sound of the otoacoustic emission is detected by means of a microphone.
- a compensation or anti-noise signal is generated by means of a receiver in the auditory canal, so that the otoacoustic emissions of objective tinnitus are suppressed.
- the invention has for its object to provide a particularly suitable method for operating a hearing aid.
- a further object of the invention is to provide a hearing aid device suitable for carrying out such a method.
- the method according to the invention is suitable and configured for operating a hearing aid device, in particular a tinnitus therapy device.
- the hearing aid device comprises a microphone and a handset, which are arranged at least partially in an (external) auditory canal of a user or in the auditory canal can be used (can be introduced).
- the handset is intended and set up for insertion into the auditory canal of a user.
- ANR Active Noise Canceling of an ambient acoustic signal received by the microphone is effected at one or more tinnitus frequencies of a user by generating an acoustic compensation signal of the listener in the auditory canal of the user.
- the acoustic compensation signal is also referred to below as an anti-sound.
- an active sound compensation is provided to reduce the tinnitus sensation, in which an extinction or at least suppression or attenuation of the or each tinnitus frequency is sought by an additional radiation of the acoustic compensation signal by the listener by superimposing the compensation signal with the ambient acoustic signal in the ear canal.
- an audible compensation signal is output by the listener which, when superimposed on the ambient signal in the auditory canal of the user, produces a resulting overall signal in which the or each tinnitus frequency of the user is suppressed or at least reduced.
- frequencies with frequency values less than 2 kHz (kilohertz) can be efficiently attenuated.
- the hearing aid is designed to suppress or reduce perceived subjective tinnitus (non-physical tinnitus).
- the or each tinnitus frequency is a subjectively perceived tinnitus frequency of the sufferer.
- the subjective tinnitus frequencies of the affected person are therefore not caused by a measurable sound source.
- the term tinnitus is to be understood as meaning, in particular, a subjective tinnitus, and the term tinnitus frequency in particular as meaning a corresponding (subjective) tinnitus frequency of such a subjective tinnitus.
- the active sound compensation does not take place by means of a compensation signal for an otoacoustic emission of a objective tinnitus. Rather, the or each (subjective) tinnitus frequency is suppressed or reduced by the compensation signal in an ambient signal coming from outside the ear, so that a neuroplastic reorganization of the affected person's central auditory system is made possible.
- the auditory canal of the user is thus not closed by sound blocking means of the hearing aid device, so that in addition to the compensation signal generated by the listener and the ambient acoustic signal penetrates into the ear canal.
- the occlusion effect is thus substantially completely avoided. This is particularly advantageous for non or little hearing impaired users, as thus a more natural sound image of the environment is possible.
- the acoustic compensation signal or the anti-sound is hereby generated in particular as a phase-inverted acoustic signal.
- the anti-sound is generated in such a way that it corresponds to a sound corresponding to the or each tinnitus frequency in the ambient signal with as exactly opposite polarity as possible. This allows a reliable extinction or suppression of the corresponding frequency ranges in the perceived ambient signal in the ear canal.
- the hearing aid device is designed in particular as a tinnitus therapy device.
- the hearing aid is with a microphone for receiving an acoustic ambient signal and conversion of this ambient signal into an electrical input signal, and with a signal processing device for processing the electrical input signal into an electrical output signal, and with a at least partially arranged in an auditory canal of a user handset for converting the electrical output signal into an acoustic compensation signal ,
- the acoustic compensation signal is used here for active sound compensation at the or each tinnitus frequency of the user.
- an acoustic ambient signal is detected by the microphone, which is analyzed as an electrical input signal from the signal processing device.
- the signal processing device processes the electrical input signal into an electrical output signal with which the acoustic compensation signal is generated.
- the acoustic compensation signal generated by the listener overlaps or interferes with the ambient acoustic signal in the user's ear canal in such a manner that the sound pressure at a user's tympanic membrane is reduced or completely suppressed at the or each tinnitus frequency.
- the remaining sound spectrum or frequency spectrum of the ambient acoustic signal is substantially unchanged in this case, so that a high sound quality is guaranteed for the user.
- the or each tinnitus frequency of the user is stored in a memory of the signal processing device.
- For detecting and determining the or each tinnitus frequency is determined at which frequency or at which frequencies the user perceives acoustic signal, which does not have its cause in reaching from the outside of the ear acoustic signals (ambient signals). This detection takes place, for example, with a doctor or a hearing care professional.
- the or each detected tinnitus frequency is then stored in the memory of the signal processing device.
- the signal processing means uses the or each deposited tinnitus frequency to process the input signal.
- a suitable compensation signal for extinguishing or suppressing the tinnitus frequencies can always be generated.
- the signal processing device preferably also takes into account the internal hearing aid transmission path including the phase differences between the ambient signal and the compensation signal to be generated.
- a model of the acoustic transmission path of the hearing aid device it is possible, for example, for a model of the acoustic transmission path of the hearing aid device to be stored in the memory.
- the microphone and the handset are arranged as close to each other as possible, so that occurring signal delays and phase differences are largely reduced.
- the signal processing device preferably has a controller (that is to say a control device).
- the controller is in this case generally - program and / or circuitry - set up to carry out the inventive method described above.
- the controller is thus in particular configured to generate an electronic output signal based on the received input signal and the stored tinnitus frequencies and preferably with the help of a model of the transfer function of the hearing aid by adjusting the filter means, which causes an acoustic compensation signal of the listener for active sound compensation of the tinnitus frequencies in the ambient signal ,
- the controller is formed at least in the core by a respective microcontroller with a processor and a data memory, in which the functionality for implementing the method according to the invention in the form of operating software (firmware) is implemented by programming, so that the method - optionally in interaction with a user - when running the operating software in the microcontroller is performed automatically.
- operating software firmware
- the controller is in a possible embodiment within the scope of the invention alternatively but also by programmable electronic components, for example an application-specific integrated circuit (ASIC) is formed, in which the functionality for carrying out the method according to the invention is implemented by circuitry means.
- ASIC application-specific integrated circuit
- the controller is designed, in particular, to effectively suppress tinnitus frequencies with frequency values greater than 2 kHz. Frequently there is already a significant hearing loss in this frequency range, so that even with an open, ie not sound-blocking, auditory canal, a clear attenuation of the tinnitus frequencies is already achieved. In the case of users or patients without significant hearing loss, it is still possible to close the auditory canal with acoustic filter means which in particular acoustically attenuate frequencies greater than 2 kHz.
- the filter means are designed, for example, as electronic filters, in particular notch filters (notch filters).
- the active sound compensation or the anti-sound is used in particular for the attenuation of tinnitus frequencies less than about 2 kHz, for frequencies greater than about 2 kHz in particular the filter means for acoustic attenuation of the tinnitus frequencies of the ambient signal are provided.
- the filter means for acoustic attenuation of the tinnitus frequencies of the ambient signal are provided.
- FIGURE shows a schematic and simplified representation of a worn on an ear of a user tinnitus therapy device with a microphone and a handset and with a running for active sound compensation signal processing device.
- a hearing aid device designed as a tinnitus therapy device 2 is shown.
- the tinnitus therapy device 2 is worn in operation on an ear 4 of a tinnitus-affected user.
- the tinnitus therapy device 2 comprises a microphone 6 as acousto-electrical converter and a Handset 8 as an electro-acoustic transducer and a signal processing device 10, which is designed in particular for active sound compensation of one or more tinnitus frequencies.
- the microphone 6 is arranged at an input 12 of an (external) ear canal 14 of the ear 4. However, it is also conceivable that the microphone 6 is at least partially disposed within the ear canal 14. However, the earphone 8 is always positioned further inside or lower in the ear canal 14 than the microphone 6. The ear canal 14 extends from the entrance 12 to a tympanic membrane 16 of the user.
- the or each tinnitus frequency of the user is stored in a memory 18 of the signal processing device 10.
- an acoustic ambient signal 20 is actively sound-compensated such that the or each stored in the memory 18 Tinnitusfrequenz is suppressed or reduced.
- the tinnitus therapy device 2 is designed to be open, which means that the entrance 12 of the ear canal 14 is not (completely) acoustically blocked by the tinnitus therapy device 2.
- the ambient acoustic signal 20 enters the ear canal 14.
- the environmental signal 20 is further received by the microphone 6 and converted into an electrical input signal 22.
- the electrical input signal 22 is supplied to the signal processing device 10, which processes it into an electrical output signal 24.
- the electrical output signal 24 is supplied to the handset 8, which emits it as an acoustic compensation signal (anti-noise) 26 in the ear canal 14.
- the acoustic ambient signal 20 and the acoustic compensation signal 26 superimpose or interfere with each other to form a resulting total signal 28 which has a reduced sound pressure at the eardrum 16 in the frequency range of the or each tinnitus frequency.
- the acoustic compensation signal 26 is hereby generated as a phase-inverted acoustic signal of the ambient acoustic signal 20. This means that the acoustic compensation signal 26, at least in the frequency range of the or each tinnitus frequency, corresponds to a sound of the ambient acoustic signal 20 with as exactly opposite an opposite polarity as possible.
- the acoustic ambient signal 20 is detected with the microphone 6. Based on the stored tinnitus frequency (s) and with the aid of a model of an acoustic transfer function of the tinnitus therapy device 2 stored in the memory 18, the signal processing device 10 calculates the signal component of the environmental signal 20 which would still remain on the eardrum 16. For this signal component, the opposite-polarity acoustic compensation signal 26 is then generated in the receiver 8 for compensation.
- the tinnitus therapy device 2 further comprises filter means, for example in the form of electronic notch filters (notch filters) 30, for the acoustic attenuation, in particular of tinnitus frequencies greater than 2 kHz.
- filter means for example in the form of electronic notch filters (notch filters) 30, for the acoustic attenuation, in particular of tinnitus frequencies greater than 2 kHz.
Abstract
