Abstract
Description
- Die Erfindung betrifft ein Therapiegerät gemäß den Merkmalen des Patentanspruchs 1.
- Aus der Patentschrift
DE 195 07 927 C2 ist ein schwingfähiger Stehboden bekannt, der aus einem stehenden Unterteil und einem beweglichen Oberteil besteht und der im Wesentlichen in horizontaler Ebene schwingfähig ist. Schwingböden dieser Art werden in der medizinischen Therapie insbesondere zum posturalen Training verwendet. - Sensormotorisches Training ist eine bekannte Methode um traumatische, krankheits- oder altersbedingte Gleichgewichtsstörungen zu therapieren. Dabei ist die zu theraplerende Person durch die gezielte Schaffung eines labilen Standuntergrundes gefordert durch ausgleichende Bewegungen den Körperschwerpunkt zu verlagern um, im Extremfall, ein Fallen zu verhindern. Fallverletzungen sind speziell im Alter die mit Abstand führende Verletzungsart bezüglich Therapiekosten bzw. Pflegekosten. Bei Sportverletzungen sowie Erkrankungen des menschlichen Gleichgewichtsapparates werden entsprechende Übungen genutzt um durch Training anderer physiologischer (Ersatz-)Mechanismen eine Verbesserung der Leistungsfähigkeit bzw. Lebensqualität zu erreichen. Einer der Faktoren für das Training ist es auf das Unterbewusstsein zu wirken, d.h. ein bewusstes, willentliches Eingreifen zur Gleichgewichtsfindung zu verhindern. Nur so kann die Person im täglichen Leben Unfälle durch spontane Ausgleichsreaktionen auf unerwartete Ereignisse (nicht bemerkte Bodenunebenheiten o.a.) vermeiden.
- Schon heute werden Übungen mit dem o.g. Hintergrund durchgeführt. Dazu sind heute bereits einfache Balancierplattformen (Wippen, Kegel, Schaumstoffunterlagen) wie auch komplizierter Aufbauten, z.B. mit kardanischer Federaufhängung im Einsatz. Allen gemeinsam ist aber, dass es zum Training des Unterbewusstseins notwendig ist, den zu Therapierenden aktiv abzulenken; zum Beispiel durch Gespräche bis hin zum Lösen von Rechenaufgaben etc. Dies ist speziell wichtig wenn die zerebrale Leistungsfähigkeit zur Verarbeitung der Reize bzw. Befehle an den Bewegungsapparat für die Probleme verantwortlich ist.
- Aus
DE 601 33 286 T2 ist ein Gleichgewichtstrainer bekannt bei welchem die Bewegung durch Messfühler erfasst und zu einer elektronischen Schnittstelle geleitet, welche die aktuelle Position der Testperson in Maus-, Joystick- oder ähnliche Zeigegerätsignale umwandelt, die dann einem Personalcomputer (PC) zugeführt werden. Auf diese Weise kann eine Testperson durch freiwilliges Ändern der Körperhaltung verschiedene auf dem PC laufende Anwendungsprogramme steuern. - Es ist Aufgabe der Erfindung eine Therapieanordnung mit einem schwingfähigen Stehboden anzugeben, mit welcher die Effektivität einer posturalen Trainingseinheit gesteigert werden kann.
- Die Aufgabe wird mit der Therapieanordnung gemäß den Merkmalen des geltenden Patentanspruchs 1 gelöst. Vorteilhafte Ausgestaltungen der Erfindung sind Gegenstand von Unteransprüchen.
- Die erfindungsgemäße Therapieanordnung weist einen schwingfähigen Stehboden auf. Die Erfindung zeichnet sich dadurch aus, dass ein Computer zur Ansteuerung des schwingfähigen Stehbodens vorhanden ist. Die Therapieanordnung umfasst erfindungsgemäß ferner ein Gerät, welches die Bewegungen einer Testperson auf dem schwingfähigen Stehboden erfasst und in ein erstes Steuersignal für einen mit dem Bewegungserfassungsgerät verbundenen Computer umwandelt. Erfindungsgemäß ist ein mit dem Computer verbundenes erstes Element vorhanden, welches ein zweites Steuersignal des Computers in mechanische Auslenkungen des beweglichen Oberteils umwandelt. Dieses erste Element kann z.B. ein aktives Element sein, welches auf geeignete Weise eine Auslenkung des schwingfähigen Stehbodens bewirkt.
- Zweckmäßig ist ein mit dem Computer verbundenes zweites Element vorhanden. Dieses zweite Element kann z.B. ein dämpfendes Element sein, welches auf geeignete Weise eine Dämpfung des schwingfähigen Stehbodens bewirkt. Der Computer wertet zweckmäßig das durch das erste Steuersignal übermittelte Balancierverhalten des Patienten aus und passt die Dämpfung an den Trainingszustand an. Hierbei erzeugt der Computer zweckmäßig ein drittes Steuersignal, welches an das zweite Element geleitet wird.
- Es hat sich gezeigt, dass durch das Zusammenwirken der bestehenden Trainingsansätze mit Geräten für computerunterstützte Balanzierübungen die Effektivität des Trainings gesteigert werden kann. Zudem kann der zeitintensive Einsatz von Therapiepersonal reduziert werden.
- Idee der Erfindung ist die Kombination einer Plattform zum posturalen Training mit einem Computer, welcher geeignet ist Anwendungsprogramme auszuführen. Als Plattform zum posturalen Training kann z.B. das Produkt Posturomed® der Fa. Haider verwendet werden. Als Computer kann jede kommerziell erhältliche Spielkonsole verwendet werden.
- Sowohl die Anwendung einer Spielkonsole, als auch die Anwendung einer posturalen Plattform führen zu bewusstem Steuern. Die Kombination der beiden Anlagen führt jedoch zu einer unterbewussten Gleichgewichtsfindung, d.h. ein bewusstes, willentliches Eingreifen zur Gleichgewichtsfindung wird verhindert. Dieser Effekt kann noch durch die Reaktion auf von außen aufgeprägte Störungen verstärkt werden, bzw. der Schwierigkeitsgrad adaptiert und damit die Trainingsintensität gesteigert werden.
- In einer ersten Ausführungsform der Erfindung ist das Bewegungserfassungsgerät ein Balancebrett für eine Spielkonsole. Bei dieser Ausführungsform dient das Balancebrett als Standfläche für die Testperson. Handelsübliche Balancebretter für Spielkonsolen umfassen eine Mehrzahl von Sensoren, mit welchen die Bewegungen der Testpersonen auf dem Balancebrett erfasst und in elektrische Steuersignale zur Weiterleitung an die Spielkonsole umgewandelt werden.
- In der zweiten Ausführungsform der Erfindung ist das Bewegungserfassungsgerät eine visuelle Bewegungserkennungseinrichtung für eine Spielkonsole. Bei dieser Ausführungsform steht die Testperson vor der visuellen Bewegungserkennungseinrichtung. Handelsübliche visuelle Bewegungserkennungseinrichtungen für Spielkonsolen umfassen üblicherweise eine Kamera, mit welcher die Bewegungen einer Testperson erfasst und in elektrische Steuersignale zur Weiterleitung an die Spielkonsole umgewandelt werden.
- Die Erfindung wird im Weiteren anhand von Figuren näher erläutert. Es zeigen:

Fig. 1 undFig. 2 zeigen eine Therapieanordnung mit einem schwingfähigen Stehboden. Der schwingfähige Stehboden 1 umfasst beispielhaft ein auf dem Boden B stehendes Unterteil 3 und ein relativ zum Boden B bewegliches Oberteil 2. Der Stehboden 1 kann dabei auf verschiedene Weise schwingfähig gelagert sein. InFig. 1 undFig. 2 sind das Oberteil 2 und das Unterteil 3 beispielhaft durch zwei Schwingungselemente 4a, 4b miteinander verbunden. Andere Varianten für eine schwingfähige Lagerung des Stehbodens sind ausDE 19507927 C2 bekannt bzw. sind inFig. 3 und4 beispielhaft gezeigt. Zwischen dem Oberteil 2 und dem Unterteil 3 sind in den inFig. 1 undFig. 2 dargestellten Ausführungsformen ferner ein aktives Element 5 und ein dämpfendes Element 6 angeordnet. Das aktive Element 5, z.B. ein elektrischer Antrieb oder ein hydraulischer Aktuator und das dämpfende Element 6, z.B. ein pneumatischer oder elektroviskoser Dämpfer, sind jeweils mit einem Computer 7, z.B. einer Spielkonsole verbunden. Auf der Spielkonsole 7 können Anwendungsprogramme ausgeführt werden, welche auf einem Bildschirm 9 für eine Testperson 8 dargestellt werden können. - Die Testperson 8 verfolgt die auf dem Bildschirm 9 dargestellten, visuellen Reize. Die Reize werden durch eine Übung (z.B. durch ein Spiel, Fragestellungen, Aufgaben etc.) dargestellt. Die Verwendung von spielerischen Übungen (z.B. durch angepasste, kommerzielle PC- oder Konsolenspiele kann die Motivation zur Durchführung und damit die Effektivität deutlich erhöhen. Die Bewegungen werden im Bewegungserfassungsgerät 10 erfasst, in ein erstes elektrisches Steuersignal umgewandelt und an die Spielkonsole 7 weitergegeben. Die Spielkonsole 7 erzeugt in Abhängigkeit des ersten Steuersignals ein zweites Steuersignal und steuert damit den schwingfähigen Stehboden 1 über das aktive Element 5 an, um z.B. in Abhängigkeit des jeweiligen PC- oder Konsolenspiels eine Auslenkung des Oberteils 2 des schwingfähigen Stehbodens 1 in Pfeilrichtung zu bewirken.
- Zusätzlich erzeugt die Spielkonsole 7 in Abhängigkeit z.B. des ersten Steuersignals oder des Fortschritts, des auf der Spielkonsole abgespielten Anwendungsprogramms, ein drittes Steuersignal und steuert damit das dämpfende Element 6 an. Dadurch kann z.B. das Dämpfungsverhalten des schwingfähigen Stehbodens 1 je nach Level des Spiels oder je nach Therapiefortschritt aktiv beeinflusst werden kann. So kann bei Anfängern z.B. die Dämpfung erhöht werden, während bei Fortgeschrittenen (oder in einem fortgeschrittenem Spiellevel) die Dämpfung reduziert werden kann. Eine permanente Analyse der Variabilität des Körperschwerpunkts erlaubt es darüber hinaus, die Dämpfung dynamisch zu regeln. Außerdem können Ereignisse, welche auf dem Bildschirm angezeigt werden, auf dem schwingfähigen Stehboden 1 simuliert werden (z.B. Kippbewegung, Seitwärtsbewegung beim Ausweichen eines Hindernisses). Zusätzlich können Störungen ohne visuelle Vorwarnung aufgeprägt werden.
- In der in
Fig. 1 dargestellten ersten Ausführungsform steht die Trainingsperson 8 auf einem Bewegungserfassungsgerät 10, wobei das Bewegungserfassungsgerät 10 auf dem Oberteil 2 des Stehbodens 1 befestigt ist. - Das Bewegungserfassungsgerät 10, welches eine Art Eingabegerät für Bewegungen ist, kann ein elektronisches Balancebrett mit internen oder externen Sensoren 11 sein. Das Bewegungserfassungsgerät 10 ist mit der Spielkonsole 7, zweckmäßig über eine entsprechende Schnittstelle (nicht dargestellt) verbunden. Die Sensoren 11 des Bewegungserfassungsgeräts 10 erfassen die Verlagerung des Körpergewichts der Testperson 8 und wandeln sie zur Weiterleitung an die Spielkonsole 7 in elektrische Signale um.
- In der in
Fig. 2 dargestellten zweiten Ausführungsform steht die Trainingsperson 8 vor einem Bewegungserfassungsgerät 10 auf dem schwingfähigen Stehboden 1. Das Bewegungserfassungsgerät 10 ist ein visuelles Bewegungserkennungsgerät, z.B. eine Kamera. Dieses Gerät 10 erfasst die Bewegungen der auf dem schwingfähigen Stehboden 1 stehenden Testperson 8, wandelt sie in elektrische Signale um und leitet sie an die Spielkonsole 7 weiter. - In einer weiteren Ausgestaltung des schwingfähigen Stehbodens, wie in
Fig. 3 und4 gezeigt, ist es möglich, dass die Schwingungselemente 4a, 4b, das aktive Element 5 und das dämpfende Element 6 durch eine Kombination von Schwingungs- und Dämpfungselemente ersetzt sind.Fig. 3 zeigt, dass zwischen dem auf dem Boden B stehenden Unterteil 3 und dem schwingenden Oberteil 2 Formlegierungselemente 4c aus einstellbaren elastischen Materialen angeordnet sind. Ein sich auf dem Oberteil 2 befindliche Bewegungserfassungsgerät 10 ist nicht dargestellt. Die Formlegierungselemente 4c können von einem Computer 7 (nicht dargestellt) wie inFig. 1 bzw. 2 dargestellt, angesteuert werden. -
Fig. 4 zeigt eine weitere Variante, bei welcher zwischen dem Boden B und dem schwingenden Oberteil 2 einstellbare pneumatische Aktuatoren 4d angeordnet sind. Diese pneumatischen Aktuatoren können z.B. Fluidmuskel in Kissenform sein. Über die Leitungen LL ist es möglich z.B. Luft L in die Kissen 4d zu pumpen. Zweckmäßig können die Kissen mit unterschiedlichen Volumina gefüllt werden. Dadurch ist es möglich, dass der schwingfähigen Stehboden 1 eine entsprechende Neigung gegenüber dem Boden B einnimmt. Die pneumatischen Aktuatoren 4d können von einem Computer 7 (nicht dargestellt) wie inFig. 1 bzw. 2 erläutert, mittels eines zweiten bzw. dritten Steuersignals angesteuert werden. - Es ist auch möglich, dass die Formiegierungselemente 4c in
Fig. 3 z.B. durch Fluidmuskel in Schlauchform ersetzt werden, welche nach demselben Prinzip befüllt und angesteuert werden können wie unterFig. 4 beschrieben.
Claims (6)
- Therapieanordnung mit einem schwingfähigen Stehboden (1),
dadurch gekennzeichnet, dass
ein Computer (7) zur Ansteuerung des schwingfähigen Stehbodens (1) vorhanden ist,
ein Gerät (10) zur Erfassung der Bewegungen einer Testperson (8) auf dem schwingfähigen Stehboden (1) und zur Umwandlung der erfassten Bewegungen in ein erstes Steuersignal für den mit dem Gerät (10) verbundenen Computer (7) vorhanden ist,
ein mit dem Computer (7) verbundenes erstes Element (4c, 4d, 5) vorhanden ist zur Umwandlung eines zweiten Steuersignals des Computers (7) in mechanische Auslenkungen des schwingfähigen Stehbodens (1). - Therapieanordnung nach Anspruch 1,
dadurch gekennzeichnet, dass
ein mit dem Computer (7) verbundenes zweites Element (4c, 4d, 6) vorhanden ist zur Umwandlung eines dritten Steuersignals des Computers (7) in eine Dämpfung der Auslenkung des schwingfähigen Stehbodens (1). - Therapieanordnung nach einem der Ansprüche 1 bis 2,
dadurch gekennzeichnet, dass
das Bewegungserfassungsgerät (10) ein Balancenbrett für eine elektronische Spielkonsole ist. - Therapieanordnung nach einem der Ansprüche 1 bis 2,
dadurch gekennzeichnet, dass
das Bewegungserfassungsgerät (10) eine visuelle Bewegungserkennungseinrichtung z.B. eine Kamera für eine Spielkonsole ist. - Therapieanordnung nach einem der vorangehenden Ansprüche,
dadurch gekennzeichnet, dass
das erste Element (4c, 4d, 5) ein elektroviskoser Dämpfer, ein elektrischer Antrieb oder ein hydraulischer Aktuator ist. - Therapieanordnung nach einem der Ansprüche 1 bis 4,
dadurch gekennzeichnet, dass
das erste Element (4c, 4d, 5) eine Kombination aus einem Schwingungs- und Dämpfungselement ist.
