Abstract
Um bei einem Implantat zur Festlegung von zwei miteinander zu verbindenden Knochenteilen mit einem ersten plattenförmigen Anlageelement zur Anlage an Innenflächen der Knochenteile, mit mindestens zwei daran festgelegten, quer von diesem abstehenden Spannelementen und mit einem zweiten plattenförmigen Anlageelement zur Anlage an Außenflächen der Knochenteile, welches mittels der durch den Zwischenraum zwischen den Knochenteilen hindurchgeführten Spannelemente gegen das erste Anlageelement spannbar ist, die Handhabbarkeit des Implantates beim Spannen zu verbessern, wird vorgeschlagen, daß die Spannelemente an ihrem dem ersten Anlageelement gegenüberliegenden Ende durch eine Brücke miteinander verbunden sind.
Description
IMPLANTAT ZUR FESTLEGUNG VON ZWEI MITEINANDER ZU VERBINDENDEN KNOCHENTEILEN
Die Erfindung betrifft ein Implantat zur Festlegung von zwei miteinander zu verbindenden Knochenteilen mit einem ersten plattenförmigen Anlageelement zur Anlage an Innenflächen der Knochenteile, mit mindestens zwei daran festgelegten, quer von diesem Anlageelement abstehenden Spannelementen, mit einem zweiten plattenförmigen Anlageelement zur Anlage an Außenflächen der Knochenteile, welches mittels der durch den Zwischenraum zwischen den Knochenteilen hindurchgeführten Spannelemente gegen das erste Anlageelement spannbar ist.
Ein solches Implantat ist beispielsweise in der DE 103 26 690 B4 beschrieben. Die Anlageelemente sind dabei rechteckig ausgebildet und werden durch zwei stiftförmige Spannelemente gegeneinander gespannt, die durch den Zwischenraum zwischen den zusammenzuspannenden Knochenteilen hindurchragen. Dadurch wird sichergestellt, daß die rechteckigen Anlageelemente parallel zu diesem spaltförmigen Zwischenraum zwischen den Knochenteilen angeordnet werden und auch parallel zueinander, das heißt die relative Orientierung der Anlageelemente kann durch das Vorsehen von zwei derartigen Spannelementen sichergestellt werden.
Es ist jedoch schwierig, bei zwei derartigen Spannelementen, an denen das zweite Anlageelement verschiebbar gelagert ist, eine verkantungsfreie Verspannung des zweiten Anlageelementes relativ zum ersten Anlageelement zu erreichen. Bei der bekannten Vorrichtung muß dazu ein Spannwerkzeug nacheinander abwechselnd an den beiden Spannelementen angelegt werden, um an jedem der beiden Spannelemente das zweite Anlageelement um eine be-
stimmte Wegstrecke in Richtung auf das erste Anlageelement zu verschieben. Dies ist umständlich, und außerdem besteht die Gefahr, daß an den freien Enden der freien stiftförmigen Spannelemente Instrumente, Gewebe etc. hängenbleibt oder Verletzung durch diese freien Enden erzeugt werden.
Es ist Aufgabe der Erfindung, ein gattungsgemäßes Implantat so weiterzubilden, daß die Handhabung vereinfacht wird, insbesondere beim Spannen der Anlageelemente.
Diese Aufgabe wird bei einem Implantat der eingangs beschriebenen Art erfindungsgemäß dadurch gelöst, daß die Spannelemente an ihrem dem ersten Anlageelement gegenüberliegenden Ende durch eine Brücke miteinander verbunden sind.
Dadurch werden die Spannelement stabilisiert und die freien Enden der Spannelemente werden abgedeckt, so daß eine Verletzung an diesen freien Enden oder ein Hängenbleiben ausgeschlossen ist.
Insbesondere kann vorgesehen sein, daß die Brücke mit den Spannelementen in Spannrichtung unverschieblich verbunden ist. Dann kann die Brücke als Widerlager für ein Spannwerkzeug dienen, mit dem das zweite Anlageelement gegen das ersten Anlageelement gespannt wird.
Insbesondere können die Spannelemente als Stifte ausgebildet sein.
Zur Festlegung der Brücke an den Spannelementen ist es gemäß einer bevorzugten Ausführungsform der Erfindung vorgesehen, daß die Brücke mit den Spannelementen durch eine Verquetschung unverschieblich verbunden ist.
Die Spannelemente können in Bohrungen in der Brücke eingesteckt sein, so daß dadurch eine exakte Parallelführung der Spannelemente erreicht werden kann.
Bei einer bevorzugten Ausführungsform der Erfindung ist vorgesehen, daß die Brücke eine Anlagefläche für eine Spanneinrichtung aufweist, welche mit zwei Spannwerkzeugen einerseits an der Anlagefläche und andererseits an der Aus- senfläche des zweiten Anlageelementes anlegbar ist und beim Auseinanderschieben der Spannwerkzeuge das zweite Anlageelement gegen das erste Anlageelement spannt.
Dabei ist es insbesondere vorteilhaft, wenn die Anlagefläche und das an ihr anliegende Spannwerkzeug derart geformt sind, daß sich das Spannwerkzeug an der Brücke in der Mitte zwischen zwei Spannelementen abstützt. Man erhält damit einen eindeutigen Abstützpunkt für die Spannwerkzeuge, der an der Brücke in der Mitte zwischen den beiden Spannelementen liegt, und daher können die Spannkräfte symmetrisch zu den Spannelementen am zweiten Anlageelement angreifen.
Weiterhin ist es vorteilhaft, wenn das an dem zweiten Anlageelement anlegbare Spannwerkzeug an zwei Punkten an dem zweiten Anlageelement anlegbar ist, die in der von zwei Spannelementen aufgespannten Ebene im Abstand zueinander und von der Mitte zwischen den zwei Spannelementen entfernt angeordnet sind. Man erhält insgesamt damit eine Drei-Punkt-Anlage der Spannwerkzeuge, nämlich an der Brücke in der Mitte zwischen den Spannelementen und an dem zweiten Anlageelement in der Ebene der beiden Spannelemente, jedoch im Abstand zur Mitte. Dadurch wird eine Verkippung des zweiten AnIa-
geelementes bei der Verschiebung vermieden, und das zweite Anlageelement kann ohne Schwierigkeiten bis in die Spannstellung verschoben werden.
Insbesondere kann die Anlagefläche an der Brücke kreisbogenförmig verlaufen, so daß das an der Anlagefläche anliegende Spannwerkzeug beim Spannen zwangsläufig in die tiefste Stelle der Anlagefläche und damit in die Mitte zwischen den beiden Spannelementen geschoben wird, insbesondere wenn das anliegende Spannwerkzeug in ähnlicher Weise kreisbogenförmig ausgebildet ist.
Gemäß einer weiteren vorteilhaften Ausgestaltung ist vorgesehen, daß die Anlageelemente quer von ihnen abstehende, jeweils zum anderen Anlageelement weisende Fixiervorsprünge tragen und daß diese Fixiervorsprünge so geneigt sind, daß ihr Abstand von den Fixiervorsprüngen auf der gegenüberliegenden Seite des Anlageelementes von dem Anlageelement zum freien Ende der Fixiervorsprünge hin zunimmt. Eine solche Ausgestaltung führt beim Zusammenspannen der beiden Anlageelemente dazu, daß die Knochenteile beim Eindringen der Fixiervorsprünge in die Knochenteile aufeinander zugeschoben werden, da die Knochenteile in Richtung auf die Ansatzpunkte der Fixiervorsprünge an den Anlageelementen verschoben werden und da der Abstand der Fixiervorsprünge an den Fußpunkten der Fixiervorsprünge kleiner ist als an den freien Enden.
Die nachfolgende Beschreibung bevorzugter Ausführungsformen der Erfindung dient im Zusammenhang mit der Zeichnung der näheren Erläuterung. Es zeigen:

Bei den beiden miteinander zu verbindenden Knochenteilen 1, 2 kann es sich beispielsweise um zwei plattenförmige Teile des Schädelknochens handeln oder um die bei einer Herzoperation voneinander getrennten Teile der Rippenbögen, die längs des Sternums getrennt sind. Bei dem dargestellten Ausführungsbeispiel sind die Knochenteile 1, 2 sehr vereinfacht schematisch dargestellt, wesentlich ist lediglich, daß die Knochenteile 1, 2 auf gleicher Höhe und längs eines Zwischenraumes 3 zwischen den Knochenteilen 1, 2 angenähert relativ zueinander fixiert werden sollen.
Diese Fixierung erfolgt mit Hilfe eines Implantates 4, welches eine rechteck- förmige erste Anlageplatte 5 und eine ebenfalls rechteckförmige zweite Anlageplatte 6 umfaßt. Die erste Anlageplatte 5 weist senkrecht von ihr abstehende und in Richtung auf die zweite Anlageplatte 6 weisende, zahnförmige Fixiervorsprünge 7 auf, die längs der Kanten der ersten Anlageplatte 5 angeordnet sind, und in gleicher Weise sind an der zweiten Anlageplatte 6 in Richtung auf die erste Anlageplatte 5 weisende Fixiervorsprünge 8 angeordnet. Alle Fixiervorsprünge 7, 8 laufen spitz aus und sind daher geeignet, beim Anpressen der
Anlageplatten 5, 6 an den Knochenteilen 1, 2 in das Knochenmaterial einzudringen. Dabei sind alle Fixiervorsprünge 7, 8 so geneigt, daß der Abstand der Fixiervorsprünge an gegenüberliegenden Kanten der Anlageplatten ausgehend von den Fußpunkten der Fixiervorsprünge 7, 8 an den Anlageplatten 5, 6 hin zu den freien Enden der Fixiervorsprünge 7, 8 größer wird, das heißt die Fixiervorsprünge 7, 8 sind geringfügig nach außen geneigt. Dies führt dazu, daß beim Gegeneinanderspannen der Anlageplatten 5, 6 und beim Eindringen der Fixiervorsprünge 7, 8 in die Knochenteile 1, 2 diese einander angenähert werden, das heißt in der Ebene der Anlageplatten 5, 6 gegeneinander gespannt werden.
Von der ersten Anlageplatte 5 stehen zwei im Abstand zueinander und in der Längsmittelebene der Anlageplatte 5 liegende Spannstifte 9, 10 ab, die parallel zueinander verlaufen und deren Länge wesentlich größer ist als die Dicke der miteinander zu verbindenden Knochenteile 1, 2. Diese Spannstifte 9, 10 durchsetzen Öffnungen 11, 12 in der zweiten Anlageplatte 6, diese zweite Anlageplatte 6 ist längs der Spannstifte 9, 10 auf diesen verschiebbar gelagert.
In dem unteren, der ersten Anlageplatte 5 zugewandten Teil tragen die Spannstifte 9, 10 eine Profilierung, beispielsweise in Umfangsrichtung verlaufende Rippen 13, und diese dienen der Fixierung der zweiten Anlageplatte 6 in unterschiedlichen Stellungen längs der Spannstifte 9, 10. Dazu weisen die Öffnungen 11, 12 radiale Einschnitte auf, durch die die an die Öffnungen 11, 12 anschließenden Randbereiche 14 in einzelne elastisch verbiegbare Zungen unterteilt werden, die beim Verschieben der zweiten Anlageplatte 6 in Richtung auf die erste Anlageplatte 5 elastisch an den Rippen 13 vorbeigleiten und in einer Stellung verbleiben, die ein Zurückschieben unmöglich macht.
Der der ersten Anlageplatte 5 abgewandte Abschnitt der Spannstifte 9, 10 trägt keine derartigen Rippen 13, sondern ist glatt ausgebildet, und die freien Enden der beiden Spannstifte 9, 10 sind in Bohrungen 15, 16 einer die beiden Spannstifte 9, 10 miteinander verbindenden Brücke 17 eingeschoben, welche mit den freien Enden der Spannstifte 9, 10 abschließt. Die Brücke 17 ist durch ein Quetschwerkzeug im Bereich der Bohrungen 15, 16 so verformt, daß die Brücke 17 in axialer Richtung unverschiebbar auf den Spannstiften 9, 10 gehalten ist, durch diese Verquetschung werden auf der Außenseite der Brücke 17 Quetschlinien 18 erzeugt, die die Form von umlaufenden Nuten aufweisen (Figur 3).
Die Brücke 17 fixiert die beiden Spannstifte 9, 10 relativ zueinander im selben Abstand, in dem die Spannstifte 9, 10 an der ersten Anlageplatte 5 gehalten sind. Auf ihrer den beiden Anlageplatten 5, 6 zugewandten Seite weist die Brücke 17 eine im Querschnitt kreisbogenförmige Anlagefläche 19 auf, die genau zwischen den beiden Bohrungen 15 und 16 angeordnet ist, so daß der tiefste Punkt der Anlagefläche 19 genau in der Mitte zwischen den Bohrungen 15 und 16 liegt.
Diese Anlagefläche 19 bildet eine Abstützfläche für ein oberes Spannwerkzeug 20 einer in Figur 1 nur ganz schematisch dargestellten Spanneinrichtung 21, die mit einem unteren Spannwerkzeug 22 an der Oberseite der zweiten Anlageplatte 6 anlegbar ist, und zwar unmittelbar neben den Öffnungen 11, 12 und außerhalb der beiden Spannstifte 9, 10. Auf diese Weise können die Spannwerkzeuge 20 und 22 in der von den Spannstiften 9, 10 aufgespannten Ebene insgesamt an drei Punkten zur Anlage kommen, so daß beim Auseinanderbewegen der Spannwerkzeuge 20 und 22 an drei Punkten Kräfte in das Implantat 4 eingeleitet werden, nämlich eine nach oben gerichtete Gegenkraft K in der
Mitte zwischen den beiden Spannstiften 9, 10 an der Anlagefläche 19 der Brücke 17 und in Richtung auf die erste Anlageplatte gerichtete Spannkräfte L1 und L2 im Bereich zwischen den schmalen Kanten der zweiten Anlageplatte 6 und den Öffnungen 11, 12 an der Oberseite der zweiten Anlageplatte 6. Es ist dadurch möglich, die zweite Anlageplatte 6 verkantungsfrei längs der Spannstifte 9, 10 gegen die erste Anlageplatte 5 zu verschieben und dadurch die Knochenteile 1, 2 zwischen den beiden Anlageplatten 5, 6 einzuspannen.
Damit übernimmt die Brücke 17 nicht nur die Aufgabe, die Spannstifte 9, 10 gegeneinander zu stabilisieren und die freien Enden der Spannstifte 9, 10 abzudecken, sondern die Brücke 17 wirkt gleichzeitig als Anlageelement für das Spannwerkzeug 20 der Spanneinrichtung 21.
Nach dem Spannen der beiden Anlageplatten 5, 6 können die über die zweite Anlageplatte 6 vorstehenden Teile der Spannstifte 9, 10 entfernt werden, beispielsweise mit Hilfe einer Schneidvorrichtung. Durch die Brücke 17 ist dabei sichergestellt, daß die entfernten Teile als eine einteilige Baueinheit verbleiben, so daß die Gefahr herabgesetzt wird, daß beim Abschneiden Teile der Spannstifte 9, 10 im Operationsgebiet verbleiben.
Außerdem können die Spannstifte 9, 10 bei diesem Schneidvorgang an der Brücke 17 gehalten werden, gegebenenfalls auch mit Hilfe eines geeigneten Werkzeuges, das unmittelbar an der Brücke 17 angreift.
Claims
Implantat zur Festlegung von zwei miteinander zu verbindenden Knochenteilen (1, 2) mit einem ersten pJattenförmigeπ Anlageelement (5) zur Anlage an Innenflächen der Knochenteile (1, 2), mit mindestens zwei daran festgelegten, quer von diesem abstehenden Spannelementen (9, 10) und mit einem zweiten plattenförmigen Anlageelement (6) zur Anlage an Außenflächen der Knochenteile (1, 2), welches mittels der durch den Zwischenraum (3) zwischen den Knochenteilen (1, 2) hindurchgeführten Spannelemente (9, 10) gegen das erste Anlageelement spannbar ist, dadurch gekennzeichnet, daß die Spannelemente (9, 10) an ihrem dem ersten AnlageeJement (5) gegenüberliegenden Ende durch eine Brücke (17) miteinander verbunden sind.
2. Implantat nach Anspruch 1, dadurch gekennzeichnet, daß die Brücke
(17) mit den Spannelementen (9, 10) in Spannrichtung unverschieblich verbunden ist.
3. Implantat nach Anspruch 1 oder 2, dadurch gekennzeichnet, daß die Spannelemente (9, 10) Stifte sind.
4. Implantat nach einem der Ansprüche 2 oder 3, dadurch gekennzeichnet, daß die Brücke (17) mit den Spannelementen (9, 10) durch eine Verquetschung (18) unverschieblich verbunden ist.
5. Implantat nach Anspruch 4, dadurch gekennzeichnet, daß die Spannelemente (9, 10) in Bohrungen (15, 16) in der Brücke (17) eingesteckt sind.
6. Implantat nach einem der voranstehenden Ansprüche, dadurch gekennzeichnet, daß die Brücke (17) eine Anlagefläche für eine Spanneinrichtung (21) aufweist, welche mit zwei Spannwerkzeugen (20, 22) einerseits an der Anlagefläche (19) und andererseits an der Außenfläche des zweiten Anlageelementes (6) anlegbar ist und beim Auseinanderschieben der Spannwerkzeuge (20, 22) das zweite Anlageelement (6) gegen das erste Anlageelement (5) spannt.
7. Implantat nach Anspruch 5 oder 6, dadurch gekennzeichnet, daß die
Anlagefläche (19) und das an ihr anliegende Spannwerkzeug (20) derart geformt sind, daß sich das Spannwerkzeug (20) an der Brücke (17) in der Mitte zwischen zwei Spannelementen (9, 10) abstützt.
8. Implantat nach Anspruch 7, dadurch gekennzeichnet, daß das an dem zweiten Anlageelement (6) anlegbare Spannwerkzeug (22) an zwei Punkten an dem zweiten Anlageelement (6) anlegbar ist, die in der von zwei Spannelementen (9, 10) aufgespannten Ebene im Abstand zueinander und von der Mitte zwischen den zwei Spannelementen (9, 10) entfernt angeordnet sind.
9. Implantat nach einem der Ansprüche 7 oder 8, dadurch gekennzeichnet, daß die Anlagefläche (19) an der Brücke (17) kreisbogenförmig verläuft.
10. Implantat nach einem der voranstehenden Ansprüche, dadurch gekennzeichnet, daß die Anlageelemente (5, 6) quer von ihnen abstehende, jeweils zum anderen Anlageelement (6, 5) weisende Fixiervorsprünge (7, 8) tragen und daß die Fixiervorsprünge (7, 8) so geneigt sind, daß ihr Abstand von den Fixiervorsprüngen (7, 8) auf der gegenüberliegenden Seite des Anlageelementes (5, 6) von dem Anlageelement (5, 6) zum freien Ende der Fixiervorsprünge (7, 8) zunimmt.
