Abstract
Es wird ein Transversalträger-Tibiaplateau beschrieben, das aus zwei Gelenkflächen .(3), zwei Gelenkflächenträgern (1) sowie einem Transversalträger (4) besteht. Dieses TTTP ist so angeordnet, daß die Gelenkflächen (3) separat von vorn in das Kniegelenk eingebracht werden können. Diese Anordnung ermöglicht ein einfacheres operatives Vorgehen mit Erhalt der ventralen Kortikalis. Durch die Ausfuhrung mit separaten Gelenkflächenträgern (1) und Gelenkflächen (3) können zunächst die Gelenkflächenträger (1) auf dem Transversalträger (4) z.B. mit Hilfe von Trägerschrauben (2) fixiert werden. Als Alternative können die Gelenkflächenträger (1) bzw. die Gelenkflächeneinheiten (321) zunächst von schräg oben mit Erhalt der ventralen Kortikalis implantiert und anschließend von der Seite her durch einen entsprechend ausgebildeten Transversalträger (4) aufgenommen und abgestützt werden.

Beschreibung
Transversalträger-Tibiaplateau (TTTP) Die Erfindung betrifft ein Transversalträger-Tibiaplateau (TTTP), das aus zwei
Gelenkflächen, zwei Gelenkflächenträgern sowie einem Transversalträger besteht, so daß die Gelenkflächen separat von vorn in das Kniegelenk eingebracht werden können. In einer weiteren Ausfuhrungsform können die zwei Gelenkflächen mit den zwei Gelenkflächenträgern zwei Gelenkflächeneinheiten bilden, welche nach dem Einbringen von oben durch den Transversalträger von der Seite her aufgenommen und abgestützt werden.
Die Vorteile der Anordnung eines Tibiaplateaus nach dem Transversalträger-Prinzip sind in
WO0241809 aufgezeigt. Bekannt sind die erhöhten Anforderungen an den Operateur bei der Implantation einer
Knieprothese mit Erhalt beider Kreuzbänder durch die geringere intraoperative
Aufklappbarkeit des Kniegelenks bei Belassen des vorderen Kreuzbandes.
Während bei einem lateralen Zugang mit Tuberositasosteotomie die ventrale Kortikalis für das Einbringen der Gelenkflächenträger nicht weiter bearbeitet werden muss, wird das Operationsverfahren mit dem häufiger verwendeten medialen Zugang schwierig und aufwendig.
Die Aufgabe der Erfindung ist es, ein Transversalträger-Tibiaplateau zu schaffen, welches den Anforderungen an ein Implantat nach dem Stand der Technik in möglichst hohem Maße genügt. Zudem soll durch entsprechende Anordnung der Komponenten die Implantation vereinfacht werden.
Diese Aufgabe wird erfindungsgemäß durch ein Transversalträger-Tibiaplateau gelöst¬ weiches aus zwei Gelenkflächen, zwei Gelenkflächenträgern sowie einem Transversalträger besteht, so daß die Gelenkflächen separat von vorn in das Kniegelenk eingebracht werden können.
Der Grundgedanke besteht darin, tibiaseitig zunächst zwei einzelne Gelenkflächenträger auf einem eingebrachten Transversalträger auszurichten und zu fixieren. Das Einbringen der Gelenkflächenträger kann von oben nach Außenrotation der Tibia gegenüber dem Femur medial bzw. nach Innenrotation lateral erfolgen. Anschließend können die Gelenkflächen separat direkt von vorne in das Kniegelenk eingebracht und auf den Gelenkflächenträgern fixiert und abgestützt werden. Diese Vorgehensweise ermöglicht den Erhalt der kompletten ventralen Kortikalis, auch beim medialen Zugang. In einer weiteren Ausfuhrungsform können alternativ die Gelenkflächen mit den zwei Gelenkflächenträgern zwei Gelenkflächeneinheiten bilden, welche nach dem Einbringen von oben durch den Transversalträger von der Seite her aufgenommen und abgestützt werden. Dieses kann entweder mit Durchfuhren des Transversalträgers durch entsprechende Ausnehmungen der Gelenkflächeneinheiten geschehen, oder der Träger umschließt bei Ausbildung in entsprechender Form die unteren Enden der Gelenkflächeneinheiten. Eine korrekte dauerhafte Ausrichtung und Abstützung kann durch formschlüssige Ausbildung der Verbindung z.B. mit Hilfe von Führungen oder zusätzlich von ventral einzubringenden Schrauben oder Bolzen erreicht werden.
Nachfolgend wird die Erfindung an Ausfuhrungsbeispielen beschrieben.
Fig. 1 zeigt schematisch ein Transversalträger-Tibiaplateau gemäß einer Ausfuhrungsform nach der Erfindung. Fig. 2 (a, b) zeigt schematisch eine weitere Ausfuhrungsform eines Transversalträger-
Tibiaplateaus nach der Erfindung.
Fig. 3 zeigt schematisch einen Frontalschnitt gemäß einer Ausfuhrungsform nach der
Erfindung mit einer weiteren Ausführungsform der Gelenkflächen und Gelenkflächenträger.
Fig. 4 zeigt schematisch einen seitlichen Schnitt gemäß einer Ausfuhrungsform nach der Erfindung mit einer weiteren Ausfuhrungsform der Gelenkflächen und Gelenkflächenträger.
Die Figur 1 zeigt einen Transversalträger (4) auf welchem zwei Gelenkflächenträger (1) mit Hilfe von Trägerschrauben (2) fixiert werden.
Die Gelenkflächenträger (1) wiederum nehmen je eine Gelenkfläche (3) auf. Dazu sind die Ausnehmungen (31) unter den Gelenkflächen (3) so ausgebildet, daß sie von vorne über eine entsprechende Führung (11) der Gelenkflächenträger (1) eingebracht und fixiert werden können. Durch entsprechende Ausbildung dieser Verbindung mit Anschlag oder Einrastung kann eine mittige Position über dem Transversalträger gewährleistet werden. Nach dieser Ausführungsform sind die Gelenkflächenträger (1) konisch ausgeprägt, wobei das untere Ende (12) in einer Ausnehmung (42) des Transversalträgers (4) platz findet. Um eine korrekte Rotationsausrichtung der hier abgewinkelt zu den Gelenkflächen (3) ausgebildeten Gelenkflächenträger (1) zu erreichen, kann neben der Schraubenverbindung zwischen der Ausnehmung (42) und dem Trägerschraubengewinde (22) eine zusätzliche Nut oder Führung zwischen dem unteren Ende (12) der Gelenkflächenträger (1) und der Ausnehmung (42) vorhanden sein.
In dieser Ausfuhrungsform bleibt der Trägerschraubenkopf (21) innerhalb der Gelenkflächenträger (1). Er kann auch zusätzlich zur Führung der Knieprothesen-Gleitlager verwendet werden (Fig. 3). Hier wird als Bewegungseinschränkung der Gleitlager ein Anschlag (32) ausgebildet. Durch die Schraubenverbindung (22, 42), wird die Höhe (h) zwischen den Gelenkflächen (3) und dem Transversalträger (4) konstant festgelegt. Über weitere besondere Ausbildungen der Führungen (11, 31) kann ein gewisser Druck auf den Knochenstock erzeugt werden, was die Primärstabilität erhöht und die ossäre Integration fordert. Es sind aber auch einfachere Verbindungen im Sinne von formschlüssigen
Steckverbindungen zwischen Gelenkflächenträger (l) und Transversalträger (4) denkbar. Neben einer Schlüsselfläche (41) zum Einbringen und Ausrichten des Transversalträgers (4), kann dieser, entsprechend der anatomischen Form der Tibia, abgeschrägte Stirnflächen aufweisen.
Die Figur 2a zeigt zwei Gelenkflächeneinheiten (321), welche durch einen Transversalträger (4) abgestützt werden. Um das Einbringen des Transversalträgers (4) über die Schlüsselfläche (41) nach Implantation der Gelenkflächeneinheiten (321) zu erleichtern, können die beiden Ausnehmungen (322) mit den zugehörigen Lagerflächen (42) unterschiedlich dimensioniert werden.
Die Figur 2b zeigt eine Seitenansicht der Figur 2a in welcher die Ausnehmungen (322) der Gelenkflächeneinheiten (321) als runde Lager zu den Lagerflächen (42) eines zylindrischen Transversalträgers (4) dargestellt werden. Hier sind weitere formschlüssige Verbindungen, wie Gewinde, zusätzliche Bolzen, formschlüssiges Umgreifen eines z.B. u-förmig ausgeprägten Transversalträgers (4) etc. denkbar.
In der Figur 3 wird eine weitere Verbindung jzwischen jGßlenkfläche (3) -und Gelenkflächenträger (1) dargestellt. Mit Hilfe der Trägerschraube (2) werden die Gelenkflächen (3) fixiert, wodurch bei geeigneter Ausfuhrung ebenfalls die Höhe (h, Fig. 1) positiv für einen entsprechenden Druck auf den Knochen genutzt werden kann.
Der Trägerschraubenkopf (21) kann durch eine entsprechende Gleitlagerführung (211) auch gleichzeitig zur Führung oder Befestigung der Knieprothesen-Gleitlager dienen.
Die Figur 4 zeigt noch eine weitere Verbindung zwischen Gelenkfläche (3) und Gelenkflächenträger (1). Durch die unter einem Winkel (ß) ansteigende Ausbildung der formschlüssigen Verbindung (11, 31) kann die Höhe (h, Fig. 1) ebenfalls positiv beeinflusst werden.
Claims
1. Transversalträger-Tibiaplateau mit einem Transversalträger (4), zwei Gelenkflächenträgern (1), sowie zwei Gelenkflächen (3),
05 dadurch gekennzeichnet, daß die Gelenkflächen (3) separat von vorne auf den Gelenkflächenträgern (1), welche formschlüssig mit dem Transversalträger (4) verbunden sind, fixiert werden können. lθ
2. Transversalträger-Tibiaplateau nach Anspruch 1, dadurch gekennzeichnet,
15 daß die Gelenkflächenträger (1) mit Hilfe von Trägerschrauben (2) auf dem Transversalträger (4) befestigt werden. Bei abgewinkelten Gelerikflächenträgern (1) können zusätzliche Führungen die Rotation der Gelenkflächenträger (1) verhindern. Bei nicht rotationssymmetrischen Querschnitten bzw. senkrechten Gelenkflächenträgem (1) kann
20 darauf verzichtet werden.
-LTransversalträger-Tibiarilateau nach Anspruch 1 oder 2, 25 dadurch gekennzeichnet, daß die Gelenkflächen (3) ebenfalls mit Hilfe der Trägerschrauben (2) auf den Gelenkflächenträgern (1) fixiert werden. Die Trägerschraubenköpfe (21) können außerdem zur Führung oder Fixierung der Knieprothesen-Gleitlager Gleitruhrungen (211) aufweisen. 30

Transversalträger-Tibiaplateau nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, 35 daß die Verbindungen (11,31) zwischen Gelenkflächen (3) und Gelenkflächenträgern (1) unter einem Winkel (ß) ausgebildet werden. Zudem können Anschläge oder Einrastmechanismen die Position etwa mittig über dem Transversalträger gewährleisten.

5. Transversalträger-Tibiaplateau nach einem der Ansprüche 1 bis 4, dadurch gekennzeichnet,
45 daß die Schraubenverbindungen (22, 42) zwischen Trägerschrauben (2) und
Transversalträger (4) direkt durch die Gelenkflächenträger (1) übernommen werden. Es können alternativ auch Steckverbindungen zwischen Gelenkflächenträger (1) und Transversalträger (4) ausgebildet werden. Bei nicht senkrechter Ausrichtung der Gelenkflächenträger (1) zu den Gelenkflächen (3) sollten diese formschlüssigen
50 Verbindungen rotationsstabil sein.

Transversalträger-Tibiaplateau nach einem der Ansprüche 1 bis 5, dadurch gekennzeichnet, daß die Gelenkflächenträger (1) Ausnehmungen (322) oder Führungen am unteren Ende (12) aufweisen, so daß der Transversalträger von der Seite her eingebracht bzw. zur Fixierung darüber geschoben werden kann. Bei dieser Anordnung können die Gelenkflächenträger (1) mit den Gelenkflächen (3) eine Gelenkflächeneinheit (321) bilden. Eine zusätzliche Arretierung bei einfachen Verbindungen mit Hilfe von Schrauben oder Bolzen kann ebenfalls vorgenommen werden.
7. Transversalträger-Tibiaplateau nach einem der Ansprüche 1 bis 6, dadurch gekennzeichnet, daß die Verbindungen (322/42) unterschiedlich dimensioniert sind, um ein einfacheres Einbringen des Transversalträgers über entsprechendes Hilfswerkzeug an der Schlüsselfläche/Flansch (41) zu ermöglichen.
