Abstract
Dämpfungselement (1) zur dynamischen Stabilisierung von zwei Knochen, insbesondere von zwei benachbarten Wirbelkörpern mit einer Zentralachse (11) und mit A) einem ersten die Zentralachse (11) schneidenden Ende (12), einem zweiten die Zentralachse (11) schneidenden Ende (13) und einem dazwischen angeordneten, zur Zentralachse (11) koaxialen Federelement (10), wobei B) mindestens an einem Ende (12;13) des Federelementes (10) ein mittels Spannmitteln (40) lösbar blockierbares, zur Zentralachse (11) konzentrisches Kugelgelenk (20) integriert ist.
Description
Dämpfungselement
Die Erfindung bezieht sich auf ein Dämpfungselement gemäss dem Oberbegriff des Patentanspruchs 1.
Ein gattungsgemässes Dämpfungselement zur dynamischen Stabilisierung zweier benachbarter Wirbelkörper ist aus der EP-A 0 516 567 NAVAS bekannt. Dieses bekannte Dämpfungselement umfasst einen koaxialen Dämpfungskörper mit je einem axial aussenstehenden sphärisch konvexen Verbindungsteil, mittels welcher das Dämpfungselement an zwei Pedikelschrauben befestigbar ist. Durch die kugelgelenkartige Verbindung zwischen den zwei Verbindungsteilen und den Köpfen der Pedikelschrauben lässt sich das Dämpfungselement mit variablem Winkel zwischen den Längsachsen der Pedikelschrauben und der Zentralachse des Dämpfungselementes mit den Pedikelschrauben verbinden. Nachteilig an diesem bekannten Dämpfungselement ist, dass durch die Geometrie des Dämpfungselementes der Abstand zwischen den Pedikelschrauben vorgegeben ist.
Hier will die Erfindung Abhilfe schaffen. Der Erfindung liegt die Aufgabe zugrunde, ein Dämpfungselement zu schaffen, welches mindestens an einem seiner Enden polyaxial schwenkbar und axial teleskopierbar mit einem Längsträger verbindbar ist.
Die Erfindung löst die gestellte Aufgabe mit einem Dämpfungselement, welches die Merkmale des Anspruchs 1 aufweist.
Die durch die Erfindung erreichten Vorteile sind im wesentlichen darin zu sehen, dass dank des Dämpfungselementes mittels mindestens einer an einem Ende des Dämpfungselementes integrierten Kugelgelenkverbindung:
- das Dämpfungselement bei deblockiertem Zustand des Kugelgelenkes polyaxial schwenkbar mit einem stabförmigen Längsträger einer Vorrichtung zur Stabilisierung von Wirbelkörpern verbindbar ist. Daher müssen bei der Implantation eines Längsträgers innerhalb einer Wirbelsäulenstabilisierungsvorrichtung keine Längsträger gebogen werden; und
- das Dämpfungselement axial teleskopierbar mit einem stabförmigen Längsträger einer Vorrichtung zur Stabilisierung von Wirbelkörpern verbindbar ist.
Weitere vorteilhafte Ausgestaltungen der Erfindung sind in den abhängigen Ansprüchen gekennzeichnet.
In einer bevorzugten Ausführungsform umfasst das Kugelgelenk einen sphärisch konvexen, radial elastisch komprimierbaren Klemnnkörper mit einer diametralen, eine Bohrungsachse aufweisenden Zentralbohrung, so dass ein in die Zentralbohrung eingeführter stabförmiger Längsträger bei komprimiertem Klemmkörper in der Zentralbohrung relativ zum Klemmkörper blockiert ist.
Das Kugelgelenk gestattet vorzugsweise eine Rotation des Klemmkörpers um einen zwischen der Bohrungsachse der Zentralbohrung im Klemmkörper und der Zentralachse des Federelementes gemessenen Winkel α innerhalb eines Bereiches von 0° bis ± 25°. Damit ist der Vorteil erreichbar, dass ein in die Zentralbohrung des Klemmkörpers eingeführter stabförmiger Längsträger relativ zum Federelement schwenkbar ist und daher der stabförmige Längsträger nicht angebogen werden muss.
Vorzugsweise umfasst das Kugelgelenk zwei axial getrennte, den Klemmkörper mindestens teilweise aufnehmende Lagerschalen, so dass der Klemmkörper beim Zusammenpressen der Lagerschalen gleichmässig komprimiert wird und somit das Kugelgelenk rigid blockierbar ist.
In einer anderen Ausführungsform sind die Lagerschalen sind mittels der Spannmittel axial gegen den Klemmkörper pressbar sind, wobei das Federelement vorzugsweise an seinem ersten Ende einen koaxialen, ein Gewinde aufweisenden Zapfen aufweist und dass die erste Lagerschale axial endständig in den Zapfen integriert ist, derart, dass sich die Lagerschale gegen das zweite Ende des Federelementes verjüngt. Die Spannmittel sind vorzugsweise als Mutter ausgebildet, welche über das Gewinde des Zapfens schraubbar ist. Vorzugsweise ist die zweite Lagerschale konzentrisch in der Bohrung der Mutter integriert.
In einer weiteren Ausführungsform umfasst die Mutter eine koaxiale Bohrung mit mindestens zwei axial benachbarte Längsabschnitten, wobei der gegen das Federelement gerichtete, aussenstehende Längsabschnitt ein zum Gewinde des Zapfens komplementäres Innengewinde aufweist und im daneben angeordneten Längsabschnitt die zweite Lagerschale integriert ist, derart, dass sie sich gegen den aussenstehenden Längsabschnitt erweitert.
Zur Durchführung eines stabförmigen Längsträgers durch die Spannmittel sind diese koaxial durchbohrt.
In wiederum einer weiteren Ausführungsform weist der Klemmkörper einen zur Bohrungsachse der Zentralbohrung parallelen Schlitz auf, welcher die Wand des Klemmkörpers von dessen Aussenwand bis zur Zentralbohrung durchdringt.
In einer anderen Ausführungsform umfasst das Dämpfungselement einen an einem seiner Enden koaxial stabförmiges Verbindungsteil, welches mit einem weiteren Teil innerhalb einer osteosynthetischen Stabilisierungsvorrichtung verbindbar ist.
In wiederum einer anderen Ausführungsform umfasst das Dämpfungselement zusätzlich einen in die Zentralbohrung des Klemmkörpers einführbaren und im Klemmkörper lösbar fixierbaren, stabförmigen Längsträger.
Die Erfindung und Weiterbildungen der Erfindung werden im folgenden anhand der teilweise schematischen Darstellungen mehrerer Ausführungsbeispiele noch näher erläutert.
Es zeigen:
Fig. 1 einen Längsschnitt durch eine Ausführungsform des erfindungsgemässen Dämpfungselementes; und
Fig. 2 eine Vergrösserung des in Fig. 1 durch den Kreis A markierten Segmentes.
In den Fig. 1 und 2 ist eine Ausführungsform dargestellt, welche ein hohlzylindrisches Dämpfungselement 1 mit einer Zentralachse 11 und einem lösbar blockierbaren Kugelgelenk 20 zur polyaxial schwenkbaren Verbindung des Dämpfungselementes 1 mit einem eine Längsachse 4 aufweisenden, stabförmigen Längsträger 3 umfasst. Das Dämpfungselement 1 umfasst neben dem Kugelgelenk 20 ein Federelement 10, welches in der hier dargestellten Ausführungsform aus einer metallischen Schraubenfeder und einem in den Spalt 30 zwischen den Federwindungen eindringenden und den Hohlraum 15 diametral verengenden Kunststoffteil 31 besteht. Das Kugelgelenk 20 ist am ersten Ende 12 des Federelementes 10 angeordnet, während am zweiten, axial entgegengesetzten Ende 13 des Federelementes 10 ein koaxiales stabförmiges Verbindungsteil 16 angeordnet ist, welches zur Verbindung mit einem weiteren Teil (nicht gezeichnet) einer Wirbelsäulenstabilisationsvorrichtung geeignet ist. Das Kugelgelenk 20 besteht hier aus einem sphärisch konvexen Klemmkörper 21 mit einer eine Bohrungsachse 27 aufweisenden Zentralbohrung 22 und aus zwei zum Klemmkörper 21 komplementär konkaven Lagerschalen 23;24. Die erste Lagerschale 23 ist zur Zentralachse 11 konzentrisch in den Gewindezapfen 39 am ersten Ende 12 des Federelementes 10 integriert, derart, dass sie sich gegen den Hohlraum 15 im Dämpfungselement 1 verjüngt. Der Gewindezapfen 39 wird von einer zur Zentralachse 11 koaxialen, in den Hohlraum 15 mündenden Bohrung 14 durchdrungen, so dass die Bohrung 14 zur Aufnahme eines durch die Zentralbohrung 22 im Klemmkörper 21 durchgeführten, stabförmigen Längsträgers 3 geeignet ist. Die zweite Lagerschale 24 ist in eine Mutter 25 integriert, welche über das Gewinde 26 am Gewindezapfen 39 schraubbar ist. Am zweiten Ende 13 des Federelementes 10 ist der Hohlraum 15 geschlossen. Zur Verbindung des zweiten Endes 13 des Federelementes 10 mit einem weiteren Teil, beispielsweise dem Kopf einer Pedikelschraube oder eines Pedikelhakens (nicht gezeichnet) ist am zweiten Ende 13 des Federelementes 10 ein zur Zentralachse 11 koaxiales, stabförmiges Verbindungsteil 16 angeordnet.
Wie in Fig. 2 gezeigt ist der Klemmkörper 21 mit einem zur Bohrungsachse 27 parallelen Schlitzen 28 versehen, welcher den Klemmkörper 21 von der Aussenwand 29 des Klemmkörpers 21 bis zur Zentralbohrung 22 durchdringt. Beim Anziehen der Mutter 25 wird somit der zwischen den Lagerschalen 23;24 angeordnete Klemmkörper 21 zwischen den Lagerschalen 23;24 festgeklemmt und gleichzeitig radial zur
Bohrungsachse 27 der Zentralbohrung 22 komprimiert, so dass der in die Zentralbohrung 22 eingeführte Längsträger 3 blockiert wird.
Die Mutter 25 wird von einer zur Zentralachse 11 koaxialen Bohrung 32 durchdrungen, welche mehrere axial aneinandergrenzende Längsabschnitte 34;35;36 mit verschiedener Geometrie aufweist. Der dem ersten Ende 12 des Dämpfungselementes 1 benachbarte Längsabschnitt 34 ist mit einem zum Gewinde 26 am ersten Ende 12 des Federelementes 10 komplementären Innengewinde 33 versehen, während der mittlere Längsabschnitt 35 die zweite, ebenfalls durchbohrte Lagerschale 24 umfasst und der aussenliegende Längsabschnitt 36 konisch ausgebildet ist. Dabei ist die zweite Lagerschale 24 so angeordnet, dass sie sich gegen den aussenliegenden Längenabschnitt 36 verjüngt. Der Konus 38 in dem aussenliegenden Längenabschnitt 36 erweitert sich gegen die aussenstehende Stirnfläche 37 der Mutter 25, so dass ein stabförmiger Längsträger 3 im Kugelgelenk 20 schwenkbar aufnehmbar ist.
Claims
1. Dämpfungselement (1) zur dynamischen Stabilisierung von zwei Knochen, insbesondere von zwei benachbarten Wirbelkörpern mit einer Zentralachse (11 ) und mit
A) einem ersten die Zentralachse (11) schneidenden Ende (12), einem zweiten die Zentralachse (11) schneidenden Ende (13) und einem dazwischen angeordneten, zur Zentralachse (11) koaxialen Federelement (10), dadurch gekennzeichnet, dass
B) mindestens an einem Ende (12;13) des Federelementes (10) ein mittels Spannmitteln (40) lösbar blockierbares, zur Zentralachse (11) konzentrisches Kugelgelenk (20) integriert ist.
2. Dämpfungselement (1) nach Anspruch 1 , dadurch gekennzeichnet, dass das Kugelgelenk (20) einen sphärisch konvexen Klemmkörper (21) mit einer diametralen, eine Bohrungsachse (27) aufweisenden Zentralbohrung (22) umfasst.
3. Dämpfungselement (1) nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass das Kugelgelenk (20) eine Rotation des Klemmkörpers (21) um einen zwischen der Bohrungsachse (27) und der Zentralachse (11) gemessenen Winkel α innerhalb eines Bereiches von 0° bis + 25° gestattet.
4. Dämpfungselement (1) nach Anspruch 2 oder 3, dadurch gekennzeichnet, dass das Kugelgelenk (20) zwei axial getrennte, den Klemmkörper (21 ) mindestens teilweise aufnehmende Lagerschalen (23;24) umfasst.
5. Dämpfungselement (1) nach Anspruch 4, dadurch gekennzeichnet, dass die Lagerschalen (23;24) mittels der Spannmittel (40) axial gegen den Klemmkörper (21) pressbar sind.
6. Dämpfungselement (1) nach Anspruch 4 oder 5, dadurch gekennzeichnet, dass das Federelement (10) an seinem ersten Ende (12) einen koaxialen, ein Gewinde (26) aufweisenden Zapfen (39) aufweist und dass die erste Lagerschale (23) axial endständig in den Zapfen (39) integriert ist, derart, dass sich die Lagerschale (23) gegen das zweite Ende (13) des Federelementes (10) verjüngt.
7. Dämpfungselement (1) nach Anspruch 6, dadurch gekennzeichnet, dass das Spannmittel (40) eine Mutter (25) ist, welche über das Gewinde (26) des Zapfens (39) schraubbar ist und dass die zweite Lagerschale (24) konzentrisch mit der Mutter (25) verbunden ist.

Dämpfungselement (1) nach Anspruch 7, dadurch gekennzeichnet, dass die Mutter (25) eine koaxiale Bohrung (32) mit mindestens zwei axial benachbarte Längsabschnitten (34;35) umfasst, wobei der gegen das Federelement (10) gerichtete, aussenstehende Längsabschnitt (34) ein zum Gewinde (26) komplementäres Innengewinde (33) aufweist und im daneben angeordneten Längsabschnitt (35) die zweite Lagerschale (24) integriert ist, derart, dass sie sich gegen den aussenstehenden Längsabschnitt (34) erweitert.
9. Dämpfungselement (1) nach Anspruch 1 bis 8, dadurch gekennzeichnet, dass die Spannmittel (40) koaxial durchbohrt sind.
10. Dämpfungselement (1) nach einem der Ansprüche 2 bis 9, dadurch gekennzeichnet, dass der Klemmkörper (21) eine Aussenwand (29) aufweist und dass von dieser Aussenwand (29) zur Bohrungsachse (27) paralleler Schlitz (28) die Wand des Klemmkörpers (21) durchdringt.
11 . Dämpfungselement (1) nach einem der Ansprüche 1 bis 10, dadurch gekennzeichnet, dass es an seinem zweiten Ende (13) ein koaxial endständig angrenzendes, stabförmiges Verbindungsteil (16) umfasst, welches mit einem weiteren Teil innerhalb einer osteosynthetischen Stabilisierungsvorrichtung verbindbar ist.
12. Dämpfungselement (1) nach einem der Ansprüche 2 bis 11 , dadurch gekennzeichnet, dass es zusätzlich einen in die Zentralbohrung (22) einführbaren und im Klemmkörper (21) lösbar fixierbaren, stabförmigen Längsträger (3) umfasst.
