Dispositif d'aide à la manipulation de cornées animales, notamment humaines, visant à optimiser la conservation des cellules cornéennes. Le dispositif est introduit dans les différents flacons et milieux utilisés avant une greffe de cornée. L'invention concerne un dispositif qui minimise les surfaces de contact pendant la conservation de la cornée et évite sa manipulation directe afin de conserver une qualité cellulaire optimale avant la greffe. Il est constitué d'un support (1) sur lequel est déposé la cornée (11), ledit support est maintenu à une tirette (9) par des clips (4).
Description
Dispositif d'aide à la manipulation de cornée animale, notamment humaine
La présente invention concerne un dispositif d'aide à la manipulation des cornées animales, notamment humaines, et vise à optimiser la conservation des cellules cornéennes jusqu'à la greffe. Le dispositif selon l'invention est introduit dans les différents flacons contenant les milieux liquides utilisés avant une greffe de cornée.
Dans le cas des humains, la greffe d'une cornée sur un patient nécessite tout d'abord son prélèvement sur une personne décédée. La cornée prélevée est ensuite introduite par le chirurgien dans un premier flacon contenant un milieu dit de prélèvement. Elle est ensuite contrôlée au microscope puis introduite dans un second flacon contenant un liquide de conservation. Enfin, quelques heures avant la greffe, un troisième flacon, contenant un milieu déturgescent, c'est-à-dire réduisant le gonflement de la cornée, est utilisé.
Actuellement, dans le flacon de prélèvement, la cornée est déposée librement dans la solution. Dans le flacon de conservation et de déturgescence, la technique est de maintenir la cornée en suspension au bout d'un fil que l'on fixe sur la périphérie de la cornée en la perçant à l'aide d'une aiguille. Cette technique actuelle présente de nombreux inconvénients :
- la manipulation est peu pratique,
- la manipulation de la cornée doit être réalisée avec des instruments stérilisés ou à usage unique,
- le risque de contamination et/ou d'infection est important,
- les contacts multiples contre les parois des flacons peuvent altérer les cellules cornéennes,
- la stagnation de la cornée au fond du flacon de prélèvement forme un risque d'adhérence de la cornée sur les parois du flacon, et implique un contact avec les cellules dégénérées qui stagnent au fond d'un flacon.
Le document EP-0262 766 divulgue un dispositif de support de cornée formé sur le bouchon d'un flacon. Ce dispositif impose l'utilisation de flacons spécifiques modifiés, les flacons standards n'étant pas adaptables à ce système.
La présente invention a pour but de fournir un dispositif qui ne reproduit pas les inconvénients de l'art antérieur.
La présente invention a notamment pour but de fournir un tel dispositif qui soit simple et peu coûteux à fabriquer, à assembler et à utiliser, tout en permettant l'utilisation des flacons standards non modifiés.
La présente invention a donc pour objet un dispositif d'aide à la manipulation de cornée humaine visant à optimiser la conservation des cellules cornéennes dans différents flacons et milieux utilisés avant la greffe de la cornée, caractérisé en ce qu'il comporte un support sur lequel est déposé la cornée et une tirette pour mamtenir et extraire ledit support des différents flacons et milieux.
Avantageusement, ledit dispositif est entièrement indépendant des flacons et/ou de leurs bouchons.
Avantageusement, la cornée, sans être comprimée, reste contenue dans un espace réduit au minimum.
Avantageusement, les surfaces en contact avec la cornée sont minimisées. Avantageusement, le support est maintenu par la tirette de façon à ce que la cornée soit positionnée au-dessus du fond du flacon, où stagnent des cellules dégénérées, par l'intermédiaire d'éléments de supports, tels que des pieds.
Avantageusement, le support présente au moins un orifice qui laisse passer la lumière directement sur la cornée lors de l'analyse microscopique.
D'autres caractéristiques et avantages de la présente invention apparaîtront plus clairement dans la description détaillée suivante d'un mode de réalisation particulier de l'invention, faite en référence aux dessins joints, donnés à titre d'exemples non limitatifs, et sur lesquels :
- la figure la représente une vue de dessus du support de cornée selon un mode de réalisation de la présente invention, - la figure 1b représente une vue en coupe selon la ligne A-A de la figure la,
- la figure le représente une vue en coupe selon la ligne S-S de la figure la,
- la figure 2a représente une vue de dessus de la tirette correspondante,
- la figure 2b représente une vue en coupe selon la ligne B-B de la ligne 2a,
- la figure 2c représente une vue en coupe selon la ligne X-X de la figure 2a, et
- la figure 3 représente une vue en coupe du dispositif assemblé.
En référence à ces dessins et selon un mode de réalisation avantageux de la présente invention, le dispositif comporte un support 1, ayant environ la forme d'une roue avec des rayons 2, et percée en son centre.
Les rayons 2 sont inclinés vers le bas (dans la position des figures lb et 3) de façon à centrer la cornée 11, et ils présentent de préférence un profil arrondi afin de minimiser la surface de contact. La hauteur du support 1 est de préférence Umitée par celle des boites stériles transparentes dans lesquelles il est introduit avec la cornée lors de l'analyse microscopique, ceci afin de ne pas toucher la cornée. Le trou central 3 du support 1 laisse passer la lumière du microscope et autorise une visée directe sur la cornée. Une tirette est prévue pour manipuler le support 1. Cette tirette 9 peut être fixée de manière amovible audit support 1. Par exemple, deux clips de fixation 4 peuvent être utilisés pour maintenir le support 1 sur la tirette 9. D'autres moyens de fixation sont aussi envisageables. La tirette comporte avantageusement des pieds 5 qui viennent se glisser dans des encoches 6 du support 1, de façon à ne pas gêner la préhension du support. Les pieds sont avantageusement suffisamment longs pour mamtenir le support 1 au-dessus du fond du flacon, où stagnent des cellules dégénérées.
La tirette 9 du dispositif de l'invention peut aussi présenter une roue 7 avec des rayons 8 à profil arrondi pour minimiser la surface de contact. La tirette peut comporter une tige 10 en forme de T, de longueur et de largeur adaptées
aux flacons standards pour permettre de centrer le dispositif au niveau du col dudit flacon.
Le dispositif de l'invention est donc avantageusement complètement indépendant des flacons et/ou de leurs bouchons, ce qui permet l'utilisation des flacons standards existant et utilisés habituellement sans le dispositif de l'invention. Ceci est très rentable, puisque l'utilisation de flacons spécifiques modifiés est évitée.
Le dispositif est de préférence constitué de matière plastique et peut être fabriqué en une seule opération de moulage par injection sur presse. Un dispositif métallique, ou d'autres matériaux, tels que les céramiques, sont aussi envisageables.
Le dispositif est de préférence stérilisé et à usage unique.
Le dispositif selon l'invention est particulièrement adapté à la manipulation des cornées humaines avant une greffe. Ainsi, le dispositif selon l'invention facilite la manipulation, réduit les surfaces de contacts avec la cornée et élimine les contacts tactiles. Il permet notamment :
- de maintenir le support à une hauteur déterminée,
- de centrer le dispositif dans le flacon, - d'introduire et d'extraire sainement le support du flacon, sans risquer d'abîmer la cornée.
Bien que la présente invention a été décrite en référence à un mode de réalisation particulier de celle-ci, il est entendu qu'elle ne se limite pas à cette mise en œuvre. Ainsi, la forme en roue à rayons pourrait être modifiée. D'autres modifications sont aussi envisageables par l'homme du métier sans sortir du cadre de la présente invention défini par les revendications annexées.
Claims
1.- Dispositif d'aide à la manipulation de cornée humaine visant à optimiser la conservation des cellules cornéennes dans différents flacons et milieux utilisés avant la greffe de la cornée, caractérisé en ce qu'il comporte un support (1) sur lequel est déposé la cornée (11) et une tirette (9) pour mamtenir et extraire ledit support des différents flacons et milieux.
2.- Dispositif selon la revendication 1, dans lequel ledit dispositif est entièrement indépendant des flacons et/ou de leurs bouchons.
3.- Dispositif selon la revendication 1 ou 2, caractérisé en ce que la cornée (11), sans être comprimée, reste contenue dans un espace réduit au minimum.
4.- Dispositif selon l'une quelconque des revendications précédentes, caractérisé en ce que les surfaces en contact avec la cornée (11) sont minimisées.
5.- Dispositif selon l'une quelconque des revendications précédentes, caractérisé en ce que le support (1) est maintenu par la tirette (9) de façon à ce que la cornée (11) soit positionnée au-dessus du fond du flacon, où stagnent des cellules dégénérées, par intermédiaire d'éléments de supports, tels que des pieds (5).
6.- Dispositif selon l'une quelconque des revendications précédentes, caractérisé en ce que le support (1) présente au moins un orifice (3) qui laisse passer la lumière directement sur la cornée (11) lors de l'analyse microscopique.
