Abstract

Automatisches Biopsiegerät, bestehend aus einem Gehäuse mit einer Biopsienadel, einer Kanüle, welche auf der Biopsienadel gelagert ist sowie je einen manuell vorspannbaren und manuell auslösbaren Federspeicher im Gehäuse für die Vorschubbewegung der Biopsienaddel und der Kanüle. Aufgabe der Erfindung ist es, das Biopsiegerät unter Verwendung neuer Federspeicherkonzepte so weiter zu entwickeln, dass es bei gleichzeitiger Verbesserung der kinematischen Eigenschaften für einen bildgestützten Einsatz in einem Magnetresonanztomographen (MRT) geeignet ist. Die Aufgabe wird dadurch gelöst, dass Biopsienadel und Kanüle bei vorgespannten Federspeichern zentrisch zur Symmetrieachse über eine lösbare Rastverbindung gehalten werden, aus einem für einen Einsatz im MRT geeigneten nicht magnetisierbaren und nicht elektrisch leitfähigen Werkstoff, vorzugsweise aus einem faserverstärkten Kunststoff bestehen, sowie die beiden Federspeicher sich aus für einen Einsatz im MRT geeigneten nich magnetisierbaren und nicht elektrisch leitfähigen Komponenten zusammensetzen.

Description


  



  Biopsiegerät für den MRT-Einsatz Die vorliegende Erfindung betrifft ein automatisches Biopsie  rät    für die Entnahme von weichen Gewebeproben aus einem lebenden Organismus gemäss des Oberbegriffs des ersten Anspruchs.



  In der Human-und Tiermedizin ist eine gezielte Entnahme weicher Gewebeproben aus dem lebenden Organismus, genannt Biopsie, bei. zahlreichen Krankheitsbildern ein fester Bestandteil in der Diagnose. Letztendlich gibt erst das Untersuchungsergebnis an einer entnommenen Gewebeprobe die endgültige Sicherheit bei der Diagnosestellung. Während eine Biopsie an von aussen zugänglichen   Körperbereichten    meist visuell mit einem   Operationsbesteck    erfolgt, kommen bei einer Biopsie im Körperinnern endoskopische Verfahren, beispielsweise mit Hilfe einer Biopsienadel, zum Einsatz. Dabei wird die Biopsienadel, welche nahe der Nadelspitze eine seitliche Aussparung aufweist, in das zu untersuchende Gewebe eingestochen, wobei sich das Gewebe in die Aussparung eindrückt.

   Im Anschluss daran wird eine Kanüle über die Biopsienadel zur Nadelspitze nachgeschoben, wobei das Gewebe in der Aussparung für eine weitere Untersuchung abgetrennt wird.



  Bei   Biopsiegeräten    unterscheidet man drei verschiedene Typen.



  Bei manuellen   Biopsiegeräten    erfolgt sowohl der Einstich der Biopsienadel als auch das Nachschieben der Kanüle manuell, während bei halbautomatischen   Biopsiegeräten    nur der Einstich der Biopsienadel zwar noch manuell erfolgt, während hingegen die Kanüle mit dem Auslösen eines Federspeicher durch die freigesetzte Federkraft mit hoher Geschwindigkeit über die Aussparung der Biopsienadel geschoben wird. Automatische Biopsiegeräte weisen dagegen meist zwei Federspeicher jeweils für das Vorschnellen der Biopsienadel in die vorgegebene Zielregion mit unmittelbar anschliessenden blitzartigem Nachschieben der Kanüle auf.



  In W096/39941 wird ein automatisches   Biopsiegerät    beschrieben, welches aus einem Gehäuse mit einer in diesem gelagerten   Biop-    sienadel und einer auf dieser   übergeschobenen    und axial zu dieser verschiebbaren Kanüle sowie je einem Federspeicher mit je einer Metallfeder für eine federkraftgetriebene Vorschubbewegung von Biopsienadel und Kanüle bestehen. Für die automatische   Betä-    tigung werden zunächst beide Federspeicher manuell angespannt, wobei die beiden Führungsschlitten, an die die Biopsienadel und die Kanüle befestigt sind, in Endposition mittels eines einfachen Einrastmechanismusses exzentrisch zu der Kanüle und der Biopsienadel einrasten.

   Nach Ausrichtung des   Biopsiegerätes    wird die Einrastverbindung für die Biopsienadel ausgelöst, wobei diese mit der Federkraft in den Zielbereich schnellt. Kurz vor Erreichen des Zielbereiches löst die Vorschubbewegung der Biopsienadel im Gehäuse die Einrastverbindung für die Kanüle und gibt damit die federkraftgesteuert Nachschubbewegung für die Kanüle frei. Das   Biopsiegerät    weist jedoch mehrere metallische Komponenten auf, welche beispielsweise im Falle der Verwendung von Federn aus Federstahl im MRT Bildstörungen (Artefakte) verursachen und damit für den MRT-Einsatz nicht geeignet erscheinen. Die Biopsienadel mit der Kanüle ist als austauschbare Einheit in dem wiederverwendbaren Gehäuse mit den beiden Federspeichern eingesetzt.



  Werden jedoch beispielsweise bei MRT-Untersuchungen Karzinome erkannt und sollen mit dem Biopsiegerät   bildgestützt    im MRT eine Gewebeproben entnommen werden, so muss das Biopsiegerät und der Manipulator so beschaffen sein, dass keine Artefakte entstehen.



  Aufgabe der Erfindung ist es, das zuvor beschriebene Biopsiegerät auch unter Verwendung neuer Federspeicherkonzepte so weiter zu entwickeln, dass es für einen   bildgestützten    Einsatz in einem MRT, in dem starke Magnetfelder ( > 1 Tesla) vorhanden sind, geeignet ist. Ferner soll die Kinematik des   Biopsiegerätes    zuguns ten eines   störungsfreieren    Auswechselns und Betriebs austauschbarer Biopsienadeln und Kanülen prinzipiell verbessert werden. 



  Die Aufgabe wird mit der in Anspruch 1 beschriebene Vorrichtung gelöst. Die weiteren Ansprüche geben bevorzugte Ausgestaltungen der Vorrichtung an.



  Das erfindungsgemässe   Biopsiegerät    wird im folgenden anhand von Zeichnungen dreier Ausführungsbeispiele erläutert. Es zeigen Fig.   1    einen Schnitt durch die erste Ausführungsform mit zwei   Fluidfederspeicher,    Fig. 2   a    bis c den Spannvorgang des Biopsiegeräts am Beispiel der ersten Ausführungsform, Fig. 3 a bis c den Schussvorgang des   Biopsiegeräts    am Beispiel der ersten Ausführungsform, Fig. 4 einen Schnitt durch die zweite Ausführungsform mit zwei   Spiralfederspeicher,    Fig. 5 einen Schnitt durch die dritte Ausführungsform mit zwei Federspeichern mit Gummizugfedern Fig. 6 a bis e einen Aufklappmechanismus für die Bohrung 21 am Beispiel der dritten Ausführungsform gem.

  In Fig.   1    ist das Biopsiegerät der ersten Ausführungsform in Ruhelage, d. h. mit entspannten Federspeichern, dargestellt. Im Gehäuse   1    befinden sich zwei Federspeicher, bestehend aus zwei Bohrungen 2 und 3 in denen zentrisch je ein Kolben 4 und 5 mit Dichtungen 6 und 7 angeordnet sind. Hierbei bietet es sich an, die beiden Bohrungen 2 und 3 als Sacklöcher zu gestalten. Die Kolben 4 und 5 werden durch das von diesen eingeschlossene vorgespannte Druckmedium 9, welches aus mindestens einem kompri mierbaren Fluid, vorzugsweise Gas, besteht, gegen die Anschläge 10 und 11 im Gehäuse 1 gedrückt. Die Bohrungen 12 und 13 in den Kolben 4 und 5 sind mit den Bohrungen 2 und 3 des Gehäuses   1    verbunden und bilden gemeinsam einen vergrösserten Volumenspeicher für das Druckmedium 9.

   Durch die Vergrösserung des Volumenspeichers reduzieren sich die relativen Volumenänderungen und damit die Druckgradienten des Druckmediums während eines Verschiebens der Kolben 4 und 5 in vorteilhafter Weise. Die Kolben 4 und 5 durchdringen das Gehäuse 1 im vorderen Bereich über den Anschlag 10 und 11 hinaus und sind mit je einer Kappe 14 und 15 fest verbunden und mit dieser bewegbar.



  Der Kolben 5 greift über einen Zapfen 16 in eine umlaufende Nut des um die Symmetrielinie 8 rotationssymmetrischen   Kupplungsstü-    ckes 17, welches als Mitnehmer für die Biopsienadel 18 dient, fest mit dieser verbunden ist sowie   in.    der Bohrung 21 geführt wird, ein. An der zur Biopsienadel 18 abgewandten Seite des Kupplungsstücks 17 sind mindestens zwei elastische Stege 19 angeordnet, welche an deren Ende als Rasthaken ausgebildet sind.



  An der der Biopsienadel zugeordneten Seite weist das Kupplungsstücks 17 dagegen eine nach innen gerichtete, zur Biopsienadel hin offenen Kegelfläche 20 auf.



     In ganz ähnlicher    Weise greift ein Zapfen 22, welcher mit Kolben 4 verbunden ist, in eine umlaufende Nut des um die Symmetrielinie 8 rotationssymmetrischen Kupplungsstücks 23, welches als Mitnehmer für die Kanüle 24 dient, fest mit dieser verbunden ist sowie ebenfalls in der Bohrung 21 geführt wird, ein. An der zur Kanüle 24 abgewandten Seite des Kupplungsstücks sind mindestens zwei elastische Stege 25 angeordnet, welche an deren Ende als Rasthaken ausgebildet sind.



  Wie in Fig.   1    dargestellt, ist die Biopsienadel 18 in die Kanüle 24   kupplungsstückseitig    eingeschoben und wird in dieser gleitend nahezu spielfrei geführt. Die beiden Kupplungsstücke 17 und 23, welche hintereinander angeordnet in der Bohrung 21 geführt sind und mit der Biopsienadel 18 bzw. mit der Kanüle 24 fest verbun den sind, gewährleisten, dass sowohl die Biopsienadel 18 als auch die Kanüle 24 in vorteilhafter Weise ohne weitere Führungelemente exakt auf der Symmetrieachse 8 geführt sind.



  Im mittleren Bereich der Bohrung 21 zwischen den   Kupplungsstü-    cken 17 und 23 ist ein in dieser umlaufender Absatz 26 im hinteren Bereich ein weiterer, ebenfalls ein in dieser umlaufender Absatz 27 angebracht, welche als gehäuseseitige Einraststufen für die Rasthaken auf den Enden der Stege 19 und 25 in Spannlage, d. h. bei angespannten Federspeichern, dienen. Beide Kupp  lungsstücke    weisen dabei jeweils mindestens zwei Stege auf, welche symmetrisch zu der Symmetrieachse 8 über den gesamten Umfang des jeweiligen Kupplungsstückes angeordnet sind. Damit werden die Kupplungsstücke in Spannlage in vorteilhafter Weise redundant und zentrisch zur Symmetrieachse 8, d. h. ohne die Gefahr eines Verkantens in der Bohrung 21, gehalten.

   Durch eine Gestaltung der beiden Zapfen 16 und 22 als eine in die umlaufenden Nut der Kupplungsstücke 17 und 23 eingreifende Gabel erhält man ebenso in vorteilhafter Weise ein zu der Symmetrieachse 8 symmetrische Verteilung der Angriffsfläche dieser formschlüssigen Verbindung und damit eine weitgehende Eliminierung der auf die Nadel wirkenden Drehmomente sowie ein erheblich geringere Verkantungsgefahr der Kupplungsstücke 17 und 23 in der Bohrung 21.



  Ein in das Gehäuse eindrückbarer   Betätigungsbolzen    28 dient zum Auslösen des   Biopsievorganges    und ist hierfür mit seinem vorderen Ansatz in der Bohrung 21, im mittleren Bereich in einer Bohrung 30 im Gehäuse 1 axial verschiebbar gelagert. Auf der vorderen, der Bohrung 21 zugewandte Seite ist im   Betätigungsbolzen    28 eine Kegelfläche 29 eingearbeitet, die zum Entsperren der Raste 19 dient.



  In Fig. 2 a bis c wird der Spannvorgang des   Biopsiegeräts    der ersten Ausführungsform dargestellt. Zunächst zeigt Fig. 2 a das   Biopsiegerät    in entspannter Ruhelage ; beide Federspeicher sind entspannt, d. h. die beiden Kolben 4 und 5 drücken mit dem vorgespannten Druckmedium 9 gegen den Gehäuseanschlag 10 und 11   (vgl. auch Fig. 1). In einem folgenden Schritt wird, wie in Fig.



  2 b dargestellt, der Kolben 5 von Hand über die Kappe 15 in die Bohrung 3 des Gehäuses 1 eingedrückt, wodurch das Druckmedium 9 komprimiert wird. Mit dem Kolben 5 wird gleichzeitig das Kupp  lungsstück    17 mit der Biopsienadel 18 so weit zurückgezogen, bis die Rasthaken auf den Enden der Stege 19 in den Absatz 27 einrasten. Im Anschluss daran, möglich ist aber auch gleichzeitig hierzu, wird die Kanüle 24 mit dem Kupplungsstück 23 und dem Kolben 4 über die Kappe 14 gegen den Druckanstieg des Druckmediums 9 manuell zurückgezogen, bis die Rasthaken auf den Enden der Stege 25 in den Absatz 26 einrasten   (Fig.    2c). Beide Federspeicher sind nun angespannt ; das   Biopsiegerät    befindet sich jetzt in Spannlage.



  In Fig. 3 a bis c wird der Schussvorgang des   Biopsiegerats    dargestellt. Die Kolben 4 und 5 sind zunächst über das Druckmedium 9 vorgespannt, die Rasthaken der Stege 25 und 19 an den Kupplungsstücken 17 und 23 halten die Biopsienadel   18    und   dieKanüle    24 in Spannlage. Durch manuelles Eindrücken des   Betätigungsbol-    zens 28 in das Gehäuse 1 werden die elastischen Stege 19 des Kupplungsstücks 17 über die Kegelfläche 29 soweit zusammen gedrückt bis die Rasthaken vom Absatz 27 gedrückt werden und die Rastverbindung gelöst wird. Angetrieben durch den Druck des Druckmediums schnellt daraufhin die Biopsienadel 18 in das zu biopsierende Gewebe 32 des Patienten, bis der Kolben auf den Anschlag 11 auftrifft.

   Kurz zuvor trifft die Kegelfläche 20 im Kupplungsstück 17 auf die Stege 25, biegt diese gegeneinander, sodass die Rasthaken von dem Absatz 26 geschoben werden und damit ein anschliessendes Nachschnellen der Kanüle über die Biopsienadel 18 ausgelöst wird (Fig. 3 b). Die Vorschubbewegung der Kanüle endet mit dem Auftreffen des Kolbens 4 auf dem Anschlag 10 (Fig. 3 c). Durch die hohe Geschwindigkeit der Kanüle 24 wird das in der Aussparung 31 der Biopsienadel 18 befindliche Gewebe 32 dabei abgeschert, wobei die Kanüle 24 die Gewebeprobe 33 umschliesst. Das   Biopsiegerät    befindet sich nun in Ruhelage und kann für eine Nachuntersuchung der Gewebeprobe 33 aus dem Gewebe gezogen werden. 



  Zur Entnahme der Gewebeprobe 33 wird die Kanüle 24 manuell über die Kappe 14 zurückgeschoben, wobei die Aussparung 31 freigelegt wird und die Gewebeprobe 33 entnommen werden kann.



  Das   Biopsiegerät    in der zweiten Ausführungsform zeigt Fig. 4 in Ruhelage. Diese unterscheidet sich von der zuvor beschriebenen Ausführungsform in der Art der Federspeicher, welche anstatt eines komprimierten Druckmediums 9 mit Druckfedern 34 und 35, welche auf die Kolben 36 und 37 wirken, ausgestattet ist. Die Bedienung der zweiten Ausführungsform, insbesondere der zuvor beschriebene Spann-und Schussvorgang gemäss den Figuren 2 a bis c und 3 a bis c entsprechen denen der ersten Ausführungsform.



  Fig. 5 zeigt das   Biopsiegerät    der dritten Ausführungsform in Ruhelage, d. h. mit entspanntem Federspeicher. Im Gegensatz zu den beiden zuvor beschriebenen Ausführungsform enthält jeder der beiden Federspeicher mindestens eine bereits in Ruhelage vorgespannte, hochelastische Gummizugfeder 38, welche die beiden Kolben 4 und 5 über eine in diese eingebrachte längs durchgängige Bohrung durchdringt, beidseitig jeweils einen Kopf aufweist und mit diesen. formschlüssig auf der einen Seite im Gehäuse 1 nahe den Anschlägen 10 bzw. 11 sowie mit der anderen Seite in den Kolben 4 bzw. 5 fixiert sind. Konstruktiv bedingt entfallen bei der dritten Ausführungsform die beiden Kappen (vgl. 14 und 15 in Fig. 1), mit denen die Federspeicher der beiden zuvor genannten Ausführungsformen manuell gespannt werden.

   Statt dessen weisen die Kolben 4 und 5 je eine Spannnocke 39 bzw. 40 auf, welche durch je einen Führungsschlitz 41 bzw. 42 geführt aus dem Gehäuse 1 herausragen.



  Ferner eignen sich als Federkraftspeicherelemente des Federspeichers auch hochelastische Bänder, vorzugsweise Gummibänder, welche ausserhalb des Gehäuses geführt an dieses befestigt sind, und auf Mitnehmer, welche die Vorschubbewegungen für die Biopsienadel und die Kanüle in das Gehäuse übertragen, wirken. Konstruktiv ist eine Führung der Mitnehmer gemeinsam mit einem Schlitten parallel zu der Vorschubbewegung der Biopsienadel und der Kanüle denkbar. Wie bei den zuvor beschriebenen Ausführungsformen wird die Vorschubbewegung formschlüssig über Zapfen (vg. Pos. 16 und 22 in den Figuren) auf die Kupplungsstücke der Biopsienadel und der Kanüle übertragen. Funktionsmässig ersetzen dabei diese Schlitten die Kolben 4 und 5 der zuvor beschriebenen   Ausfüh-    rungsformen. Diese Variante ist in den Figuren nicht dargestellt.



  Bei allen beschriebenen Ausführungsformen bilden Biopsienadel 18 und Kanüle 24 gemeinsam mit den Kupplungsstücken 17 und 23 kon  struktiv eine    separate Einheit. Sie ist beispielsweise als sterile Einwegkomponente im Biopsiegerät in vorteilhafter Weise austauschbar integriert. Wird ein Austausch dieser Einheit gefordert, ist das Gehäuse 1 des   Biopsiegeräts    so zu gestalten, dass die Bohrung 21 für den Austausch zugänglich ist. Im Folgenden werden hierzu anhand der Figuren 6 und 7 zwei Gestaltungskonzepte dargestellt. Stellvertretend für alle zuvor beschriebene Ausführungsformen zeigen die Figuren 6 und 7 die   Ausfüh-    rungsform mit Gummizugfeder (Fig. 5).



  Fig. 6 a bis e zeigen einen Aufklappmechanismus für die Bohrung 21 im Grund-und Aufriss, bei dem das Gehäuse mit den mit den beiden angedeuteten Federspeichern 43 mit einer Klappe 44 versehen ist, welche um das Mittelteil eines der beiden Federspeicher 43 schwenkbar gelagert ist und auf selber Höhe am anderen Federspeicher einen Schnappverschluss 45 aufweist. Fig. 6 a und b zeigen das Biopsiegerät im zugeklappten, Fig. 6 c bis e im aufgeklappten Zustand, wobei die Bohrung 21 seitlich zugänglich wird. Wie man den Figuren 6 b und e, ist ein Aufklappen des der Biopsienadel 18 abgewandten Seite des Gehäuses für ein Einlegen der im vorigen Absatz genannten Einheit nicht erforderlich.



  Hierdurch erfährt dieser Bereich des Gehäuses keine Schwächung, sodass der Aufklappmechanismus in der beschriebenen Art auch für das Biopsiegerät der ersten Ausführungsform mit   Druckmediumge-      stützten    Federspeichern (gem. Fig.   1    bis 3), bei denen gerade dieser Bereich des Gehäuses 1 durch den Innendruck des Druckme diums 9 besonders belastet ist, eignet.



  Als Alternative eignet sich auch eine zweite Variante des Aufklappmechanismusses, bei dem, wie in den Figuren 7 a bis d dargestellt ist, das Gehäuse 1 längs entlang der Bohrung   21    so geteilt ist, dass zwei Gehäusehälften mit jeweils einem Federspeicher 43 entstehen. Fig. 7 a und b zeigen das Biopsiegerät im zugeklappten Zustand, Fig. 7 c und d im aufgeklappten Zustand.



  Wie in den Schnittbildern 7 a und 7 c zu erkennen ist, sind die beiden Gehäusehälften über ein Scharnier 46 schwenkbar miteinander verbunden und wobei im geschlossenen Zustand (Fig. 7 a) ein Schnappverschluss 45 der einen Gehäusehälfte in die andere Gehäusehälfte einrastet. Als weitere Gestaltungsvariante bietet sich an, die Substitution des Scharniers 46 durch ein Biegeelement oder einen zweiten Schnappverschluss an.

   Automatisches   Biopsiegerät    für die Entnahme von weichen Gewe beproben aus einem lebenden Organismus, bestehend aus a) einem Gehäuse (1), b) einer Biopsienadel   (18)    mit einer seitlichen Aus sparung (31) nahe der Nadelspitze, welche axial im Ge häuse   (1)    gelagert ist,    c)    einer Kanüle (24), welche axial bewegbar auf der
Biopsienadel   (18)    gelagert ist, d) einen manuell vorspannbaren und manuell auslösbaren
Federspeicher im Gehause (1.) seitlich der über einen

Rastverbindung gehalten wird,    g)    die Biopsienadel (18) und die Kanüle (24) aus einem für einen Einsatz im Magnetresonanztomographen MRT ge eigneten nicht magnetisierbaren und nicht elektrisch leitfähigen Werkstoff, vorzugsweise aus einem faser verstärkten Kunststoff bestehen, sowie h) die beiden Federspeicher sich aus für einen Einsatz im Magnetresonanztomographen MRT geeigneten nicht mag netisierbaren und nicht elektrisch leitfähigen Kompo nenten zusammensetzen. 



  2. Automatisches   Biopsiegerat    nacn Anspruch 1, dadurch gekenn zeichnet, dass Biopsienadel (18) und Kanüle (24) gemeinsam mit jeweils einem Kupplungsstück (17) und (23) als eine Ein heit austauschbar im   Biopsiegerat    integriert ist.



  3. Automatisches   Biopsiegerat    nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Federspeicher aus je einem Kolben  (4) und (5), welche in je einer verschlossenen, vorzugsweise als Sackloch ausgeführten Bohrung   (2)    und (3) gegen ein ein geschlossenes Druckmedium (9), vorzugsweise mindestens einem komprimierbaren Fluid, federnd dichtend geführt ist, besteht.



  4. Automatisches   Biopsiegerat    nach Anspruch 3, dadurch gekenn zeichnet, dass die Kolben (4) und (5) druckmediumseitig mit
Bohrungen (12) und (13) versehen sind, wodurch ein vergrösser ter Volumenspeicher für das Druckmedium (9) im Federspeicher bei sonst unveränderten Abmessungen entsteht.



  5. Automatisches   Biopsiegerät    nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Federspeicher für die Realisierung der Federkraft je eine nichmetallischen Druckfeder (34) und  (35), vorzugsweise aus einem faserverstärkten Kunststoff oder
Keramik, enthalten.



  6. Automatisches   Biopsiegerat    nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Federspeicher für die Realisierung der Federkraft jeweils mindestens eine Gummizugfeder (38) enthalten.



  7. Automatisches   Biopsiegerat    nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass die Federspeicher je einen auf einer
Schlittenführung parallel zu der Biopsienadel bewegbaren
Schlitten enthält und die Federkraft über jeweils mindestens ein hochelastisches Gummiband, welches zwischen dem Schlitten und dem Gehäuse gespannt ist, realisiert ist.
